hether they go by Grandma and Grandpa, Meemaw and Peepaw, Nana and Grandpop – or as my kids call my parents, Meme and Papa– there’s no question that grandparents are the greatest. And it’s not just us saying it – throughout the centuries, countless things have changed. There is something to be said for the experience their previous lifestyles offer.

It’s that time of year where we recognize the importance of grandparents in our lives and the need to cherish their wisdom, experience and love. On Grandparents Day, we remember special grandparents with things grandparents have that your don’t.

1. Courting: Until well into the 1920s, modern dating didn’t really exist. A gentleman would court a young lady by asking her or her parents for permission to call on the family.

2. Hunting, Fishing, and Foraging: Even city dwellers in your great-grandparents’ generation had experience hunting, fishing, and foraging for food.

10. Ironing: Although many of your grandmothers probably ironed just about everything — underwear, pillowcases, handkerchiefs — you’d be hard-pressed to find a 20-something who did so.

Break the Cycles of Pain

P

ain is the body’s natural way of communicating a problem. It’s supposed to resolve as the body heals itself. For most aches or injuries, pain goes away with treatment and time. When it doesn’t, doctors may find themselves at a loss, and patients can get caught in a cycle of treatments or longterm prescriptions.

But real pain relief isn’t measured in doses.

“It’s about listening to what your body is trying to tell you, then addressing the underlying cause,” says Dr. Suleya Cortes of Chi-Rho Family Wellness. “Medications take away the symptoms, but don’t correct the cause. Usually patients come to my office when their medications are no longer working.”

A chiropractor, Dr. Cortes primarily treats people with neck, back and sciatic nerve pain.

“When the spine is out of alignment it can pinch the nerves and cause pain,” she explains. “Everybody experiences pain differently, but after only one adjustment some of my patients say, ‘Wow, I feel great.’

Others may take several months to feel improvement.”

Pain Management is a branch of medicine that uses an interdisciplinary approach to ease suffering and improve the quality of life of those living with chronic pain. The typical pain management team may include medical practitioners, pharmacists, mental health specialists, physiotherapists, occupational therapists and massage therapists.

Pain sometimes stops once the underlying trauma has healed. But long-term pain frequently requires the coordinated efforts of the entire management team.

When a painful injury resists treatment, when pain persists after the injury has healed, or when medical science can’t identify the cause of pain, it’s still important to relieve suffering. Chronic pain is commonly defined as between 3 months and 6 months since onset, though some researchers place the transition from acute to chronic pain at 12 months. Treatment may include pharmacological measures, such as analgesics or antidepressants, interventional procedures, physical therapy, exercise, application of ice and/or heat, and psychological measures such as biofeedback and cognitive behavioral therapy.

“Pain is subjective,” says Dr. Chris Lechmaier of Family Chiropractic Center. “I can’t measure it like I would blood pressure.”

Here’s what to tell your doctor: How Intense It Is: You’ll first be asked to rate your pain, usually on a scale of 0 (no pain) to 10 (the worst you’ve ever had). Otherwise, say that it feels mild, moderate, intense, or the maximum—the worse pain possible.

“More than 50 percent of my patients suffer from chronic pain,” says Dr. Lechmaier. “Over the course of one or two years, they’ve tried many different approaches to help it. We may suggest a spinal adjustment, specific exercises or therapeutic massage. A small percentage may be referred to an orthopedist or a neurologist. We manage their chronic pain symptoms conservatively.”

Dr. Lechmaier frequently sees patients in their eighties. “We use very gentle procedures which are very safe and effective for seniors,” he says. “Sometimes a patient has experienced symptoms for more than 20 years, so the underlying cause cannot be reversed or corrected. But we can manage their pain.”

He also sees men and women who want to end long-term use of medications.

“I had a patient who had been on morphine and we worked to alleviate her symptoms so she didn’t need it any longer.”

According to the National Institutes of Health, studies have shown that opioid analgesic compounds (taken exactly as prescribed) are safe, can manage pain effectively, and rarely cause addiction. Yet the nightly news is filled with reports of abuse among drugs such as OxyContin, oxycodone and morphine.

“Unfortunately, many people use these drugs inappropriately,” explains Dr. Roger Accardi, owner of Accardi Clinical Pharmacy. “We have a protocol in place: If someone brings a prescription for a fairly large quantity of opioid pain medication, they must face an interview process, meet with one of our doctors, answer a questionnaire and listen to a review of the risk/benefit ratio for the use of narcotics.”

In addition, Dr. Accardi does not accept prescriptions from physicians outside of Volusia, Seminole and Orange Counties.

“We want patients to understand the addiction potential, as well as the long-term effects of opioids on the neurological, respiratory, cardio-vascular and psychological systems,” he says. “Certainly, some accidents necessitate the use of narcotics to restore daily functions. Just look at the number of traffic accidents which involve head, neck and spinal injuries or broken legs.

But there are many ways that pain can be addressed.”

In addition to opioid therapy, Dr. Accardi looks for alternative ways “to get to the source of the problem.” He points to physical therapy as well as cognitive behavioral therapy, which helps patients understand the relationship between one’s physiology (pain and muscle tension), thoughts, emotions,

and behaviors. A main goal in psychological therapy is to encourage helpful thought patterns and healthy activities such as regular exercise. Lifestyle changes improve sleep patterns and better coping skills for pain,

include techniques such as deep breathing, meditation and biofeedback.

A study published in the January 2012 issue of the Archives of Internal Medicine found cognitive behavioral therapy is significantly more effective than standard care in treatment of people with body-wide pain, like fibromyalgia.

Another option is acupuncture, which involves the insertion and manipulation of tiny needles into specific points on the body to relieve pain. It’s believed to restore the

energy balance in the body through stimulation of energy channels called the meridians. According to Dr. Debra (Rusty) Gaffney, acupuncture has been proven to reduce pain signals through production of endorphins that are known to be the natural painkillers.

“I see 40-50 people daily and two-thirds of them suffer from chronic pain,” Dr. Gaffney says.

“Headaches, sinus, neck, joint and back pain. Recently, I’ve seen a lot of patients with plantar fasciitis in their feet.

“Acupuncture is usually their last resort,” Dr. Gaffney adds.

“People are afraid because they think we use hypodermic needles. These are so small they hurt a lot less than if you stick your finger with a sewing needle.

Most people fall asleep while we treat them.”

Dr. Gaffney agrees that lifestyle changes are an important part of treatment for chronic pain. According to FamilyDoctor.org, it’s important to include relaxing activities in your daily life, even if you are already taking medicine for pain. Relaxation can change the body’s chemicals that produce pain. However, it’s important to remember the results may not be immediate: You may use stress-reduction methods for several weeks before you notice a decrease in pain.

Along with reducing stress, physical therapy, exercise, prescription medications and alternative medicine (such as chiropractic, acupuncture and cognitive behavioral therapy,) there are many options for managing chronic pain.

The Invisible Epidemic

D

id you know that 1 in 4 older Americans suffer from poor nutrition? I suspect the number is even higher, though 3.7 million malnourished seniors paint the picture of an invisible epidemic of great magnitude. Caring for my sweet mother-inlaw in our home, I began to notice last year the increasing amount of time it was taking to finish her meals. Now, at age 93, it is necessary to demonstrate how to use a spoon to eat her soup and how to pick up her cheese and tomato sandwich which was a go to favorite.

Her all-time favorite breakfast egg dish is met with smiling eyes that ask for help with what to do next. This is a snapshot of how it is possible to become severely malnourished even in the presence of a wholesome meal. The good news for now is that encouragement throughout the meal together with small frequent nutrient dense offerings, whole food in a capsule, snacks and smoothies throughout the day have allowed her to remain well-nourished and in good health. For so many though the

risk for illness and a shortened lifespan is very real.

How can we increase the visibility of this epidemic? What are the signs that contribute to and occur as a result of malnutrition? Here are several clues that present opportunities

for intervention. One of the more obvious signs is significant weight loss however the early clue is loose fitting clothes, especially in the waist area. You may notice a new and smaller belt buckle position. Conversely, being moderately overweight is not necessarily a sign of better health and increased longevity, a commonly held myth. A diet high in calories but low in nutrients will result in weight gain but often with nutritional deficiencies for the vital nutrients needed for a healthy body. Since these high calorie, low fat foods often provide excess sugar and fat there is increased risk for diabetes and heart disease.

Certain medications can cause a loss of appetite. Significant muscle weakness, often noticed getting in and out of a chair or walking for long distances, might be another clue as a certain amount of metabolic energy from food and adequate hydration is required to

Out of date items may stress the immune system or cause a gastrointestinal response that depletes the body of necessary nutrients. Getting sick often, bruising, dry, cracked skin or slow healing wounds will be improved by adequate nutrition. Adequate hydration requires a conscious effort to meet your daily goal as thirst is an unreliable indicator of the need to drink and the body is less able to conserve water. Depression, lack of energy, memory issues and even dementia will be positively impacted by adequate hydration as the brain is approximately 75% water. As little as a 2% drop in hydration can result in confusion, altered thinking and short term memory loss. In addition to liquids, foods such as grapes, tomatoes or melons are also hydrating. What can happen with even 5 glasses of water per day? In addition to all the body processes working more effectively the research tells us that your risk of heart disease, stroke and blood clots will decrease.

This is a side effect worth tapping into!

Oral health problems such as poorly fitting dentures, tooth loss, dry mouth due to medications, dehydration, or gum problems can make eating a painful chore. Oral hygiene can become more difficult as we age and battery operated toothbrushes offer a solution to both ability and technique challenges. Trouble chewing and swallowing are closely related which presents both a choking risk and an overall less pleasurable activity.

A decreased sense of smell or taste can make food taste bland though the addition of spices and herbs will make food more interesting and healthful.

Physical limitations such as difficulty opening cans or packages may limit the foods that can be prepared without assistance. Vision problems will make cooking directions or adjusting the settings on the microwave or oven a challenge. The simple act of standing to prepare meals or carrying heavy pots can make meal preparation difficult or impossible. People who live alone may not be inclined to make meals for one. You may be one of the 30 million Americans that suffer from dry eye disease, and not even know it. Dry eye is a condition in which a person doesn’t have enough quality tears to lubricate and nourish the eye. A healthy tear film is necessary for good vision and overall eye health. Chronic dryness can cause the surface of the eye to become inflamed, which may reduce tear production even more. This results in a continuous cycle of dryness.

There are many causes of dry eye age, environment, medications, medical conditions, a history of eye surgery, and contact lens use. Elderly people frequently experience dry eye, but it can occur at any age. Symptoms can vary from blurriness, burning, scratchiness, redness, and discomfort from wearing contact lenses.

While it may sound strange, people with dry eye often have tearing, because the eye is responding to the underlying irri-

tation. It is diagnosed in an ophthalmologist’s office with a comprehensive exam and various tests.

Depending on the cause of dry eye, your doctor may use various approaches to relieve the symptoms. It is important to know that this is a chronic condition and there is no “quick fix.” The first step is avoiding the environment factors that aggravate the condition such as smoke and fans. A humidifier can help add moisture to dry indoor air. Remembering to take breaks when working at a computer for long periods of time can go a long way.

Artificial tears are available over-thecounter and help to lubricate the eye.

Cyclosporine, lifitegrast, and steroids are all prescription eye drops that help to reduce inflammation. Punctal plugs help to conserve your own tears by blocking the drainage channel.

As we age, we all face the difficulties of dealing with the aches and pains that come along with it. For most of us, we can “get by” treating pain with over the counter anti-inflammatories.

For others, pain can sometimes be debilitating and those patients may require the usage of prescribed medication to manage pain. A goal for every physician is to help the patient be comfortable.

However, pain medication, is intended for short time usage.

Your physician may determine that the only treatment solution remaining for you is surgery.

As an Administrator for a local orthopaedic clinic, I can tell you that one of the most common comments I have heard patients make to our physicians are, “Doc, please help me not hurt!” When they are

told they need surgery, they are sometimes reluctant and think pain medication is an easier route to take. Eventually, once

Low vision has been identified as one of the most significant contributors to the loss of independence among the senior population.

Organic vs. Conventional Farming, What is in Your Food?

I

n last month’s issue we looked at where our food comes from and how it impacts the quality of our food. This month I want to continue the discussion by looking at what goes into our foods or how safe are they? I so often hear the tired old mantra of “There is no difference between organic and conventionally grown produce and then the person will trot out some studies showing how the conventional raised vegetables are just as safe as organically raised ones. The problem with that argument is there are just as many studies showing organic vegetables are safer. This discussion is growing in agricultural and health circles and entails far more than I can cover in this column, so I will cover the general concerns.

More and more the produce and meats we eat are raised on large corporate farms. The focus is on quantity rather than quality. More and more you hear people say how bad meat and vegetables taste. All you need to do is come taste some of our produce and then compare it to any store bought produce to see there is a marked decline in the flavor of fresh produce.

We constantly hear at our farm how sweet the

vegetables are or how bitter the store bought ones are. I am convinced this has a lot with how they are grown.

The large corporate and conventional farms push synthetic chemical sprays and fertilizers at the plants to get them to grow faster and

produce larger and greater quantities of product. They do it under the claim we need to increase production to feed a growing world population, when really the incentive behind it is profit. I am all for profit but not at the expense of a healthy and safe product. What I believe is happening is the chemicals that

That is to say they are not only on the produce but they get into the produce. They cannot be washed off. And when you eat the produce you also get the chemicals. If you doubt me just Google “The dirty dozen.” You will find a list of vegetables and fruits that are the worst for absorbing pesticides and other chemicals. We are told over and over how safe these products are. But how many times have we seen approved chemical sprays later taken off the market because of health effects? (Google discontinues pesticides or go to http://agriculture.ks.gov/docs/ default-source/pest-fert/pest_disc_ rpt.pdf?sfvrsn) On our farm we use only organically certified products for pest and disease control. These by USDA definition must be non-systemic. In other words they don’t get into the product. It can be washed off.

These products also carry the advantage (or disadvantage depending on your view) of having a short life. Each spray has a reentry time assigned to it. This is the time it takes before it is safe to go back into the field. The reentry time on our products run from 0 to 60 minutes. By that time the product has either broken down or is harmless to people. This is compared to conventional sprays where a reentry time may be as long as 4 or 5 days.

Most organic products are made of naturally occurring substances like neem oil, garlic oil, and other extracts or they are simple salts like potassium bicarbonate. When needed we apply them but we do not blanket spray as they often do on a conventional farm.

Folks, this is just some of the generals.

There is so much more I could share but my editor is telling me I am over my word count.

So for now I encourage you to begin to consider what is in your food and remember today is a good day to have a good day.

When our loved ones become seniors, we may worry about all kinds of things regarding their wellbeing. Their health is usually at the top of the list along with safety and emotional and mental stability. Good nutrition is essential for good health. Unfortunately, it is a common problem to see seniors with malnutrition. Many factors can contribute to this problem. Assisted living facilities and nursing homes are usually understaffed and just cannot provide the nutritional requirements of an aging adult. Another factor is that the senior may be on prescription medication that can reduce their appetite and make them sleepy and they just don’t feel like eating. Many seniors are dehydrated as well. It is a known fact that many prescription drugs can reduce or even block the absorption of some vitamins and minerals which can lead to severe deficiencies. Depression is another issue that reduces the appetite make them lethargic and antisocial. It becomes apparent how very important making sure your loved one is receiving all the nutrition they so desperately require to be well.

Some helpful tips include: 1. Make them a healthy nutritional smoothie that contains protein, vitamins, minerals, and other super foods and vegetables.

2. Garden of life makes a wonderful shake base called “Raw Meal” or just a protein mix that you can either use alone with any liquid such as almond milk or juice or you can add healthy things to it. It comes in a powder that can be shaken or blended into a delicious and healthy shake.

3. Make sure they are taking a good food –based multi vitamin and mineral supplement for seniors to fill in anything lacking in their diet.

4. Bring them healthy meals or snacks including fresh fruit bowls, healthy treats like muffins made with whole grains, super seeds like flax, chia, quinoa, coconut and nuts and berries.

5. Always monitor their fluid intake and make sure they are getting enough liquids to avoid dehydration.

Delicious Apple Dumplings

I love desserts that are quick and easy to prepare. This recipe was given to me by a friend several years ago. It is so yummy and perfect for this time of year when Fall apples are ready for picking. Enjoy!

2 Granny Smith Apples 1 8-count package Crescent rolls 1/8 - 1/4 tsp. Cinnamon 1/2 cup butter 3/4 cup sugar 1 cup orange juice 1 tsp. Vanilla 1/2 cup chopped pecans

DIRECTIONS: Preheat oven to 350 degrees. Lightly grease an 8” x 8” baking dish. Peel and core the apples. Cut each into fourths lengthwise. Unroll and separate dough. Wrap each apple slice in crescent roll and place in pan. Sprinkle cinnamon evenly over top.

In a medium sauce pan, combine sugar, butter and orange juice. Stir and bring to a boil. Remove from heat and stir in vanilla. Pour sauce over dumplings. Sprinkle pecans over top. Bake approximately 30 minutes or until golden brown.
