Have you ever done that, and noticed that?

Of course, it’s unwise to leave life’s big decisions to chance, but the exercise is intriguing. What do you do when you know what you really want, but aren’t quite sure it can become a reality?

When it comes to love, most of us think of our family and friends. We want the best for them -- and everyone we love -- but sometimes the decisions can be difficult. It’s no different when we are considering senior living options for our parents or aging loved ones.

That’s why in the February issue of Aging Tree Illustrated we are continuing the three-part series on senior living options by looking at memory care and continuing care retirement communities in detail. What are the options available, and are they the best fit for your loved one?

I counsel hundreds of families a year about the options that exist for assisted living, memory care and other senior living arrangements. Of course, the costs of such living arrangements are a big consideration, but the second biggest stumbling block can be how very uncertain you feel about making the decision in the first place. Is it the right move? Is it the right time? Is this a huge mistake?

I recommend to them – and to you – to take the LOVE approach. Ask these questions: 1) What does my loved one want? 2) What do I want? 3) What is the safest scenario for everyone involved? Time and again, I have found that senior living decisions require us to access a level of love that’s deeper than what’s needed for other lighter, less future-directing decisions most of us make every day. The answers to the three questions above can help guide you and them on the correct path.

Love is the answer. Working together to understand what’s best for your loved ones’ future is a serious endeavor. Loving your family member enough to make hard decisions for them can feel like a burden, but it’s also an opportunity – an opportunity to access love the love between you and use it to make the best decision for everyone involved.

In this issue, we discuss the benefits and costs of memory care and continuing care retirement communities, while contributors add to our learning. Cheree enlightens us with insight on how to make good decisions for memory-affected love ones, and tools available to help us along the way; the always-informative Kim Denoff offers an insight into Heart Health AwarenessMonth; and Scott Selis brings some great advice on working with Medicaid and VA aid and attendance.

It Takes a Village--Continuing Care Retirement Communities

As mentioned in last month’s feature focusing on assisted living, Continuing Care Retirement Communities (CCRCs) are large communities with a neighborhood feel composed of single-family homes, villas, apartments, and other facilities that provide a variety of senior care options in one setting.

According to Aging Tree’s CEO Corey Shenk, the major benefit of a CCRC is the opportunity for residents to advance in treatment and care as their needs grow throughout the aging process. This smooths the transition for a resident who needs to move from one type of senior living facility to another as his care index increases.

Costs for CCRC Living

Considered a premier option, CCRCs can offer peace of mind but also come with higher costs.

Shenk explains that typically, CCRCs are set up as “buy-ins” requiring a down payment, or entrance fee, with dues paid monthly. A contract or agreement between the resident and the CCRC guarantees assistance and care for an extended period of

time.

Entrance fees can range widely, from $35,000 to $1 million, with monthly charges ranging from $3,000 to $5,000 or more. (These costs can increase as needs change.) Fees depend on the type of housing, the option of renting or purchasing, and the type of service contract. Housekeeping, meals, activities, and transportation may involve additional costs.

Different types of contracts may be available. For instance, one type of contract may require a high entrance fee but limit the increase in costs for future care. Conversely, the second type of contract could require a lower entrance fee while the resident would later incur higher fees as their need for care increases.

With this option, prospective residents may be able to negotiate fees and contract provisions. Either way, it is imperative to request a breakdown of costs and what is included, as well as a history of fee increases.

Memory Care--Attending to a Special Need

Memory loss is a common occurrence as we age. It can also be a disconcerting one, both for the person experiencing it and for loved ones.

Dementia, according to the Alzheimer’s Association, is a “general term for loss of memory and other mental abilities severe enough to interfere with daily life.” Alzheimer’s, the most common form of dementia, is a progressive disease that accounts for 60 to 80 percent of dementia cases. People who deal with various types and stages of dementia also require different levels of attention, care, and security as part of their living environment.

While some CCRC communities provide care for all stages of a resident’s memory care, even if it progresses over time, the level of care offered at assisted living and nursing homes varies.

In general, features to be aware of when considering memory care options include the expertise and training of staff, building and grounds security, care offered during the night (as Alzheimer’s and dementia can interfere with sleep patterns), activities that encourage sensory stimulation, the availability of care for those exhibiting disruptive or aggressive behavior, and cost.

Assisted Living

Assisted Living facilities support the autonomy of residents through help with daily tasks such as housekeeping, meals, laundry, transportation, and other services.

This option may be sufficient for those in the early stages of Alzheimer’s who still function independently. Unless designated as an “assisted living memory care”, however, such facilities may not accommodate residents who are considered “exit-seeking,” need a secured environment, or require higher levels of monitoring and medical care. When choosing an assisted living facility, it’s important to understand the level of memory care a resident needs and what each assisted living community provides.

Assisted living memory care facilities combine the amenities of an assisted living environment with the higher levels of monitoring and medical care residents with dementia require. These may include: specially-trained, licensed nurses on-site 24/7, a safe and secure building and outdoor area, cont. pg 10 |8|

an environment that encourages interaction and socialization, and a choice of private or companion apartments.

Secure Memory Care

Also referred to as Alzheimer Special Care Units (SCUs), these facilities offer “secure memory care.” Similar to assisted living memory care, this living option also provides 24-hour nursing, guaranteed secure grounds, staff who have received dementia training, and activities that help residents exercise their skills and interests. Although brand new “Memory Care Only” communities are being built as stand-alones, these units are generally set up as a group or floor within a larger residential facility.

Nursing Home Memory Care

Nursing homes, also called Skilled Nursing Facilities (SNFs), provide for the basic needs of residents, including housekeeping, meals, showering, and using the bathroom.

In addition, many SNFs provide 24-hour medical care from Registered Nurses and on-call physicians and custodial care for residents with late-stage Alzheimer’s or other debilitating conditions. Typically, Skilled Nursing Facility Secured Memory Care units are designated for the end stages of dementia and provide the highest level of

care available. Prior to this “highest level of care” in a Nursing Facility, most individuals afflicted with memory loss will have received memory care either through Assisted Living Memory Care or Home Care first.

Costs for Memory Care Facilities

Costs for senior living vary as widely as the level of care and services needed. Nationwide, the average cost of assisted living in 2016 was approximately $3,600 per month with the average cost of Alzheimer’s assisted living in Florida at about $4,300 per month.

Paying for an Alzheimer’s or memory care facility involves many of the same funding sources we outlined in last month’s issue about assisted living. These include personal resources such as savings accounts, reverse mortgages, life insurance conversions, and loans. Are you a veteran or survivor covered under a VA pension? Then you may be eligible for the Aid and Attendance Benefit. This program works better for those choosing assisted living, while Medicaid is preferred for nursing home care. Be aware that receiving a VA pension may disqualify a veteran for Medicaid or VA disability compensation.

In general, Medicare does not cover the costs of assisted living or other forms of longterm care and services. However, a person with a severe or disabling chronic condition, such as dementia, can join a Medicare Special Needs Plan (SNP) if they have Medicare Part A and Medicare Part B and meet other eligibility requirements. SNPs are approved

by Medicare and run by private companies.

Top 10 Strategies for Caring for Your Parents with Dementia

D

uring the month of February we are reminded that “love is in the air”. This can be bittersweet, as one of the hardest things is watching someone you love forget that they love you. Tia Walker, author of the Inspired Caregiver: Finding Joy While Caring for Those You Love, shares that “Caregiving often calls us to lean into love we didn’t know possible”. I believe this is true. Often families share they are challenged by the ever-present feeling of not doing enough or the right things in the right timing. Here are a few practical ideas that I have found to be helpful in this regard.

Get Organized: Create a caregiving book or update the digital system you have on your phone or computer. This will include copies of a photo identification, insurance and social security cards and a list of emergency contacts. Legal document copies such as a Durable Medical Power of Attorney, General Power of Attorney and Living Will should be included. Medical information to include: a list of doctors, specialty, address and phone number; history, medication dosage, routine and information on each will make life easier.

A brief description of the daily routine which is updated regularly together with any specific instructions will facilitate communication with other caregivers. A contact list of willing neighbors or church friends who are dear to your loved one will be important.

Use Technology: There are a variety of mobile monitoring devices that will track a GPS location, monitor calls and messages.

Camera monitoring systems, some of which are interactive, can offer peace of mind.

Create Less Risk: While it may not be possible to protect your loved one from all injury, it is possible to create a safer environment.

Minimizing fall risk is a priority and removing objects that could harm a loved one, for example knives, car keys and matches can help.

Label and Provide Cues: The need to explain everyday things and routines will be ever present. Using a white board, photos and labeling objects can minimize frustration.

Posting a picture of the night clothes or socks on the drawer where they reside may help your love one locate when needed. Verbal cues may be effective. In anticipation of the inevitable “Who are you? And where am I? I greet our sweet 94-year-old each morning

with a “Good Morning! I am Cheree and this is my home.”

Scheduling: As time goes by you will have more information about the ebb and flow of energy and mood throughout the day and plan trips outside the home accordingly.

Limit Choices: When stress increases as choices are presented you can simply say, “Time to eat, here is your food” or “Put on these clothes, I will help you”. Understanding what is possible to do independently and what requires assistance is a delicate and changing balance. Helping before needed supports loss of independence. Finding that fine line between needing encouragement and causing frustration is the goal.

Redirect and Reassure: Resist the urge to keep your loved one grounded in reality for those circumstances that cause distress and pain. An example of this in our home is the shift of conversation from the serious illness and impending death of a sibling to a general request for prayer for that individual which has a peaceful and calming effect. There are

Denis Waitley writes that a “Smile is the light in your window that tells others there is a caring, sharing person inside”. Apparently, this fills the gap where words fall short.

Discerning Honesty to Reduce Stress: Despite our desire to be meticulously honest there are times when Plan B will work better to decrease stress for your loved one. Many years ago, while cutting the hair of my own mother I moved from the daughter she knew to a hairdresser at a shop. After a deep breath, we had a lovely conversation about my children and then her grandchildren with a promise to return for her next haircut. Several caregivers I know if asked what is happening that day will present a “really fun” activity, such as lunch out and on the way home just happen to stop by the “less fun” doctor’s office.

Remember it’s the Disease not the Person: It is healing to forgive both your loved one and yourself for the full range of emotions that are normal. It is not unusual for there to be moments of perfect sense and more appropriate responses. You are not imagining things, rather you have been given a moment to treasure.

Be Proactive with Your Own Health and Self Care: Remember it can be more difficult to ask for help than accept help when it is offered. Just say yes. You cannot pour from an empty cup.

Be kind to yourself this month and “lean into love”. As Mother Teresa beautifully reminds us, “It’s not how much you do, it is how much love you put in the doing”.

Macular Degeneration

Prevent Vision Loss Age-related macular degeneration (AMD) is the leading cause of blindness in people older than 65 years, and affects more than 11 million Americans in the United States.

Because overall life expectancy has continued to increase, AMD has become a major eye problem.

Macular degeneration is caused when part of the retina deteriorates. The retina is the interior layer of the eye. The macula is the central portion of the retina and is responsible for your central vision.

There are 2 main types: Dry and Wet. Dry AMD is the more common type and makes up 90% of cases. This results in

the gradual loss of central vision. There currently is no cure, but the Age-Related Eye Disease Study (AREDS) found that

supplementation with vitamins that include antioxidants (lutein, zeaxanthin, vitamin C, vitamin E, zinc) decreases the likelihood of progression. Wet AMD makes up the other 10% of cases and involves abnormal vessels that leak blood and fluid in the macula. This causes a rapid decrease or distortion of your cen-

tral vision. It can be slowed with injections of medication into the eye. The dry form can change to the wet form at any point.

Risk factors for developing AMD include a family history, older age, Caucasians, females, UV light exposure, smoking, and a poor diet. The best way to prevent vision loss is to have an annual dilated exam by an ophthalmologist, wearing sunglasses, not smoking, and eating a healthy diet. If you have any of the risk factors, get checked today!

NINE TIPS FOR FIRST TIME RIDERS

1. Plan ahead. Know the route and schedule before leaving the house. If a transfer is necessary, know ahead of time the transfer location and corresponding bus. Route and schedule information are available online at Votran.org and by calling the Customer Service Center. Maps may be picked up at sales outlets, libraries, city halls and county administration buildings.

2. Schedule extra time for each trip. Plan to arrive at the bus stop or designated pick-up location 10 - 15 minutes before the scheduled departure.

3. Stand in the designated boarding location and wave to the bus operator. To signal the operator that a new rider is ready to board, itâ&#x20AC;&#x2122;s important to be on the correct side of the street and stand up or wave when the bus is in view.

4. Bring exact change or purchase a pass in advance. Because bus operators cannot provide change, they require exact fare amounts. Passes can be purchased in advance at Votran.org and in Votran administrative offices, the transfer plaza, the county administration building in DeLand, and at several libraries. 5. Confirm the bus destination before boarding. Check the windshield of the approaching bus to make sure the route matches the desired travel destination.

6. Have the bus fare ready before boarding. When boarding the bus give the exact fare to the operator or slide a pass through the value pass reader.

7. Signal the bus operator to stop within one block of the travel destination. Signal the bus operator by pushing the yellow strip next to the window or by pulling the cord running the length of the bus. Gather belongings before the bus stops and exit promptly when the doors open.

8. Know the time of the next bus. Before leaving home, make note of the route, time, distance and fare required to transfer to another bus. There may be some walking required when making a transfer.

9. Ask for operator assistance when boarding. All fixed-route buses are wheelchair accessible with lifts or ramps to make boarding easier. Each Votran bus operator is specially trained in wheelchair securement and aiding individuals who require additional assistance on a bus.

Most people incorrectly think that they have to be destitute to be eligible for Medicaid. However, by pre-planning, understanding, and following Federal and State law, people of modest means can become

eligible for Medicaid to get help with expenses in their senior years. This is legal and does not involve “hiding” anything from the government. In fact, President Trump, when pressed about taking advantage of Federal Rules to reduce his taxes said that made him “smart.” So I encourage you to do the same thing. Take advantage of federal and state laws that will help make your senior years more affordable.

Take it To Heart February is Heart Health Awareness Month. According to the CDC, heart disease is the leading cause of death for men and women in the United States. The three biggest reasons for this are high blood pressure (stress), high cholesterol (eating fatty foods), and smoking. Basically, we are stressing our hearts due to the lifestyle choices we are making.

How can we greatly improve our heart health outcomes and truly take this information to heart? The CDC recommends that we get some type of physical exercise daily, eat a nutrient rich diet, drink plenty of water, reduce stress and quit smoking. Adopting one or all of these lifestyle

changes could greatly reduce your risk of heart disease.

What are the common signs of a heart attack?

• Chest pain or discomfort. Most heart attacks involve discomfort in

the center or left side of the chest that lasts for more than a few minutes, or that goes away and comes back. The discomfort can feel like uncomfortable pressure, squeezing, fullness, or pain.

• Discomfort in other areas of the upper body. Can include pain or

discomfort in one or both arms, the back, neck, jaw, or stomach.

• Shortness of breath. Often comes along with chest discomfort. But it also can occur before chest discomfort.

• Other symptoms. May include breaking out in a cold sweat, nausea, or light-headedness.

nowing the “Real”story behind a destination: Mount Rushmore Have you ever wondered what the “real story” is behind a destination, piece of art, or monument?

If you are like most, you have probably learned about famous sights in history from

a book, magazine articles, history lesson, or television show. It is a unique opportunity to put that knowledge to the test when you see the sight in person.

One of the most recognizable monuments in the US is Mount Rushmore- nearly three million people each year travel worldwide to marvel at the majestic beauty of the Black Hills of South Dakota and see the National Monument to four of our nation’s most influential leaders. What may surprise you, is that it was not the original idea of artist Gutzon Borglum to create a monument to the president to the badlands of South Dakota… rather, South Dakota historian Doane Robinson was initially looking to bring tourists to the badlands by celebrating the West. They were to commission an artist to sculpt Western heroes like Oglala Lakota leader Red Cloud, explorers Lewis and Clark, and Buffalo Bill Cody into the nearby stone pinnacles known as the Needles. But the dream of making this tourist attraction large enough to draw people from around the globe needed to be larger than the Needles…. It needed to be carved into the side of a mountain. And so, it was.

The carving was originally meant to include the bodies of the presidents down to their waists.

In 1937, a bill was introduced to Congress to add the image of women’s rights leader Susan B. Anthony to the mountain… But then, Congress then passed a bill requiring only the heads that had already been started be completed.

400 men worked on Mount Rushmore, only one of whom was hired an artist, the rest were miners who were good with dynamite. Surprisingly, no man died during this project.

The one artist Borglums did hire Korczak Ziolkowski, to work as an assistant on the mountain. But after 19 days and a heated argument with Borglum, Korczak Ziolkowski left the project. He would later begin another mountain carving nearby, Crazy Horse Memorial, which today is the world’s largest mountain sculpture in progress.

High-temperature Eye-of-Round Roast

By John Sobol

So simple, yet so delicious. This classic recipe is the easiest roast you will ever cook. This recipe takes a very tough piece of meat and makes it tender and delicious. Feel free to improvise with seasonings and use garlic salt instead of regular salt. I added a shallot & a tbsp. of bourbon.

Serves:4-6 Recipe: Eye of Round Roast From the Kitchen of: John Ingredients: 1 Eye of Round Beef Roast (2½ to 4 lbs) Salt & Pepper to taste Other Options: Roast Seasoning 1 shallot 1 tbsp bourbon

DIRECTIONS:

1. Preheat oven to 480° salt and pepper the roast 2. Place roast in hot oven for 7 minutes per pound (3lbs =21 mins) no water no cover.

3. Turn off oven & let sit for 2½ hrs. DO NOT open oven door!

4. Remove from oven & check with thermometer. Rest meat for 10minutes. Slice against the grain thinly. Enjoy!!!

This recipe will roast to a med-rare temperature of 145° if you desire more medium turn oven back on at 200° for another 25 mins. Also in place of salt and pepper you may use your favorite roast seasoning. This dry roasting method produces very little fat or juice for gravy as shrinkage is minimal.
