The Importance Of Preventive Eye Care

O

f all the human senses, vision is often named as the most important and precious – that which would be missed most if lost.

According to the CDC, approximately 11 million Americans over the age of 12 could improve their vision through proper refractive correction, and over 3 million (3%) over 40 are either legally blind or have low vision.

People often seek eye care if their eyes are red or painful, or if vision becomes blurry. But routine eye examinations are important even if eyes and vision are fine – because many blinding eye diseases have few or no warning signs until vision loss is severe and possibly irreversible.

The most common cause of

blindness in the United States is diabetic eye disease. In the early stages, when most easily treated, diabetic eye disease has no symptoms. Diabetics should have a comprehensive, dilated eye examination at least once a year to prevent serious complications.

Maintaining good blood sugar control is also important to the health of the eyes.

The second most common cause of blindness in the US is glaucoma.

Vision loss from glaucoma often goes undetected until the disease is advanced, and once vision is lost, it can never be regained. Also, progression of more advanced glaucoma is more difficult to arrest.

Glaucoma is more common in older adults, but it can occur even in the young. Regular examinations include screening to detect glaucoma.

The most common cause of blindness among Americans over age 50 is age-related macular degeneration. While early macular degeneration has no cure, early detection, vitamins and a healthy diet can help to avoid progression.

In cases which do progress, prompt intervention can improve outcomes.

Worldwide, cataracts are by far the most common cause of blindness. Fortunately, this condition is reversible, and treatment is widely available in

developed countries. Technical advances have made surgery more successful than ever before.

The American Academy of Ophthalmology has revised its recommendations for routine eye examinations, stating children should have routine screening and vision testing during regular pediatric visits, with referral to a specialist in the event of a finding or a family history or other risk factor.

Young adults should have a baseline evaluation, and then periodic routine visits. “The frequency of ocular examinations should depend on the individual’s age, race, past ocular history, medical history, family history of eye disease, and the types of symptoms or ocular findings encountered.” Your eye care professional should recommend an appropriate interval.

In addition to periodic screening examinations, prevention is important for maintaining good ocular health. Always wear eye protection when working with power tools or dangerous chemicals, or when playing sports posing a risk of eye injury. Fireworks and paintball injuries are also common causes of vision loss. Ultraviolet protection (glasses or sunglasses) can help to prevent cataracts, macular degeneration and ocular tumors.

A diet healthy in fish and green vegetables can reduce the risk of macular degeneration. Finally, good blood pressure and blood sugar control, and smoking cessation helps avoid eye disease in addition to other health benefits.

While great advances have been made in ophthalmic treatments including cataract surgery and macular degeneration therapy, prevention and early detection can help to ensure the best possible vision for a lifetime.

Jeffrey Minkovitz, MD is an ophthalmologist with Eye Physicians and Surgeons in Wilmington, Delaware. Dr. Minkovitz specializes in cataract and refractive surgery, and is one of only a few Delaware surgeons specially trained in corneal transplantation. He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins.

Previously, he graduated magna cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. Auburn Hospital, and served his ophthalmology residency at Washington University.

Dr. Minkovitz participated in the early research on Excimer Laser refractive surgery, and has offered this procedure to Delawareans since FDA approval in 1996, before Delaware had its own laser. These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. In addition to introducing laser vision correction to Delaware, he also introduced NearVision CK to eliminate reading glasses, DSAEK (a newer, less invasive type of corneal transplant) and most recently the newest advance: laser cataract surgery.

Dr. Minkovitz is a co-founder of the Center for Advanced Surgical Arts and serves on its medical advisory board. He is also on active staff at Christiana Care. While on staff of Wilmer at Johns Hopkins University, Dr. Minkovitz received the Outstanding Teacher award. He was also named Top Doc in ophthalmology

by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

It finally occurred to me what has been missing during the last 2 days. Since Richard’s surgery, he isn’t complaining about his inability to read something on the fly without searching for his glasses. He wired 6 recessed lights (NO GLASSES), he read the Sunday paper sections A & B and menus (NO GLASSES), and is using the computer (NO GLASSES). I suppose for Richard, needing to look for glasses when he wanted to see something, has been the hardest adjustment of becoming older. His dependence was so gradual that I never related to why he would get so angry over needing them.

My house has been quieter. I now have 2 less things to do everyday: find the glasses and make sure they were clean!

Dear Readers

As I am trying to get the paper ready to go to press I am looking out my back French doors at my beautiful garden and pool, wishing I could go outside and play. It is supposed to reach over 90° today, and I am not sure that it hasn’t already reached that temperature. But, I will not complain, because after the winter and spring that we just had I feel very fortunate to be enjoying this beautiful summer season finally. I hope that each of you are out enjoying your summer as well with plenty of family time.

Enjoy your family wherever and whenever you can because you never know when one of those family members won’t be around. Take me for example, in the last year I have lost several of my close family members. This alone is a very stressful and sad time, but one item that came up in two of their deaths was that they did not have a Will, nor had they made funeral arrangements for their burial. Though they both could certainly have afforded to pay for a funeral, once they died their family’s hands were tied, because nothing from the estate could be touched to pay for the burial. Both had loans that were attached to their other properties, thus my sister-in-law was forced to sell investment property, so that she did not lose everything she had. The other was my daughters’ father he had no Living Will, no Will, and no money. My girls not only lost their father, but they also had great pain in trying to deal with the huge decision of whether or not to put their father on life support. We all knew his wishes, but he had not taken the time to do a Living Will and put those wishes on paper. Even if you have nothing to leave for your family, it is important to put in writing what you want should you become ill or die suddenly and put it in the hands of someone you trust, or at least file it with the courts so that it can be found. Don’t wait until it is too late, and your family has to suffer even more because they have to make the decisions about your remains or other important health decisions that each of us should make long before the end of our time. Even if you go on-line and download a form… please do yourself and your family a favor and do something before it’s too late!

Women Profile in

Catherine M Marino

Catherine M Marino, AuD joined the Riddle Hospital Audiology team in October of 2011. She has been providing audiology services for more than 35 years in both medical and rehabilitation settings. Her goal is to help people hear their best over their lifetime. This starts with hearing evaluations early in life so that individuals can appreciate the value of their hearing and preserve what they have. “Many people still do not realize the potentially harmful affects of loud noise and certain drugs, especially if not prescribed by a physician.” She believes that hearing checks are just as important as dental and vision checks.

Hearing and balance disorders can occur at any age and can be temporary or permanent. Audiologists are professionally trained to evaluate auditory and vestibular (hearing and balance) function. If the problem has a potential medical cause, the individual is referred immediately to a physician for evaluation and treatment.

Dr. Marino most enjoys helping people find the best hearing solutions for their individual needs while employing best practices. “Hearing devices today are both light and cosmetically appealing. They can automatically deliver the desired speech signal above background noise.” Her passion is to help people communicate effortlessly in all facets of their life using the amazing new hearing technology that is currently available.

Background: Dr. Marino earned her Doctor of Audiology degree from Salus University in 2010. She received her Master of Arts degree in Audiology from Northwestern University in 1975 followed by an externship at the Mayo Clinic in Rochester, Minnesota. She received her Bachelor of Arts degree in Speech Pathology and Audiology from The George Washington University. She is former Director of Audiology at Ear Nose and Throat Associates of Chester County, Wireless Technology Specialist, and staff audiologist at Elwyn. In addition, Dr. Marino is a fellow in the American Academy of Audiology, a member of the Pennsylvania Academy of Audiology and has her Certificate of Clinical Competence from the American Speech-Language-Hearing Association. She is licensed to practice audiology in the Commonwealth of Pennsylvania since 1988.

Dr. Melissa P. Broyles

Melissa P. Broyles, D.O. has been a first-rate doctor for over 16 years. In 1993, she graduated from the Catholic University of America on the Dean’s List with a B.S. in Biology. She received her D.O. from the Philadelphia College of Osteopathic Medicine where she graduated in 1997. After obtaining her D.O. she did a general rotating internship at Mercy Suburban Hospital where she was elected chief intern. She then did a three year Family Medicine residency at Abington Memorial Hospital where she was named the Women’s Board Scholar and received the Resident Teacher Award. In January 2014, Dr. Broyles completed a two year Integrative Medicine fellowship directed by legendary Dr. Andrew Weil at the University of Arizona.

Dr. Broyles continues to learn about Integrative Medicine including topics focusing on nutrition, natural supplements, stress reduction, bioidentical hormone replacement and digestive health. Her emphasis is on holistic care. Dr. Broyles believes that “drugs are not the answer to every illness and complex problems require complex solutions - including mind, body and spirit approaches to wellness.” Dr. Broyles has chosen a career in Integrative Medicine because she wants to be able to spend more time with patients and “get to the root of the problem.”

Dr. Broyles has a wide range of experience in different areas of the medical field. She is currently a member of the American Osteopathic Association, the Pennsylvania Osteopathic Medical Association, the Bioidentical Hormone Society, and a Fellow in Integrative Medicine. Rest assured that Dr. Broyles has the skills to address your health needs.

In her spare time, Dr. Broyles enjoys being part of the Glen Mills community where she has practiced and lived in for 12 years with her husband and two children. She particularly enjoys running, camping, and hiking. When she’s not in Thornton, you can find her on medical missions across the world. Joanne Graves started her career in the spa industry as the only esthetician and massage therapist at a salon focused on nail services. Dr. Barbara J. Madden Dr. Barbara J. Madden is a doctor of audiology at Riddle Hospital in Media, Pennsylvania. Her experience includes more than 25 years of direct patient care, including diagnostic hearing assessments and comprehensive hearing aid care in this community. As well, she directs the Audiology Department & Hearing Aid Center at Riddle Hospital.

Dr. Madden is a results-oriented audiologist, who listens to her patients in order to formulate the best hearing solutions based on their individual needs and communication goals. She is committed to investigating and identifying issues and problems to recommend innovative and creative solutions for her patients. Her areas of special interest include practice development, marketing, community education, customer service training and public speaking. She is a certified facilitator for leadership and management training.

Dr. Madden received her Master’s Degree in Audiology from Penn State University and her Doctoral Degree in Audiology from the Pennsylvania College of Optometry, School of Audiology. She is board-certified by the American Board of Audiology, a fellow of the American Academy of Audiology and certified by the American Speech-Language-Hearing Association.

In recent years, Dr. Madden established an Audiology & Hearing Aid Center at Maris Grove, a local retirement community.

Dr. Madden and her husband enjoy travel and exploring the greatness of our country, particularly the National Parks. She resides in Chester Heights and has lived most of her life in Delaware County. She is an avid reader and participates in numerous volunteer activities for the hospital and the community.

Denise E. Stewart, M.S., CCC-A, is a Pennsylvania licensed audiologist who has been practicing for over 26 years. She received her Master’s Degree in Communication Disorders from Pennsylvania State University and her Bachelor’s Degree in Communications and Secondary Education from West Chester University. Denise is a fellow of the American Academy of Audiology and certified by the American Speech-Language-Hearing Association. Her areas of special interest include hearing conservation and aural rehabilitation, hearing aid dispensing, diagnostic audiology and balance assessment. Denise has worked as an audiologist in private practice, in hospital and rehabilitation settings during her career. She has been a staff audiologist at Riddle Hospital for 13 years.

Denise’s practice philosophy places hearing loss prevention and hearing conservation as a priority. She believes that an audiologist’s first responsibility to patients is to counsel them to protect their hearing whether it is impaired or not. Denise reminds patients that it is never too late to protect your hearing.

Lisa C. Mackenzie

Lisa C. Mackenzie, M.S., CCC-A, has been a clinical audiologist in Delaware County for 25 years. She has provided comprehensive diagnostic and rehabilitative services in the Audiology Department at Riddle Hospital for the past 18 years. Lisa received both her Bachelor of Science degree and her Master’s Degree in Audiology from Ithaca College in Ithaca, NY. Lisa is certified in audiology through the American Speech-LanguageHearing Association and licensed to practice audiology in the Commonwealth of Pennsylvania.

Lisa performs diagnostic testing to identify hearing loss and assess patients’ communication abilities, and she also provides balance assessment evaluations to patients who suffer from vertigo and dizziness. Lisa enjoys the diverse caseload of patients at Riddle Hospital; their ages range from newborn through geriatric adults.

Lisa is committed to assessing her patients’ unique listening needs and guiding them towards appropriate rehabilitation with advanced hearing aid technology. Her work has helped patients to communicate more effectively and experience improvements in quality of life. She is rewarded when she sees their progress and enjoys the relationships that she has established with her patients.

Lisa is a volunteer in her community, involved in activities for both the church and school at St. Thomas the Apostle. She enjoys time reading, traveling, and spending time with her family. She and her husband are the proud parents of three children and they reside in Glen Mills.

Severe Tooth Wear: The Premature Aging Of A Smile O

ne might expect to see severely worn teeth in the elderly.

Unfortunately I see a great number of patients far from being considered elderly with short, discolored, worn teeth. Some don’t show teeth at all when they smile.

The loss of tooth structure ages these patients.

In addition to the cosmetic impact of tooth wear, there are health issues. Patients may experience sensitivity, fractures of teeth, jaw joint and facial muscle pain. Severe rapid wear may even wear through enough hard tooth structure to expose the nerve of the tooth!

The outermost layer of tooth structure known as enamel is the hardest substance in the human body. Once this enamel is worn through the next layer, dentin, is exposed. Dentin has about the same hardness as bone and is much darker than enamel. The exposure of the darker dentin is why teeth get darker as they wear.

Also, since the dentin is softer than enamel the wear of teeth accelerates. Sometimes wear is vertical, making teeth shorter.

At times the wear is front to back making teeth thin, almost transparent.

Severe wear isn’t the result of the

normal function of chewing. It is, rather, the result of parafunction, habits such as grinding, clenching, nail biting. A special type of tooth wear known as erosion occurs as a result of acid attack on teeth.

Stomach acids come in contact with the teeth when patients suffer from acid reflux diseases or bulimia. A highly acidic diet can also cause erosion of the teeth.

Ideally stress reduction is the best solution.

Sometimes counseling, medication or biofeedback help.

Most often stress can not be eliminated and our ability to deal with it is less than perfect. In this case a “damage control” appliance known as an occlusal guard can be worn. There are many designs of occlusal guards. Some are worn at night when most people parafunction without even knowing they do.

What can be done?

First, have regular dental check –ups so that your dentist can detect the early signs of abnormal tooth contact and help you either eliminate the cause or limit the damage.

When erosion is the type of wear, a change of diet or referral to a medical specialist is appropriate. If attrition of vertical height of teeth is noted, then grinding or clenching is most often the cause. Improper tooth/jaw position can trigger

It is important to recognize some signs and symptoms of hearing loss in order to seek treatment. Here are a few guidelines to help you decide if you may need audiologic evaluation: • You frequently ask others to repeat themselves or find yourself missing parts of ongoing conversation in a small group of people.

udiologists frequently see patients who come to the office for a baseline hearing test.

These patients often report that their spouse prompted them to have their hearing checked. Yet most of the time the patient fails to relay observations of their own communication difficulties. Only when asked specifically about certain situations do they admit to difficulty hearing in a large group of people and in noisy restaurants.

These patients often negate the importance of these situations citing that they rarely attend large parties or noisy restaurants. The people they can’t hear well are described as being unusually soft spoken. Why do so many people fail to recognize their own hearing difficulties? One reason is hearing loss usually occurs very gradually and it often starts first in the high frequency range where clarity of conversation is

You are unable to follow conversation in noisy places where others are able to converse without the same level of difficulty that you are experiencing.

•

You use a higher TV volume than others in your family or your spouse

•

You are hearing noises in your ears called tinnitus such as ringing, chirping ,buzzing or wind noise frequently or on a constant basis

•

You have frequent difficulty understanding telephone conversation and find that you avoid answering the phone.

If you find that one or more of the above conditions are true of you, you should ask your physician to refer you for an audiologic evaluation. The audiologic evaluation is completely painless and some patients even find the test to be fun! So, it is difficult to understand why so many people

delay having their hearing evaluated for years. Research has shown that delaying evaluation and treatment of hearing and communication difficulties has the following negative effects: •

Increased stress resulting in irritability, a negative attitude and anger

•

Fatigue, tension and depression

•

Avoidance of many social situations resulting in loneliness

•

Impairment of memory and ability to learn new tasks

•

Reduced job performance and earning power

•

Reduced alertness and awareness of one’s environment resulting in increased risk of personal safety

•

Diminished emotional health

•

Diminished overall health due to effects of ongoing stress on blood pressure and the immune system

So you can see that “a little bit of hearing loss” can be a dangerous thing if not recognized and treated.

After your hearing is evaluated, your audiologist will counsel you regarding your treatment options.

Treatment may involve use of hearing aid technology, assistive communication devices, training in tinnitus management strategies and training in communication strategies to address difficult listening situations.

ith a healthy uneventful pregnancy, Kelly expected the birth of her second child to be free of complications. But after 24 hours of labor, Kelly began to hemorrhage and lost 40 percent of her blood volume. She was rushed to the operating room and received four blood transfusions.

“The blood transfusions saved my life,” said Kelly.

“Thanks to the blood donors and Blood Bank of Delmarva, my two beautiful children still have their mom!”

Unfortunately, stories like Kelly’s are not uncommon and don’t take a vacation during the summer months, a time when blood donations are typically slower.

Last year, a record 241 companies participated in the Summer Blood Challenge. This resulted in 14,308 blood donations.

Blood Bank of Delmarva is a non-profit community service organization that provides blood and blood products to 16 hospitals in the Delmarva region.

More than 83,000 blood donations were needed last year for about 20,000 patients across the Delmarva Peninsula.

Gallstones more common in women and may require surgical removal

80 percent of all gallstones and consists mostly of undissolved cholesterol. Because most gallstones result from high levels of cholesterol, diet plays a very important role in the prevention of gallstone development.

ach year, approximately 800,000 people. are admitted to the hospital for complications with their gallbladders – most often because of gallstones. They occur more commonly in women, and more frequently in those who have been exposed to higher amounts of estrogen during their lifetime through birth-control pills, hormone therapy, or pregnancy.

The gallbladder is a small pearshaped organ located just below the liver. It plays an important role in food digestion by storing bile produced by the liver and providing it to the small intestine where it breaks down fat. The gallbladder generally goes unnoticed unless a patient experiences symptoms of a disorder, such as gallstones.

Gallstones are like grains of sand or small rocks that develop in the gallbladder as a result of bile becoming chemically unbalanced.

Only about 60 percent of people with gallstones experience symptoms, as they can develop and pass through the gallbladder and intestines without causing symptoms. However, when a gallstone is large it may become caught in the passageways, or ducts, of the gallbladder, pancreas or liver, resulting in extreme pain.

Gallstones generally fall into one of two categories, depending on the cause. Pigment gallstones develop when bile contains too much bilirubin, a naturally-occurring chemical normally removed by the liver. Pigment gallstones generally develop in individuals with liver conditions such as cirrhosis or sickle cell anemia. Another type, cholesterol gallstones, comprises

Diets high in fats and cholesterol are a main contributor to gallstone development. However, cholesterol levels in your bile do not always correspond to cholesterol levels in your blood, especially since cholesterol-lowering drugs do not prevent gallstones even though they lower blood cholesterol levels.

Crash diets that include too little fat, skipping meals, too few calories, rapid weight loss or going long periods without eating prohibit the gallbladder from emptying often enough or completely, increasing the risk of gallstone development.

In addition to diet, other risk factors for developing gallstones include gender, body weight, age and ethnicity. Women are more likely to develop gallstones because estrogen causes more cholesterol to be excreted in bile. Women who have high levels of estrogen in their bodies, such as those who are pregnant, taking birth control pills or on hormone therapy, are especially at risk. Overweight individuals may also have higher levels of cholesterol excreted into their bile. Individuals older than 60 years of age and those of Native American and Mexican-American descent are also high-risk groups.

A person with a healthy liver can function normally without a gallbladder, however, lifestyle modifications such as limiting fat intake and increasing dietary fiber may be necessary for normal digestive function. Although surgery is the most common treatment for large gallstones and blocked ducts, non-surgical options to dissolve or break up the stones may be appropriate for individuals unable to undergo surgery due to complications such as cardiovascular conditions or liver disease.

ven today, many health care providers do not fully appreciate the benefits of higher tech MRI studies. We love to answer doctors’ questions. For instance, we found a patient with marrow abnormalities on a lumbar MRI. In addition to being like a CT scan image, MRI shows vascular structures and bone marrow much better than CT scans. The abnormality we found was not seen on x-ray. The doctor asked if a bone scan would add any information.

The answer was NO. We saved the patient $900 dollars because the doctor asked a question.

Doing the bone scan would have given us no useful additional information. Dr. Chao has 30 years experience which can help you and your doctor save money. We love to save money. A recent review article in Medscape, (http://www. medscape.org/ viewarticle/776272), completely turns everything we know about MR imaging upside down. We offer diffusion tensor imaging and want to do resting state fMRI exams. These exams are useful in patients with both Parkinson’s disease and autism. There are new techniques in imaging—3T MRI, MR spectroscopy, diffusion tensor imaging, DWI and perfusion imaging—all of which are better than the routine post-contrast MRI study for lesion margin evaluation.

We are the only facility in Delaware (and possibly the entire Delmarva Peninsula) with a 32channel head coil. The extra channels translate into better and faster brain imaging. The 32-channel coils themselves are extremely rare. Even in world capitals like Paris and NYC, only one or two facilities have such high resolution coils. MRA’s can be done at super high quality in only 4 minutes. Quantitative MRI can measure white and grey matter and CSF. Diffusion Tensor Imaging picks up subtle brain trauma, Alzheimer’s and even autism. Proton spectroscopy helps do a non-invasive biopsy (we do this for free) if the test might show a stroke or tumor. This is useful information which costs nothing if you come to us for your MRI study with contrast—we just add the 5 min scan for free. Prostate MRI which, at 3T does not require the insertion of an invasive probe/ coil. We do spectroscopic exams for no additional fee because Dr. Chao wants to help patients. Many facilities (if equipped to perform the test) charge up to 100 dollars for the same exam. 3T Breast MRI is 100% sensitive for breast cancer. We do a special lymph node detection sequence called DWIBS. This can change your diagnosis up to 30% of the time. It even adds lymph node detection without using IV contrast. We offer a super high quality abdomen study for $500 that is able to pick up smaller pancreatic cancers, smaller liver cancers and includes diffusion weighted imaging for free. This new technique is up to 96% sensitive for cancer. Ask us about our Outof-Pocket discount $700 for studies that use safe contrast.

Another development has been in the area of the contrasts used for MRI. While the contrasts used for MRI are very different from those used for CT scans, they are not all the same. Some have been linked to a very rare, but very serious, debilitating and incurable illness called Nephrogenic Systemic Fibrosis (“NSF”). We have always been very careful to select contrasts that have NEVER been connected to NSF. The safer contrasts cost a little more, but you’re worth it! We use Multihance (at 1.5T), Prohance (at 3T) and Eovist (for patients with kidney problems).

He continued his studies at the University of Rochester School of Medicine and Dentistry, receiving his medical degree in 1983. After a transitional internship year at the Mary Imogene Bassett Hospital in Cooperstown, NY, Dr. Chao did his residency in Radiology at Brooklyn’s SUNY Health Science Center (19841987), where he became Chief Resident. His advanced research and specializations complete, Dr. Chao left the University of Pennsylvania for a position as Director of MRI in Wilmington, DE – a position he held for 14 years. Dr. Chao eventually left that position to create the best MRI Center in Delaware which uses the very latest technology: 3T MRI. While 3T (tesla) scanners have been used for scanning research patients since 2002, these advanced, more powerful MRI scanners only became available for clinical use in 2004.

Veins Can Be A Pain, Big Or Small I

often reflect back on my experience at First State Vein and Laser Center, thinking about what matters most to my patients.

Obviously, patients that have “big” unsightly, bulging, winding painful veins are concerned about them. But I realize that it’s the “small” things that really matter to the majority of the patients.

“Small” things like reducing downtime, lessening discomfort and the possibility of scarring - matter. To most, it is also the small veins that frequently matter.

Even after ablation (closure) of an incompetent great saphenous vein, the source of their initial complaint, I am often asked what can be done about the remaining bulging vein, or about the small stubborn blue-purple spider veins.

Obviously, we are concerned about life threatening D.V.T. (deep vein thrombosis) or leg ulcers. But for most patients, it is irrelevant; they don’t have them. Most are bothered by non-critical vein problems. As ‘vein doctors’ we should address the entire spectrum of vein problems, should not ignore patient’s strong wishes and treat what matters to them - the big and the small veins.

What is Venous Insufficiency?

Uni-directional valves are essential to insure the flow of blood from superficial to deep veins and from the legs to the heart.

These valves are present in ALL veins, the tiniest and the largest alike. When these valves leak or become incompetent, the blood flows backwards, pools within the veins and weakens their walls.

This condition, called venous insufficiency, may occur in veins of all sizes. In large ‘trunks’ like the great saphenous vein, this will result in what is commonly known as varicose veins, while in smaller (2-3 mm) veins under the skin (reticular veins) it will produce spider veins.

Spider Veins - Often Part of a Bigger Problem As mentioned above, leaky valves in reticular veins result in spider veins. These can be isolated phenomena. But spider veins can also be a manifestation of a deeper, larger vein’s insufficiency that over the years has gradually affected more superficial veins and eventually, reaches the reticular veins. A patient who comes in for the treatment of spider veins, should be first assessed to determine whether these are an

isolated issue, or part of a broader and deeper, problem.

Isolated Spider Veins Can Be Treated By:

Sclerotherapy – closure of the veins by injection of a chemical solution called sclerosant, ideal for spider veins 0.8 - 1.2mm in width.

Ohmic Thermolysis - works by a heat generated by radiofrequency device called “Vein Gogh”, best for spider veins smaller than 0.8mm in width.

L.H.E. Therapy – combined Light and Heat Energy Technology – effective for very tiny veins.

When spider veins are a manifestation of an underlying large veins venous insufficiency, which can happen even without the presence of large varicose veins, the underlying problem should be addressed first.

60 Magical Mommy Minutes

very new mom wants what is best for her baby.

The best food for your baby is mother’s milk. Breastfeeding is one of the best, most loving things you and your baby will share. Earlier this year, the American Academy of Pediatrics (AAP) came out with a formal opinion for new parents, that exclusive breastfeeding is best for babies for the first six months of life, followed by cereal and table food with breast milk, if possible.

Studies have found that babies who are breastfed are less likely to have diarrhea, ear infections and respiratory infections. Later in life, they have less risk of asthma, obesity and diabetes.

Breastfeeding also is an incredibly rewarding experience. As a mom myself, I can tell you that breastfeeding helps to bring you and your baby even closer. Before you know it, nursing will be a natural part of your routine.

Some moms have difficulty getting the knack of breastfeeding, and Skin-to-Skin helps babies to breastfeed even better. Our nurses are trained in lactation to help you begin to breastfeed right in the delivery room and throughout your hospital stay. Christiana Hospital is one of only 88 hospitals in the U.S. participating in Best Fed Beginnings, a program designed to increase the number of hospitals who support moms that breastfeed by becoming a Baby Friendly, USA designated facility.

Nursing moms are often concerned that their babies aren’t getting enough milk. You can’t precisely measure the amount of milk that goes from a mother’s breast into a baby’s tummy but there are several ways you can make sure your baby is well fed.

Through the Baby Friendly Initiative, moms are learning many valuable facts that make breastfeeding easier, such as, using pillows to raise the baby to a comfortable level. You also will learn tips for encouraging your baby to “latch” appropriately to your breast.

Don’t be discouraged if you were not successful in trying to breastfeed your baby. A lot of moms are, in fact, successful the second time around– or the third.

Help for nursing moms At feeding time, make certain your baby is alert. Adults can’t eat properly when they are sleepy -and neither can a baby. Undress your baby, down to the diaper, so you have a lot of skin contact. Rub your baby’s feet and talk softly. This stimulates your baby and also is a time for mother and child to feel really close.

First, check the diaper. If your baby is peeing and pooping, as indicated by your baby’s doctor, (you should receive a sheet indicating how many times each is expected, depending on the day of life), that means your little one is processing food. Many smart phones have easy to use breast feeding apps to help record the number of nursing sessions, the time between feedings, and diaper changes. The apps are especially helpful during the first few weeks until regular nursing is established.

Your baby’s weight also will confirm that he or she is not going hungry. If your baby is gaining weight and continuing to grow, you shouldn’t worry. If you are concerned about your baby’s weight, call your baby’s doctor for a weight check.

Even if you can’t be with your baby all the time, you can still provide milk. If you are separated from your baby for some reason, pumping milk can help with milk production. Many insurance companies pay for breast pumps, so be certain to check with your provider.

Nursing your baby is a natural followup to giving birth. That means starting to breastfeed within an hour of delivery.

A great way to begin is called “Skin-to-Skin,” in which your baby is placed on your chest immediately after birth. This keeps your baby warm and eases baby’s transition into the world, and it’s soothing for your baby after the excitement of being born. O

bstructive Sleep Apnea (OSA) can be found in every segment of population, even in children. One distinction to be made is that OSA in adults can only be managed, not cured. However, OSA in children can be cured.

Consequence of airway obstruction is reduced oxygen available for the body. Oxygen deprivation is especially critical for the rapidly growing body and brain. Approximately 3 to 12 percent of children do snore.

One to ten percent of children go on to develop various forms of sleep breathing disorder.

The mild form is called Upper Airway Resistance Symptom (UARS). UARS causes brain to arouse from sleep to correct the problem then falls back to sleep again. This cycle occurs many times during the sleep; therefore, it destroys quality of sleep. Here are some symptoms often associated with UARS: • Hypertension • Fatigue • Cognitive impairment • Unrefreshing sleep • Chronic pain (often misdiagnosed as Fibromyalgia in adults) Some children do go on to develop full OSA. In OSA, there are more physiological symptoms.

Some of the symptoms in children include: • Failure to thrive

• Bed wetting • Obesity

There are many other signs and symptoms associated with OSA. The list above is, by no means, a complete list. They are mentioned to illustrate how easy it is for the physicians to be misguided to wrong diagnosis. A child with OSA symptoms can be very easily misdiagnosed as other behavior disorders such as ADHD.

The most common cause of childhood OSA is enlarged tonsils and adenoid tissues.

These are soft tissues that line the upper airway and the back of the mouth. Intuitively speaking, they simply “clog the pipe”. Surgically removing these enlarged tissues has proven effective in improving airway. Numerous studies have documented improvement in snoring, OSA, frequent urination, behavior and growth following adenotonsillectomy.

Bony upper airway obstruction is the second category of causes. In this category, bony structures of the nasal/palatal physically impede airflow. Examples in this category are: deviated nasal septum, midface hypoplasia, high vault shape palate. Early intervention to correct these abnormalities, using orthodontics and surgery, would afford a child the chance of normal development; hence avoid living with airway obstruction when he or she is an adult.

Diagnosis starts with

careful review of history.

Unusual behavior problems are usually the first clue.

Be sure to discuss with your physician and dentist about your child’s behavior pattern. If there is a definite sleep disorder pattern, the physician then can do a detailed examination of the airway. He would be looking for any soft or hard tissue blocking the airway. Should there be enough evidence from physical exam to support the OSA diagnosis, polysomnography (sleep test) is warranted to get the definitive diagnosis.

Treatment options vary greatly depending on the cause and severity.

For the most common cause of obstruction which is enlarged tonsils and adenoids, surgical removal is the first line of treatment.

When facial and palatal bone structure is the cause of constriction, orthodontic manipulation may be called for to expand the bony air passage. Childhood obesity is increasingly playing a bigger role in this disease. Healthy eating lifestyle is a prominent requirement in any treatment plan.

Early detection and treatment for childhood OSA allows a child to have normal development both physically and behaviorally/emotionally.

Treatments at early stage of the growth curve tend to have greater and more permanent effects. This represents an opportunity to unlock a child’s growth potential.

Flashes And Floaters – Symptoms Of Possible Retinal Tear Or Detachment T

hink of the eye like a camera: it is a very small organ with many different parts to it. The eye has a lens that focuses the light on the retina.

The retina is like the film of a camera – it lines the back wall of the eye similar to wallpaper. When a patient complains of flashes and floaters, as a retina specialist I am always concerned about problems with retinal tears and /or detachments.

The retina can tear or detach and the initial symptoms may start as flashes and floaters. If the retina starts to detach, then a patient may notice a dark shadow or shade that descends over their vision. If left untreated, a patient will go blind.

Floaters are a common symptom that many patients may experience. Typically floaters are dark, small, and float back and forth. Many times floaters get in the way of vision, but if the eye looks back and forth, the floater can float out of the way. Acute floaters can occasionally be harmless however more commonly acute floaters are a symptom of something more serious. If one has had floaters for many years and they have not gotten worse, then usually it is nothing to worry about. If one experiences floaters acutely, and they have not experienced them before. it usually signifies a problem. The only real way to tell the difference is to get a complete eye examination including dilation, allowing the entire retina to be checked. If a tear develops, many times it is in the periphery of the retina which can not be seen unless the pupil is dilated. If a patient complains of new floaters in one eye and the floaters do not go away s/he should be seen immediately.

Flashes are not as common and are typically associated with floaters. Flashes that are caused by a retinal tear or detachment usually occur in the periphery of the vision (side vision) and are noted at night or in the dark, usually lasting for less than a second. They can occur frequently but the actual flash lasts for a very short period of time. Flashes can also be induced when one looks quickly from side to side.

The most common cause of flashes and floaters is a posterior vitreous detachment (PVD). This occurs as a normal aging process. The process usually begins by age 50, and by age 70 more than 50% of people have had a PVD. The back inside cavity of the eye is filled with vitreous fluid. When you are born, the fluid is clear and gelatinous, like egg white before it is cooked. As one ages, the vitreous starts to liquefy. As it liquefies, the vitreous starts to contract and pull away from the retina. When it pulls away, the vitreous tugs on the retina and causes flashes – the retina is stimulated by the vitreous tugging on the retina. When a PVD occurs, patients will commonly notice flashes and floaters. It is usually during this 6-8 week period of the vitreous pulling away from the retina that a patient is at risk for retinal tears or detachments.

Sometimes when the vitreous jelly is pulling loose, it will cause a tear or a retinal detachment. If a retinal tear goes untreated it commonly will lead to

a retinal detachment. The initial symptoms of a PVD should be checked out immediately.

A retinal tear is much easier to treat (requires laser in the office) than a retinal detachment (requires more involved surgery). Hence, catching a problem earlier and before vision loss is always better. While a PVD usually occurs spontaneously, it can also be caused by blunt trauma to the eye or a hard hit to the back of the head that causes secondary injury to the eye.

In summary, acute flashes and floaters should always be checked out with a dilated eye exam immediately, usually within 1–2 days from when they start.

The most common cause is a PVD, which causes very little damage, but a tear or retinal detachment may be the cause and immediate treatment may be needed.

Role Of Age And Gender In The Early Recognition O Of Heart Attack Victims ptimal treatment for heart attack victims is based on early recognition and institution of therapy, particularly reperfusion.

However, our ability to institute treatment rapidly is determined by how quickly we recognize the presence of the heart attack (myocardial infarction/ MI). Typically, chest pain is the initial symptom alerting us to the possibility of an MI. However, it is possible to suffer a heart attack without chest pain. It now appears age and gender impact the likelihood of this scenario. A study was published this month in the Journal of

the American Medical Association evaluating this phenomenon and its consequences. This registry study included over one million heart attack patients and evaluated for the presence or absence of chest pain. The results are striking. Over one-third (35.4%) of all MI patients presented without chest pain.

However, the percentage was significantly higher for women than men (42% vs. 30%). (In addition, lack of chest pain was more likely in diabetics). The patients without chest pain were less likely to receive reperfusion therapy such as thrombolytics (clot busting medications)

or percutaneous intervention (balloon or stent). They also were less likely to receive appropriate medical therapy such as aspirin, beta blockers, etc. This delay translated into a significantly higher mortality rates for women compared to men (14.6% vs. 10.3%).

The mortality difference was greatest in younger women (less than age 55) suggesting the absence of chest pain in young women with MI is an important predictor of survival.

Interestingly, these differences tended to dissipate with increasing age. This variance may be explained by different

pathophysiology of younger vs. older women with heart attacks. It has been postulated that younger woman with an MI typically have less narrowing of the coronary arteries with concomitant hypercoagulability, inflammation, spasm or plaque erosion/rupture creating the event. This scenario is particularly more common in young female smokers.

This study raises an important question in how to accurately diagnose a heart attack without the presence of chest pain, particularly in young females.

Recognition of atypical symptoms such as shortness of breath, nausea, indigestion, diaphoresis (sweating), pain in other areas (back, neck, jaw, shoulder) is important. Smoking cessation is critical.

Physician awareness of the possibility of heart attack in young women and the fact that it is quite common for them not to have chest pain will aid in earlier recognition and institution of therapy which, in turn, will lead to better outcomes.

Ultimately, however, further research is needed to improve our understanding of the pathophysiology of these differences and provide the general public and physicians with the best information to allow them to seek early recognition and treatment. In the meantime, patients and physicians should remain vigilant in evaluation of all patients at risk, even with atypical symptoms, particularly the young female who is more likely to present in this fashion.

Meet the Physicians of Cardiology Physicians P.A. ALAN MICKLIN, M.D., F.A.C.C. Dr. Alan Micklin joined Cardiology Physicians, P.A. in July of 2006. He began his studies as an undergraduate biology major at the University of Delaware and graduated in 1986. He then went on to attend the Hahnemann University Dr. Alan Micklin, School of Medicine M.D., F.A.C.C. from 1986 to 1990.

Upon his graduation in 1990, he started his residency in Internal Medicine at Hahnemann University. He began his fellowship in cardiovascular disease at Hahnemann in 1993 and was the chief cardiology fellow from 1994-1995. Dr. Micklin is certified in internal medicine and cardiovascular disease, as well as interventional cardiology. He has also been certified to perform nuclear cardiology procedures since 1996 and is certified to interpret vascular ultrasound studies.

Dr. Micklin specializes in both nuclear and interventional cardiology and had been practicing since 1996 in Charlotte, North Carolina before joining Cardiology Physicians. At the University of Delaware he was a member of the Phi Beta Kappa and at Hahnemann University he was a member of Alpha Omega Alpha Honor Societies. He also received a commendation for Superior Performance during his residency at Hahnemann. Some of his clinical responsibilities include nuclear stress testing, echocardiography, and vascular ultrasound. As an interventional cardiologist, Dr. Micklin also performs diagnostic cardiac catheterization including intravascular ultrasound as well as stent placement and angioplasty.

How Spinal Misalignment May Be Affecting Your Golf Game By Dr. Katie Schuler D.C.

T

he spinal column is made of bones called vertebrae, which are composed of soft, cushioning disks that act as shock absorbers, and strong, heavy ligaments that add form and stability. Attached to the spine are many muscles that give strength and movement to the spine. The spine houses the spinal cord and spinal nerves that carry information to the entire body. The spine is a complex structure designed to move and is susceptible to dysfunction. When the spine becomes misaligned, it can affect function in a variety of ways. There are several key signs that your spine, or the supporting structures, may be out of alignment.

Golf can be one of the most technically demanding sports to play. The slightest thing can change a golfer’s game, whether it’s equipment, course conditions or body mechanics. With the use of chiropractic care, those variables related to body mechanics can become more repetitive and predictable. Consistency and accuracy are the keys to a good golf game.

Golfers and athletes from many other sports use chiropractic care to help improve their performance.

There is a Sacro-iliac (SI) joint on each side of the pelvis. Most people have what is called a “Subluxation” www.TheWomensJournal.com

of these joints – a “mis-alignment”.

If one of the SI joints have microtears or weakness in the ligaments, the pelvis can not bear weight on that side. It is comparable to having a sprained ankle, but in the pelvis.

This condition could be painless, but it may include pain in that joint, the lower back or hips.

This injury creates a poor golf swing. If the SI joint is weak, the golfer will not be able to finish their backswing, and will not create enough torque in the body to generate power. The ball will either fade or slice and the distance of the shot will be reduced in an unhealthy individual.

If the front side of your pelvis is unstable, this will also create a severe loss of power.

Chiropractic care can re-align the pelvic bones and give those ligaments a chance to heal.

A chiropractic adjustment can also re-align your lower back, improving the muscle action of your lower body. The combination of chiropractic treatment and core stability training should help you improve your game considerably.

You now have a strong foundation for your swing. The next thing you need is flexibility in the torso and shoulders to create that whip action.

Nerve supply to your upper body comes from your neck. Subluxations in your neck, decreased cervical curvature or poor body posture all limit nerve flow to your upper body.

The stance of most golfers is head forward, shoulders rounded forward and chest drooping downward. This affects the consistency of the swing.

If you look at the posture of many of the young golfers today, they stand with chest out, shoulders square and head in alignment with the rest of their spine. Regular chiropractic adjustments to the cervical spine can improve nerve flow to the upper body. This helps improve posture, flexibility and strength, which in turn helps to create a more repetitive golf swing. Strengthening the muscles in your shoulder is also extremely important.

Rotational stretches for the entire spine can help create the flexibility needed for a powerful, fluid swing.

In 1990 the Human Genome Project launched.

It was an international research project whose goal was to identify and sequence all of the genes of the human genome. The project concluded in 2003 after 92% of the human genome was sequenced. This is a major scientific breakthrough. The hope is that genomic studies will lead to advances in the diagnosis and treatment of diseases, including obesity.

eight loss diets come and go. There was the Atkins, South beach, No-fat, and No-carb diet. However, most people regained weight after stopping these diets and their popularity ceased. Now, a diet based on an individual’s DNA is available. The good news is our DNA will not change, as do fad diets.

The “genome” (the complete set of genetic information) of any individual is unique.

In less than 40 years, the prevalence of obesity in the U.S. has increased by over 50%.

Currently, two out of every three American adults are overweight or obese. Obesity and overweight account for nearly one in every ten American deaths, and costs our nation $223 billion dollars per year. Obesity is a complex problem, and now the epidemic has spread to our children. It is estimated that today’s children will be the first generation to live shorter life spans than their parents. In some cases of obesity, genetics seem to be responsible; in others, various combinations of hormonal, metabolic and behavioral factors may play a role.

We are now able to identify a variety of genes that influence an individual’s eating behaviors, metabolism, response to diet and exercise, as well as identify an individual’s propensity to develop certain medical conditions. DNA testing is a great catalyst to help people make lifestyle changes, especially in respect to diet. A single saliva sample can identify how a person metabolizes fats, carbohydrates and protein. This information can be very helpful for a person trying to lose weight. For example, if an individual is a slow metabolizer of fats, then it’s recommended he/she be on a low fat diet to achieve weight loss. Clinical studies show that individuals who follow a genetically appropriate diet lose weight more easily.

Most insurance companies pay for a portion of the DNA testing. However, the physician must submit necessary paperwork which includes the patient’s vital signs and a valid reason (i.e. weight gain, fatigue) for ordering the test. It takes 2-3 weeks for the lab to process the DNA sample. The patient is then provided a personalized genetic report and an individualized diet plan according to their DNA.

So remember, one diet is not right for everyone.

Swelling In Your Body May Be A Condition Known As Lymphedema Article by Joanna Mikhail, DPT, ATC, CLT Doctor of Physical Therapy Certified Lymphedema Therapist

or Many people are unaware that they are living with a condition known as lymphedema. If you notice swelling anywhere in your body you should ask your doctor if a referral for an evaluation for lymphedema treatment is appropriate.

When a patient receives a diagnosis of lymphedema, they may experience relief that there is a name for their bewildering cluster of symptoms, but there may also be fear and anxiety. How will lymphedema affect my quality of life?

At Lymphedema Clinics of America, our certified lymphedema therapists will help you understand the nature and progression of this condition and provide you with a highly effective treatment plan.

We will initially address relieving your acute symptoms, and then provide you with tools and therapies to keep those symptoms under control in order to improve your quality of life.

The treatment protocol used at Lymphedema Clinics of America is Complete Decongestive Therapy (CDT). This therapy program was developed in Europe and has been used in the United States for 20 years.

“Numerous studies have shown the effectiveness of this approach and is now commonly the treatment of choice.”

There are 2 phases of CDT. The goal of Phase I is to reduce the swelling in the affected limbs. This phase is done at the clinic with the patient continuing some aspects of the treatment at home between sessions.

The goal of Phase II is to prevent the swelling from reoccurring. This phase is continued at home using an individualized program designed by the lymphedema therapist.

Phase 1 of Complete Decongestive Therapy (CDT) is composed of four components. Manual Lymphatic Drainage (MLD), compression bandaging, exercising and skin care.

Manual Lymphatic Drainage is a gentle, manual technique that moves

the lymphatic fluid responsible for the swelling of the limbs, away from the affected limb and into the pathway of healthy lymph nodes, which will release the fluid into the venous system.

Following the MLD, the therapist applies short, stretch cotton compression bandages to the affected limbs. These bandages improve the return of blood to the heart in both the venous and lymphatic systems and help to break up some hardened deposits in the skin that occurs from lymphedema – thus improving skin tone and appearance. The therapist then introduces exercises to be done while wearing the compression bandages. These exercises involve the contraction and relaxing of the muscles in the affected limb, which assists in moving the lymphatic fluid from the area.

The important final aspect of Phase I is skin care. The body part affected with lymphedema is more prone to infections. The skin breaks down making it easier for bacteria to enter the body and the lymphatic fluid is protein rich, making that area an ideal breeding ground for bacteria.

In addition, the immune defenses in that area are reduced making it more difficult for the body to naturally fight the infection. The skin care regime prescribed in Complete Decongestive Therapy addresses these issues and reduces the incidents and severity of skin infections. After the patient has completed the Phase I protocol of CDT, which takes place at the clinic, they move into Phase II. Phase II contains the same elements as Phase I, but the patient is taught to perform these aspects at home, using a program that is tailored to their particular needs and abilities.

Patients visit the clinic for routine progress evaluations and additional assistance when required.

Lymphedema cannot be cured, but through therapy the symptoms can be controlled. The patient experiences improved physical appearance, greater mobility, and an enhanced quality of life. For instance, Reverse Namaste (or prayer hands behind your back), is a pose that no matter how many times I tried in my regular practice, I was not able to get fully into. The support of the fabric and the space created in my shoulders from my aerial practice helped me fully into this mudra (even without the fabric) within 5 times of practicing it.

Another reason I enjoy Aerial Yoga is that no matter what physical limitations a student may enter the room with (sciatica issues, shoulder strain, broken wrist), there are always adaptations that can be made to allow the student to create movement and space in the body. The ability to surrender into postures makes it the perfect way to create more flexibility naturally, and gives the practitioner a chance to feel support where the body needs it.

Today, many people associate Pilates with yoga-like exercises or stretching. That concept may be partially true, but it is a definition that is so simplistic that it’s nearly inaccurate.

Pilates can be performed on the mat or on complex apparatus, or both, during a single training session. Pilates requires concentration, focus, and practice in order to build strength and develop flexibility.

A well-educated trainer encourages clients of become more aware of her body because in addition to the adjusting springs on the apparatus, clients use their own body as resistance.

Many clients are so conditioned to sitting and walking out of alignment, they find correction and balance a bit uncomfortable at first.

hy ballroom dancing? Let me share a bit of my personal experience. Attending my very first ballroom dance class well over a dozen years ago, I found myself in a situation where I felt shy, awkward and unsure that I really wanted to pursue this activity. I struggled to find my place in this new arena. Patient teachers, repetition, and a stick to it attitude helped me gain confidence. I find I now hold my head higher, dress differently and walk with greater assurance. I want the same for my students. I really like to see the light in their eyes when they find joy in movement. My goal is the same; teach, encourage, equip students to experience and enjoy the Gifts of Dance. Including Physical, Mental, & Social Benefits “Dance is a very powerful drug Mr. Keane. According to Elliott M. Antman, MD, a heart specialist at Harvard Medical School, “Dancing appears to be an attractive and fun way for heart failure patients to get their exercise,” Antman said. “I highly recommend it.”

A group of 469 men and women were studied between the years 1980 and 2001. Each member gave details about participation in six brain stimulating hobbies; reading, writing for pleasure, doing crosswords, playing musical instruments, taking part in group discussions and playing board games and 11 physical activities including: ballroom dancing, team sports, swimming and bicycling.

The researchers found no significant association between physical activities and the risk of dementia – except for ballroom dancing. The amazing 76% risk reduction from frequent participation in ballroom dancing by 130 avid dancers was the highest score of all hobbies and physical activities measured in the study.

Dr. Joe Verghese, assistant professor of neurology at Albert Einstein College of Medicine of Yeshiva University, New York, offered the theory that the requirements of ballroom dancing: remembering the steps, moving in precise time to the music and adapting to the movements of one’s partner – are mentally demanding exercises. Ballroom dancing offers both physical and mental stimulation.

Researchers at the Washington University in St. Louis medical school compared the effects of Argentine tango lessons and exercise classes on the functional mobility of 19 Parkinson’s patients. The patients were randomly assigned to do 20 one-hour tango classes or group strength and exercise sessions designed for Parkinson’s patients and the elderly.

All the patients were assessed prior to the start of these activities and after they completed the sessions. The dance classes included stretching, balance exercises, tango-style walking, footwork patterns, experimenting of timing of steps, and dance with and without a partner. By the end of the sessions, both groups showed significant improvements in standard tests designed to measure mobility. People in the tango group showed more improvement in balance than those in the exercise group.

“This type of therapy looks to be superior to what is currently offered,” Researcher Madeline E. Hackney, a pre-doctoral trainee in movement science, said in a prepared statement. “The quality of life improved in these patients because of the social aspect of dancing.”

hen the last school bell of the year rings, your child probably feels excited for a relaxing summer vacation. Every student needs a break from the routine and rigor of a busy school year. Although summer break should certainly be enjoyed, too long of a pause in learning will result in learning loss – otherwise known as regression.

According to the Johns Hopkins School of Education students lose 2.6 months of grade-level equivalency in math computation skills over summer break.

The area that suffers the most for all students is factual/ procedural knowledge.

Summer learning loss is a serious issue, but one that can be mitigated. How can you prevent your child from losing ground during summer vacation? Here are several tips:

• Keep up the reading. Have your child read for 30 minutes a day.

If you child needs motivation, explore summer reading programs at your library, school or local bookstore, which often offer younger children rewards for completing certain reading milestones.

• Write often. Encourage your child to write letters, keep a journal, get together with friends to start a neighborhood newsletter or even collaborate with siblings or cousins to create a family newsletter. Encourage your child’s inner storyteller.

Don’t focus on correcting mistakes – keep it fun and simply allow your child to practice.

• Review prior year assignments.

If you’ve saved some of your child’s work throughout the school year (or textbooks), spend a few minutes each evening reviewing past assignments.

Before school lets out, ask the teacher what concepts are especially important to brush up on throughout the summer.

he value of education in the workplace is well documented. Bureau of Labor Statistics data show that earnings consistently increase with the level of education. On average, workers over 25 years old with bachelor’s degrees earn an astonishing 70% more than their counterparts with high school diplomas. Education beyond a bachelor’s degree increases earnings even further.

Presidential initiatives underscore the importance of higher education for individuals to succeed and compete. As emphasized in White House statements on education, “Earning a post-secondary degree or credential is no longer just a pathway to opportunity for a talented few; rather, it is a prerequisite for the growing jobs of the new economy.”

In addition to completing a college degree, people are turning to certificate programs to boost their earnings, add competencies to their professional toolboxes and improve their career prospects. Certificates are nondegree programs that offer practical, concentrated study in a particular professional area. They provide a way to earn a credential in a relatively short time— generally between four months and a year.

A recent study by Georgetown University’s Center on Education and the Workforce called certificate programs a gateway to gainful employment and college degrees. “Certificates

are a homegrown American invention and are expanding rapidly in response to a wide range of educational and labor market demands,” reported the Georgetown researchers. A major finding of the study is that the postsecondary certificate has become a cost-effective tool for increasing educational attainment and gainful employment .

The University of Delaware’s Division of Professional and Continuing Studies offers a range of certificates in the health care, legal and business fields: Analytics--Optimizing Big Data, Business Analyst, Clinical Trials Management, Lean Six Sigma Green Belt, Paralegal, Advanced Paralegal, Project Management and Social Media Marketing Strategy.Certificates meet in the evenings in Wilmington starting in September.

A certificate information session is scheduled for July 17 at the Doubletree Hotel in

Camp Pecometh Grows Oyster Babies Camp Pecometh and Retreat Ministries, located in Centreville, Maryland, recently made an impact on the nearby environment, both with the cleanup of the Chesapeake Bay and on the local oyster population.

Last summer, an associate from the Chester River Association suggested that the camp might want to be involved with the Marylanders Grow Oysters program. Why oysters? They play a vital role in helping to strengthen the life of the Bay by feeding on plankton and filtering it out of the water. It’s just like a filtration system for the water that comes from your tap at home.

The Pecometh team jumped at the chance, and knew it could definitely be an opportunity they could share with guests, school groups and also the

day campers that attend their “Schools Out, We’re In” program during the school year. Soon after, four cages of “spat on shell” (baby/larval oysters attached to recycled oyster shells) were “adopted”. In October 2013, the day campers did experiments in which they observed turbidity (how cloudy the water was due to particles of dirt and other matter in the water).

Megan Shitama, Retreat Programs Coordinator, with Pecometh commented that it was a great “hands-on thing to do with the kids.” After nine long months, in May of this year, the spat, now growing oysters, were placed in a new home in Lankford Bay, at the southern end of the Chester River. It will take about three years for them to grow to “market” size.

This recent effort shows us that we can all take time to improve the quality of our rivers and streams, and ultimately, the life of the Chesapeake Bay. The Pecometh team will also keep the public updated on the progress of the oysters as they continue to thrive in their new habitat.

“It’s Just A Sign, It Doesn’t Have To Be Professional.” ? y l l a e R , h O

By Carol Mack Warner to the digital billboard President you see on the highway while going to work, signs are so much a part of your everyday life you take them for granted. Outside to inside, lobby to office, in the home, everywhere, signs are there to announce, inform, direct, warn and guide. Signs are a visual, non-verbal way to communicate and not just by the words alone. The type of sign, location, choice of font or letter style, sizing, layout and design all play a part in getting the message and your best image across.

It’s not just a sign – it’s tells others who you are and if designed correctly, will attract the type of client you want. I have a friend who decides where she will eat if she likes the sign outside. If it’s neglected, she wonders what else is being neglected.

This applies to all the signage you have in your business. Does your print material reflect the type of business you do?

A comic book store will likely suffer using signage that a funeral home would use and vice versa. A comic book letter style and super hero are as out of place as somber tones and lettering are for the comic book store.

Where do you go and what do you look for when searching for a graphics professional?

Often word of mouth is a good way to start but don’t stop there unless they make you feel welcome and comfortable to discuss your needs. A full service sign company can save time and money if they fit. They can design and produce the final product without having to search out a company to produce your sign. A good relationship should form and they can provide other types of signs that follow your look without having to repeat the introduction process.

If you want a logo designed, determine the purchase price of the design package so you can own the artwork. This is standard in the industry and should be discussed prior to work being done.

This will allow you to take and use your artwork freely. Remember it is your business and you are the driver, so don’t be intimidated if it doesn’t work for you.

If you work with the designer, you should be asked a variety of questions including the type of client you want to attract. Yes, that is what a sign can do by the way it is designed. All you have to do is watch TV advertisements to see how that works. Other questions include a discussion of the following topics: Type of sign, where will it be located and mounted, what are you trying to say and conveying the message using the visual clues of font style, colors and images. www.TheWomensJournal.com

Type of Sign: The type of sign is determined by the purpose and what you want to say: Large format printing, posters, banners, A.D.A. interior signage, real estate site signs, traffic signs, business cards, engraving, raised lettering, cast bronze plaques, flags, specialty items, vehicle lettering, car wraps, window lettering, labels, decals, magnets, directories, trade show, floor graphics and custom items all have different properties and uses.

It is amazing to think that many weren’t in existence even 20 years ago. New products are coming out all the time.

Location: What distance away from the sign will viewer stand? How will it be mounted? Please check with your township for the appropriate permits and regulations for exterior signs. A.D.A.(American Disabilities Act) signage with raised lettering and Braille II translation also have specifications for production, installation and use.

Layout - Image This is where your message will work or fail.

The layout of your message is influenced by many considerations including purpose, fonts or letter style, graphics or icons, color and size.

If your message is to announce your place of business, then it is important to be visible from

the street quickly – so cars passing by can see the sign easily within the few seconds they pass by.

Remember, this applies to vehicle lettering as well.

The back door is a “billboard” and gets the greatest length of viewing time from the people traveling or stopping behind the vehicle. Drivers pass by the sides quickly, their eyes on traffic ahead. This valuable “real estate” needs to be optimized for quick viewing.

Window lettering, banners, interior placards, business cards and brochures are saved for more detailed information.

Traffic, parking and warning signs are designed to impact immediately and are simple and direct.

Choice of font or letter style, graphics and materials express more information than the written message. The prospective client gets a feeling about the business from the sign: “It looks like it’s a good, clean and friendly place to eat.”

“Too cheap looking – the furniture will probably fall apart.” “Outdated and bad maintenance – probably cutting back on quality.” “New sign – they must be doing well –let’s try it.”

Handwritten signs are pretty much a thing of the past except in “back in 5 minutes” type of situations. Do not confuse this with professional hand lettering, almost a lost art in the industry and highly prized.

The office printer is common place and can be forgiven to be used as temporary signage.

After a short period of time, the sign should be removed or replaced if the message is a permanent one. Temporary signage gets tattered, worn and generally lacks the authority of your company image.

There is more to it than just written words. It’s an opportunity for prospective and current customers to identify with your business and image. An experienced, professional graphic designer and sign maker will make your signs work for you.

So it isn’t just a sign. Really.

Adjusting To Life Financially After A Divorce Review your insurance needs

here’s no doubt about it – going through a divorce can be an emotionally trying time. In addition to the emotional impact a divorce can have, it’s important to be aware of how your financial position will be impacted.

Assess your current financial situation Following a divorce, you’ll need to get a handle on your finances and assess your current financial situation, taking into account the likely loss of your former spouse’s income.

Establish a budget

Typically, insurance coverage for one or both spouses is negotiated as part of a divorce settlement.

Evaluate your health, disability and life insurance coverage. Finally, make sure that your property insurance coverage is updated.

Change your beneficiary designations After a divorce, you’ll likely want to change the beneficiary designations on any life insurance policies, retirement accounts, and bank or credit union accounts. Keep in mind that a divorce settlement may require you to keep a former spouse as a beneficiary on a policy.

This is also a good time to review your will. Make sure that your former spouse isn’t still named as a personal representative, successor trustee, beneficiary, or holder of a power of attorney in any of your estate planning documents.

Consider tax implications

A good place to start is to establish a budget that reflects your current monthly income and expenses. Include your regular salary and wages, and any monthly alimony and/or child support you may receive

You’ll also need to consider the tax implications of your divorce. If you are receiving alimony, it will be considered taxable income to you.

Child support, on the other hand, will not be considered taxable income.

Consult a financial professional

Consider taking steps to try to protect your credit record and/or establish credit in your own name.

In the end it keeps us from the life that we really want. Even after we know the effects of fear and we experience the effects of fear we do little or nothing to change. Our response usually comes in the form of an “I tried.” We say, “I tried to change,” or “I am really trying to get better,” but the truth is that “trying” will never get you anything other than a reason why things didn’t work out. So roll up your sleeves and get ready to break through the barrier of your fear.

5 Keys Of Breaking Through The Barrier Of Fear 1) See it as it is, not worse

Too often we make our fears bigger than they really are. It always reminds me of the wizard in the Wizard of Oz. The big scary Wizard was really a small little scared man. We can’t dismiss the fear, because fear is ultimately here to serve us, but we want to hear its message and then move forward. Most of the times we can handle the fear of normal proportion. It’s when it becomes larger than life that we become overwhelmed by its presence.

2) See it better than it is

4) Take massive action

This is the step where people stop. It is also called approach avoidance. I conduct a seminar called Breaking Through The Barrier.

It is a seminar in which we get people to move past their fears. That is the day where they take everything they learn and apply it to break a pine board with their bare hand.

As we get closer to the time when we break boards, you can see the fear begin to rise.

Even though they now have the resources, they become scared to take the action. You can imagine how powerful it is when they push past the fear and take action and they see the board is broken! Don’t stop at the doorway, go through the door of opportunity.

5) Don’t give up

Even when we take action, it doesn’t mean we will automatically get the results. We could be doing the wrong action, not enough action, not doing the action long enough yet.

We cannot always measure success by the results we get. We must keep focused on what is important and keep moving forward. If our approach doesn’t work, we must try something else. If that doesn’t work, we need to try something else. One quality that is so common in successful people is that they don’t give up.

The acronym for FEAR is False Evidence Appearing Real. We give fear life or more life in our mind. The more we avoid it, the greater it is until it becomes a 10,000 pound gorilla. We must step up and walk through the fear. Not later, not tomorrow but now. Live Free.

We all have resources. Sometimes it is a friend or co-worker, other times it is within us from a time in the past we overcame fear, or it is a place where we can go to get more information and help. kay, we need to get serious about Custody in Delaware and we need to do it now.

I am constantly faced with questions about parents’ rights and children’s rights and what will happen in Court. This article continues our discussion on Custody and will explain three main topics in the hopes that it will allow you to better deal with your Custody issue – not to mention better understand the outcome. These three topics are:

1. Expectations of the Court 2. Co-Parenting 3. Considering your Actions from your Child’s Point of View Number one, the Family Court and its officials deal with families breaking up on a daily basis. When there are two parents fighting over an issue pertaining to their child or children they will expect the following: 1. That the parents have talked about the issue before coming to Court; 2. That the parents have tried to come to some agreement by acting reasonably; and 3. That the parents are using the Court as a last resort.

Do not expect either a happy Judge or a decision in your favor when you’ve done nothing to help move along the situation before taking it to Court.

Number two, Co-Parenting: this is a BIG issue. The Court expects you and the other parent of your child or children to continue to raise this child or children together in a respectful, mature, thoughtful, unresentful, non-spiteful, reasonable manner.

However you chose to have your child or children – be it a long term relationship, marriage, or a one night stand (to quote Dr. Phil “When you choose your actions, you choose your consequences” and we all must understand that in every action we take we are making a choice. I’ll continue to have my “Dr. Phil” hat on throughout this article, so be forewarned.) – you and your child or children’s other parent are tied together for life. It doesn’t stop when the Custody order comes down, it doesn’t stop when she, he or they turn 18. It lasts forever, and Ever, and Ever.

have to communicate with the other parent somehow. So many people I speak with state that they just can’t talk to the other person – they either get yelled at or berated; agreed with so they’ll just shut up; or ignored altogether.

Of course you can’t communicate! You’re divorced, broken-up or barely know each other in the first place. But guess what? You will get absolutely nowhere if you don’t continue to try to communicate. You have to make decisions together. You have to agree on deviations from the Custody and Visitation schedule together. You have to sometimes make alternate arrangements for care for the kids together.

You have to agree on extracurricular activities, summer camp/care, schooling together.

It comes down to one thing – you remain the parents of this child or these children and no matter how much you may hate the other parent or how stupid you think they are or how they are acting, you have to be able to communicate somehow and keep at it until you find a way that works for both of you.

Third is considering your actions through the eyes of your child or children. I think we can all agree that a divorce/break-up is a decision made by parents and children get no say or control in the matter. You must constantly consider how your actions against/ about/involving the other parent looks from your child or children’s point of view. Kids are sponges and absorb so much of their

surroundings, much more than they let on.

They for sure are taking in how you treat their other parent. This not only puts the child in the middle of your relationship and creates feelings of guilt and sadness, but also affects how they are learning how to be married and have children in the future. Is it hard to be respectful of the other parent when you have a lot of resentment? Of course, but you have to do it anyway for the health and welfare of your child or children.

What Happens When Good Employers Go Bad?

Delaware Workers’ Compensation Benefits

urphy’s Law states that whatever can go wrong will go wrong. Much to our dismay, this is true in life, and also with regard to work injuries. Sometimes you can do all the right things but, through no fault of your own, still face difficulties navigating your work injury claim. This is especially true if your employer, for whatever reason, did not have the requisite workers’ compensation insurance in place at the time you were injured.

Thankfully, most employers in the State of Delaware carry the required workers’ compensation insurance.

However, employers are not infallible.

Sometimes they make innocent mistakes like forgetting to pay their insurance premiums, sending their payment to the wrong address, or sending an insufficient check. Other times they purposely evade paying their insurance premiums. For whatever reason, if you’re one of the unlucky few that are injured while working for an uninsured employer, this article is for you!

Normally, if a worker is injured on the job, the employer is very forthcoming with turning over their workers’ compensation insurance information, and filing the First Report of Injury. (See previous articles explaining what the “First Report of Injury” is, and what it does.) If, however, the employer is uninsured, they will usually either use delay tactics in hopes that the injured worker doesn’t press the issue, or they will attempt to bargain with the injured worker. Both tactics are reprehensible, and both have the same effect on the injured workerthey make obtaining workers’ compensation benefits very difficult, but not impossible.

In my practice, I’ve seen a growing number of employers fail to carry the required workers’ compensation insurance. In those instances, when an employee is injured we still press forward as if the employer did have insurance. One of the goals is to get to a hearing with the Industrial Accident Board (the State of Delaware administrative agency that oversees work-related injuries) as quickly as possible. Again, there will be more obstacles placed in your path before you can get to a hearing, but a savvy attorney will be able to guide you past the obstacles to reach your first goal- presenting your case at the hearing.

After the hearing is conducted, the Industrial Accident Board will issue a written decision to the injured worker and also to the employer. If the decision awards benefits to the injured worker, it is now time to put the tenacity of the attorney to the test. The attorney then goes into “collection mode” in an attempt to collect your debt- your workers’ compensation benefits. In doing so, there are several things that can happen: the employer’s assets can be collected, they can be sold at a sheriff sale, their earnings and tax returns can be garnished and they can be forced to sell company and personal assets to compensate you.

ecently, I was summoned for jury duty in the United States District Court. Not every person summoned to jury duty is selected to participate.

Like many, I did not think I would be chosen. However, I was. Instead of being the attorney in the courtroom, I was now Juror Number 2. Over the years, I have heard others complain about having to serve on a jury, or sometimes ask how they could be excluded. The Jury Act allows courts to excuse a juror from service if summoned on the grounds of “undue hardship or extreme inconvenience.”

From my own personal viewpoint, I realize jury service is one of the most important civic duties you can perform. Though candidly, I was very concerned about being out of the office and absent from my clients for a period of time. I am honored to work with a really dedicated, caring team of personal injury attorneys, who also understand how important jury service is. My partners readily stepped in to help me and expertly cover my tasks. While I was out of the

Federal Statute 28 U.S. Code Section 1875 mandates that no employer shall discharge, intimidate, or coerce any permanent employee by reason of such employee’s jury service in any court of the United States.

In the end, I was glad I was chosen to serve on the jury. It was a wonderful experience. I had the opportunity to learn about a whole new area of law I had never practiced, Patent Law. I heard talented expert engineer witnesses from all over the country speak about technology, inventions, and patent infringement. I was fortunate to view highly skilled legal teams for both parties. Each legal team utilized state-of-the-art technology in the courtroom while arguing their positions to the jury. Candidly, it was a whole new experience and change for me to be in a courtroom and be silent.

I will not be called to serve again for two years in Federal District Court under the Jury Act. I am grateful though for learning about an area in which I do not regularly practice, and for the new friendships I made.

However, I am happy to be back to my comfort zone – helping clients who have been injured at work or in motor vehicle accidents.

One of the most rewarding aspects of jury duty for me was meeting and getting to know my fellow jurors. We were a quite diverse group of all ages, from all walks of life, and from all three counties in Delaware.

As an attorney, I had previously heard stories that jurors may rush to judgment in order to be able to get

Your Rights Under The Fair Debt Collection Practices Act M

any of us have consumer debt: credit cards, a car loan, a mortgage, or other personal bills, and we may know what it is like to fall behind. We

By Mary Higgins, Esq. may not know, however, that we have rights under the Fair Debt Collection Practices Act (FDCPA) that can protect us from harassing, threatening, and illegal behavior by debt collectors. (Business debts are not subject to the FDCPA.) Before discussing the basic protections provided by the FDCPA, it is important to have a clear understanding of what debt collectors are. Debt Collectors are hired on commission to collect accounts. They may also purchase debts outright from creditors.

All contact between a debt collector and a debtor must include what is known as a “miniMiranda” disclosure (alluding to the rights told to suspects when arrested, known as Miranda rights).

Any oral or written communication must include language that the “communication is an attempt to collect a debt and any information obtained will be used for that purpose.”

If the debt collector does not provide the original creditor’s identity, the collection must cease.

After initial contact, the FDCPA continues to protect the debtor from overreaching, harassing, and illegal activities.

Threatening violence Debts are private matters; not only is revelation to third parties a violation of the FDCPA, but communication that identifies the sender/caller as a debt collector or allows the contents to be read (e.g. postcards) is also

o matter how extravagant or simple a wedding is, the bride and groom are usually working within a budget. The Flower Factor is merely one of the considerations that affect the overall cost of any wedding ceremony and reception.

When you sit down to discuss your vision for your wedding day be sure that your florist knows whether your budget is $900 or $10,000. The venue that you have chosen is an important factor in selecting the size and style of your wedding party’s bouquets, corsages, boutonnieres and ceremony pieces as well as the table centerpieces and serving table arrangements. A cathedral will require a larger and more elaborate floral display than a chapel. If the ceremony and reception are held in the same location it is possible for some arrangements to do “double duty.” Consider the ceiling height in both the ceremony and reception locations. A simple, low arrangement may look perfect in a “country inn” atmosphere, but would be insignificant in a large banquet room. Remember that whatever venue you choose you will want the flowers to make a statement.

The bride’s personal preference in flowers is another major factor.

Choosing seasonal flowers means more value for each dollar spent.

Deciding on peonies in April before they are readily available is not a budget-minded decision.

Neither is having your heart set on lilies of the valley in October.

The season for certain blooms can be quite short and including them in your flower plan means that you will not get them for a budget-friendly price. The size of each bloom naturally affects how many of any particular flower you

When flowers are important to you and the selection for both ceremony and reception is a huge priority but your budget is limited, we suggest that you use your favorite blooms in your own bouquet and in the groom’s boutonniere. Then, if you wish, they can be used sparingly in your attendant’s bouquets, with just a touch reserved for the centerpieces. The flowers used for the altar or pews or even the serving tables at the reception should be in the same color family and theme as the bridal party flowers, but it is not necessary to incorporate the more expensive, favored blooms in these pieces.

Over the years I have learned a few things from the brides that have chosen Belak Flowers for their weddings: • A bride’s bouquet should never be larger than her head! I used to think it had more to do with how embellished the wedding gown was or how tall the bride is.

• To some wedding guests the table numbering system at the reception is an indication of the bride or groom’s favor. No one wants to sit at Table #18 thinking it is probably back in a corner. Using random high numbers like 102 or 303 or 701 adds a little mystique to the day when guests come to pick up their table numbers!

A final thing to remember if your flower budget is limited: Do not decorate spaces that will not be photographed. It is unlikely that all 12 of the stained glass windows in the church will be included in the wedding album.

For the Love of Flowers

It is possible to be quite budgetminded when your planning allows you to re-use some of the ceremony flowers for the reception. We have designed centerpieces so that they can first be used to decorate the aisles in the church. We have also created pomander balls for the ceremony, which can later be used during the reception. It is also possible to use the bridal attendant’s bouquets for head table flowers at the reception and using large ceremony arrangements for the seating card table or in a prominent niche near the dance floor makes great financial sense.

Today there are many venues that provide centerpieces in their “package”. If you like what is offered it could be a significant amount that is not subtracted from your flower budget.

Redesign and Reinventing: I can’t begin to tell you how many times I have had someone in my store who is there to sell some of their Jewelry and they are just clutching onto one piece. Usually, this is the piece that they don’t want to sell because they do have some attachment to it, but know it will just continue to sit there in the box, unused. Nine times out of ten they are just saving that piece to show me last as they are sad to part with it. I always recognize this and beat them to it by asking: “That’s one you really don’t want to sell isn’t it?, Tell me about it”…. I then will recommend a redesign almost to the point of refusing to let them sell it. We usually spend a little time chatting and drawing up a few different options and even incorporate other items they already own to save them some money. Hate My Hair, Can’t Leave My Stylist The One Sure Thing

The first requirement for many is chemistry. Not the color kind, the personality kind. Women relax in the salon; they open up, let their guard down. They need a person that can offer a comfort-zone.

When focused on honing their skills; staying current with advances in the industry; networking to benefit of your community, your client and yourself; and when they can talk about current events with their clients without becoming deeply personal, they’ve breached the threshold of professionalism.

This ability to loosely bond with the client assists the stylist in creating a unique, personalized look for them as they learn about lifestyle, personal preferences, and personality.

ala season is wrapping up. Prom season is winding down. At this point I’ve seen tons of new clients in the salon for coifs, curls and make-up and I’ve chatted with a fair number of women dressed to the nines at galas for some worthy charity and I’m still baffled.

It’s the nature of my industry, once people figure out I’m a hair artist, they launch into questions about their style, my opinion on their color and cut, they solicit my ideas about changes. Giving it some thought, I usually respond, careful not to disparage their current stylist but the number of times I hear some version or another of “hate my hair, can’t leave my stylist” would floor you.

Here I stand, eye-to-eye with a blonde woman, hair nearly waist length, fried from ammonia and over processing, the color is brassy almost orange, no depth and dimension, she’s been seeing her stylist for umpteen years and “can’t possibly hurt her feelings” by making a change.

Inevitably, let me say that again, inevitably when a stylist crosses the boundaries of professionalism with their clients FRICTION will be the result. Why? Stylist are human, they make errors , many are young and the only work experience they may have includes bartender, waitress, or high school student.

Stylists aren’t salaried like employees in commercial industries. Their world hasn’t depended on networking, team work, and professional polish until/ unless they work in a salon that requires these traits in addition to the technical skills of hairdressing.

Hairdressers ARE entrepreneurs.

They must build and retain a book of business (client list).

Many mistake building a book for ”making lots for friends who come to get their hair done”. That can get you started, but it’s a shaky foundation.

It’s an easy question, would you rather your client view you as the ultimate source for all things hair or would you rather they see you as unsettled, unpredictable, chaotic and inconsistent?

Don & Jessie Brown Found Their Journey Easier With Help From Delaware Hospice’s Transitions Program Donald received various treatments; went in and out of remission and in and out of the hospitals. As Donald became weaker, Jessie’s anxieties grew, as did her need for help.

r. Donald Brown was an extraordinary person, and he and his wife, Jessie Brown, had an extraordinary love for each other.

Sometimes you wonder at the fate that works to bring two souls together. Donald was living in Washington DC and Jessie in Virginia Beach when a mutual friend introduced them. Jessie thought she had never met a man who had so much—looks, personality, intelligence and manners. They ended up sharing the next 36 years of their lives.

Donald served on the faculty of several universities and was a lifelong practitioner of theatre arts. Jessie said, “Delaware State University offered a faculty position to Donald in 1997. Jessie and Donald adjusted well to the Dover area, raised their son, and enjoyed their Church and the community around them. Donald became a very popular professor. Jessie said, “Young people know when a professor really cares about them, and he did.”

Eleven years ago, Jessie decided that they both needed to schedule physical exams. Jessie’s nagging finally forced Donald to have a routine colonoscopy. Results revealed cancerous polyps, which marked the beginning of a long journey. During the ensuing years,

Fate acted again in their favor, Jessie believes, when a friend told her to call Delaware Hospice for help from their Transitions Program. Neither Donald nor Jessie were prepared to forego curative treatments, and therefore were not ready for hospice care.

Transitions Program was established by Delaware Hospice to be supportive to patients and family members who were coping with a serious illness through visits, advice on where to find practical assistance, volunteer help, and phone calls.

Kristy Handley, Transitions Program Coordinator, remembers her first visit to the Browns. “I met with the family and talked to them for quite a while. They thought about it and called a few days later and asked me to sign them up.”

Delaware Hospice. Jessie said, “They took good care of both of us, prepared me well, and have kept in touch and offered help dealing with my loss.”

Jessie said, “Kristy somehow brought light into our lives.

She would check on us regularly, and Donald always seemed stronger after talking with her. She gave us hope for tomorrow and helped find solutions to practical things.

She arranged for a volunteer to help transport him to the doctor.

I remember one day Don left his appointment saying he also needed to get to a lab for blood work. That wonderful volunteer offered to come back the same day, after another engagement, to take him!”

What To Ask Before Selecting A Senior Living Community A

s the Director of Sales and Marketing at Foulk Manor South, it is fair to say that I am knowledgeable of important questions that should be asked before selecting a senior living community. However, until I have truly walked in the shoes of seniors facing the important decision of choosing the right senior living, I cannot fully surmise what matters most to seniors considering Independent Living or Assisted Living. In an effort to dig deeper into what potential senior shoppers should consider before selecting a senior living community, I sought the help of Foulk Manor South resident Floyd Jury. Mr. Jury, age 80, has been a resident of Foulk Manor South for 2 years.

He vividly remembers making the transition into Assisted Living with his wife; his input will prove to be invaluable in presenting seniors with important questions and factors to consider when choosing a senior living community. He has agreed to be honest and forthright in his answers.

“Many people give more thought to planning their young child’s birthday party than they do selecting a Senior Living Facility : for themselves or their elderly parents. That’s a mistake, since it is likely one of the last important decisions to be made for yourself or for your parents,” said Jury.

In my interview with Floyd Jury, he suggested that the following topics be considered:

Meals are an important part of your life. Therefore, you should ask to retain a copy of the lunch and dinner menus for the past week. In addition, you should sample both the lunch and dinner meals the day of your visit to the Senior Living Facility. The Host will likely want to isolate you to a separate table, but you should insist on sitting with at least four different residents at each meal.

If you are a couple, ask what happens if one person’s health declines to the point where more intensive care is needed on a permanent basis. Will that care be available in the same building?

What happens to the other person; will they have to move?

This was a very personal issue for me before my wife died. I chose Foulk Manor South primarily for this reason.
