HOW TO USE THE INSURANCE? You have 2 alternatives of use:

Do you need a medical consultation at a clinic or hospital?

Choose any doctor from our network provider list and schedule an appointment.

Inform the doctor you have insurance with Adisa,

3.

The doctor will inform Adisa and request the autrorization letter

4.

Attend the consultation and pay the copayment.

Direct Payment

The providers network are: Hospitals, Medical Centers, Doctors, Diagnostic Centers and Laboratories which the insured can use through the presentation of their insurance card to obtain direct payment and not have to cancel more than their participation.

Call 4101-0050 and choose option 2: "Atención Médica a Domicilio" Request a home consult and indicate that you have insurance with Adisa, provide your full name and ID They will send you a general doctor to your home.

Copayment: $10 per visit.

Médicas visitas is available 24/7

After a medical consultation, you may need medications, laboratory tests or have further medical advice.

If that is your case, you may follow these steps:   



Present the insurance card and ID in the emergency service admission.



At the end of your visit, cancel the participation (if any) before leaving the emergency service.

Take a picture of your prescription and send us via WhatsApp at 7111-1050 Adisa will validate and provide the authorization letter.

With your authorization letter you can buy the medications or do the lab tests and pay only 20% of coinsurance

Important: 

Pharmacy network with direct payment is available in Farmacia Santa Lucia and Fishel.



With Fishel, you can request home delivery of medications, coordinate delivery with the pharmacy by dialing 800-800-4000



Network labs for direct payment: Laboratorios Echandi and Laboratorios San José.



Assistance with a specialist in psychology. Six consultations per year (1 hour each).

Reimbursement 

SPA ASSISTANCE AT HOME 

If you go to a doctor not included in the provider network, you can request a reimbursement of your expenses. Reimbursement, also applies if you buy medication in pharmacies outside the network. Just follow these steps: 

Always carry a reimbursement form for your doctor to complete.



Submit your reimbursement form to Comercial de Seguros, representatives and remember to include the following:

Bills, Medical records, Medical references, Test results, and any other document Adisa requested.

Reimbursement forms are avaialble in our page, through our whatsapp or through the account executive.

Adisa reimburses your claim within 10 business days, after having the complete documentation Remember that for reimbursement, you must discount an annual deductible of $ 100

Relaxing massage with physiotherapy. Three times per year.
