Primary Care Team 2: 915 N. Grand Blvd. (Moving to new location summer 2012.) 314.289.7600 stlouis.va.gov What We’re Known For: The Veterans Affairs St. Louis Healthcare System is fully accredited and committed to providing the most reliable, effective and efficient healthcare treatment for our welldeserving veterans. Bigger and Better: Our new, expanded spaces will provide improved access to care for St. Louis’ veteran population, with eight Primary Care teams located at each facility. In addition to primary care, additional services at both locations include Mental Health, Laboratory, Pharmacy Counseling, Nutrition Counseling and Telehealth.

On the Cutting Edge: The VA St. Louis Healthcare System is joining a growing number of VA healthcare facilities in adopting a new method of health delivery called Patient Aligned Care Teams, or PACTs. This system is even more patient-centered than ever, and focuses heavily on quick and easy communication between patients and their healthcare providers. A revolutionary new service called Telehealth facilitates this, allowing easy communication between patient and doctor, as well as among multiple doctors treating the same patient. Our Focus: PACT is personalized care to meet the veterans’ individual healthcare goals, looking at all aspects of their health. In addition to personal visits with primary healthcare providers, veterans may schedule visits with other members of the team. This also includes access to group clinics, educational seminars, plus information through the My HealtheVet website.
