Group Adult Foster Care (GAFC) is a Persona Care Assistance Program sponsored and pai for by MassHealth (Massachusetts Medicaid
This program keeps individuals safe at hom and reduces the need for hospitalizations an admissions to nursing homes for more care.
There is no out of pocket expense and MassHealth pays for all of the cost of the program.
Eligibility Criteria:
Must be 22 years old +;
Live in Subsidized Housing;
Have MassHealth Standard and/or SCO Insurance;
Have a Primary Care Physician;
Require assistance with at least 2 ADLs;
Agree to have personal care services daily.

Must be 16 years old or older;
Must be a Massachusetts resident;
Must be 18 years old or older;
Must pass CORI, SORI, OIG check;
Requires assistance with at least 1 ADL;
Must have a family member (can’t be spouse, parent if the member is a minor or guardian) who lives in the same home.

Must have TB Screening within the past 12 months.
