EMERGENCY CONSENT FORM

All parents must fill out an Emergency Consent Form when they register their child. Please enter the names of any person(s) whom you would trust to pick up your child in case of an emergency. The designated alternates will be allowed to pick up your child only if they properly identify themselves to the teacher in charge.

AMBULANCE SERVICE

All AST students and staff are covered by the ambulance service PorSalud while on the school campus and on school-sponsored field trips.

EMERGENCY NUMBERS

If the school decides students need to leave campus the following actions will take place:

• Parents will be contacted to pick up their children in the back and front parking lots.

• Staff will evacuate after all students have been accounted for and properly dismissed.

CAMPUS EVACUATION

If inside:

• Students and staff should drop to the ground and take cover under or next to a table or desk.

• Hold on until the shaking stops. Cover your head and face with your arms.

• Stay away from glass, windows, outside doors and walls.

• Stay inside until the shaking stops and it is safe to go outside.

• Research has shown that most injuries occur when people inside buildings attempt to move to a different location inside the building or try to leave the building.

• Be aware that the electricity may go out or the sprinkler systems or fire alarms may turn on.

If outside:

• Stay there. Move away from buildings and utility wires.

• Once in the open, stay there until the shaking stops. The greatest danger exists directly outside buildings, at exits and alongside exterior walls.

• Once the tremor or movement stops, teachers should start the evacuation procedures according to plan.

If in a moving vehicle:

• Stop as quickly as safety permits and stay in the vehicle. Avoid stopping near or under buildings, trees, overpasses, and utility wires.

• Proceed cautiously once the earthquake has stopped. Avoid roads, bridges, or ramps that might have been damaged by the earthquake.

If you hear, “LOCKDOWN” over the intercom or an administrator announces the lockdown in person:

• Everyone is to stay where they are.

Classroom teachers are to:

• Quickly glance outside the room to direct any students or staff members in the hall into your room immediately.

• Lock your door.

• Lower or close any blinds.

• Place students against the wall, so that the intruder cannot see them looking in the door. Look for the ‘Safe Corner’.

• Turn out lights and computer monitors.

• Keep students quiet.

Note: All staff members should locate and hold on to their roll book prior to turning out the lights. This will aid in accounting for all students should an evacuation be necessary.

– Physical education classes being held in the gym (or fields) should move into a locker room, lock all doors, and find a safe area.

– Students and staff in the cafeterias should move to the kitchen and/or stairwell area.

– If students and teachers are outside the school building, they should stop, drop, and remain still. You will be directed where to relocate depending on the situation.

– If teachers and students are in the bathrooms, they should move to a stall, lock it and stand on the toilet.

– Anyone in the hallway should move to the closest classroom immediately.

– Nurses/cafeteria workers/support staff should stay in the area they are in, secure the doors, and turn out the lights.

– Students and staff in the library should remain in the library. Librarians should lock the doors, turn out the lights, and locate a safe area.

Stay in safe areas until directed by law enforcement officers or an administrator to move or evacuate.

Never open doors during a lockdown, even in the event of a fire alarm.

For further directives, law enforcement officers and administrators will have keys to open the doors or announcements will be made over the intercom.

An administrator will signal all personnel if the lockdown has been lifted.

If an evacuation occurs, all persons/classrooms will be directed by a law enforcement officer or administrator to a safe location. Once evacuated from the building, teachers should take roll to account for all students present in class.

Administrators will divide and keep in communication with radios or cell phones.

• Stay Calm.

• Take a good look at the place and the victim.

• Call: School Nurse (9985-4067) and notify principal. (Principal notifies Superintendent)

• Aid the victim – Always attend those injuries that seem critical first.

• Do not leave victim unattended. Stay with victim and send a staff member or responsible student to notify nurse.

• The entire class should never be left unsupervised to tend to an injured student. In the event the injured student must be moved, the whole class should accompany the adult and injured student.

• A student with injuries to the neck or spine should not be moved.
