The reinstatement of Private Health Insurance rebates by the Government in April this year for seven natural therapies represents a watershed moment. Through NTREAP advocacy, ANTA successfully demonstrated that these therapies are safe, effective and valued

As we celebrate ANTA’s 70th anniversary in 2025, I reflect on what makes ANTA a unique and enduring association.

From my short time as CEO, what’s now clear to me, is that the answer lies in ANTA’s authentic, unwavering commitment to our fundamental ethos: we work with the natural processes of the body to bring about improved states of harmony and function. This is who we are. This principle guides everything we do to enhance practitioner conduct and clinical effectiveness, across multiple natural medicine disciplines, to the highest professional standards.

And this year has exemplified this longstanding commitment of ANTA, through unprecedented achievement and long-needed strategic clarity.

Strategic Vision and Member Engagement

In October, we launched our inaugural ANTA Strategy - a transformational milestone establishing our compass for the Association’s next chapter. This Strategy is authored by you, our valued members.

Through extensive engagement and collaboration with key opinion leaders across all modalities, we co-created a strategic framework authentically reflecting your professional needs, aspirations and the daily challenges you navigate.

Our inaugural ANTA Natural Therapists Symposium in October was extraordinary. Held on the Sunshine Coast, this milestone event brought our diverse practitioner community together to share insights in clinical excellence, and to connect across multiple modalities, strengthening our collective community and professional standing. As one delegate told us: “I truly felt like I had found my tribe - people who share the same interests and passions and are driven to make the world a better place through natural health and wellness.”

We also launched four ANTA Director CPD Roundtable Webinars, which attracted over 850 member delegates across all sessions. Led by our Directors, Branch Chairs and industry leaders, these sessions directly addressed critical member learning needs: • Clinical record-keeping to safeguard practitioner • Deepening ethical practice and building public trust • Obtaining patient informed consent with confidence • Navigating virtual consultation guidelines effectively.

Our Clinical Nutrition members have benefited from ANTA’s proactive engagement with regulatory bodies on critical safety matters. Practitioners are now better equipped with evidence-based guidance on therapeutic product prescribing and TGA regulatory developments.

Our Naturopathy and Western Herbal Medicine practitioners have contributed substantially to the global evidence base. Their research into herbal medicine applications for ADHD, long COVID, autism, women’s health and thyroid conditions continues to thrive.

And beyond clinical outcomes, the vital role our members play in enhancing peoples’ health literacy will also be actively championed - as a real and profound contribution to public health and individual empowerment. By enabling patients and communities to better understand and use health information to make informed decisions about their wellbeing, ANTA members extend meaningful impact, far beyond the consultation room.

Our Myotherapy and Remedial Therapy branches have achieved significant educational course recognition milestones in collaboration with other associations sharing aligned standards. We also submitted comprehensive NDIS advocacy to government, calling for reinstatement of funding eligibility for qualified practitioners.

Our Chinese Medicine professions are navigating evolving AHPRA regulatory frameworks and implementing proactive harm prevention strategies. Our Acupuncture Branch made good progress in establishing enhanced collaborative approaches to provide a truly united voice to government and regulatory stakeholders. And our emerging Ayurveda community strengthened its visibility and recognition, supported by enhanced professional development initiatives.

Myotherapy

Branch Chair Report Shaun Brewster Myotherapy Chair

As 2025 draws to a close, we reflect on a positive and impactful year at ANTA. This, our 70th birthday year, was filled with much activity, and for Myotherapists the professional landscape has continued to develop and improve in many areas.

ANTA’s commitment to collaborating with a range of other professional associations has resulted in a thorough assessment and approval of several training providers now being endorsed to deliver the updated Advanced Diploma of Myotherapy training package.

This collaborative work has also resulted in combined efforts in advocacy, aimed at improving the recognition and professional standing of Myotherapists and other members at ANTA. Much work still exists to be done to ensure Myotherapy is recognised equally alongside other Allied Health professions. We take this role seriously and commit to continuing this work.

As Chair of the Myotherapy Branch, I have had the great pleasure this year of contributing to development and execution of two major and important pieces of work.

One being the ANTA Strategic Plan, which will inform the direction of the Association for the next 5 years. This task, which has involved the entire ANTA National Council, our CEO, David Liesegang, and our Member Engagement Lead, Danielle Svensson, has produced a clear map for PAGE 06 | The Natural Therapist

us to chart the future course of the association and the members we represent.

The second major project that I have participated in this year was the planning and delivery of ANTA’s inaugural Natural Therapists Symposium, which was held on the Sunshine Coast in October.

The Symposium saw us host an engaging and impactful two days of learning, with world class educators, thought provoking topics and an excited and inspired group of members and guests.

The event showcased two renowned and expert Myotherapists, Rob Granter and Bodine Ledden. Their workshops and lectures were incredibly well received, and the feedback has been outstanding.

2025 also marks the end of my current tenure as National President. I have thoroughly enjoyed this role over the past few years, and respectfully step down as our charter requires a change of roles every three years. I look forward to continuing my role on the board in 2026 and participating in the good work that ANTA does for our professions and our members.

Considerations relating to scope of practice (such as herbalists prescribing nutritional products, as well as vice versa) and ‘practitioner-only’ products (intended for dispensing by qualified health professionals only) are important concepts for all practitioners to be well versed in. If you would like to improve your own clarity and confidence in this area, I recommend you review the ANTA prescribing guidelines and this year’s ‘Clinical Practice Prescribing’ workshop content for more information. This may help you go into 2026 with a stronger awareness of your rights and responsibilities as a Clinical Nutritionist specifically.

2025 has continued to be an eventful year in the field of Nutrition, especially in relation to topics that are highly relevant to ANTA’s Clinical Nutrition members. Here is a recap of some of the most important items from the year.

Clarifying Nutrition Titles

After some confusion and concern seemed to emerge online this year (following certain online discussions) around the topic of which Nutrition practitioner titles are required to practice, it became apparent that some clarifications were necessary for ANTA to reassure Nutrition members of their own status as a fully qualified health professionals, and as practising clinical nutritionists, who have attained the full and complete right to practice as a Clinical Nutritionist in Australia. If you or any of your colleagues feel that any confusion remains, or continue to encounter content from various parties that causes uncertainty in this regard, I would encourage you to read the statements that ANTA has sent to you as a Nutrition member on this topic this year, or otherwise invite you to let ANTA Head Office know if you feel any further direct clarifications continue to be needed.

Prescribing Guidelines

The sale of therapeutic products in certain settings can be confusing for many to navigate. Even for practitioners it can be unclear at times where the boundaries lie, and what the best practices might be, especially in

The longstanding and widespread awareness that persistently high doses of certain forms of Vitamin B6 can cause nerve damage was heightened this year when it was brought to the TGA’s attention that certain highly sensitive individuals may experience this issue even at lower doses. After extensive investigation, the TGA released an interim report on this issue, which was heavily discussed across all forms of media. To help all ANTA ingestive modality members understand this important phenomena and the regulatory considerations that surround it, I wrote an explanatory article providing ‘Insights on the TGA’s Interim Report’ to help translate the key findings, explain some crucial points of distinction around which forms of Vitamin B6 are most implicated, and give direct recommendations to practitioners seeking to manage these risks in the most effective and responsible manner. The final determinations by the TGA will be announced in the first quarter of 2026, however in the meantime I highly recommend all practitioners prescribing nutritional medicines containing Vitamin B6 remain informed on this important topic by reviewing the summary points in the article.

ANTA’s 70th Year Symposium

For those who were not able to make it, this year ANTA’s 70th anniversary year symposium, held on the Sunshine Coast, was benefited by the very generous contributions of Jessica Cox and Jacinta Barbagallo who both gave fantastic presentations as fellow ANTA Clinical Nutrition members within the ingestive stream of the symposium. They shared personal experiences they have encountered in practice over the years, with some helpful clinical insights, and useful business building tips.

I would like to personally thank Jessica and Jacinta for their contributions to this excellent event, and thank all of ANTA’s Nutrition members who attended, as well as those of you out in the field who are bringing your own contributions to our profession and community every day of the year.

Chinese Herbal Medicine Branch Chair Report Tino D’Angelo

Chinese Herbal Medicine Chair As another year draws to a close, I’d like to take a moment to reflect on the year that’s been. As most will be aware, ANTA has introduced a new CPD delivery model and moved away from quarterly seminars in each major state to bring you regular webinars highlighting key topics of interest. Another major change was the introduction of our inaugural Symposium in October on the Sunshine Coast. This year, our Symposium coincided with ANTA’s 70th Birthday celebration. Held at a truly spectacular setting, the Symposium was a landmark event for our members, bringing together practitioners, educators and esteemed thought leaders in an inspiring and collaborative occasion.

The two days were filled with rich discussions, practical learning, and opportunities to connect, making it a truly memorable event. Members also benefitted from direct access to leading exhibitors and suppliers showcasing the latest supplements, herbal products, clinical tools and technology.

As your Chinese herbal medicine representative, part of my role and ongoing commitment to supporting our profession, I am pleased to share key insights from the recent AHPRA/CMBA Reference Group meeting. These discussions highlight important regulatory priorities and emerging issues that will assist in shaping the future of Chinese medicine in Australia: PAGE 08 | The Natural Therapist

1. National Scheme and Regulatory Reform

AHPRA is advancing a proactive regulatory approach focused on harm prevention and public safety. This includes a five-year strategic plan to strengthen cultural safety, eliminate racism, and improve access for Aboriginal and Torres Strait Islander communities. Organisational reforms are underway to enhance capability, data analytics, and stakeholder engagement.

2. Addressing Family, Domestic, and Sexual Violence (FDSV) The National Scheme is leveraging its 960,000 registered practitioners to respond to FDSV through a three-tiered strategy: building practitioner capability, enforcing zerotolerance policies, and embedding trauma-informed practices. Education initiatives emphasise the ‘Recognise, Respond, Record, and Refer’ framework, with cultural and intersectional considerations integrated into standards and CPD.

3. Emerging Regulatory Challenges

Consumer-driven healthcare trends, such as online access to medicinal cannabis, weight-loss injectables, and cosmetic procedures, are reshaping risk profiles. A new Rapid Regulatory Response Unit is addressing these challenges through proactive engagement, cross-sector collaboration, and information-sharing to safeguard public health.

4. Workforce Sustainability

Chinese medicine represents only 0.5% of Australia’s regulated health workforce and faces declining registrations, mid-career attrition, and geographic disparities. Key retention levers include remuneration, job security, and career development support. Broader collaboration is needed to address gaps in consumer demand analysis, international pathways, and graduate support.

5. Evolving Regulatory Needs

To conclude reference group meeting, a panel discussion was held that highlighted rising public expectations for safety, transparency, and accountability. Priorities include managing scope-of-practice complexities, improving communication standards, and clarifying distinctions between acupuncture and dry needling. Future directions focus on evidence-based standards, Medicare access, public education, and integration within the broader healthcare system.

These developments signal a clear shift toward proactive, evidence-based regulation and greater integration of Chinese medicine within Australia’s health system.

For practitioners, this means embracing continuous professional development, strengthening cultural safety practices, and preparing for evolving consumer expectations. It also emphasises the importance of collaboration, both within our profession and across healthcare disciplines to ensure sustainability and maintain public trust. By staying informed and engaged,

we can help shape a future where Chinese medicine continues to thrive as a respected and accessible component of holistic care.

I would also like to echo the sentiments shared by our Acupuncture Chair regarding the energy shift, and importance of unity within the Chinese medicine profession. This year Kaitlin Edin has been actively involved in discussions with other associations to establish collaborative and collective approaches to some of the challenges we face as a profession. As mentioned, these efforts are still in their early stages, however, the formation of a representative body with a united voice to government and regulatory stakeholders is a significant step forward. Such collaboration will strengthen our advocacy, ensuring our profession is well represented in policy decisions, and create strong opportunities to address shared priorities, which will ultimately benefit Chinese medicine practitioners and the communities we serve.

I’d like to sincerely thank all our members for your continued support, both of ANTA, and the broader Chinese medicine profession. Your commitment and engagement not only enable us to advocate effectively but also strengthen our community as a whole. As we look ahead to the coming year, I wish you every success in your practice and encourage you all to stay connected as we work together towards shaping a vibrant and sustainable future for Chinese medicine.

In Memoriam

Derek High

It is also with great sadness that we share the passing of Derek High – a deeply respected and much-loved Chinese Medicine practitioner, lecturer, and clinical supervisor. Derek passed on the 19th of June 2025.

Derek was an extraordinary source of knowledge whose generosity in sharing his expertise profoundly shaped the lives of countless students and colleagues. Beginning his teaching journey at Victoria University in 1995 and later joining the Southern School of Natural Therapies in 2003, Derek taught across a broad range of subjects with unwavering depth, warmth, and remarkable insight. In both the classroom and the clinic, he inspired and challenged students to strive for excellence, and his collegial spirit and passion for Chinese Medicine were evident in all that he did.

His presence will be missed beyond words. Like many treasured members of our profession who are no longer with us, we honour Derek’s legacy and express our heartfelt gratitude for his contributions to his patients, to education, and to the wider Chinese Medicine community.

We extend our sincere condolences to all who knew and loved him.

ANTA have always had high educational standards, and we have been working closely with Torrens University, Endeavour College and Southern Cross University and all other naturopathic and herbal medicine associations so that we can all ensure that the quality remains high.

Climate change and supply challenges are pushing the profession towards sustainability and local sourcing.

Australian-grown botanicals such as lemon myrtle, saltbush and Kakadu plum are gaining ground as both therapeutic and culinary resources. We need to ensure that any growth in this space remains ethical and respectful to traditional owners of land and knowledge.

Naturopathy & Western Herbal Medicine

Branch Chair Report Esther Parker

Naturopathy & Western Herbal Medicine Chair As my first year as Naturopathy and Western Herbal Medicine Branch Chair ends, it’s the perfect time to reflect on what has been happening in our industry.

The biggest news for this year was obviously the government reinstatement of private health insurance (PHI) rebates, following the NTREAP recommendations.

Although this did not translate to ‘instant claims’, we have been in regular consultation with private health funds, helping to ensure that when rebates are available, they are of the greatest value to us as practitioners, and for the public. We have been keeping you updated on this and will continue to do so.

Research is thriving, especially around the use of herbal medicines as it may support ADHD, long Covid, autism, women’s health, thyroid and more. If you have been one PAGE 10 | The Natural Therapist

Globally, herbal medicine is being taken more seriously.

Following the WHO Traditional Medicine Summit, the World Naturopathic Federation’s General Assembly (Brazil) and International Conference for Natural Medicine (Paris), countries are moving toward clearer frameworks for education, sustainability, and traditional medicine integration. Naturopaths and Herbalists all over the world are contributing strong clinical data, and international studies continue to explore herbal pharmacology and the role of plant compounds in stress resilience and inflammation.

As 2026 approaches, Naturopaths and Herbalists face both pressure and opportunity: to maintain traditional wisdom, embrace an ever-evolving health space, and meet the growing demand for evidence-informed, plantbased care. In a world rapidly changing with AI, we need to align with our traditional roots more than ever – and make space for ourselves in future healthcare models.

Our 2025 shift to ‘webinars and symposium’ program has been a huge success. By having consistent, live and online spaces to learn and connect, we feel that we are meeting the needs of busy practitioners. But it’s not all about online – we want to thank you for times where you’ve shown up in person - attending conferences or webinars, presenting research, testing, prescribing and being present for your clients. We know it’s a lot. Thank you to the practitioners who attended our first Symposium in October, on the beautiful Sunshine Coast. We had such a ball and are already busy planning the next one.

I am so honoured to help represent you in Naturopathy and Western Herbal Medicine through events, webinar presentations, community connections, regulatory and industry representation and more, and will continue to do this into 2026. Remedial Therapy Chair 2025 has been a vibrant and productive year for our Remedial Therapy (RT) branch, aligned strongly with ANTA’s broader strategic plan and underpinned by a commitment to high standards, membership growth and advocacy.

Strategic Alignment and High Standards

ANTA’s strategic plan emphasises three core goals: Develop, Support and Impact. This year we have focused our branch efforts to reflect these priorities. ANTA continues to uphold a robust Scope and Standards of Practice framework, ensuring our practitioners deliver safe, ethical, competent care.

Education providers seeking ANTA-recognition are too, subject to stringent assessment, reinforcing that our modalities meet national benchmarks - ensuring public trust and aligning with ANTA’s mission of “best-practice health care” for the public.

Membership Growth and Engagement

Membership within our branch has grown steadily this year as therapists recognise the value of ANTA accreditation - access to CPD, professional indemnity

In October 2025 the ANTA National Symposium on the Sunshine Coast brought our branch together with other modalities. The two-day event included workshops, practitioner panels and networking sessions tailored for remedial therapists, strengthening our community and visibility.

Throughout the year our branch hosted a series of Roundtable Webinars, focused on evolving clinical techniques in remedial therapy, ethical practice in private clinics and business resilience. These sessions provided peer-discussion, real-world case examples and access to national speakers.

In July 2025 ANTA made a joint submission to the National Disability Insurance Agency (NDIA) under the Support Rules consultation. In partnership with Massage & Myotherapy Australia we called for reinstatement of funding eligibility for qualified Remedial Massage Therapists and Myotherapists, highlighting the role we play in pain, mobility, stress and function for NDIS participants.

Advocacy and Representation

Our branch engagement in advocacy remains strong.

ANTA continues to represent the natural therapy sector in regulatory forums, endorsement of high-standard training providers and demonstrating the value of remedial therapy in integrative health settings. The NDIS submission underscored how our practitioners meet rigorous standards (qualifications, insurance, continuing professional development) and deliver valued care. In addition, ANTA’s regular News & Insights publications have kept our members informed of regulatory changes and evolving industry developments.

Closing Remarks

As Branch Chair, I extend my gratitude to all members, volunteers, and the ANTA national office for their dedication throughout 2025. Together, we have strengthened the recognition, professional standing, and community of remedial therapy practitioners across Australia. Branch Chair Report Neerja Ahuja Ayurveda Chair

As we come to the end of the year, I want to take a moment to acknowledge our collective efforts as Ayurveda practitioners within ANTA. Ayurveda in Australia is still in its early stages, and while the landscape may not yet be as active or widely recognised as we hope, every step we take is meaningful. Each consultation you offer, every conversation you initiate, and each moment you spend educating your community contributes to a slow but steady strengthening of our presence.

A key highlight for our community this year was the ANTA Symposium in October. Although the event covered a range of modalities, it provided an important platform for Ayurveda to be represented alongside other established fields. The engagement, questions, and interest from attendees showed that curiosity about natural health, ANTA, and Ayurveda is growing. Being part of the Symposium allows possibility for us to share our perspective, learn from colleagues in other modalities, and continue paving the way for greater visibility and understanding.

Our professional development sessions, being done online now, also continued to play a vital role. One Ayurveda session was in September, and the talk was about The Gut-Brain Secret - Unlock Emotional Strength with Ayurveda. you can. These spaces help us stay connected, deepen our understanding, and lay the groundwork for the next generation of practitioners who will carry Ayurveda forward in Australia.

As we look to the year ahead, I invite each of you to reflect on how you can continue to support and grow Ayurveda in your own way. It could be through research, interdisciplinary conversations, community education, or simply sharing small, practical insights in your networks.

Ayurveda Day celebrated globally and now celebrated on 23rd September, instead of on Dhanvantari Jayanti, remains a wonderful opportunity to increase awareness in your local community. This season also invites us to slow down and honour our own wellbeing. I hope you will find moments for rest, reflection, and renewal. When we nurture ourselves, we become better able to nurture the growth of Ayurveda in Australia.

As your Ayurveda Chair and ANTA Board Member, I remain committed to advocating for our modality, working alongside the National Council, government, and regulatory bodies to create more opportunities and recognition for Ayurveda. While progress may be gradual, it is made possible by your steady dedication. Thank you for your professionalism, your commitment, and for being part of this growing community. I look forward to continuing this journey with you in the coming year.

Across the year, as your ANTA representative, I have been working with other associations within the Chinese Medicine profession to establish a collaborative and collective approach to the professional issues that affect us all. While it is early days, a representative body that provides a united voice to government and other regulatory stakeholders is being formed and its agenda considered. If you have anything to add to that conversation and the agenda, please contact me care of the National Administration Office at admin@anta.com.au As part of our celebration of 70 years since ANTA’s inception, our inaugural practice conference was held on the Sunshine Coast in early October. It was both a fond retrospective and acknowledgement of where we have come from, as well as a renewal of our hopes for natural and human-centred energy medicine more broadly.

Acupuncture Branch Chair Report Kaitlin Edin

Acupuncture Chair I have spoken to practitioners, clients, and colleagues this year, and there are a few themes that have been the catch cry of the Yin Wood Snake, 2025.

Challenge and its intensity have come through from nearly everyone. Whether for good or ill, I am hearing of change, a shifting of gears, the relentless concentration and intention of having to ride the merry-go-round until it stops on its own. Yes, we might have finally arrived on the shore, (or are close) but it’s been pretty exhausting holding on. If you are still being flung around the carousel, hang in there and know it will slow. Your community is here.

Things have definitely felt like they are shifting this year in all kinds of spheres, and while change is an ever constant in our lives, the intensity of it seems to have been particularly significant. The intuitives have recognised it as a year of realignment and adjustment, requiring both the individual and collective to transform from the timeworn patterning into different ways of being and seeing.

Whether the transformation represents better days is up to us, for the choice to love is ours alone.

Paul Coelho in The Alchemist, reminds us “when we love, we always strive to become better than we are. When we strive to become better than we are, everything around us becomes better too.”

After the losses and the trespasses of the COVID years, it was truly wonderful to reconnect and deepen our gratitude for the medicine that our members and speakers enact. The art and soul of learning, healing and community was manifest, and it lit the fire of the love and intention in action that makes us better as people, practitioners and communities of practice.

In looking to the Yang Fire Horse year of 2026, we should remember that Fire is an alchemy, for it is transformative power at its very source. Fire as we know is the element of the heart and associated organs, it’s four-fold dynamic is our own powerhouse of love, transformation, integration and mastery, requiring both maturity and generosity.

Water, earth, metal, wood are true to their nature without fire, but one cannot make the cup, the spoon, the tea, without it. What will you make with the fire of the coming year?

The nature of Horse is as an animal of beauty, power and agency, and it knows its own mind. In relationship with Horse, humanity has gone further and faster than we could have on our own. It reminds us that we are better and stronger together, and the relationship we have with ourselves and others is the sacred ground.

Perhaps with those energies of mastery next year, we can put our individual and collective intentions into the parts of the map that we seek to grow, strengthen and maintain. Yang years after all are generally about action.

Connection, gratitude, reflection and determination will be required.

But then I haven’t met an acupuncturist who didn’t bring an abundance of all of those qualities to the table.

Our strategic review for ANTA’s future direction was completed and shared at the ANTA National Conference in October 2025. Under the expert leadership of ANTA CEO, David Liesegang, and his dedicated team, the NAO continues to serve as a vital first point of contact for members.

We are expanding our messaging across social media and other platforms to keep you informed and connected.

As we end the year, I’d like to take a moment to reflect on our shared progress, acknowledge key contributions, and outline the path ahead. This season has brought steady growth, renewed engagement, and thoughtful recalibration, and I’m proud of the resilience and professionalism our multi-modality branch continues to demonstrate.

Advocacy and Standards

Over the past year, we’ve made meaningful strides in advocacy. The ANTA National Administrative Office (NAO), alongside directors / branch chairs, has actively represented our professions in discussions with AHPRA, government bodies, universities, colleges, and other professional organisations. We’ve stood firm on practice standards in training and education, reinforcing the credibility and integrity of our field. These efforts reflect the clarity, evidence, and unity of voice we’ve cultivated together. We have been working on enhancing professional entry requirements and communicating these steps with the colleges and universities in a collaborative approach.

at the International Congress on Naturopathic Medicine (ICNM), Paris Esther Parker

Naturopath, ANTA Naturopathic and Western Herbal Medicine Chair

Earlier this year, I had the privilege of representing ANTA and the Australian naturopathic community at the International Congress on Naturopathic Medicine (ICNM) in Paris. The experience offered an inspiring glimpse into the diversity of global naturopathy - from rigorous, evidence-informed practice to the more unconventional corners of our profession - it was an amazing privilege to be a part of it.

The ICNM is one of the largest global naturopathic events, drawing practitioners from Europe, North America, and beyond. The ICNM featured a broad mix of speakers and topics. Attendance hovered around 300, with sessions delivering powerful insights and valuable connections.

Some of these practitioner connections I look forward to bringing to ANTA in 2026.

Several speakers stood out for their professionalism and clinical depth and I encourage you to reach out to them, if they align with your own practice here in Australia: • Dr Jacquel Patterson (USA) spoke on neuroinflammation,

Dr Nasha Winters (USA) discussed integrative oncology and metabolic therapies for cancer care, offering an inspiring, evidence-based approach.

Dr Kim Callaghan (Canada) shared balanced, insightful perspectives on menopause, brain chemistry, and

bio-identical hormones - content that would resonate strongly with Australian audiences.

Dr Sonja Jensen (Canada) delivered a powerful talk on hormonal health, intergenerational trauma, and spirituality, weaving herbs like ashwagandha, shatavari, and vitex into a deeply moving presentation.

Australia was proudly represented by our very own experienced practitioner and ANTA Member, Tara Nelson, who presented on the brain–thyroid connection. Her clarity, clinical precision, and professionalism reflected the high calibre of Australian naturopathic education and practice.

Not all presentations met the same standard that we have in Australia. This was a good reminder of the importance of critical thinking and scientific integrity in our field, married with traditional wisdom. These moments underscored how vital Australia’s evidence-based approach is, and how well our practitioners hold their own internationally. The ICNM demonstrated that Australian naturopaths have a respected global reputation, and I believe that it is our commitment to quality education, regulation, and clinical excellence which sets us apart.

For practitioners interested in expanding their horizons, international conferences offer extraordinary opportunities to share research, learn, and connect.

ANTA is exploring ways to support members who wish to present abroad - an ideal way to elevate your profile and strengthen Australia’s contribution to global naturopathy.

Attending the ICNM reaffirmed my pride in our profession. Australia has a unique, credible voice in naturopathy and herbal medicine — one grounded in science, compassion, and integrity. It’s time we shared it more widely.

Harnessing the Metabolic Microbiome Through a food-first approach in the Naturopathic Management of Type 2 Diabetes

Type 2 diabetes (T2D) is a complex, chronic metabolic disorder characterised by insulin resistance, hyperglycaemia and

impaired metabolic flexibility. Despite significant pharmaceutical developments, particularly in the arena of glucagonlike peptide-1 (GLP-1) receptor agonists, its global prevalence continues to rise. This persistent increase signals a need to re-centre treatment strategies on upstream determinants of health, particularly those involving diet, lifestyle and environmental factors. Central to this is the gut microbiome - a dynamic microbial ecosystem that interfaces directly with metabolic regulation, inflammation and endocrine function.

The gut microbiome, a dynamic and metabolically active microbial ecosystem, has emerged as a key modulator of metabolic health. Its influence on inflammation, hormone signalling and nutrient metabolism positions it as a foundational element in the prevention and management of T2D. Modern microbiome research reaffirms what traditional systems of medicine have long understood: food is not only nourishment but also medicine, influencing the body’s inner ecology in profound ways. What we eat is one of the most rapid and potent modulators of the gut microbiota - resonating strongly with the naturopathic principles of food as medicine and the healing power of nature (vis medicatrix naturae).

A food-first, microbiome-centred approach focussing on improving microbial diversity and endogenous GLP1 production, encompasses whole foods, prebiotic and probiotic foods, herbal medicines and traditional dietary wisdom. Modulating the metabolic microbiome to support glucose regulation, reduce inflammation and improve metabolic resilience, moves beyond addressing glycaemic control alone.

The Metabolic Microbiome: A Functional Overview

The term “metabolic microbiome” refers to the microbial community within the gastrointestinal tract that plays PAGE 20 | The Natural Therapist

a functional role in host metabolism. These microbes contribute to dietary fibre fermentation, short-chain fatty acid (SCFA) production, bile acid transformation, modulation of systemic inflammation and immune regulation – each tightly linked with insulin sensitivity and glucose regulation.

In individuals with T2D, studies consistently show: • Reduced microbial diversity; • Depletion of beneficial SCFA-producing bacteria such as Faecalibacterium prausnitzii and Akkermansia muciniphila; • Elevated levels of pro-inflammatory, endotoxinproducing pathobionts.

These changes are not merely a consequence of metabolic dysfunction – they may actively contribute to disease progression. For example, Akkermansia muciniphila is associated with improved glucose homeostasis and gut barrier function and its absence may facilitate both inflammation and dysglycaemia.

The Food-Microbiome-Metabolism Axis in T2D

The composition of the gut microbiome responds quickly and dynamically to dietary patterns. Diets rich in diverse, whole plant food fibres and minimally processed whole foods promote microbial diversity and increase the production of SCFAs which play a central role in metabolic health.

The Mediterranean diet, characterised by a high intake of vegetables, legumes, nuts, seeds, fish and extra virgin olive oil, has shown consistent benefits in improving HbA1c, reducing insulin resistance and enhancing microbiome composition.

In contrast, processed and ultra-processed foods, laden with artificial sweeteners, emulsifiers, preservatives and food additives, disrupt microbial balance, increase gut permeability and drive systemic inflammation impairing glucose regulation.

Conventional diabetes care often focuses on carbohydrate counting or caloric restriction. A more holistic approach prioritises food quality, source, traditional preparation and seasonality, restoring food as a central pillar of healing and shifts the narrative from one of constraint to reconnection and empowerment.

SCFAs and GLP-1: Microbial Metabolites with Metabolic Power

The microbiome’s most potent SCFAs acetate, propionate and butyrate are metabolites produced by microbial fermentation of indigestible plant fibres and play a central role in metabolic regulation.

These SCFA’s interact with G-protein coupled receptors on enteroendocrine L-cells stimulating GLP-1 secretion.

L-cells are primarily found in the ileum and colon and are highly responsive to nutrients and microbiota-derived metabolites like SCFAs.

Acetate - lipogensis and cholesterol metabolism Propionate - regulates gluconeogenesis and satiety Butyrate - primary energy source for colonocytes; anti-inflammatory; enhances insulin sensitivity and intestinal barier integrity.

GLP-1 is an incretin hormone pivotal in regulating: • Postprandial insulin release • Glucose uptake and storage • Satiety via stimulation of Peptide YY (reduces appetite and slows gastric Emptying) • Inflammation Beyond SCFAs, the microbiome also modulates GLP-1 secretion via bile acid metabolism, tryptophan-derived indoles (produced by bacteria like Bacteroides and Clostridium) and nutrient sensing mechanisms.

Personalised Nutrition, Lifestyle and the Microbiome

Microbiome composition varies significantly between individuals, which helps explain the variability in glycaemic responses to identical foods. Highly processed foods, alcohol and stress negatively impact the metabolic microbiome leading to disruption in the diversity and composition of the microbiome and increase intestinal barrier dysfunction. These changes contribute to inflammation and metabolic imbalances increasing the risk of Type 2 Diabetes.

In clinical practice, even small shifts in fibre intake or specific food inclusions and / or exclusions can have measurable effects on blood sugar control, satiety and inflammatory markers. Enhancing endogenous GLP-1 secretion through diet by increasing various plant-foods presents a powerful preventative and / or adjust to pharmacotherapy especially in pre-diabetes or early-stage T2D. Physical exercise has positive effects on microbiome diversity which is believed to be associated with cardio-respiratory fitness attributed to higher abundance of butyrateproducing bacteria by the Clostridiales, Erysipelotrichaceae, Lachnospiraceae and Roseburia families. An increase in butyrate-producing bacteria influences the metabolic pathways involved in fat accumulation helping to prevent obesity and T2D, reduces insulin resistance, inflammation and improved exercise tolerance.

Minimising Microbiome Disruptors

To foster microbial and metabolic resilience, practitioners should advise patients to reduce: • Ultra processed foods • Excessive alcohol and caffeine • Industrial seed oils and trans fats • Emulsifiers and artificial sweeteners • High glycaemic, low fibre carbohydrates

Polyphenol-rich Foods Poorly absorbed in the small intestine, polyphenols reach the colon where they are transformed into anti-inflammatory, GLP-1-promoting metabolites: •

Berries (blueberries, blackberries), Apples (with skin), Green tea, Cocao (dark chocolate > 70%) Herbs and spices, eg: turmeric, cinnamon, rosemary

These all contribute to inflammation, dysbiosis and glycaemic instability.

Naturopathic note: emphasise ‘crowding out’ – adding in nourishing foods rather than focussing solely on restriction.

Naturopathic Recommendation: embrace culinary herbalism – encourage the daily use of medicinal spices and teas for therapeutic benefit.

Traditional Diets as Microbiome Medicine

A high dietary fibre intake daily (30 – 50g), especially fermentable fibres from legumes, vegetables and whole grains, is the cornerstone of any food-first strategy targeting the microbiome. Across cultures, traditional dietary patterns - Mediterranean, East Asian, Ayurvedic and Indigenous Australian - emphasise: • Fresh, seasonal, plant-centric meals • Fermented and cultured foods • Bitter and astringent plants and herbs • Dietary diversity These principles align closely with today’s microbiome science and support a return to food as medicine.

A diverse, plant-rich diet that includes prebiotic fibres, polyphenols, fermented foods and healthy fats supports microbiome-mediated GLP-1 secretion, improving satiety, glucose regulation and insulin sensitivity. Meta-analyses show that higher total fibre intake correlates with reduced T2D incidence and improved HbA1c levels in established T2D.

Clinical Applications: Dietary Strategies to Modulate the Metabolic Microbiome

Prebiotic-Rich Foods Non-digestible fibres (e.g.: inulin, fructo-oligosaccharides, galacto-oligosaccharides, resistant starch) selectively fuel beneficial bacteria (like Bifidobacterium and Faecalibacterium), increasing SCFA production and GLP-1 activity. Key sources include: • Chicory root, Jerusalem artichokes, garlic, onions, leeks, asparagus • Oats, barley (ϐ-glucans) • Legumes (lentils, chickpeas, black beans), green bananas and cooled potatoes (resistant starch) • Flaxseed, Psyllium Naturopathic Recommendation: encourage clients to gradually increase intake of these foods, often beginning with cooked versions to minimise digestive discomfort in cases of dysbiosis.

Fermented and Cultured Foods Introduce live beneficial microbes, enhance microbial diversity and upregulate L-cell activity increasing SCFA production: • Sauerkraut, Kimchi, Yoghurt, Kefir, Miso, Tempeh, Natto Naturopathic recommendation: introduce fermented foods slowly, aiming for daily consumption. Encourage homemade preparations to enhance probiotic diversity and patient engagement.

Resistant Starch Resistant starch escapes digestion and is fermented in the colon into butyrate which supports L-cell function and GLP-1 secretion.

• Cooked and cooled potatoes or rice • Green bananas • Legumes and Whole grains, eg: rye, barley Healthy Fats Long-chain fatty acids stimulate GLP-1 secretion and also via bile acid conversion regulating lipid metabolism, fat storage, glucose homoeostasis and energy expenditure.

• Extra virgin olive oil, avocado, nuts and seeds (walnuts, almonds, flaxseeds), fatty fish (salmon, sardines, mackerel).

Protein-Rich Foods Amino acids, particularly arginine, leucine and glutamine stimulate GLP-1 secretion directly via nutrient-sensing pathways in L-cells.

• Eggs, dairy, fish, lean meats, legumes Bitter Compounds Bitter taste receptors in the gut also stimulate GLP-release: • Bitter melon, dandelion greens, rocket, citrus peel, hops

Herbal Allies: Phytomedicines Supporting GLP-1 and Microbial Health

Conclusion

Where modern medicine often intervenes downstream - after disease has manifested - naturopathic approaches seek upstream determinants of health.

Supporting Clients with Anxiety:

Beyond the Perfect Treatment Plan Georgie Collinson

For nearly a decade I’ve been practising as a Naturopath, and for most of those years I’ve specialised in working with

women with anxiety. In that time, I’ve learnt a great deal about both the challenges and the unique opportunities that come with supporting this group of clients. My intention in sharing my own insights here is that you’ll take something valuable away to use the next time an overwhelmed, stressed-out woman walks in your door.

When clients have anxiety, it is not uncommon to find they struggle to consistently do loving things for themselves.

Their health routines may come from a place of pressure or perfectionism, rather than genuine self-care. That was the case for me personally; many years following strict food rules in the pursuit of health, that only added to my stress. Others face deep resistance to self-kindness, because it’s not something they’ve ever been taught to practise.

When we, as practitioners, understand this dynamic, we can guide our clients to much better results and create the kind of consistency in their healing journey that actually moves them forward.

I am very passionate about my work. I’ve written two books, The Anxiety Reset Method, which was featured

by Oprah and published internationally, and Master Your Relationship Anxiety, released this year. Through my programs, I’ve helped thousands of women around the world learn to master their anxious minds and become their calmest, most confident selves. My approach to anxiety combines three core elements, working with the subconscious mind and deeper emotions, optimising physical health and leaning into a more spiritual, supportive belief system about life. Through this work,

I’ve come to believe that anxiety requires much more than the traditional model of talk therapy and medication. We need to go much deeper, to the root cause - and natural health practitioners have a key role to play.

When the Perfect Plan Isn’t Enough

As practitioners, we often create the perfect plan: a tailored diet, specific supplements, lifestyle strategies and meditation practices. In my early days in clinic, I quickly realised that no matter how well-researched, detailed or personalised the plan was, many clients simply couldn’t follow through. Perhaps you’ve experienced something similar?

Some didn’t want to help themselves. Others desperately wanted to and had the best intentions, but struggled to maintain consistency, or life simply got in the way. This was a turning point for me. I realised I needed to broaden my skill set and find ways to support people beyond just creating the “perfect plan.”

I bring my own personal experience into this too. Having recovered from an eating disorder, years of binge eating and bulimia on top of my own anxiety, I’m acutely aware of how strict diets and “perfect” health rules have the potential to cause harm rather than heal. This has made me particularly mindful of the words I use with clients and The Natural Therapist | PAGE 27

the importance of sensitivity, especially when discussing food. Asking key questions about a client’s relationship with food has become a standard procedure in my initial consultations. It’s simply saying, “Before I give any dietary advice, I’m curious to know, what’s your relationship like with food?”.

But asking for more information isn’t the end of the story either. I went on to study hypnotherapy and the subconscious mind with Marisa Peer at the School of Rapid Transformational Therapy. This training allowed me to address the deeper emotional patterns and habits driving my clients’ anxiety. While I don’t suggest every practitioner needs to train in hypnotherapy, I do believe there are principles we can all integrate into our practice to achieve better results.

The Emotional Component

One of the most consistent findings in my work is that anxiety nearly always has an emotional root, and it’s usually not conscious to the client themselves. Behind the magnesium deficiency or gut microbiome imbalance, there is often unresolved grief, shame or suppressed emotion. It can take some digging to really understand what that emotion is all about. A woman who is suddenly experiencing anxiety in perimenopause might have some hormonal imbalances, but she also has genuine unresolved emotion coming to the surface as well. The hormone imbalance thins the veil and makes it less possible for her at this time of her life to keep those old, hidden emotions locked away.

I’ve found clients with anxiety typically fall into two categories: • Those who suppress or avoid emotions altogether.

Those who believe they are ‘too emotional’ or ‘too sensitive’, but who still don’t allow themselves a full emotional release.

Both groups end up stuck. The sensitive ones may cry briefly, then quickly shut it down with shame, never completing the process. They have usually been told from a young age that they are ‘too much’ in some way, and so they try to keep the tears in. Helping clients build a healthier relationship with their emotions – and referring to psychology, energy healing, kinesiology, Reiki, or other safe spaces - is essential.

A question I always ask is: “How comfortable are you with feeling your feelings?” This alone often opens up powerful dialogue and insight. Even though you may be trained in supplements or a manual therapy, supporting clients to consider how their emotional world may be contributing to anxiety can make a huge difference.

Anxiety is a messenger. It’s asking us to sit with the parts of ourselves we’ve long suppressed. And unless we address this, no amount of gut-healing or supplementation will resolve the issue fully.

The Role of Natural Medicine

Of course, natural medicine plays a vital role. Sleep deprivation, skipped meals, hormone imbalances, nutrient deficiencies, blood sugar crashes - these can all make emotional processing much more challenging.

Our role is to optimise the system so that clients are wellnourished and supported with the energy to face their inner world. This means coming back to the basics: quality sleep, balanced meals and daily movement. Even simple adjustments like eating regularly can dramatically reduce

common anxiety symptoms such as dizziness, headaches or panic attacks.

I always recommend eliminating all sources of caffeine for at least two weeks, including green tea (yes, the L-theanine is not enough to counterbalance the effects for someone who is sensitive); this way, clients are able to establish a baseline of how they feel without any stimulation. It’s easy to find delicious caffeine-free alternatives, so after some hesitation, my clients have always been fine with the experiment. They rarely go back to drinking it again, and if they do, they consume much less. Stimulants add fuel to the anxiety fire, often in ways clients don’t realise until they experience life without them.

Equally, simple nervous system practices can be lifechanging recommendations to add to your plan. There are two practices that I most like to advise (and are great supports for us as practitioners too): • The physiological sigh - a double inhale (one deep breath in, then a sip more of air) followed by a long exhale. Just a few rounds can instantly calm the nervous system.

• Legs up the wall - easy, accessible, and powerful in activating the parasympathetic nervous system, even when the mind is racing. Simply lie down in bed or on the couch with legs up and resting against the wall/ back of couch, allowing the parasympathetic nervous system to activate.

I love that these practices cost nothing, can be implemented anywhere, and teach clients the empowering truth that they have tools within themselves to shift anxiety.

Supplements: Safety Nets, Not Solutions

Calming supplements and herbs absolutely have their place, but they must be used wisely. We don’t want clients to believe that they need the supplements and herbs to feel safe. I encourage clients to see them as short-term support or ‘safety nets’ for particularly stressful times, rather than long-term crutches.

My favourites: • Kava: potent and effective, though requires careful guidance and strict adherence to dosage.

• Passionflower: safe and reliable, with a palpable calming effect.

• Skullcap: better for clients who feel irritable as well as anxious, which really works.

But sometimes, these herbs can be used to hold in what needs to come out as part of the body’s built-in healing process. Nothing compares to the power of an emotional release. Sometimes the best prescription is something we resist the most, simply to have a good cry.

Language Matters

Finally, I urge all practitioners to consider the language we use with clients. The words we say to our clients can

become stamped in their minds. If we tell someone “you’ll have to manage anxiety forever,” we can unintentionally tattoo that belief into their identity.

Instead, I remind my clients that anxiety is not a life sentence. It is a message from the body, asking us to pay attention. We can learn to develop a relationship with that anxiety, seeing it as a positive influence reminding you to reconnect and come home to yourself.

Transformation is always possible. While anxiety will always be a part of the human experience to some degree, with the right support, anxiety can become much less frequent, less intense, and feel like it doesn’t impact your life at all.

As practitioners, we hold a profound responsibility. Our clients hear every word we say. If we can offer empowering guidance, remind them that healing is possible, and equip them with practical tools alongside natural medicine, we can support them in creating lasting transformation.

The milestone event marked an exciting new chapter for ANTA - one that celebrated excellence, showcased innovation, and strengthened the collective voice of natural medicine practitioners.

Across the two-day program, delegates engaged in expertled presentations, hands-on workshops, multidisciplinary Q&A forums, and vibrant networking opportunities.

The Symposium opened with an energising keynote from Mark Bunn, who explored the habits and mindset of healthy high performers. On Day 2, mental health educator Dr Jodi Richardson delivered a powerful keynote on practitioner wellbeing, resilience, and building thriving practices – a key professional development theme moving into 2026.

A major highlight of the weekend was the launch of ANTA’s new organisational Strategy, presented by ANTA CEO David Liesegang, as a roadmap for the association’s vision leading into 2030. Members welcomed the forwardthinking focus on professional recognition, practitioner support, and leadership within the broader healthcare landscape.

It was a tremendous honor to be invited to participate in the ANTA Symposium and the celebration of 70 years this

past October. The event was truly heartfelt - every participant demonstrated remarkable professionalism, collegiality, and respect. The program was exceptionally well designed, allowing ample time to visit the exhibitors, connect with colleagues, and build new friendships and professional networks.

The venue itself was stunning-welcomed by kangaroos grazing on the grass and native birds in song, all with a beautiful water view upon arrival. The Novotel offered an outstanding environment, equipped with state-of-the-art technology that supported each presenter in delivering their sessions with clarity and precision. Exhibitors were incredibly generous, providing thoughtful welcome packages, product samples, and door prizes that added to the celebratory spirit.

Among the many informative presentations, two key themes stood out: practitioner wellbeing and burnout prevention, and the integration of AI to support time management and enhance clinical efficiency. These topics were presented with both insight and humour, keeping the sessions engaging and uplifting for everyone in attendance.

A highlight of the symposium was the unveiling of ANTA’s forward-looking vision for the future - both for the association and its members. It was inspiring to witness an organisation so deeply committed to its members’ wellbeing, while simultaneously embracing innovation and preparing for the challenges and opportunities of the future.

Having attended many professional gatherings throughout my 40-year career, I can say with sincerity that this event was among the most impressive. My heartfelt gratitude goes to the ANTA team and staff whose dedication, warmth, and attention to detail made the symposium such a success.

1. Microbiome - Thyroid Axis Gut microbes modulate iodine, selenium, zinc, iron, and vitamin D absorption - all essential for thyroid hormone synthesis and immune regulation. Dysbiosis disrupts these pathways, contributing to both hypothyroid and autoimmune thyroid disorders.

• Iodine: Microbes metabolise and regulate availability; imbalance linked to both deficiency and excessrelated thyroid issues.

Why I spoke: I love to show how AI can quietly remove

friction in practice while lifting standards of care. Not hype; useful, safe, measurable improvements that therapists can adopt easily.

1. Start where it helps patients most

The fastest wins are in the dull-but-critical parts of care: intake, consent, note taking, treatment plans, and follow up. AI can pre fill case histories from patient forms, generate clean SOAP notes, and draft home care instructions written in plain English. That frees practitioner attention for the therapeutic relationship while improving consistency between visits.

2. Keep the human firmly in the loop

AI should assist, not replace. I recommend a simple rule: AI drafts, practitioner approves. Use templates that match your modality, then add clinical nuance only you can provide. This reduces variability without flattening professional judgement. Patients value the combination of warm rapport and well structured care plans.

3. Evidence and safety first

Every AI output should be traceable to a source or guideline. Prefer tools that cite references, allow custom practice policies, and log changes. Build a short “clinical guardrail” checklist: scope of practice, red flags, referral thresholds, and contraindications. If the AI suggests an action outside scope or contrary to your policy, it stops and flags for review.

4. Data ethics made practical

Explain to patients, in writing, what data is collected, how it is used, and who can access it. Store client notes in systems that meet Australian privacy law, allow local data storage, and offer role based access controls. Keep a two minute script for verbal consent and a one page plain language consent form. Trust grows when people understand the pipeline.

5. Measure what matters

Choose a small set of outcome measures that make sense for your modality: pain scores, range of movement, sleep quality, digestive comfort, or stress markers. Let AI plot trends across visits and surface early warnings.

This is where natural therapies can shine: demonstrating change over time with clear, human readable charts.

6. A simple adoption plan

Week 1: map your workflow and consent.

Week 2: pilot AI for admin tasks only.

Week 3: add drafted treatment plans with clinician approval.

Week 4: introduce outcome tracking and patient education handouts.

The bottom line

Good AI in a clinic is quiet, safe, and boring in the best way: fewer errors, clearer notes, faster referrals when needed, and more face to face care. That is how curiosity becomes clinic ready practice.

Collaborative Approaches to

Understanding and Managing Women’s Pain

My recent presentation at the ANTA symposium focused on reimagining women’s pain management through a collaborative lens, which is less about individual pain syndromes, and more about the systemic, cultural, and clinical reforms needed in women’s healthcare. The central question I asked was: Why does women’s pain need new conversations? The answer is because the old ones have failed. For generations, women’s reports of pain have been downplayed, psychologised, or dismissed entirely.

The underlying problem is systemic bias: women’s pain is too often underrecognised and viewed through a lens of emotion rather than evidence. This minimisation leads to delayed diagnoses, inappropriate treatments, and higher rates of mental health referrals instead of pain management. Despite chronic pain being more prevalent in women, medical education still offers minimal training in pain conditions that predominantly affect them. The result is a system that overlooks the biological and hormonal nuances of female pain, the psychosocial context in which it occurs, and the cumulative harm of being unheard. Research on endometriosis exemplifies this: in a survey of Australian women, participants described being “dismissed and chastised,” likening the experience to being a “second-class citizen.” Many reported having their pain normalised (“period pain is normal”) or doubted entirely. The consequences extend beyond the clinic, women become reluctant to seek care, risk medical trauma, and often manage complex conditions in isolation.

Throughout the talk I emphasise that pain is complex, but also that complexity can be liberating as it provides numerous opportunities for change. Beyond tissue damage, chronic pain often intertwines with neurodivergence, connective tissue disorders, dysautonomia, and immune dysfunction.

Studies now show high comorbidity between ADHD, autism, and chronic pain, with overlapping mechanisms involving the stress response, neuroinflammation, and dopamine regulation. Conditions such as hypermobility and POTS (Postural Orthostatic Tachycardia Syndrome) frequently co-occur, creating what researchers call the “Super Pentad”, a cluster of interrelated syndromes that require integrated, multidisciplinary approaches.

Strategies such as case conferencing, direct referral pathways, shared consultations, and clear communication about treatment priorities help dissolve silos. Collaboration does not always require full integration, but integration cannot exist without collaboration.

At the heart of this model is the woman herself. Effective care places her at the centre of decision-making, not at the periphery. When women are supported to understand their health, interpret symptoms, and participate in decisions, outcomes improve. Knowledge and critical health literacy are key: when clinicians share information clearly and meaningfully, they build trust, reduce anxiety, and increase adherence. This process of “sense-making” helps women contextualise symptoms, regain control, and stop chasing elusive single causes.

I concluded by circling back to its opening premise: women’s pain has been pathologised, ignored, or silenced for too long. The path forward requires science that includes women, clinicians who listen, and care models that reflect the intricate interplay of biology, psychology, and social reality. Collaborative care is not simply good practice, it is the ethical foundation of women’s healthcare, and the most direct route to restoring trust, equity, and healing.

These patterns expose the inadequacy of fragmented care.

Rethinking the Origins of Adolescent Scoliosis

Adolescent Idiopathic Scoliosis (AIS) is traditionally considered a progressive and largely untreatable condition without surgery or bracing. However, both past and emerging clinical observations challenge this assumption.

This case report presents a radiographically confirmed reversal of thoraco-lumbar scoliosis in a growing adolescent, achieved through correction of a primary biomechanical driver, an anatomical leg length discrepancy without the use of bracing or surgery.

The patient demonstrated a 15° Cobb angle reduction during active growth despite rapid deterioration prior to intervention. This strongly refutes the deterministic model of idiopathic scoliosis and supports the concept of reducible curves driven by modifiable asymmetries.

Introduction

AIS is defined as a spinal curvature greater than 10° of unknown cause, usually identified between ages 10 and 18. The label “idiopathic” often leads to passive treatment protocols, such as observation, bracing, or ultimately surgery (Weinstein et al., 2008). Despite decades of research, the emphasis has remained on controlling curvature rather than addressing causality.

Conservative care, including Thoraco-Lumbo-Sacral Orthosis (TLSO) bracing and Physiotherapeutic ScoliosisSpecific Exercises (PSSE) techniques like Schroth, has demonstrated limited long-term structural benefit and focuses primarily on halting progression (Negrini et al., 2018; Monticone et al., 2016).

Surgical intervention, commonly posterior spinal fusion, is often recommended for curves exceeding 45°, even when cardiopulmonary function remains normal (Lonstein, 2006; Pehrsson et al., 2001). The orthopaedic field continues to treat the curvature as a primary disorder rather than investigating it as a potential response to

foundational asymmetries (Weiss et al 2006).

Purpose

This report aims to present clinical and radiographic evidence that challenges the conventional understanding of AIS as an irreversible condition. By identifying and correcting a structural leg length discrepancy, a meaningful and sustained reduction in the patient’s Cobb angle is evidenced to have been achieved during active skeletal growth, highlighting the possibility that many cases labelled “idiopathic” may, in fact, be secondary and reversible.

Patient Profile: Adolescent 13yo female, Risser 3 at baseline, progressing to Risser 4 over 6 months with evidence of anatomical leg length discrepancy and asymmetric sacral ala development.

Initial Cobb Angle: ~28° in May 2024, progressing to 39° November 2024.

Intervention: A structural leg length discrepancy and unilateral sacral ala imbalance were corrected through the use of a heel lift, which was progressively increased to 7 mm. This adjustment was guided by postural and pelvic asymmetry findings indicating an anatomical deficiency of approximately 1.1 cm. The intervention was combined with weekly soft tissue and osseous physiological therapeutics, initiated in early December 2024 and continued until the followup radiographic examination on 6 May 2025.

approximately ±3–5° (Morrissy et al., 1990; Vrtovec et al., 2009). Consistent with this expected margin of error, two independent orthopaedic specialists who earlier reviewed the patient’s November 2024 radiographs reported Cobb angles of 36° and 41°, compared to my measurement of 39°at initial consultation December 2024.

One of these attending orthopaedic specialists, who ordered the May 2025 follow-up radiographs to monitor the scoliosis, agreed to the patient’s request to assess the impact of my heel lift intervention through both lift and no-lift imaging. I did not request or even recommend the follow-up film due to the obvious improvement. This specialist subsequently confirmed the radiographic

improvements documented in this case study, and based on these findings, recommended that the patient continue wearing the prescribed lift and receiving the ongoing treatment I provided.

The Cobb angle reduction of 15° documented may, in fact, understate the full extent of structural improvement, given the use of high-resolution image magnification analysis to trace clearly defined endplates. Additionally, the marked reduction in rib and lumbar prominences observed on the Adams forward bend test confirms a shift in the structural basis of the curve. These clinical findings, along with radiographic signs of levelling, are consistent with the correction of an anatomical leg length discrepancy

and reduced Heuter-Volkmann loading asymmetry at the sacral ala. Importantly, no bracing, surgery, or PSSE were used during this period.

Lifestyle counselling to always wear the lift and to move regularly if not lying down, and engage in weight-bearing hetero-lateral gait activities such as walking, running, and dancing were advised. Additionally, the patient was counselled to avoid excessive side-bending and rotation activities, as well as predominantly unilateral sports like tennis, and activities involving heavy lifting or high-impact loading such as high and long jump.

Non-specific stretching and mobilisation were also advised against, as time and gravity offer little respect for freedom and laxity in structurally imbalanced cases as described in models of spinal dysfunction making unstructured mobility potentially detrimental in cases with foundational imbalance (Panjabi, 1992). Physiological therapeutic interventions with specific soft tissue and osseous techniques targeted to reduce the structurally bound nature of Adams-positive scoliosis were applied at roughly weekly intervals.

Discussion

The case demonstrates key features of a functional, reducible scoliosis: • Curve Progression During Growth: Consistent with natural history associated with pubertal growth spurt, the Cobb angle increased from ~28° to 39° in the six months prior to intervention (Weinstein et al., 2013).

• Curve Reversal with Mechanical Correction: Following heel lift and physiological interventions, Cobb angle reduced by 15°, even during continued skeletal growth (Risser 4). Improvement was visible even in the absence of the lift at time of filming, suggesting structural change rather than mere positional artifact.

These findings directly support the hypothesis that many cases of scoliosis represent secondary adaptations to modifiable biomechanical drivers, such as leg length discrepancies, sacral ala asymmetry, or functional leg deficiencies like increased Q-angle resulting from excessive foot pronation. This perspective is grounded in 44 years of clinical experience, during which I have frequently employed heel lifts to correct non-functional imbalances, often alongside treatment for concurrent functional asymmetries.

In my experience, these three biomechanical factors account for the vast majority of cases labelled as idiopathic scoliosis, while other causes such as tumours and congenital anomalies are comparatively rare.

Accordingly, “idiopathic scoliosis” is, in most cases, a diagnosis that reflects a failure to identify the underlying predisposing etiological factors (Barge, 1981).

Contemporary treatment often overlooks such root causes. Instead, scoliosis is approached as a condition to be restrained rather than resolved. This case adds to a growing body of literature suggesting that early biomechanical correction can yield structural reversal, even during peak risk periods (Herbert et al., 2002; Romano et al., 2023).

The Heuter-Volkmann principle further explains how asymmetric loading during growth can deform vertebrae over time, reinforcing the need for early identification and correction of loading asymmetries (Stokes, I.A.F. (2007).

No adverse reactions to the heel lift were reported or observed, other than the practical need to wear it consistently during weight-bearing activities. Compared to one consulted orthopaedic specialist’s recommendation of a TLSO brace, and the other’s suggestion of possible surgical fixation if the curve progressed, the heel lift was a welcome alternative.

Over three decades ago, I routinely employed annual spinal radiographs in the clinical follow-up of adolescents with suspected biomechanical drivers of scoliosis, often tracking individual cases over three or more years.

However, I later discontinued this practice in light of evolving awareness around the risks of cumulative radiation exposure, even at low levels, and shifted toward conservative management unless imaging was clearly warranted as it arguably was in this case. I did not request or initiate the follow-up radiographs; rather, they were ordered independently by an attending orthopaedic specialist to monitor the patient’s progression toward a previously anticipated surgical referral who also complied with the patient’s request to assess the effects of the employed heel lift intervention.

Conclusion

This case provides compelling evidence that structural scoliosis can reverse during active growth when its mechanical origin is corrected. A 15° Cobb angle reduction was documented in an adolescent within 6 months without bracing or surgery, demonstrating that not all curves are irreversible. Clinicians must remain vigilant in assessing foundational biomechanics before accepting an idiopathic label and committing to long-term passive care or irreversible intervention.

Closing Remarks

A diagnosis of idiopathic scoliosis should never foreclose inquiry into correctable causes. This case underscores the importance of re-examining AIS through a functional lens. By realigning clinical focus toward identifying and correcting the origin of biomechanical stress, we open the door to non-invasive, sustainable solutions that honour the body’s inherent capacity for adaptation and healing. Recognising the difference between fixed and functional curves is essential for timely intervention, and may prevent thousands of adolescents from unnecessary dependency on bracing or surgery.

The future of scoliosis care, as reported herein, is strongly aligned with timely, targeted, biomechanically informed correction, rather than resignation to a default of irreversible deformities arising from failures to consider fundamental biomechanical and physiological etiological factors.

Differential Diagnosis and Management of Cervicogenic Headache and Migraine A Case Report and Clinical Analysis

Background: Cervicogenic Headache (CGH) is a secondary headache originating from cervical spine dysfunction, often mimicking migraine in its clinical presentation. Accurate differentiation between CGH and migraine is essential for effective management.

Case Summary: A 61-year-old male presented with recurrent headaches primarily localised to the upper cervical and occipital regions, radiating to the temporal and frontal areas. Pain intensity reached 9/10, unresponsive to oral analgesics but aggravated by cervical motion. Neurological examination and imaging were unremarkable. The diagnosis of cervicogenic headache was established based on cervical tenderness, pain pattern, and poor medication response. The patient underwent three sessions of cervical traction, massage, and proprioceptive neuromuscular facilitation (PNF) therapy, resulting in complete pain resolution without pharmacologic intervention.

pain is typically referred from the upper cervical region to the occipital, temporal, and frontal areas, often mimicking migraine in presentation.

However, unlike migraine, CGH is precipitated by cervical movement, sustained posture, or mechanical stress and is poorly responsive to common analgesics or triptans. Because of these overlapping symptoms, CGH is frequently misdiagnosed as migraine, resulting in delayed or inappropriate treatment. This report presents a clinically representative case of CGH successfully managed with non-pharmacologic therapy, emphasising differential diagnostic features and the therapeutic rationale for physical interventions.

Conclusion: This case highlights the key diagnostic features distinguishing cervicogenic headache from migraine and supports the effectiveness of conservative physical therapy modalities in achieving complete remission.

Introduction

Cervicogenic headache (CGH) is a secondary headache disorder caused by musculoskeletal dysfunction of the cervical spine and its associated neural structures. The PAGE 50 | The Natural Therapist

Case Report

A 61-year-old male presented with sudden-onset headache on the afternoon of April 26. The pain intensity progressively increased from 5/10 to 9/10 and did not respond to oral ibuprofen. The patient attended the emergency department twice (April 27 and 29), where intravenous analgesics temporarily reduced pain to 2/10.

However, the headache recurred within 24–48 hours. On May 1, the headache returned with intensity rated 5–7/10.

The patient denied hypertension, cardiac disease, nausea, photophobia, or vomiting. He reported a similar headache episode 25 years earlier, which resolved after morphine injection.

Physical examination revealed significant tenderness over the upper cervical region with limited range of motion. Brain CT and laboratory investigations were normal. Blood pressure and heart rate were within normal limits. Considering the following features - poor response to analgesics, pain aggravated by neck movement, occipital onset radiating to the frontal area, and distinct cervical tenderness - a diagnosis of cervicogenic headache was made.

Treatment 1.

Cervical traction: every other day, 15 minutes per session.

2. Cervical massage: gentle upward strokes along both sides of the cervical spine using thumb pads, with focused pressure on the occipital region.

3. Proprioceptive Neuromuscular Facilitation (PNF) for cervical stabilisation and muscular coordination.

Discussion

Cervicogenic Headache and migraine share overlapping clinical features, but their pathophysiological mechanisms differ. CGH arises from dysfunction of the upper cervical joints and associated nerves - particularly the greater, lesser, and third occipital nerves. In contrast, migraine involves trigeminovascular activation and neurogenic inflammation. Clinically, CGH is characterised by pain triggered by neck motion or posture, localised tenderness, and poor response to NSAIDs, whereas migraine is associated with nausea, photophobia, and pulsating pain.

Physical therapy aims to restore cervical joint alignment, relax hypertonic muscles, and relieve nerve irritation. This case demonstrates the efficacy of nonpharmacologic treatment modalities - cervical traction, massage, and PNF - inresolving CGH without the need for analgesic medication. Accurate diagnosis and targeted manual therapy can prevent chronicity and improve patient outcomes.

Conclusion

Cervicogenic Headache should be differentiated from migraine based on pain origin, trigger factors, and cervical findings. Non-pharmacologic interventions such as traction, massage, and proprioceptive neuromuscular facilitation provide effective, lasting relief and improve cervical function. Early recognition and management are critical to preventing misdiagnosis and unnecessary pharmacologic use.

Treatment Outcome

After the first treatment on May 2, pain intensity decreased to 1–2/10. Following the second treatment on May 4, the headache completely resolved. After three sessions, the patient achieved complete remission without medication.

At a follow-up visit on May 14, he remained headache-free with normal cervical motion and no tenderness.

An Interview with Medical Herbalist Jaynaya D’Esterre Atkins

Ethnobotany - the study of how people use plants for medicine, food, and culture - offers a unique lens into both

ancient traditions and modern clinical practice. In this interview, I speak with Jaynaya D’Esterre Atkins, a medical herbalist, mentor and researcher specialising in ethnobotany, to explore how traditional plant knowledge continues to inform scientific research and natural health care in Australia today.

Welcome Jaynaya! Could you start by explaining what ethnobotany is, and how it differs from general herbal medicine or phytotherapy?

In short, ethnobotany examines the relationships between plants, fungi, and people. It incorporates a range of disciplines including anthropology, botany, archaeology, pharmacology, linguistics, and history.

Where the application of herbal medicine contributes to healing the body, ethnobotany studies how such healing is understood – not merely looking at how people use plants, but how we perceive and relate to plants. Phytotherapy is still the medicinal use of plants, but centres around measurable efficacy, often focusing on specific chemical compounds in the plant.

What first drew you to study ethnobotany - was there a personal or cultural influence behind it?

My personal journey has always been deeply embedded in “the wild”. Without getting carried away in mentioning the myriad of happenings that led me to where I am today, the main theme is that I’ve always felt the most comfortable in solitude, exploring natural landscapes.

Travel has shaped me profoundly – listening, observing, and, when permitted, participating alongside traditional custodians across the Americas and some of Australia.

This has taught me more than any textbook could about the depth of relationship between people and land. To be frank, what’s kept me alive through challenging periods of life has been my relationships to plant, fungal, and animal life.

Australia has such a rich Indigenous plant heritage.How is traditional knowledge being recognised and incorporated into contemporary herbal research?

Australia’s indigenous plant heritage is one of, if not the oldest and most ecologically attuned knowledge systems on Earth. Aboriginal and Torres Strait Islander peoples have preserved their intricate relationships with plants for tens of thousands of years encompassing medicine, ceremony, food, and Country stewardship. This knowledge is increasingly recognised not just as “folk tradition” but as a sophisticated empirical science.

Across universities and research institutes, initiatives such as collaborative ethnopharmacology, biocultural knowledge registers, and community-led research protocols have been developed to ensure that indigenous intellectual property and agency remains central. Plant efficacy cannot be divorced from ecological context, spiritual significance, and cultural custodianship.

What are some of the challenges in bridging traditional plant use with evidence-based research frameworks?

Bridging traditional plant use with modern evidence-based models remains an ethically complex space, particularly in Australia. The frameworks underpinning contemporary PAGE 52 | The Natural Therapist

science (i.e., peer-reviewed trials, pharmacological isolation, and quantitative data) are historically rooted in colonial epistemologies that privileged Western models of knowledge.

A legacy of trust-breaking persists. Plants once shared in good faith for cultural exchange, were (and are) often patented or commercialised without consent, while sacred knowledge was (and is) decontextualised and exploited.

Moreover, ecological issues compound this tension in the way of overharvesting, habitat loss, and the commodification of medicinal species. We see a severance in the complementary relationship between land and community, contributing to what could be described as ‘land memory sickness’ – where environmental degradation reverberates through plant/fungal life, geology, and collective wellbeing.

How can researchers and clinicians respectfully collaborate with Indigenous communities in this area?

Respectful collaboration means allowing Country and its custodians to lead the research conversation, not adapting indigenous knowledge to fit scientific structures.

Some practical actions can include: Sovereignty: Hold indigenous sovereignty at the forefront in research design, data ownership, and dissemination.

Consent: Make sure to adhere to community protocols, including seeking consent from Elders and local custodians before collecting, testing, or publishing any information.

Take Time: Build long-term relationships rooted in respect (e.g., ensuring that any economic, intellectual, or environmental benefits are returned to the communities and lands of origin). Remember, for many indigenous communities, plants are not simply biological resources, they are ancestors, teachers, and sentient beings.

Support the Future: Be part of community-led education and land care initiatives that sustain both traditional and ecological health.

Could you share an example of a recent ethnobotanical study that has influenced clinical practice or herbal formulation?

An interesting study published earlier this year examined how colonial-era pharmacopoeias universalised indigenous plant knowledge by translating culturally embedded “substances with bodily effect” into standardised “medicaments”. Using Andean Qollahuaya pharmacopoeias as a case study, it shows how social and ritual contexts were completely lost when indigenous substances

entered Galenic frameworks.

History of Pharmacy and Pharmaceuticals, 66(2), 236–256. https://doi. org/10.3368/hopp.66.2.236 Rather than stripping knowledge from its place and lineage in the form of universalisation, the most promising research directions are those that re-centre relationship between people, plants, ecosystems, and ways of knowing.

Many practitioners feel that research can sometimes strip away the “spirit” of herbal medicine. How do you balance science and tradition in your own work?

My practice is heavily rooted in the Hermetic principles of the ancient West – an alchemical worldview that regards nature as animated, intelligent, and reflective of universal order. It’s too expansive to unpack fully here, but to paint a small snapshot: before modern chemistry emerged, laboratory alchemy was as much a spiritual discipline as it was a scientific one.

The spirit – that invisible animating principle within matter – is considered essential to both transmutation and depth of understanding.

For me, science and tradition are not opposing forces, they are two languages describing the same mystery.

The laboratory processes involved in spagyric practice, for example, is both measurable chemistry and ritual participation. So rather than trying to balance science and spirit, I let them converse. I hold empirical evidence and good manufacturing practice at high regard, and I also maintain ritual, prayer, and relationship as integral parts of the work.

With increasing interest in plant-based medicines, how do we ensure sustainability and ethical sourcing of traditional herbs?

Firstly, we need to learn more about bioregional herbalism (understanding the ecological habitats of the plants) and identify what grows abundantly within our own region.

Sustainability will look different depending on the scale and nature of your practice - a small-batch herbalist might grow or wildcraft much of their Materia Medica if they have the means, while larger companies need to prioritise regenerative and transparent supply chains.

Collaborative partnerships with farms growing seasonally and sustainably can support manufacturing purposes.

Ethical sourcing requires that reputable suppliers be willing to discuss their methods, origin, and ecological impact openly.

Some good questions to ask include: Is this species invasive or endangered in its natural habitat?

• What are the harvesting or farming practices being employed by the source?

Are workers paid fairly, and is there a sustainable land care plan enacted?

Does the company have personal partnerships with local farmers, harvesters, or communities? If so, what is the nature of their agreement?

Are there any traditions that you find especially meaningful or clinically relevant in your practice or research?

The Unani Tibb system has influenced me the most as I have come to know humoral medicine, temperament, and energetics both correlating to the body and the herbs themselves. Its framework is closely aligned with both Hermetic philosophy and practical alchemy, giving language to that delicate dance between the material and immaterial aspects of healing and health.

What has been the most surprising or inspiring lesson you’ve learned from working at the intersection of plants, people, and culture?

The most surprising lesson I’ve learned is that many people from Western civilisation today believe we’ve “lost” or become “disconnected” from our animist or nature-based roots. While this sense of loss is understandable – the historical evidence is fractured and oral traditions often survive only as fragments, if any – what’s more revealing is what has endured.

When we look at the archaeological and anthropological records of north-western Europe as one of many examples, we begin to see that the instinct to commune with the more-than-human world has never really vanished; it’s only shapeshifted overtime. Disconnection is never absolute, it’s just a matter of remembrance and reactivation in our DNA.

How do you see the future of ethnobotany in Australia?

The future of ethnobotany in Australia will be determined by the extent to which we maintain integrity within indigenous spaces and knowledge systems. It means continuing the legacy of ethnobotanists such as Dr Beth Gott (1922 – 2022), whose life’s work honoured Aboriginal plant knowledge through careful, respectful documentation and collaboration rather than extraction.

Going forward, I believe the most meaningful progress will come from collaborative and community-based initiatives,

projects that cross disciplines and social boundaries to restore balance between people, plants, and place.

Finally, what message would you like to share with practitioners who want to deepen their connection with traditional plant wisdom while maintaining scientific rigour?

Understand your scope, beliefs, and cultural alignment or complexity. By committing to discover more about yourself (including your bloodlines) and unravelling learned limitations, you can identify your biases when reading and interpreting traditional and modern scientific research.

I would say the most important skills would be curiosity and getting your hands in the soil! Direct engagement might look like joining a bushland or community garden group or spending time with elders and local custodians.

Spend some time learning to grow, identify, and observe even a handful of plants in your local landscape.

Chinese Herbal Medicine has always emphasised prevention as much as treatment. While herbs like Huang Qin and

Ban Lan Gen get a lot of attention during flu season, one herb I return to time and time again - both in clinic and at home - is Jie Geng (桔梗), also known as Platycodon grandiflorus or balloon flower root.

Jie Geng has long been used to disperse Lung Qi, resolve phlegm, relieve sore throats, and drain pus. It’s commonly used in cases of cough, chest congestion, tonsillitis, and lung abscesses, and is a go-to herb when patients present with phlegm-heavy conditions or excess in the Lung channel.

It also plays a key role in opening and regulating the Lung Qi pathway, making it easier for other herbs in a formula to reach the upper burner. It’s often paired with Gan Cao (Licorice root) to enhance its throat-soothing and immunesupportive actions.

What Does the Research Say?

Recent pharmacological studies confirm that Jie Geng is far more than a traditional throat herb. A 2022 study published in Pharmaceutical Biology found that saponins extracted from Platycodon grandiflorus have antiinflammatory, antitussive, and expectorant effects that modulate immune function and inhibit pro-inflammatory cytokines like IL-6 and TNF-α—both of which play key roles in respiratory infections, including COVID-19.

A 2021 study in Frontiers in Pharmacology highlighted Jie Geng’s antibacterial and antiviral effects, noting its ability to inhibit lung inflammation, protect alveolar cells, and regulate oxidative stress. This makes Jie Geng highly relevant for COVID-19 recovery, post-viral fatigue, and chronic cough. give it dual strength as both a preventive and restorative herb.

Jie Geng’s saponins have also shown antitumor, antiasthmatic, and even neuroprotective properties, making this root a potential candidate for broader applications in integrative medicine.

Clinical Use and Daily Support

In my clinical practice, Jie Geng is a trusted ally during cold and flu season, but also year-round as an immune tonic. Naturopaths widely use it as a gentle, daily immune support for children and vulnerable populations, especially those prone to respiratory infections. And to be honest, I use it daily for my own kids - it sits on the same shelf as ginseng and echinacea in our home. It’s safe, mild, and surprisingly powerful.

We start using Jie Geng at the first sign of a sore throat or stuffy chest, and I often include it in prevention protocols for families during high-risk periods. Its ability to protect the lungs from pathogen invasion and prevent complications like pneumonia makes it especially important during outbreaks.

Jie Geng might not be flashy, but its consistent, proven benefits make it one of the most valuable herbs in our toolbox today. It’s a beautiful reminder that ancient herbs still have a vital role in modern health - especially when supported by today’s science.

In the contemporary healthcare scene, a growing consensus acknowledges that true well-being extends beyond the mere absence of disease. Modern medical frameworks, though highly effective for acute conditions, are increasingly integrating a whole-person approach that addresses the physical, mental, emotional, and spiritual dimensions of health.

This movement, often termed integrative medicine, look to combine evidence-based conventional treatments with complementary therapies to provide comprehensive patient care. It is within this paradigm that the ancient science of Ayurveda finds its most potent modern-day relevance.

For health practitioners trained in conventional systems, Ayurveda may initially appear to be a philosophical curiosity rather than a structured medical science. The traditional view holds that medicine must be based on the scientific method, with a focus on repeatable, quantifiable results from clinical trials. However, this perspective is shifting to a more nuanced understanding. Ayurvedic epistemology offers a sophisticated, qualitative framework for interpreting the complex data of an individual’s health.

It provides a language to bridge the gap between bodily experiences and systemic patterns.

For example, the Ayurvedic concept of Gunas, qualities such as Guru (heavy) and Tikshna (sharp) can be used to interpret the actions of modern drugs. A drug with a Tikshna quality, such as an aggressive antibiotic, is understood to have a rapid, penetrating action, while a sedative with Manda (slow) and Guru qualities is seen to have a gradual, prolonged effect. allows practitioners to proactively anticipate how a drug’s emergent effects (Karma) might manifest differently in a patient based on their constitutional context (Prakruti/ Vikruti).

For example, a drug that is inherently Ruksha (drying) might aggravate a susceptible Vata-predominant individual, leading to secondary adverse effects. This demonstrates that Ayurveda is not just an ancient belief system but a structured, logical framework that can enhance a practitioner’s understanding of pharmacological effects and patient outcomes.

The practitioner-patient relationship is vital in this integrative model. Clinical communication strategies that are increasingly taught in medical schools, such as asking open-ended questions, sitting at the patient’s level, and maintaining a non-judgmental attitude, are foundational to the Ayurvedic consultation.

Acknowledging a patient’s efforts toward self-care and asking directly about their use of complementary therapies can build a stronger therapeutic alliance and encourage openness which is crucial for safety and coordinated care.

Deconstructing the Ayurvedic Consultation A Practitioner’s Guide

An Ayurvedic consultation is not a brief, time-constrained interaction. It is an extensive, multi-layered inquiry designed to capture a complete picture of the patient’s health. This is achieved through the Trividhapariksha, or three-step examination, which includes observation, touch, and questioning.

The Diagnostic Process Darshan, Sparsha, Prashna

The diagnostic process begins with Darshan (Observation), where the practitioner assesses the patient’s general physical health, including their movements, body contour, skin and hair qualities, and facial lines.

This is followed by Sparsha (Touch), which involves palpation, listening to internal sounds, and percussion.

A key component of Sparsha is Nadi Pariksha (pulse diagnosis), a sophisticated technique that, in the hands of an expert, provides a real-time window into the body’s internal workings.

While conventional medicine uses pulse to measure heart rate and rhythm, Ayurveda interprets it as a “subtle symphony” of qualities that reveal an individual’s unique constitution and state of imbalance. Practitioners identify

distinct rhythms, such as the snake-like movement for Vata, the frog-like rhythm for Pitta, and the swanlike movement for Kapha. This is not a replacement for modern diagnostics but rather a quick, non-invasive tool that serves as a valuable bio-feedback loop, often confirming insights gained during the verbal intake.

The most extensive and critical step is Prashna (Questioning), the holistic intake. This process goes far beyond a typical medical history, considering all areas of a person’s life.

The practitioner inquires about: • Daily Routine Questions about the regularity of sleep, waking times, and meal schedules, which can directly indicate the degree of Vata’s irregular qualities.

• Diet and Digestion Detailed questions about appetite, digestion, food sensitivities, and bowel habits, a central focus in Ayurvedic diagnostics.

• Mental and Emotional State Open-ended questions about current state of mind, reaction to stress, concentration, and emotional patterns.

• Physical Qualities Inquiries about body temperature, skin/hair condition, weight, and energy levels.

This deep inquiry serves a dual purpose. From a diagnostic standpoint, it provides the comprehensive data needed to pinpoint the root cause of an imbalance.

From a therapeutic perspective, it is an intervention in and of itself.

By creating a space for the patient to share their concerns about their relationships, work, and mental health, not

just their physical symptoms, the practitioner helps the patient feel seen as a whole person, building a strong and trusting therapeutic partnership. This comprehensive questioning is the first step in cultivating the patient’s own body awareness, empowering them to become an active partner in their healing journey.

The Vata Dosha Aggravation A Case Study in Holistic Diagnosis

Vata is the dosha associated with the elements of Space and Air. It is the energetic principle of movement, governing everything from the pulsation of the heart to the flow of thoughts. The primary qualities of Vata are dry, light, cold, rough, subtle, and mobile. When Vata becomes aggravated, its qualities increase, leading to a state of imbalance that manifests across the entire mindbody continuum.

The unifying principle of Vata aggravation is mobility, coldness and dryness. The same excessive mobility that causes a racing mind and feelings of restlessness also disrupts the physical body, leading to irregular appetite, inconsistent digestion, and spasms. Likewise, the inherent dryness of Vata can cause dry skin, brittle hair, and constipation. To illustrate this interconnectedness, consider the following holistic symptom map for Vata aggravation: This Vata-centric approach provides a powerful framework for understanding and addressing chronic conditions that

often lack a clear diagnosis in conventional medicine. For example, the pathogenesis of Irritable Bowel Syndrome (IBS) is directly linked to an aggravation of Vata, where its dry and mobile qualities lead to gas, bloating, and irregular bowel movements.

Similarly, fibromyalgia is classified in Ayurveda as a Vatavyadhi, a disorder of Vata, where the dosha’s mobile nature affects the muscles (Mamsa Dhatu), leading to widespread pain, stiffness, and fatigue. Even a condition like Generalised Anxiety Disorder, which is typically viewed as a psychological condition, is seen in Ayurveda as a classic manifestation of Vata imbalance in the mind, characterised by restlessness and a racing mind.

By understanding this unifying principle, practitioners can move beyond treating isolated symptoms and address the single, underlying root cause of a patient’s multisystemic complaints.

A Tailored Path to Balance Recommendations for Vata Imbalance

Ayurveda’s therapeutic approach is based on the principle that “opposites balance”. To pacify Vata’s dry, cold, and mobile nature, a practitioner recommends therapies and lifestyle changes that incorporate warmth, nourishment, and stability. This therapeutic logic applies to every aspect of the patient’s care, from diet to daily routine.

Dietary Adjustments Food as a Psycho-Physiological Intervention

In Ayurveda, food is considered medicine, chosen for its qualities (Gunas) and tastes rather than its macronutrient profile. For Vata aggravation, the focus is on calming the erratic energy of the dosha by providing grounding and nourishing sustenance.

The Vata-pacifying diet emphasises warm, freshly cooked, and oily foods, such as soups, stews, and root vegetables, while minimising cold, dry, and raw foods.

The sweet, sour, and salty tastes are favoured, as they possess qualities that are opposing to Vata.

For a conventional practitioner, this approach offers a deeper understanding of food’s role. A bowl of hot, oily soup is not just a source of calories; its heavy and warm qualities physically and mentally ground an individual, soothing an agitated nervous system.

Lifestyle and Routine Cultivating Grounding Habits

To counter Vata’s inherent irregularity, establishing a consistent daily routine (Dincharya) is considered the most powerful therapeutic intervention. A predictable schedule provides a “home base” for an otherwise scattered nervous system, creating a stabilising effect that directly addresses Vata’s mobile nature.

Key recommendations may include: • Consistent Schedule; Going to bed and waking up at the same time each day, and eating meals at regular intervals.

• Self-Care Practices; Daily warm oil massage (Abhyanga) with sesame oil or other nourishing oils to soothe dry skin and calm the nerves.

• Appropriate Exercise; Gentle, flowing movements such as yoga, tai chi, or walking are recommended, as they provide grounding without the excessive movement of high-impact activities.

Vata-Pacifying Recommendations Dietary

Favour Warm, cooked, oily, and heavy foods. Grains like rice and cooked oats.

Lean meats, eggs, and dairy. Nuts, seeds, and ghee. Sweet, sour, and salty tastes. Avoid Cold, raw, and dry foods. Most beans.

Crackers and popcorn.

Pungent, bitter, and astringent tastes.

Lifestyle

Follow a consistent daily routine. Daily warm oil self-massage. Practice gentle exercises like yoga, tai chi, or walking. Stay warm and avoid cold, windy environments. Do not skip meals.

Herbal

Rejuvenatives Ashwagandha to support the body’s stress response.

Nervine Tonics Brahmi and Shankhpushpi to soothe the nervous system and enhance mental acuity.

The Spiritual Dimension Nurturing the Soul

For the conventional practitioner, the Spiritual aspect of Ayurveda may be the most challenging to grasp. It is important to frame this dimension not as a religious requirement but as a core component of psychological and physiological equilibrium. Ayurveda’s roots lie in the Vedas, ancient texts that view all of life as a Spiritual journey. In this context, the Spiritual path is about nurturing the Soul (Atma) and increasing mental clarity, a quality known as Sattva.

Ayurveda provides a clear psychological framework for this. The three Gunas, Sattva (clarity), Rajas (activity/ restlessness), and Tamas (inertia/dullness) are seen as the fundamental qualities of the mind. In this model, Rajas and Tamas are the causes of disease.

A mind dominated by Rajas is prone to overwork, anxiety, and eventual burnout, while a mind dominated by Tamas can lead to apathy, lethargy, and depression. Spiritual practices are therapeutic tools for increasing Sattva, a state of peace, balance, and clarity, which directly counters the agitation of Rajas and the dullness of Tamas.

These practices are becoming increasingly familiar and clinically validated within conventional medicine. For example, meditation, Pranayama (breathing exercises), Yoga Nidra and yoga are proven to reduce stress, lower blood pressure, improve sleep, and manage chronic pain.
