Designate “Nursing, Geriatric Technician” as your field of study. You may be required to submit the following official documents to the Registrar’s Office: ______ Submit official high school transcript (if college not attended) ______ Submit official GED Scores (if graduate of high school equivalency program) ______ Send official college transcript (from each college attended)
Step 2. Complete the Reading Comprehension and Sentence Skills Computerized Placement Tests (CPT): Obtain a referral form to take the CPT at one of the TCC Advisement Centers listed below: TCC Metro Campus, Room MC 1014 TCC Northeast Campus, Room NE S105

It is a Nursing Division requirement that all CNA Career Ladder/GT students take the Reading and Sentence Skills CPT, regardless of previous college coursework. This college placement test is used to assist your instructors in designing the program to meet your learning needs. Take your referral form to the Testing Center at any TCC Campus. Although the test is untimed, allow approximately 45 minutes for each test. There is no need for an appointment, and there is no fee. Assessment Center hours vary on each campus—call the numbers listed below for times. Please arrive in time to finish before Assessment Center closes. Step 3. Submit your CNA/Geriatric Technician application to: Nursing Certificate Programs Office Tulsa Community College 909 S. Boston Avenue, Room MC-501 Tulsa, OK 74119-2095 Telephone: (918) 595-7324 Apply early to ensure completion of all requirements before class begins. After your application is reviewed for admission, you will receive a letter informing you of your acceptance status. If accepted, you will be asked to make an enrollment appointment with the Nursing Certificate Programs Office. (If you are applying for financial aid, be sure to contact Student Financial Services to start this process early so that payment for the course will be prior to the start of classes.)
