Presentation Introduction

Biomedicine was incorporated into the 5+2 Industry Innovation Plan in 2017, and precision health became one of the Six Core Strategic Industries in 2020 in Taiwan. The strategy to actively support and strengthen biomedical industry resulted in a revenue of over US$19 billion in 2019 alone. The health care system of Taiwan was ranked as the second best (next to South Korea) by CEOWORD Magazine in 2021, and Taiwan’s health care index was ranked as the best by NUMBEO in 2022.

Precision epidemic prevention system was launched in early 2020, which has successfully contained COVID-19 epidemic and sustained economic growth in Taiwan. To cope with the widespread Omicron variants in 2022, increased coverage of vaccine boosters, rapid diagnostics and antivirals through telemedicine has kept low rates of confirmed COVID-19 deaths and estimated excess deaths from all causes during the pandemic in Taiwan.

The integration of Taiwan’s excellent biomedical R&D, digital care and smart medicine, national health insurance system, multi-centered clinical trials consortium, national biobanks, Medi-Cloud and big data in healthcare, information and communication infrastructure, ambient intelligence, and niche products is expected to allow the country to become a major hub for the precision health and digital medicine in the Asia-Pacific region.

Presentation Introduction

The global healthcare spending continues to grow and is expected to reach $15 trillion dollars in 2030, and the fastest growing area is for early diagnosis and disease prevention accounting for 30% of the projected spending. This trend indicates a shift from disease treatment to prevention. To make the prevention care more accurate and cost-effective, the technologies commonly used in the information and communication industry should be contributed to product development. Within this context, this lecture will present our recent development of broadly protective vaccines against SARS-CoV-2 and its variants.

Since the outbreak of SARS-CoV-2 in 2019, the virus continues to spread and mutate with more than ten million spike protein sequences reported, and the vaccines used to date begin to fade their protection against the emerging variants, though they are highly protective against serious conditions. To develop a more effective and broadly protective vaccine, our approach is to remove the sugar coat from the spike protein to better expose the highly conserved epitopes and use the sugar-free spike protein or its mRNA for vaccine design. Both vaccines in animal models were shown to elicit more enhanced and broadly protective responses against the variants of concern, including the delta variant and all omicron subvariants as compared to the current vac cines.

Professor Wong received his B.S. (1970) and M.S. (1977) degrees in biochemical sciences (with KT Wang) from National Taiwan University, and Ph.D. (1982) in Chemistry (with George M. Wh itesides) from MIT. After one year of postdoctoral research at Harvard University (with George M. Whitesides), he became a faculty member of the chemistry department at Texas A&M University (1983) where he was promoted to full professor in 1987. He then moved to The Scripps Re search Institute in 1989 as Professor and Ernest W. Hahn Chair in Chemistry, and from 2006 to 2016, he served as President of Academia Sinica in Taiwan. He is currently the Scripps Family Chair Professor of Chemistry at The Scripps Research Institute and holds a joint appointment as Distinguished Professor at Genomics Research Center, Academia Sinica.

Professor Wong received many honors for recognition of his accomplishments. Some represen tative honors include: Presidential Young Investigator Award in Chemistry, USA (1986), Roy Whistler Award of the International Carbohydrate Organization (1994), ACS Harrison Howe Award in Chemistry (1998), ACS Claude S. His research interests are in the field of chemical biology and synthetic chemistry, with current focus on the development of new methods and tools for the synthesis and study of complex carbohydrates and glycoproteins, elucidation of carbohydrate-mediated recognition in disease progression and immune response, and development of universal vaccines against cancer and viral pathogens such as influenza and SARS-CoV-2. He trained over 500 graduate students and postdoctoral fellows, published over 750 papers and 120 patents, and is a highly cited scientist with an h-index of 150.

The pandemic has had a profound impact on Health care system, including, financial and operational challenges, workforce shortages, behavioral health needs, digital health access, and care delivery transformation. By 2023, healthcare markets will be under pressure to find revenue and government will be entering a period of significant disruption.

According to the President’s Budget Plan released this March which will be effective from fiscal year 2023 through 2032. There’s 26.8% increase spending from the 2021 for expanded health coverage and services, such as mental health needs, healthcare access, health research and innovation, and etc.

Taichung Veterans General Hospital, a 1,624-bed hospital, is located in central Taiwan. It started medical services on 1982. Since 1991 it has been accredited "Medical Center and First-Class Teaching Hospital" by National Health Bureau. It provides safe, high-quality medical services. With the vision "To deliver intelligent and holistic medicine," we are devoted to medical AI research to develop a world-renowned Smart Hospital.

Zoe: AI+HI Smart Critical Care System by Taichung Veterans General Hospital, Tunghai University, and Advantech Co., Ltd co-create an aim to tackle the rapidly evolving and complex nature of critical care medicine and the challenges of shortage in critical care medical personnel. We centered on the main themes of critical care medicine: respiratory care, bacteremia, end-of-life care, acute kidney injury, and pain & sedation. Using international coding systems, we built the 6-year critical care database with 66.1 million records across 24 categories and 339 features. More than ten artificial intelligence (AI) models have been developed to assist clinical care deci sion-making. We anticipate the clinically effective implementation of smart medical healthcare and positive benefits to more excellent human health and welfare.

AI techniques have been applied in cardiovascular medicine by Taichung and Taipei Veterans General Hospital,to explore novel genotypes and phenotypes, improve the quality of patient care, enable cost-effectiveness, and reduce readmission and mortality rates. In Taiwan, under the cooperation of industry and academia, several AI algorithms have been developed, including the ECG AI, cardiovascular imaging, and interventional cardiology platforms. AI will soon lead to a paradigm shift toward personalized precision cardiovascular medicine. Further, the increasing use of AI prediction models will aid in deciding tailored therapies with predictive characteristics.

Shih-Ann Chen, MD, is Professor of Medicine at National Yang Ming Chiao Tung University School of Medicine, Superintendent in Taichung Veterans General Hospital, Taichung, Taiwan, also Adjunct Professor at Case Western Reserve University, Ohio, USA.

In his more than 30-year period of work in arrhythmology, Prof. Chen and his group members are devoted to investigate the mechanisms and ablation techniques for different types of cardiac arrhythmias, and published more than 769 original articles, more than 215 reviews, editorials, and textbook chapters, including more than 70 articles in top ranking cardiology journals, nature reviews cardiology, and annals of internal medicine. Further, he and Prof. Michel Haissaguerre are the first two pioneers in the world to develop the atrial fibrillation ablation technique. Prof Chen is the first person in the world to edit the textbook of "Thoracic Vein Arrhythmia", which pro vides the novel concept of atrial fibrillation in the era of interventional cardiac electrophysiology.

In 2007, 2011 and 2016, he was appointed as the chairman of the writing group for the chapter of atrial fibrillation mechanism in the consensus document of Atrial Fibrillation Ablation by Heart Rhythm Society/European Heart Rhythm Association/Asia-Pacific Heart Rhythm Society, the chapter of Pathophysiology of Atrial Fibrillation in the Venice Charter for Atrial Fibrillation Ablation. In 2010 March, he received the Simon Dack Award for Outstanding Scholarship from the American College of Cardiology, USA. Further, he is the founder and the course director of the Asia-Pacific Atrial Fibrillation Symposium since 2005, and International Forum of Ventricular Arrhythmia since 2015, founder and the president of the Asia-Pacific Heart Rhythm Society in 2012 and 2013. Speaker Introduction

Ethan Tu is most well-known as the founder of PTT, Taiwan’s largest and most popular online bulletin board system. In the US, he worked at the National Institute of Health (NIH) to develop cancer detection systems and human genetic research. He also worked as Microsoft’s Director of Research and Development for AI in the Asia Pacific Region for 11 years, and was a key figure in development of Microsoft’s Cortana.

In 2017, Ethan founded Taiwan AI Labs, Asia’s first open AI research organization specializing in next generation AI solutions. AI Labs has shown significant results in medical AI/imaging applications, which contributed to the detection of Covid 19 from chest x-rays during the pandemic. He also started The Taiwan AI Federated Learning Alliance to encourage the pooling of data samples for better AI advancement while protecting privacy rights. Ethan also leads teams to develop AI solutions for drones, Smart Cities, speech and facial recognition, and the fighting of fake news.

Building people-oriented AI technologies for everyday life has been Ethan’s biggest faith and focus, which explains AI Labs’ mission: “Innovate AI in Taiwan and for The World, bring technology to meet nature and humanity, and create impacts and improvements for human well-being. “

Speaker Introduction

Joni Cheng is the Senior Manager of V5 TECHNOLOGIES CO., LTD. She is responsible for the quality and Regulatory affairs of medical business, in cybersecurity, privacy, risk, regulatory compliance, and commercialization. She took part in formulating quality system projects and has already obtained certifications from the Taiwan government and international agencies, such as ISO 13485 and QMS system. She holds a Master of Science in Management from The Glasgow University, UK and Professional Master of Law in Business Administration from The National Taiwan University.
