Words from the Chair of the AMA Council of Doctors in Training, Will Milford AMACDT: Making sure junior doctors are not forgotten Graduation! Finally, you have arrived at your destination: internship. From both myself and the entire Australian Medical Association Council of Doctors in Training, (AMACDT) congratulations on completing your undergraduate career and welcome to your prevocational training! Just as AMSA and the medical societies were part of being a medical student, the Australian Medical Association (AMA) is part of being a doctor. The AMA is the peak representative body for medical practitioners, including junior doctors, with issues concerning doctors in training (DITs - that’s you!) forming a large part of its agenda. It also speaks out about important public health issues, such as climate change and alcohol related harms. DIT members take full benefit from the AMA’s powerful advocacy, industrial support, suite of targeted resources and range of corporate benefits tailored to the education and lifestyle needs of junior doctors. As Chair of the AMACDT, my role is to ensure that junior doctors are not forgotten, but considered a crucial stakeholder in the Australian health system. AMACDT is primarily concerned with advocacy activities concerning workforce, education and training, and workplace conditions, including safe hours, doctors’ welfare and e-health. The AMA’s broad membership base provides a platform on which we can work with other key stakeholders, meet

8

Australian Medical Students’ Association

with politicians and speak to the media about these important issues. Our other activities include producing useful resources for DITs, often collaborating with AMSA. You may have seen the AMA’s Guide to Online Professionalism as well as the Guide to Working Abroad for Australian Medical Students and Junior Doctors. You can download these at our website (www.ama.com.au/dit). You might also want to take a look at out recent policy statements to get a feel for the sort of advocacy work that we’re doing. The AMA and the AMACDT is only as strong as our membership. At this important time for medical training our voice matters and with more members, the louder it will get. It is a crucial time for junior doctors with workforce and training pressures only getting worse. I urge you to become a member of the AMA so your voice can join ours and make the future health system one that you want to work in. Good luck next year and the years to come. The AMA knows that you will make an important contribution to the health of Australian communities and your patients. Congratulations once again! Will Milford Chair, AMA Council of Doctors in Training

Information All JMOs Should Know WHAT IS AN INTERNSHIP? Internship is (usually) a one year period where a medical graduate undergoes supervised clinical training within an accredited hospital. Interns have been granted provisional registration with the Medical Board of Australia, and will gain full registration at the completion of their intern year. In general, interns are required to complete 47 weeks of clinical training, which excludes time taken off for sick or annual leave.

shift each fortnight, if not every week. These include evening shifts, night shifts and weekend shifts. You also need to look out for when you have a sick relief shift. The doctor on sick relief can be called at any time of the day or night to step in. This includes being called to step into a night shift at 11.45pm when you are about to go to sleep. Each state has a different policy on ‘unrostered overtime’. Check out your state and hospital policies on this prior to commencing work.

WHAT IS THE STRUCTURE OF THE YEAR? Most hospitals operate with five rotation blocks throughout the year for interns - some have four, ranging between 10 and 12 weeks in length.

CAN I MAKE A COUPLE APPLICATION WITH SOMEBODY ELSE? Most states accept applications for couples or joint-tickets. It is best to check with the websites listed under each state’s application information section.

WHAT TERMS AM I REQUIRED TO COMPLETE? Australian interns are required to complete five terms throughout the year, consisting of at least one surgical, medical and emergency medicine term. Each hospital offers a different selection of terms for their interns. Residents tend not to have core terms. HOW MUCH WILL I BE EXPECTED TO WORK? For all states across Australia a standard week is 38 hours, plus overtime. What you need to look out for is your particular state and hospital’s regulations about overtime. Generally expect to have at least one overtime

Can I get special consideration for my application? Each state has a separate and different policy for special consideration. Special consideration is generally reserved for applicants with extenuating circumstances such as dependant children or a serious illness. Check their websites. WHAT IS AN INTERN’S BASE SALARY? Each state has a different award salary. JMOs are paid a base salary, with professional development allowances, overtime and penalty

rates added on top of this. The amount of overtime you are paid depends on how many hours you work! The approximate starting salaries in 2013 (including known allowances, and excluding overtime) range from $55,000 to $73,000 depending on which state you apply for. WHAT ARE THE RESTRICTIONS FOR INTERNATIONAL STUDENTS WHO HAVE GRADUATED FROM AUSTRALIAN MEDICAL SCHOOLS? If you are an international student and intend to stay in Australia for internship and/ or vocational training you must lodge an application for a Business Long Stay Visa subclass 457 before the expiry of your student visa. The hospital offering your employment will assist in this process. If you wish to stay in Australia for vocational training and apply for permanent residency (under the Regional Sponsored Migration Scheme or the Employer Nomination Scheme) after completing your internship, you will need to investigate your visa options. The Department of Immigration and Citizenship (DIAC) website at www.immi.gov.au is a helpful resource. For pre-visa lodgement enquiries, the general inquiries line (13 1881) should be used.

New Rules for the Application and Allocation of Interns for 2014 The National Intern Allocation Working Party Summary for 2013 • Students who have accepted offers from multiple states and do not answer emails or phone calls regarding their circumstances and why they should be given further time to make a final decision may have one or more of these offers withdrawn, even after they have been accepted. • The First Round of offers will begin on Monday, 29 July 2013. Internship offers will continue to be made after this date, but there will be no formal date for “second” or “third” round offers, as these dates were rarely adhered to in the past anyway. • In 2012, the prioritisation order in Victoria changed at the last minute. There are rumours that some states may change their prioritisation to be the same as Victoria for 2013, although AMSA is meeting with State and Federal governments to advocate for national consistency between states. Those applying for Internship should ensure they stay up-to-date with this information as it is released. Accepting Multiple Offers The National Internship Allocation Working Party (NIAWP) is a hard-working group of people who meet via teleconferences and face-to-face meetings throughout the year to ensure that the application process and allocation of medical students to internships occurs as quickly and as fairly as it possibly can. AMSA sits on this working party, along with the

10

Australian Medical Students’ Association

AMA, Health Workforce Australia and members from each state/territory (“jurisdiction”) who will be responsible for allocating Internship positions within their jurisdiction. Last year, the work which the NIAWP had to do was unprecedented. Whilst in years past, the allocation process finished after a few months, in 2012 it stretched on into the early months of 2013. This year, the jurisdictions will be looking for ways to make this process fairer and more efficient. One of the issues which caused the process to be slow last year was that many medical students had accepted offers in multiple jurisdictions. In instances where this was identified, jurisdictions were unsure if these students were intending to accept the offer, or were intending to “hold onto a few offers” until a later date. This was compounded by the fact that many members of the NIAWP found it quite difficult to contact potential interns, via phone or via email. When these medical students finally accepted an offer, it left jurisdictions with vacant spots, which should have been filled far earlier in the process, for everyone’s benefit. In 2013, some jurisdictions will reserve the right to withdraw offers from medical students who have accepted multiple offers, and are unable to be contacted via the contact information they provided in their application.

In light of all this information, AMSA recommends: 1. If you can avoid accepting offers from multiple jurisdictions, do so; have only one accepted offer as early as possible. This will be in your own best interests, as well as making the system more efficient and fairer for everyone; 2. When you apply for internships, ensure that you will be contactable on the phone number/email address you provide in your application; and 3. If you have accepted offers from multiple jurisdictions for reasons which you can explain (e.g. family circumstances), communicate the nature of your status with these jurisdictions ASAP. Make sure you continue to be contactable by these jurisdictions until you have only accepted one offer. First round offers, but no second round In the past, there have been multiple “rounds” of offers, with set dates. However, the realities of the system have meant that these set dates were rarely adhered to, and once the first round passed, jurisdictions simply attempted to fill the spots they had remaining as quickly as possible. In 2013, there will only be one, “First Round” of offers, simply to reflect the actual nature of the allocation process. The date for this will be Monday, 29 July 2013. Prioritisation Before 2012, all jurisdictions prioritised the allocation of offers of internships in roughly the same order*: 1. Domestic, Commonwealth-supported students who are applying for internship in the same jurisdiction in which they studied medicine;

2. Domestic, Commonwealth-supported students who are applying for internship in a different jurisdiction to which they studied medicine; and 3. International students, who are full-fee paying and have studied medicine in Australia.

*Note that this is simplifying the prioritisation order (ignoring New Zealand citizens, for example), and that there were and still are differences between jurisdictions, but it is sufficient to demonstrate the key changes.

However, only a few hours before applications closed in 2012, Victoria decided to change their prioritisation order as follows: 1. Domestic, Commonwealth-supported students who are applying for internship in the same jurisdiction in which they studied medicine (Victoria); 2. International students, who are full-fee paying and have studied medicine in Victoria; and 3. Domestic, Commonwealth-supported students who are applying for internship from a different jurisdiction This was great for students in Victoria – they had the highest preference in all states! But for students who were looking to complete their internship in Victoria (e.g. Victorians studying elsewhere hoping to return home). It is entirely possible that there will be changes in the prioritisation order in the lead-up to the application process and allocation of internships in 2013. If this occurs, the jurisdictions should inform applicants, plus AMSA will communicate to students in whatever way possible. Good Luck with your applications! If you have further questions on the content of this summary, please email james.lawler@amsa.org.au

NSW

NEW SOUTH WALES Prevocational trainees have two primary obligations during their first two years as a doctor. First, as an employee, there is a service responsibility to patients and the hospital. Second, all trainees are responsible for the continuation of their medical education through clinical practice and attendance at educational activities. General registration requires satisfactory completion of internship. All doctors practicing in NSW need to be registered with the Medical Board of Australia prior to commencing work. Applications for provisional registration must be lodged in the state in which you will graduate, not the state in which you apply for internship. Prevocational trainees are allocated to a network. Each trainee can negotiate with their employing hospital network to be placed at the home hospital of their choice, in which they would like to spend the majority of their prevocational training. This will be dependent on the accreditation status of the home hospital, the network’s requirements and trainee preferences. Term rotations will be organised by the medical administration staff of the network. Trainees in NSW are encouraged to undertake a balanced mix of terms by working in a range of facilities including tertiary, metropolitan, outer metropolitan and rural hospitals.

12

There are 15 hospital networks in NSW and The Health Education and Training Institute (HETI) allocates prevocational trainees to these networks. The networks are structured to provide a broad variety of training and teaching experiences. Applicants should be aware that allocation within NSW may be to any of the 15 networks. If allocated to a Sydney metropolitan hospital, trainees may still be expected to undertake a rural term during each year of prevocational training. It is important to note that there is a federal initiative to deliver more doctors and better services to rural and regional communities. NSW HETI supports this initiative. Minor realignments of hospitals within the networks occur from time to time and new sites may become accredited and be added to networks. Thus there is a possibility that rotations may occur to hospitals that are not presently identified as a rotation for any specific network. The NSW Health Education and Training Institute (HETI) is responsible for the allocation process. Applications are submitted on-line. To apply on-line, graduates must first register with HETI. It is important to use an email

Australian Medical Students’ Association

address which is checked on a regular basis as the majority of correspondence between applicants and HETI is done through the website. There are nine steps which need to be completed in full. Required information includes personal details, university information, and any special considerations or joint applications. Other available applicant types include full time, job share and parttime. Applicants rank their hospital network preferences from 1 to 15. Some applicants require additional supporting documents so check the website. Interstate and international applicants are required to pay a bond when applying to NSW. This bond is only refunded after successful completion of a NSW internship. Applications for the main round open on the HETI website on the 1st of May and close on the 7th of June. In 2013, there were 927 places offered across the 15 networks.

Rural Preferential Recruitment (RPR) is a preferential recruitment process for final year medical graduates who wish to work in rural sites across NSW. RPR occurs prior to the main allocation round and those who are not successful in the RPR process are still eligible to gain an allocation through the main round. Those that accept a position through the RPR scheme will not be eligible for an allocation through the main round. How do I apply? You must apply directly to the rural site of your choice through ‘HealthJobs’ on the NSW Department of Health website. Applications will be shortlisted and interviews will be offered. Offers of employment are made via HETI and are sent out electronically. Up to three rounds of offers are made if necessary. Recruitment will be undertaken in line with the guidelines outlined by HETI and NSW Health recruitment policies, (please refer to the NSW Health website for these). Who is eligible to apply? All graduates of NSW medical schools (Priority Group 1.1) and graduates who completed year 12 in NSW (Priority Group 1.2) are eligible to apply for Rural Preferential Recruitment. Any positions not filled in rural hospitals as part of the Rural Preferential Recruitment process will be filled as part of the main allocation to hospital networks.

What else do I need to know about RPR? Things you should know about the RPR Scheme: • If you apply for RPR you must also apply for allocation. If you do not you will not be considered for a position through the RPR scheme • You may apply to as many rural hospitals as you wish and may receive multiple offers of employment • You can decline a position, however you then risk not being offered another position through RPR • If you are not offered a position or decline all positions that are offered, you will still be eligible to go in the main allocation round • Once you accept a RPR position, your application will be withdrawn from the main allocation round • You are not eligible to swap networks if you accept a RPR position • You must spend the majority of your PGY1 and PGY2 terms at the hospital to which you were recruited • RPR positions are still within prevocational training networks. Therefore you should still expect to undertake a rotation or two throughout the network • You should note that if you are an RPR applicant and accept a position at a three term home hospital, you must undertake two rotations outside of that hospital within the network in which you are employed since the hospital is only accredited to employ you for three terms.

The participating hospitals for 2014 are: Tamworth Rural Referral Hospital, Orange Health Service, Dubbo Base Hospital, Wagga Wagga Base Hospital, Manning Rural Referral Hospital, The Maitland Hospital, Lismore Base Hospital, The Tweed Hospital, Coffs Harbour Health Campus, Port Macquarie Base Hospital Building Capacity in the Aboriginal Medical Workforce All graduates of NSW medical schools (Priority Group 1.1) and graduates who completed year 12 in NSW (Priority Group 1.2) who are of Aboriginal or Torres Strait Islander background are eligible to apply. Specific application details and requirements are available via the HETI website.

Regional Preferential Allocation This is a preferential allocation of guaranteed applicants to regional prevocational training networks. You are required to nominate networks 2, 4, 7, 11, 12, 14, or 15 as your first and/or second preference. No separate application is required.Offers for the Regional Preferential Allocation are made two weeks

Don’t limit yourself Trying to choose your specialty? After a life full of challenge and diversity? Consider rural generalist medicine. Rural GPs need a broader set of procedural and other skills, to allow them to work confidently in rural and remote communities. With a Fellowship from the Australian College of Rural and Remote Medicine (FACRRM) you won’t limit yourself. You will receive the skills, training and registration to allow you to work independently in the specialty of general practice, anywhere in Australia. Through our Advanced Specialised Training, our registrars also select a secondary specialised skill area, such as surgery, obstetrics, anaesthetics, emergency medicine or six other areas of expertise, allowing you to broaden your capacity beyond the standard scope of general practice training.

Our Fellows work in private general practice, hospital based medicine, Aboriginal medical services, retrieval and emergency medicine, as GP anaesthetists, GP obstetricians and more, often concurrently, which means your career can be as varied and as challenging as you want it. There is no other general practice fellowship that covers this scope of practice. So, when you are looking for a career that is more than ‘a day at the office’, consider the FACRRM pathway. There is no equivalent.

SOUTH AUSTRALIA South Australia is the Australian state with a real work/life balance. Our state is growing and our famous lifestyle is more envied than ever. In 2011, Adelaide was ranked the most liveable city in the country by the Property Council of Australia.

SA

• Beautiful spaces, plenty of time and a real regard for family—the South Australian lifestyle is greatly sort after.

2014 will see a big change in internship allocation. The hospitals which previously would allocate and co-ordinate interns themselves have been put into four health networks; Southern Adelaide, Central Adelaide, Northern Adelaide, and Country Health South Australia.

• Known as the Festival State, South Australia has more than 400 events per year, including the world-renowned Adelaide Festival of Arts and the everpopular Fringe Festival.

The Postgraduate Medical Council of Western Australia (PMCWA) aims to provide leadership for the education and training of prevocational doctors in Western Australia (WA). The PMCWA works closely with the primary allocation centres to support and facilitate medical students applying for intern training positions in WA. There are three primary allocation centres in WA – Fremantle Hospital, Royal Perth Hospital and Sir Charles Gairdner Hospital. With the introduction of the Health Practitioner Regulation National Law Act 2009, Western Australia intern registration now lies with the Medical Board of Australia. Further details about the Medical Board can be obtained from their website: www.medicalboard.gov.au. Interns are required to complete 48 weeks of clinical training (exclusive of any time off for sick or annual leave). Ten days of study leave is available within the 48 weeks of clinical training. The Medical Board and PMCWA require all interns to undertake a balanced mix of clinical experience, which will include terms in medicine and surgery. A term in emergency medicine is desirable but not essential.

The PMCWA also encourages interns to gain exposure in a range of health service settings, including large teaching, outer metropolitan, regional, rural and remote and private hospitals. In addition to their award salary, interns also receive an annual professional development allowance, payable pro rata fortnightly. How do I apply? The WA Intern Application Process is an online application process consists of seven sections. Applications are made online at the WA Job Board website: www.jobs.wa.gov.au. A how-to-apply guide for a WA Intern position is available on the PMCWA website; www.pmcwa.health.wa.gov.au Who do I need to use as my Referees? You require two referees and these would normally be Consultants who know you. You should advise them that you would like them to be a referee and ensure that they will be able to speak about your performance. If you are unable to have a consultant as a referee, you may ask a Registrar to be a referee. What if I would like to go rural? The Rural Practice Pathway (RPP) was set up to provide a clear pathway into rural training and a career in rural medicine for junior doctors with a specific interest in rural medicine. The RPP comprises of a collaboration of those

organisations involved in undergraduate and postgraduate medical training. These include the WA Country Health Service (WACHS), WA General Practice Education and Training (WAGPET), the Rural Clinical School, Rural Health West (RHW), the Postgraduate Medical Council of WA (PMCWA) and Doctors In Training (DITs). The Rural Preference Program allows a rural rotation block of two or three rotations that are attached to your employing PAC. This rural rotation block will provide you with an opportunity to work closely with consultants and find about more about the region. Your PAC will still offer rural rotations to interns that are not a part of the Rural Preference Program however. You will be able to express an interest to do rural rotations and be considered in the Rural Preference Program during your internship in the intern application process. You can apply for up to 4 rotations rurally. For more information regarding the RPP, please contact A/Prof Sarah Moore at Sarah.j.moore@rcswa.edu.au.
