t Aging Tree, we search for tools to help families keep their loved ones independent. One ongoing issue is the cost of senior care. Home Care and Assisted Living are always a first choice (if one can afford these over a Medicaid funded nursing home) but what are the other options? Can technology play a role, here?

The costs for nursing-home and assisted-living care have grown at rates higher than overall inflation. Fortunately, the cost of in-home care increased at 1 percent or less over the last five years, according to Genworth Financial. In 2013, the median national cost for nursing homes grew to almost $84,000. In addition, almost 90 percent of seniors want to live at home as they age, AARP found.

As the cost of senior care continues to rise, what can folks do to help themselves and afford daily care? We looked at ideas from the business sector and found affordable technology that might just be a good solution.

Hospitals and clinicians are beginning to use a preventative care model. We discovered that these health-care businesses serving seniors offer good lessons for all families and their loved ones. “Outcome based performance standards” are becoming the norm in order to retain their payment rates from Medicare and other insurance companies. In other words, hospitals (and soon physicians) will be paid less if they don’t take a proactive approach to seniors’ care and prevent hospital readmissions or manage critical care patients to meet these standards.

How can they accomplish this? They use new technology that helps monitor and support seniors. Aging Tree found many different systems; however, one that struck our fancy is from a company called AllHealth CHOICE which offers a senior-friendly, touch-screen, home computer system called “CHARLIE.” It is an affordable, easy-to-use, 24-hour monitoring system that engages seniors in their home. It’s quite brilliant and, in many cases, has saved lives. See page 8 for more information on CHARLIE and to understand if this tool can work for you.

This month we also focus on a few local physician practices to help our readers understand additional options in our community. Bringing top-quality physicians to our area can help many of us. We are fortunate to welcome Dr. Leung, an orthopedic surgeon joining Central Florida Bone & Joint Institute to Volusia County (pg 3). We also take an inside look at Prompt Care of Central Florida, Genesis Family Practice and Home Doc’s (pg 14).

CHARLIE also recognizes changes in behavior so the system can alert family members or call 911. “We can link to motion-detectors so if Mom is not in the kitchen by 9 a.m. we can alert someone,” Patterson adds.

Many folks over 65 are trying to manage chronic conditions which is why alert monitoring and care coordination are critically important. “If someone suffers from congestive heart failure, CHARLIE monitors their blood pressure and weight,” Juliette Simpson explains. “The unit may be on the kitchen counter or on a table next to a recliner but it’s linked to the bathroom scale. Spotlight Primary Care Practice Offers Services 7 Days a Week in the Office or in your Home BUSINESS BRANCH

W

hen my dad shared that he wanted to move into his own home I was not surprised, though we had enjoyed sharing our home for that brief period of time until he settled into this new state of Florida. After all, that basic desire for independence is common to so many of us as we endeavor to age gracefully.

According to research by the American Association of Retired Persons (AARP) 90 percent of seniors want to stay in their own homes as they age. Even if they begin to need dayto-day assistance or ongoing health care during retirement, most (82 percent) would prefer to stay in their homes. Only a few express a preference for moving to a facility where care is provided (9 percent) or for moving to a relative’s home (4 percent). Living under one’s own rules is a key reason for staying in one’s own home, with 42 percent of seniors choosing it as one of their top three considerations. The decision to assist a loved one to “age in place” can be a beautiful journey that requires a willingness to be teachable and a determination to meet any challenges that might arise with an open mind. In the beginning there are many questions about what will be required and what resources will be there to help? A frequent area of concern in my experience is the question, how will I be able to get care if I am sick?

My brother, who is an Emergency Room physician, has often commented on the large number of patients, often seniors, who appear in the ER seeking care that is necessary but not typically emergent. Transportation challenges can exist for a number of our seniors. When Dr. Jay Chanmugam, D.O., board certified by the American Board of Emergency Medicine (ABEM), saw this it truly touched his heart and knew that he needed to find a solution. Imagine this…

I had the pleasure of speaking with Mustafa Ali, Administrator of Prompt Care of Central Florida, Genesis Family Practice and Home Doc’s recently and learned how they are meeting the need 7 days a week.

This might seem like a huge vision for our community however Ali shared very simply that “we just need to see the needs of the patient and follow that need”.

With increasing numbers desiring to age in place there is great need for care to meet this demand in the home. Home Doc’s currently provides outreach healthcare to over 300 families in various advanced stages of disease.

He added that being in the home enables you to see the big picture of what is happening beyond the patient and the disease process. Is there food in the fridge, running water, electricity, cleanliness and safety concerns? There is opportunity to engage the appropriate community services to make their patients lives’ easier. Another important goal is to keep people out of the hospital and emergency room. There are no longer any excuses for missing an office visit.

On a practical note, John noted that if they are running late due to spending the necessary time with each patient “what better place to wait than your own living room”. 1. Fixed Route Service – Anyone can ride a Fixed Route bus!

Our fixed route buses are the best value, have the most frequency and stops, and accommodate all riders. This service is available to seniors, persons with disabilities, and Medicare passengers at half price with a valid ID! In addition, the fleet of buses are equipped with wheelchair lifts or ramps, and they kneel at the front door for easy boarding. There is no pre-scheduling, eliminating any guess work in planning your trip. Every bus is safe and clean for every ride! Trip planning assistance and training are available for our Fixed Route systems, both online or by telephone with one of our friendly customer service representatives.

2. Gold Service – Persons who, because of a disability or other limitations, cannot use the standard fixed route bus service may be eligible for Votran’s paratransit Gold Service. This reservation based, curb-to-curb service requires an eligibility application and acceptance. Give Votran’s Customer Service Department a call or go online to votran.org to apply today!

3. NSB FLEX Service – Persons in the New Smyrna Beach area may enjoy FLEX service. FLEX is a reservation based, curbto-curb transportation service within a dedicated service area with limited, timed stops.

Are you Aware of This Valuable Financial Benefit?

Are you a wartime Veteran or a widow of a Veteran? If so, first of all, thank you for your service to our country.

Let me tell you about a veteran’s benefit that you and your family may be unaware of. It is called the Veteran Aid and Attendance Pension. It is an a benefit that will help you pay for homecare, Assisted Living, and/or Nursing Home expenses.

To qualify, you must be: a “wartime veteran or surviving spouse;” must need the assistance of another person to perform daily tasks, such as eating, dressing, undressing, taking care of the needs of nature,

etc.; and meet the meet a complicated income and asset test. Blind individuals, patients in a nursing home for mental or physical incapacity, or residents in an assisted living facility may also qualify.

The benefit amount is between about $1,100 – over $2,000 per month. Those payments are usually the difference between the independence of a veteran or surviving spouse by allowing them to stay at home with care, living in a high quality assisted living facility, or living in a nursing home.

Be proactive and apply for the benefit now, as it can take over a year to complete the process. But the VA will pay the benefit retroactively to the date of application.

If you haven’t yet scheduled an appointment with your Primary Care physician to schedule your flu shot, please do so now. Getting vaccinated prior to the peak of the flu season will give your body a chance to build immunity against the virus. The flu season runs from October to May. The Advisory Committee on Immunization Practice (ACIP) recommends the influenza vaccine to anyone 6 months of age and older. Most adults over the age of 65 have weakened immune systems. The ACIP recommends vaccinations as early as September.

Immunity against the flu can last up to a year after the vaccine has been administered.

There is a misconception that receiving the flu vaccine can “give” you the flu. This is absolutely not the case.

There is the possibility of a reaction to the flu vaccine that can include a low grade fever and aches similar to that of the flu. However, the side effects are much less severe than the symptoms caused by the actual flu virus. According to the Center for Disease Control (CDC), Flu seasons are unpredictable and can be severe. Over a period of 30 years, 3,000 to 49,000 people die from the flu annually. Each year, more than 200,000 people are hospitalized from the flu. Protect yourself and your family from the flu and get vaccinated!

INSIDE ADVISOR Do I Really Need It? When we are young and starting a family, we think about life insurance. We start a job and they offer us life insurance for a low amount out of each paycheck.

The amount chosen is more than we think is needed, but the price is good! Make the choice and then don’t think about it anymore.

Are You Eating Pesticides? Where Does Your Food Come From?

H

ave you ever taken a few minutes to think about where your food comes from? And even a few more minutes to ponder what has been done to it as our food supply becomes more globally outsourced? In this issue I want to talk about local vs. global food supply. Next month we’ll look at organic vs. conventional farming.

There are some that will say what I am writing about is a matter of opinion. But I often find that is the defense of those on the wrong side of the discussion. If you take a walk down the produce isle of your store you will find a great deal the food grown in far off places. Some of that is trough necessity.

For example, we do not have a climate to grow banana’s here in North America, at least not the ones we are accustomed to eating. And some of that is through the consumers demand. We have gotten used to having whatever we want when we want it. So when food is out of season here, we import it from other parts of the world. This poses several concerns you should be aware of.

The first concern is that not all the countries that the food is imported from have the same health concerns that we Americans

have about our food. For example, in many South American countries pesticides are still in use that has been banned in America for decades. More developed countries tend to do a better job monitoring the quality of their produce. In general, due largely to

trade agreements, there is no testing once it is in the U.S. unless it causes a serious health problem. Independent testing by some nutrition groups has found things such as heavy metals and banned pesticides in even organic produce grown outside the country. Mean-

by John Joslin while the State of Florida randomly samples and tests our produce each year for over 800 banned products. Based on that, each of us has to decide for ourselves whether to trust the food coming from outside countries.

The second concern is a nutritional value issue. Once an item of produce is picked the vitamins and nutritional value begin to breakdown. Some vegetables can lose as much as 90% of some vitamins within in just a few days once harvested. Freezing the produce tends to stop the decay, while cooling it only slows it. It is estimated that the average distance produce travels to get to your local store is about 1500 miles. Is it any wonder that often a bag of spinach or other produce is already spoiling on the shelf in the store?

Farmers try to compensate for this by picking the produce before it is fully ripe. This results in things like pale tomatoes that are gassed to turn them red. Unfortunately that means the item has not reached its full flavor or nutritional value. One way to tell if the produce is ripe is by smelling it. If it has no smell it is not ripe.

On our farm we do not pick the produce until it has reached peak ripeness. Because it is not shipped long distances and it gets to your kitchen usually within 24 to 36 hours of harvest our farm members tell us their produce lasts longer and they have less waste.

In addition you get the improved flavor and an enhanced eating experience.

But don’t take my word for it. We offer free tours from October to May, every Saturday at 10am. No reservations needed. After the tour we will give you samples of our produce to take home and try for yourself.

Some helpful tips include: • Create a calendar system to track all doctor and medical appointments and keep in a place that is easily seen and accessible.

• A bulletin board system is a good way to keep track of important phone numbers and emergency contacts written in large print, important phone numbers including neighbors, friends, all doctors phone numbers. On the bulletin board you can keep track of items such a grocery lists.

• Create meal plans for the week and to do lists and post it on the bulletin board.

• Use a pill box with daily weekly or monthly compartments can make taking vitamins easier.

Cheesy Beef Macaroni Goulash

Everyone needs dishes like this to turn to on a busy weekday or when it is too hot outside to turn on the oven. Everything is prepared in one skillet so assembling the ingredients is so easy and the result is well worth the little effort of putting them together. Not to mention clean up because that is pretty much a snap, Enjoy!

1 lb. ground beef 1 onion, finely chopped 1 tsp. salt 1/2 tsp. black pepper 1 tsp. garlic powder 1 can (14 1/2 oz) diced tomatoes with green peppers, celery and onions, undrained 1 1/4 cups elbow macaroni (uncooked) 1 1/2 cup shredded sharp Cheddar cheese

DIRECTIONS:

Brown meat and onion in large skillet, then drain. Add undrained tomatoes, salt, pepper, garlic powder and one cup of water. Bring to a boil. Stir in uncooked macaroni, cover and simmer for 10 to 15 minutes or until macaroni is cooked and liquid is absorbed. Remove from heat and stir in cheese until melted.
