Dear Members, With the end of the financial year fast approaching, so too is membership renewal. Once again, we have experienced a year of dramatic highs and lows, with our branch undergoing significant changes in response. I would encourage all members to take the time to read over the email that will accompany renewal, which tells this story in far more detail.

As I mentioned in my last President’s Report, more than half of our members have opted in for a digital-only renewal this year, which is great to see. A reminder that renewal emails are being sent out via a new system so keep an eye out for an ADAWA membership renewal email. If you did not opt-in to receive a digital renewal, please note that you will receive your documents both in hard copy and digitally.

You would have received communication recently about changes to professional indemnity insurance categories. Changes in the global insurance landscape have led to increased risk stratification and a request was made to move all members placing dental implants to our highest surgical category (YDX). We did not accept this change and through the strength of managing our group policy, we have created a new category (YD4I).

If you surgically place dental implants and cannot find the original email, please contact the branch. This category is significantly less expensive than the top-level surgical category, and we are very pleased to have been able to achieve this for our members.

In this edition of the Western Articulator, you will meet the staff working hard behind the scenes at ADAWA, and we detail some of the benefits of your ADAWA membership. When attempting to explain our branch to the uninformed, I often say that “it is the place you can find the answer to anything in your dental career”. Support underpins all the services we offer and will continue to expand. Take the time to read through this edition as there might be services you never knew existed.

We were saddened to hear about the loss of friends and colleagues in recent months, including Dr Marcelle Moody, Dr Ian Grose, Dr John Mathieson and Dr Julia Kovesi. ADAWA sends deepest sympathies to their loved ones and friends.

Finally, I would like to thank our members, our executive team and council for their support during the latest Perth lockdown. ADAWA formally opposed the Level 3 restrictions placed on dental practice, and although we received a disappointing response from the Department of Health at the time, I have no doubt that our advocacy was a major factor in avoiding an extended two-week closure. Our efforts have not stopped since then and actions are in play to minimise interruptions should this situation arise again.

The local team

to dentistry ADAWA’s latest member benefit is one that’s tailored specifically to the Australian dental industry. A digital stock management and ordering system, Invedent is a Perth-based business with an inspiring back story Luca Tizzano and Bailey Moore are not your typical Gen-Zers. At 23- and 24-years old, respectively, the duo have launched a WA-born-and-bred start-up that’s set to improve the way you run your dental practice.

For Luca, the dental industry is in his blood, so at an age where most of his peers are only starting their careers, he had already amassed a wealth of experience and understanding of how dental practices run. Luca’s dad is a dental prosthetist who runs five practices, and from an early age, Luca and his three brothers were encouraged to be a part of the family business. At 17, Luca was working as a dental technician, and it was during his time in multiple practice locations that he became aware of the challenges and inefficiencies in managing stock.

“I was essentially running my own lab across a couple of locations, and from there, I got more responsibility with managing the stock and ordering – and I realised we were running into problems on a weekly basis,” he says. “We had no visibility of what was in stock at any one time, meaning the stock would run out unexpectedly and we would either be calling the suppliers to find out the stock was on backorder, or we would be spending a lot of money on freight to get it the next day. Having these problems, like so many other dental practices, was costing the business money and I wanted to solve the problem.”

Invedent removes a task that employees dread doing. They’re sinking hours into such a tedious and manual-intensive task.

Meanwhile, Bailey, who has a background in IT, experience in education, and a degree in computer science, was looking for a new challenge. “I was working with UWA’s marine department and built a system that catalogued their marine samples that were taken around the world,” Bailey says. “It was previously a process that was very manual, paper-based, very time-consuming and something they dreaded doing. After finishing that, I was thirsting for another project, which is where Luca and my paths crossed.”

With Luca’s practice knowledge and Bailey’s tech-savviness, they realised that Luca’s pie-in-the-sky idea of a software solution to manage dental practice stock was a great business idea. “We went out to practices from North Perth to Maddington, and when we would go into the stockrooms they would just be a mess,” Bailey says. “Nothing was organised. Items were missing, and every practice tracked and ordered stock differently. It was at that point we knew the business idea could make a real difference.”

Inspired to jump into the process, Luca quit his job working for his dad. “It was a difficult time because my dad had always envisioned his four sons as part of the family business,” Luca recalls. “But I couldn’t stop thinking about this idea. My dad has run his own business for 40 years, my Mum is a dietitian and has her own dietetic practice, so it was inevitable that I would also want to start my own business.”

Together, Luca and Bailey created a digital stock management and ordering system specifically for dental practices. They joined with brothers Grant and Steven Merriel as advisors, as well as a small team of dental nurses to help onboard practices, and Invedent was born.

What is Invedent?

“Invedent is a user-friendly software application that takes the headache out of managing and ordering your stock,” Luca explains. “On a daily basis, dental practices run out of stock before they know they need to buy more – they have no visibility or tracking – and stock ordering is a time-consuming process.

"With Invedent, you can scan your products in and out; the software tells you exactly what is running low, and you can just press a button, generate your order, and send it directly to your supplier. It completely simplifies and streamlines the process – and that is just the beginning."

The dental practices currently using Invedent have saved, on average, six hours a week on stock management.

“Some practices have really bad processes,” Luca explains.

“They’re ordering three or four times a week, depending on the size of the practice. We really streamline that by getting them to order once a month or once a fortnight, depending on the size of their stock room.”

The software takes very little time to set up, with Luca saying a practice can be set up in a day because it’s so simple.

Invedent also has a very thorough support system in place.

“We put practices through our onboarding process, where we hold their hand through every step from configuration, data upload and ongoing use” he says. “Practices are also provided with physical wall posters, online step-by-step guides, live chat access to our team and more. An entire practice can be trained on the application in just a 15-minute call.”

Luca adds the system is not just built for your general dental practice. “It’s built for every type of dental practice you can think of,” he says. “We’ve got practices with one dental chair, one dentist and one receptionist – to practices with 16 locations under one company.”

Because we’re on the ground and in the practices on a daily basis, the feedback that we get is directly acted on.

Feedback The feedback the Invedent team has received has been overwhelmingly positive.

“It’s like going back to the days when we didn’t have our practice manager software, and a receptionist would write down appointments in a scrapbook,” Luca says. “Then your practice manager software came out – your Dental for Windows and your Praktikas – and you look back and cannot imagine a life without those systems; Invedent is the exact same but for stock management.”

The feedback the team receives is also taken directly into consideration and used to continually improve the software. “Because we’re on the ground and in the practices on a daily basis, the feedback that we get is directly acted on,” Luca explains. “We have a huge amount of buy-in from practice staff because in one week they say: ‘I would really like to see this

in the app,’ and the next week, they’ve got it. It’s extremely satisfying and motivational. It gets us out of bed in the morning knowing that we can go into a practice and really understand their problems and solve them quickly.”

With the feedback of participating practices, Bailey says they’re constantly improving the software, with many updates in the pipeline; the most recent improvement. being the ability for a practice to scan the barcode of a product, the application reading the expiration date and displaying it with real-time alerts in the applications.

“The more people use the software, the smarter it gets, as it is learning how to predict the volume of stock you’re going to be using over a period of time,” he adds. “As practices use the software, it will continue to improve and adapt to the practices type and volume of stock.”

Partnership with ADAWA Luca and Bailey are excited that Invedent has been added as the latest member benefit for ADAWA members.

“We are a WA born-and-bred business. Everything is based here,” Bailey says. “We are very keen on it being a WA-based solution – solving the issues for WA dental practices first.”

“We have a really good team streamlining and automating the stock tracking and ordering process as much as possible,” Luca adds. “Our drive is to save WA dental practices as much time as possible, which will in turn save them a significant amount of money as well.

“The feedback we are getting from a lot of dental practices is that they’re excited about being part of the journey,” he says. “Many industry professionals we speak to constantly say: ‘I cannot believe this did not already exist for Australian dental practices’ or ‘This is exactly what I have been looking for.' It’s quite humbling to hear that from industry leaders because we have spent so much time getting this going, and to see the results from practices is very exciting for us as a business.”

You’ll know them by their phone voices and email messages, but can you put a face to the names at ADA House? Now’s your chance: we’re introducing the member-facing staff of ADAWA

Dr David Hallett ADAWA CEO After over 30 years chairside as a dentist, Dr David Hallett became ADAWA CEO seven years ago.

David was no stranger to the workings of ADAWA when he first accepted the CEO role, as a past ADAWA councillor and president (2012/2013). As the CEO, David says his role is to implement policy and decisions of the president and council and to manage the day-to-day business of the office – all of which should predominantly be for the sole benefit of ADAWA members – which is clearly apparent if you come to ADA House and notice the comings and goings of the numerous members that he makes time to see, in between the phone calls that come throughout the day and meetings attended.

“I am part of a team who are committed to looking after our members in any way we can,” he explains. “I am here as an always-confidential confidant to help them as best I can with their day-to-day professional and personal issues.

“There is a lot of behind-the-scenes effort that goes on,” he adds. “Although an individual member might not deal with us for many years, they may suddenly get a call from AHPRA or a health fund or disaffected employee or be going through professional or personal issues; when they really need us, and we will be here.”

When asked about his favourite part of the role, he says it is the interaction with members. “The best part of dentistry is generally the interaction with your patients, and the interaction as CEO with the members is exactly the same,” he explains.

Executive Assistant

Our administrator Sue Hurley knows more about the ADAWA workings and history than most. After all, she’s worked for ADAWA for an impressive 26 years.

If you’ve ever called ADA House, you’ve likely spoken to our Executive Assistant, Jenny Catto. Jenny has worked at ADAWA for seven years and is a member’s first point of call when they contact the association.

Although her first office was in the stationery cupboard (true story!), Sue has stayed with the association for 13 presidents, four CEOs and four insurance companies.

“My job over the years has evolved from memberships and accounts,” Sue recalls. “A few years ago, the auditors said I needed some assistance because I used to do it all by myself and work weekends, so Andrea Andrys came on-board to take over most of the membership work. That left me free to deal with things behind the scenes, to ensure we have all the systems we need in place.

“The accounts have become more and more involved over the years,” Sue adds. “I also take care of the end-of-year financials and budgets, making sure everything is coded. I’m also there for my CEO.” Sue says ADAWA staff help members with everything they possibly can. “If Andrea is not dealing with a change of category, then I jump in,” she says. “We have a very hands-on approach to members, and we are always here for them. Renewal is a huge part of the job to make sure that everyone has the correct information.”

Sue’s reason for her long tenure at ADAWA is simple: the members keep her here. “We are a member-based association, and we are here for our members.

“No matter what your question, ring us and we will help; if we can’t, we’ll point them in the right direction.

We want our members to feel like part of a family, and we are all working for the good of everyone.”

Jenny answers phone queries from members and provides information to help with those queries.

She sends out pamphlets and medical questionnaire pads and assists with booking members into CPD courses, setting up Zoom meetings and Council meetings, organising bookings for the lecture theatre, arranging (always delicious) catering for events, and placing any member advertisements for the classifieds and job board on the ADAWA website.

Interestingly, the most common queries Jenny receives are about item codes (see – you weren’t the only person needing help with these!).

“We have a stack of information to give to members now, and David (Hallett) gets me to research different things for him, which I really like doing.”

An experienced journalist, Brooke has an uncanny knack of always finding the story. If you’ve ever been captivated by a story in the Western Articulator, chances are that Brooke wrote it.

Lisa, meanwhile, oversees all the communications going out of the association – via email, social media and the Western Articulator. She makes sure members are kept up-to-date with current news and issues, plus the inner workings of ADAWA.

Andrea Andrys Member Advisory Services Andrea Andrys has worked at ADAWA for six years, ensuring that all the Is are dotted and Ts crossed.

“It’s a multi-layered role,” Andrea says. “We are talking about a person’s initial onboarding as a student, welcoming them to ADAWA as a free student member, and then maintaining that to roll over to be indemnified when they are about to graduate, which rolls over when they are a graduate to being a working member.

“We support members by making sure they have access to the website, ensuring they receive the Western Articulator if they choose to, and emails from us to keep them up-to-date.”

Transferring members in and out of the State and changing members’ categories also forms a large part of Andrea’s workload. “We have members going between categories all the time so that is a constant and it never stops,” she explains. “It is very important that a member makes sure their circumstances are up-to-date so they are in the correct category for their circumstances and the hours they are working. Getting a phone call saying: ‘I started back at work last week but I’m still on maternity leave’ is a heart-in-the-mouth moment, so please call us before you go back to work.”

Andrea also assists ADAWA members’ practices going through practice accreditation because of her background as a dental nurse.

“I have all the resources to assist with infection control, so if they have questions that they don’t want to send in an email to the Infection Control Committee at the Federal level, they can call or email me,” she adds.

Andrea Paterson CPD Assistant and Australian Dental Health Foundation WA State Coordinator Andrea Paterson (yes, there are two Andreas at ADA House) started working at ADAWA nine years ago, as the CPD assistant. “I manage the course registrations and the administration,” she explains. “I liaise with members regarding WA Dental CPD courses enquiries and information, arrange the preparation of course handouts and anything that Dr Jenny Ball, the conveyor of WA Dental CPD, needs me to assist with.

“Sometimes I set up in-house if we are hosting a course in the lecture theatre or the seminar room; getting the chance to meet attendees is a beneficial side of the role, to be able to engage in-person with members.”

If someone hosts a Dental Rescue Day or adopts a patient, their out-of-pocket expenses are minimal, but the reward is immense.

In addition to her role as CPD Assistant, Andrea took on an additional role as the WA State Coordinator for the Australian Dental Health Foundation back in 2017, assisted by Volunteerism Coordinator Sandra Wood, who works in the office one day a week. Andrea says she loves the role.

“My role is to deliver pro-bono dental treatment and support to the disadvantaged in the community, via dental practices and dental teams,” she explains.

“I also help coordinate the dental service at Homeless Connect events, in collaboration with Dental Health Services. These events rely on a large number of volunteers working as a team, within the limits of the DHS dental vans. Salvation Army Rockingham host one event during Homelessness Week in August, which coincides with Dental Health Week. The second event is held at Russell Square in Northbridge in November. Both events are received extremely well by the local community, and the volunteers tend to love the experience.” Andrea is also involved in the recruitment of dentists for St Pat’s to ensure the clinic is running smoothy.

Andrea says the opportunities to volunteer are of great benefit for ADAWA members, who value the chance to give back. “Our dentists find volunteering so inspiring and fulfilling that they often become regular volunteers,” she says. “It is an empowering feeling when you can impact a life by just one dental visit.”

“I have been very fortunate to have met so many very generous and inspiring dental professionals. It’s like a community all coming together to give back.”

For members wanting to volunteer, she says she can take a lot of the organisation off their hands by providing appointments and the patient – and sponsors provide lab support. “If someone hosts a Dental Rescue Day or adopts a patient, their outof-pocket expenses are minimal, but the reward is immense,” she says. adawa.com.au

In the lead-up to her Stretched not Stressed course in July, Brooke Evans-Butler spoke to Clinical Psychologist, Alex Hof

Alex Hof has been a clinical psychologist for 26 years, but says unlike some of her colleagues, she has not always been in the profession.

Alex is no stranger to ADAWA – last year, during the COVID-19 lockdown, she was one of the presenters of a Capital Partner’s webinar for

My experience has taught me that people who are in highachieving professions do have a lot of stress on them, because of the demands and expectations of their clients/patients.

“I worked in State and Commonwealth Government and then in private enterprise before I ended up becoming a clinical psych,” Alex reveals.

“A lot of my clients come to me with issues about workplace challenges and I think I have a good sense of what it is like to be in different workplaces and different roles.

“I also ran an Employee Assistance Program for a large oil and gas company for about 15 years, which involved visiting people at various sites and providing counselling for their families. It was very interesting and helped me gain insight about the various issues that people have to manage”.

When asked why she decided to become a clinical psychologist, Alex says she was interested in what makes people tick and how to assist others to improve the quality of their lives.

Alex will be presenting Stretched Not Stressed in July – exploring what we mean by good mental health and how we can nurture it in our day-to-day lives. The interactive workshop will cover: • An overview of psychological stress, how it develops, and how it can affect our health and wellbeing.

• How to recognise the signs of psychological distress, both in ourselves and others.

• An exploration of the skills required to maintain good mental health. This will include practical day-to-day strategies that workshop participants can immediately begin to use.

• Discussion of current issues: what is adding to the stress of individuals, what is minimising stress for individuals, and how they can improve the situation.

Alex has a strong interest in preventative mental-health interventions and says she will go through what research tells us improves resilience in people’s lives.

“We are going to look at practical things and at the end of the program I hope attendees are going to walk away with a sense of, ‘Ahh, now I’m going to shift my direction in this way.’ That they can use strategies immediately that can improve the quality of their life,” she says.

“I also suspect that dentistry is a high achiever’s profession,” she adds. “My experience has taught me that people who are in high-achieving professions do have a lot of stress on them, because of the demands and expectations of their clients/patients. However, dentists can also be their own worst enemies because they put a lot of pressure on themselves. We need to talk about how we manage our expectations and the expectations of others and not be so harsh on ourselves.”

It is clear Alex loves what she does, and she says she enjoys the interaction with her clients very much.

“I enjoy problem-solving with my clients – that notion of what we can do to tweak things to get things back on track, without turning their life upside down is very rewarding.”

ABOUT THE PRESENTERS

Extracting teeth is part of everyday clinical practice.

We know that once a tooth has been removed the inevitable consequence is dimensional bone and soft tissue alterations. One treatment option, which has scientifically shown to maintain ridge volumes once teeth have been removed, is socket and ridge preservation. Socket and ridge preservation are some of the easiest methods of hard-tissue preservation and augmentation in future implant sites. If the science behind extraction socket healing is applied to our daily work, patients and dentists will benefit. This handson course will cover different techniques for hard tissue augmentation to preserve ridges and instruct attendees on some suturing specific techniques.

Pig heads will be used for the hands-on component.

Dr Leticia Algarves Miranda is the Discipline Lead in Periodontics at UWA Dental School. She obtained her dental degrees in Brazil and PhDs in Brazil and Karolinska Institute in Sweden. Leticia has held appointments at a number of universities in Brazil.

Workshop activities

The issue of mental health has become a main topic of interest in Australia over the last decade. The increasing complexity and pace of modern life means that balancing the day-to-day demands of work, home and family can be very challenging. The international pandemic has further stretched our resources. This workshop will explore what we mean by good mental health and how we can nurture it in our day to day lives.

She has a strong interest in preventative mental health interventions.

Avoid financial fraud in the dental practice

The rise of digital payments technology has sped up the process of paying and getting paid for businesses across the country – and it has also boosted the number of ways they can get fleeced.

Australia’s critical infrastructure sectors, including healthcare, represent over 35% of the incidents reported to the ACSC (Australia’s cybercrime security centre).

One in two businesses say they have been the victim of financial crime or fraud in the past, according to a PwC survey of more than 7,000 organisations across the globe.

The perpetrators of these crimes range from suppliers’ staff to foreign cybercrime syndicates who develop sophisticated schemes to redirect large business payments. Since COVID-19 has pushed organisations to work remote, internal controls have weakened. Consequently, cybercrime has increased their fraud attempt activity targeting Australian businesses.

However, as threats in the digital payments space continue to emerge, many organisations haven’t caught up, believes co-founder of eftsure, Ian Mirels.

“Internal controls in business haven’t changed that much in the past 20 years,” he says.

In the news Close to home, a Perth 102-year-old grandmother recently lost $375,000 after money for her aged-care bond was stolen via an email hack.

The woman’s granddaughter, who is her enduring power of attorney, was emailing an aged-care home about her grandmother’s care and asked for the aged care home’s bank details so she could transfer the bond money.

Scammers intercepted the email and gave details of a bank account in Sydney, which the granddaughter then transferred the money to. Perhaps if the aged care home had a system such as eftsure in place, this could have been avoided.

The company’s “Know Your Payee” technology sits over a company’s banking platform. When a business inputs payment information, the platform draws on a range of data sources to verify that payee and raises an alarm if it looks like the payment is going to the wrong person. eftsure launched to address the problem that Australian banks don’t take the full data picture, including the name of a payee, when processing transactions. The approach fights the rise of “business email compromises”, where scammers infiltrate emails or invoices and change BSB and account numbers so that funds are misdirected.

“We use a multitude of verification techniques,” Mirels says. eftsure aims to stop incorrect payments by warning businesses the minute they enter payment details.

“We’re focused on the unique way we’re verifying info, at the point of payment - so before the payment gets released,” Mirels says.

It’s not just malicious hackers that can hurt a business – suppliers’ staff can also pose a threat.

External employees are uncontrollable by your organisation, yet many schemes, either willing or unwilling, are started by suppliers’ staff.

Lately, eftsure has noticed a shift in cybercrime activity, with attacks becoming more sophisticated.

A few months ago, eftsure was able to prevent a $1 million dollar fraud attempt for one of their customers. “The company fell victim to a business email compromise scam. Luckily, several alarm bells went off and the company was able to prevent the fraudulent payment and quickly recover the compromised email account.”

Heavy is the head: case study of a crown Kristin Trafford-Wiezel, Case Manager at Dental Protection, takes us through a case study based on real events, which highlights the importance of gaining consent and maintaining open communication pathways when addressing complaints.

Mrs K was an oral health therapist working in a private practice. Six-year-old Miss A presented with her mother for a check-up due to her concerns about a possible hole on a back tooth. Examination and bite wing radiographs revealed that Miss A had a large carious lesion on her 75. Miss A’s mother was very keen to try to keep the tooth until natural exfoliation and, after extensive discussion of the options, it was decided to undertake pulpotomy and restoration with a stainless-steel crown. To ease Miss A into the treatment, a clean and polish was completed that day, and the patient was reappointed to complete the treatment on the 75.

Miss A returned in the school holidays, accompanied by her father, to complete treatment. The appointment proceeded with no issues and a great final result. Miss A seemed excited about returning to school after the holidays to show all her friends her new “princess crown” tooth.

Mrs K completed her morning of patients and, while writing the records for her final patient, she was interrupted by the practice manager,

who informed her that Miss A’s mother was on the phone, wanting to discuss Miss A’s treatment that morning. Thinking this call was likely to check on how treatment had proceeded, Mrs K happily took the call, ready to report how well things had gone.

Unfortunately, Miss A’s mother was not calling for an update. She was calling as she was quite distressed about the appearance of the stainless-steel crown in her daughter’s mouth and to complain that she had not been informed prior to treatment about the lack of aesthetics of the definitive restoration.

Mrs K was somewhat surprised by these statements due to the extensive discussions they had had prior to the appointment as part of the consent process and the mother’s insistence on wanting the best option for the long-term maintenance of Miss A’s tooth. The mother maintained that she did not realise that a stainless-steel crown would look black in her child’s mouth and would never have proceeded had she been aware.

Mrs K felt quite unprepared to respond and so invited Miss A’s mother to come to the clinic for a discussion about the situation, to give them both time to gather their thoughts. She wanted to respond in the best way to ensure a positive outcome for everyone. Mrs K called Dental Protection to discuss what had happened and to get guidance on the best way to proceed.

Miss A’s mother arrived at lunchtime and was invited into the practice manager’s office to openly discuss the situation. Mrs K began by acknowledging that there had been a miscommunication and apologised for the situation they now found themselves in.

The patient’s mother had a chance to collect herself on the way to the clinic and they were able to calmly talk through the options again and the best course of action to ensure a functional, pain-free tooth for Miss A until her tooth exfoliated.

Learning points 1. It is critical to have a clear and thorough consent process, where even things that seem obvious to you are discussed.

2. Consider the use of written information pamphlets to complement the consent process.

3. This case underlines the importance of inviting an open communication pathway to deal with the resolution of complaints.

After processing the information, the mother agreed that she wanted the best treatment for her daughter and though not optimal aesthetically, she accepted a stainless-steel crown was the best long-term option.

The meeting was concluded with Miss A’s mother feeling “heard” and reassured that her daughter was receiving the best treatment for her overall health and wellbeing.

A 78-year-old male was referred with regards to a mass in the left upper lip. The lesion had been present for a few weeks, and whilst it was not painful the lip appeared swollen and asymmetrical. His medical history was essentially non-contributory. There were no known allergies. He currently smoked eight-cigarettes per day, with an estimated 75-pack-year history. He did not consume alcohol on a regular basis.

Extra-oral examination was unremarkable and there was no evidence of regional lymphadenopathy in head and neck. Oral aperture and excursive jaw movements appeared to be normal. Intra-oral examination demonstrated a 30 x 35 x 22mm firm, fixed submucosal mass involving the left maxillary labial mucosa. There was no ulceration, and the lesion felt quite well-defined. Remaining oral soft tissues were unremarkable.

Ultrasound of the lesion demonstrated a 23 x 16 x 20mm heterogeneously hypoechoic area within the subcutaneous tissue of the left upper labial mucosa, raising the possibility of an inflammatory process possibly due to a dental abscess or nasolabial cyst. A CT was performed to further evaluate the lesion, and demonstrated

a poorly defined and heterogeneously enhancing lesion measuring 32 x 17 x 12mm. There were no periapical inflammatory lesions within the maxilla, or evidence of a nasolabial cyst to account for the lesion.

Based on the clinical and radiologic evidence, the mass was thought to be either a minor salivary gland neoplasm or lymphoproliferative neoplasm. Incisional biopsy was performed under local anaesthesia. On incision, the lesion was noted to be necrotic with suppuration present.

Histopathology was reported as a mucous plug undergoing calcification partially surrounded by an intense acute inflammatory reaction and a soft tissue abscess. There was no sign of malignancy.

Discussion Sialolithiasis is a common salivary gland disease, usually developing in the major salivary glands. The most common site is the submandibular glands (80-92%), followed by the parotid glands (16-19%), and minor salivary glands are rarely affected (2%). Sialolithiasis of the minor salivary glands most commonly occurs in the buccal mucosa and upper lip.

As in this case, sialioliths of the minor salivary glands are rarely identified on imaging such as ultrasound or computed tomography due to their small size. These are often misdiagnosed clinically due to rarity and variable presentation.

The differential diagnoses for submucosal lesions in the upper lip including salivary gland tumours, mucocoeles, non-specific sialadenitis, fibroma, foreign body, vascular malformation and malignant tumours. Surgical excision is recommended to confirm the diagnosis, and is usually curative. In this case, at two-week postoperative review, the lip swelling had completely resolved and the biopsy site had healed well.

Lumps are a common manifestation of pathology involving the palatal tissues.

These range from salivary gland tumours such as pleomorphic adenoma, basal cell adenoma, mucoepidermoid carcinoma or adenoid cystic carcinoma to locally aggressive vascular neoplasms such as Kaposi sarcoma, and haematolymphoid malignancies of either lymphoid, plasma cell, histiocytic/dendritic or myeloid origin.

Clinical differential diagnosis is complicated by overlapping clinical presentations which include raised soft tissue lumps, often taking on a blue/purple appearance, with variability in symptoms and clinical onset.

A lesser-known lesion with a similar clinical presentation previously described as a “sheep in wolf’s clothing” is adenomatoid hyperplasia. Adenomatoid hyperplasia of the oral minor salivary glands is an uncommon lesion which is mostly encountered in the palate (Fig 1), although cases have also been reported in other oral sites including the buccal mucosa, retromolar pad and lip. Its importance lies in its clinical resemblance of more sinister pathology, often mimicking salivary gland tumours. About 50 cases of adenomatoid hyperplasia have been reported in the literature, but it is likely that more cases exist, but have been misdiagnosed as salivary gland lesions such as monomorphic or pleomorphic adenomas, or salivary gland malignancies.

Adenomatoid hyperplasia is composed of hyperplastic glandular tissue with acinar morphology (Fig 2), with acini appearing hypertrophic and filled with excessive mucous, with the nuclei being pushed to the basal aspect of cells (Fig 3). The hypertrophic mucous acini are superficially located, often separated from the overlying epithelium by a thin layer of collagenous connective tissue. Additional staining beyond routine hematoxylin and eosin (Figs 2 & 3), shows abundant positivity with Periodic Acid-Schiff (Fig 4) and Alcian Blue (Fig 5) revealing mucin laden acini, and normal positive staining with CK7 (low molecular weight cytokeratin), CK14 (high molecular weight cytokeratin), and p63 and calponin (myoepithelial cells).

The exact etiology of adenomatoid hyperplasia is not known, but commonly thought to occur as a consequence of trauma to the minor salivary gland tissues in affected areas. Chromosomal aberrations seen in other malignant neoplasms such as leukaemia have been reported in adenomatoid hyperplasia, although at different break point locations.

Management of such lesions includes local excision which limits recurrence, although one case of purported malignant transformation to mucoepidermoid carcinoma has been reported.

Adenomatoid hyperplasia was first reported by Giansanti in 1971, and can affect patients of any age and gender, although the grater majority have been described in Caucasian cohorts. Inclusion of adenomatoid hyperplasia in the clinical differential diagnosis of oral lumps is important to limit invasive and inappropriate surgical management reserved for other more sinister tumours and neoplasms. More importantly, expert histopathological assessment is required to differentiate this local nonneoplastic salivary gland enlargement from other lesions such pleomorphic adenoma, mucoepidermoid carcinoma or adenoid cystic carcinoma.

With research funds becoming increasingly scarce, recent awards administered by The Australian Dental Research Foundation were particularly welcome at UWA. ADAWA proudly contributed $18,000 worth of funding towards the awards.

ADAWA - DOCTOR OF DENTAL MEDICINE (DMD) STUDENT AWARD Ruchira Chakraborty, Alexander Yap, Dr Amr Fawzy $2,973 for the project: 2458-2020 Effect of food simulating liquids on bulk-fill composites: mechanical analysis For Ruchira Chakraborty and research partner, Alexander Yap, as undergraduate dental students, it is an honour to receive such a generous grant.

“Our project involves mechanical testing of composite specimens, which are stored in various solvents,” Alexander explains. “The test specimens are fabricated using custom moulds, which were designed using a simple three-dimensional modelling program. With the assistance of the Department of Mechanical Engineering at UWA, we were able to bring these designs to fruition. Some of our specimens will need to be examined under benchtop scanning electron microscopy (SEM), which will require training at the UWA Centre for Microscopy, Characterisation and Analysis (CMCA).

“The grant has allowed us to accommodate for the extensive work needed to meet the specifications for specimen preparation, the cost of the materials, including composite and solvents, and the additional training required for the operation of benchtop SEM. By providing us access to these resources and specialist training and services, the grant has allowed us to enrich our research experience and meet the strict guidelines for mechanical testing protocols.

As dental students, it is exciting to be given the opportunity to investigate the key properties of dental biomaterials.”

“The results of our study, although limited to a laboratory setting, may provide insight into the properties of materials that are relatively new to the market, such as bulk-fill composites, and compare them to a standard universal restorative material,” Ruchira says. “Given the various commercial brands of composite materials available, we hope our project will allow dentists to make a more informed decision regarding material selection for direct restorative procedures in the clinical setting. We hope through this project we can add to the body of scientific literature available for dental materials and, from a personal point of view, improve our understanding of these materials that we will be using on a day-to-day basis.”

ADAWA - DOCTOR OF DENTAL MEDICINE (DMD) STUDENT AWARD Shashank Gadiraju, Rodwan Halimi, Dr Agnieszka Frydrych, Dr Omar Kujan $1,400 for the project: 2660-2020 Assessing the accuracy of dental team professionals and students in detecting malignant and potentially malignant oral disorders Shashank Gadiraju says their team is greatly appreciative of the funding. “It’s a good feeling to know that our work is considered valuable enough to warrant receiving it,” he adds.

“Ultimately, we hope the data we collect will enable us to review training standards for dental, oral hygiene/ therapy and DCA students with regards to preparing them to detect any suspicious oral lesions.

“We hope that the data will, in the long-term, enable improvement of the entire dental team in detecting these lesions at the earliest possible stage and hopefully lead to better outcomes for patients.”

ADAWA - DOCTOR OF CLINICAL DENTISTRY (DClinDent) STUDENT GRANT Dr Anchalee Marian Jennings-Lowe, A/Prof Leticia Algarves-Miranda, Dr Victor Haruo Matsubara, A/Prof Carlos Miranda da Silva Figueredo $3,895 for the project: NTX-I and TRAP5b as bone destruction biomarkers in individuals with peri-implantitis “Receiving the grant makes me feel like an important member of the dental community,” says Dr Anchalee Marian Jennings-Lowe. “It provides that extra bit of motivation, knowing my topic is worthwhile and my results will be of value to more than just my research team.

“Peri-implant diseases are common biological complications of dental implants, difficult to detect early, and difficult to treat,” she adds. “Current diagnosis is based on clinical and radiographic observations. My project will analyse biomarkers in peri-implant crevicular fluid and saliva. Our aim is to identify biomarkers that may represent disease progression from non-destructive peri-implant mucositis to destructive peri-implantitis. I hope the results will help improve diagnosis and prevention of peri-implant diseases.”

DOCTOR OF CLINICAL DENTISTRY (DClinDent) STUDENT AWARD Dr Zohaib Akram, Sulta Aati, Dr Amr Fawzy $7,158 for the project: Formulation of pH chlorhexidine-loaded/mesoporous silica nanoparticles modified experimental dentin adhesive Principal investigator Dr Zohaib Akram says the successful outcome of the ADRF grant will take the project to the next level – evaluating the potential use of chlorhexidine loaded mesoporous silica nanoparticles in dentin adhesive and evaluating the antimicrobial behaviour at varying pH and various amounts of the drug loaded nanocarriers to combat bacterial infections that causes tooth decay in the teeth.

“I’m really overwhelmed with the successful outcome of ADRF grant, and now I feel more confident that I could continue to contribute to the beautiful field of dentistry and science,” he says.

ADAWA - DOCTOR OF CLINICAL DENTISTRY (DClinDent) STUDENT GRANT Dr Victor Haruo Matsubara, Dr Praveen Praveen, Dr Leticia Algarves Miranda, Dr Murray Baker $13,975 for the project: 2566-2020 Fabrication of antimicrobial light-cured polymeric hydrogels doped with silver nanoparticles and chlorhexidine for management of periodontitis and peri-implantitis Dr Victor Haruo Matsubara is thankful to receive the grant this year, especially knowing that research funds are becoming increasingly difficult to obtain.

“The financial support that I am receiving from the ADRF is extremely important for my progression as a researcher at UWA and will be essential to the development of my project with hydrogels,” he says.

“We are hoping to develop a product that can be clinically efficient in the management of peri-implant and periodontal defects,” he adds. “At this stage, the research project is just starting so that all the laboratory tests will be conducted before we move ahead with animal tests in the future.”

GOOD NEWS STORIES Dental Rescue Day A huge success at another ADHF Dental Rescue Day at the Oral Health Centre of WA, with 32 disadvantaged patients being treated.

Huge thanks go to the dentists who gave up their time, including Drs Vuk Paripovik, Lida Sayadelmi, Parnian Zareie, Judit Szito, Sean Archibald, Emrik Graff, endodontist Shahrzad Nazari and Tracey Gold – as well as Curtin’s OHT students, supervisors and lecturers.

We were here being helped and now I’m the one here helping others.

A chance to give back Taking part in an ADHF Dental Rescue Day at OHCWA is a special experience. There’s something quite heartwarming about seeing so many dentists, students and OHTs giving up their time to help others – and the gratitude of the patients who have been helped.

“When we received the email asking us to volunteer, I thought ‘Oh my God, this was my family as patients seven years ago’ and I started crying. I was so happy,” she says. “We were here being helped and now I’m the one here helping others.”

For second-year Curtin Oral Health Therapy student, Noor Al-asadi, volunteering at the most recent ADHF Dental Rescue Day was extra special – because it was her turn to give back to a cause that once helped her and her family.

Being able to give back was a wonderful experience for Noor, who says she finds studying as an OHT very rewarding. “When I was younger, I was scared of the dentist and I really wanted to make it a good experience for people,” she recalls. “I remember one oral health therapy student made me feel so comfortable and I wanted to do that for someone else.

“When I was younger, we were struggling financially,” she recalls. “There was a volunteer day here at OHCWA through the Red Cross, and they helped us out. My mum had been in so much pain; I remember her on the floor crying and holding her face, but we couldn’t afford to go to the dentist. We went to a rescue day and they extracted her tooth for her.

We were so thankful.”

Noor was amazed when she received an email inviting OHT students to volunteer at an event that seven years prior had helped her family.

“My mum and I clicked to submit the application to get into the OHT together, and when we found out I was accepted we started crying and hugging. It has been a long journey, but we are making it and I am so grateful.

“It is not easy, but if it were easy, everyone else would be doing it. Volunteering at the Rescue Day was so rewarding – it was beautiful. This is really what I want to do. Seeing the patients smiling made my day.”

The next yoga sessions will be held at ADA House on Thursday June 17 (7.30pm) and Tuesday June 29 (7pm).

Yoga sessions at ADA House began under the guidance of Vanessa Smith from Vanessa Smith Yoga last year, but to meet the demand for more sessions, ADAWA member dentist, Dr Unna Childambaram, has also come on-board.

I think many people have the misconception that you have to be super flexible or have an amazing yoga body, but it is good for all body types and shapes.

“Post-COVID I had more time on my hands as I couldn’t travel, so I thought I should try something different, and I have wanted to do yoga training for a while,” Unna says. “I was not sure if I wanted to teach or to do the training for my own yoga journey, but I knew Amanda (Phoon Nguyen) was helping to organise yoga sessions at ADA House and I thought it could be an opportunity for me to give back to the dental community.”

Unna says she originally started practising yoga when she slipped a disc in high school and was told to do core exercises to prevent it happening again. She encourages other dentists to come along and give yoga a try. “It really is beneficial for our profession because we bend, we look down, we are in all sorts of positions,” she explains. “Yoga gives you a little bit of stretch and keeps you moving – including parts of the body you might not usually move much.

The volunteers behind St Pat’s Dr Lisa Standing When we call Dr Lisa Standing, she’s just finished a morning of volunteer work at St Patrick’s Community Centre in Fremantle. “Today was a busy morning,” she says. “To give you an idea, we saw nine patients in five hours and did about 25 extractions.”

“We sometimes have people drop in who may not Lisa is no stranger to volunteering. As a child, she have engaged with St Pat’s for a while and they was encouraged to volunteer as a Girl Guide, and have a toothache, and they stop at the health after finishing university, helped acquired brain clinic and notice the dentist is on. We might injury residents attend appointments. When she have a missed appointment when someone has became a mother, Lisa and her children became not turned up because some of the clients have involved in a nursing home visit program. She has also been on three CHAT trips as a volunteer dentist. difficulty with transport and health issues.

“I hadn’t done anything for a few years in a volunteering capacity, and then we moved to Fremantle a couple of years ago,” Lisa recalls.

“I had read about St Pat’s and Dom Longo [clinical lead at the time] was a mentor of mine when I graduated. I admired his work with St Pat’s and thought: ‘Now that I live in Fremantle, I’m going to go down and see if they need some help.’

Now I volunteer with St Pat’s every fortnight.”

“I saw a fellow today who lives upstairs in the men’s accommodation, and he dropped in because he hadn’t seen us for a couple of months. I sat him down and said: ‘Do you feel like having some teeth out today?’ He said he wasn’t psyched up for it and left. Then five minutes later he knocked back on our door and said, ‘You know what? Go for it.’ We took all his remaining upper teeth out and he was just calling by. You need to be a little bit flexible in nature.”

The St Pat’s dental clinic is set up as well as any private clinic. The chair is brand new, there are digital x-rays, an OPG machine – there is nothing else you could want.

Lisa says she does a spectrum of treatment at St Pat’s – including restorations, extractions, root canal and dentures.

According to Lisa, volunteering at the clinic changes the role of the dentist into a pure healthcare provider, because they do not charge for the service.

“You lose the worry about, ‘Can the patient afford this?’ It’s nice to have that total freedom of care for patients, where you don’t have to think about affordability.”

According to Lisa, the patients they see are very grateful for the treatment they receive.

“A lot of patients ask if they can give you a hug once they finish treatment,” she says. “They really want to say thanks and they always thank all of the staff.”

Volunteering at St Pat’s is very similar to a private practice because the staff are well trained, and the equipment and materials are fantastic. Lisa says there have been many satisfying moments.

“There is a really lovely lady who admitted to me she hadn’t brushed her teeth in about 10 years. She said to me: ‘Don’t you think you are going to make me change my mind and start brushing my teeth because every time I brush them, they get sore.’

her mind as to how difficult some people’s lives are in Perth. “It has made me a better listener,” she says. “I think it has improved my communication skills and it has probably helped me be a bit more flexible myself in private practice. I don’t do many extractions in private practice these days, so it also keeps my exo skills up.”

“I sat down with her and there were lots of tears and lots of anxiety and we got a little bit of cleaning done and I said: ‘There’s no point us doing this if you don’t try to brush them just a little bit to improve the health of your gums.’ After a few weeks she returned, and her gums looked so much better. She said she was brushing them twice a week. When we had the last appointment, she said: ‘Guess what? I’m brushing them on Monday, Wednesday and Friday. But I need a break in between.’

For a dentist considering volunteering with St Pat’s, Lisa says to go for it.

“We must have had a conversation about stress balls and how nice they are to hold on to during an appointment because when she came in for her last visit, she had hand-sewn these three little stress balls for us to give to patients to hold during treatment. It was the most beautiful hand-sewing you had ever seen.

She is smiling and her gums are great and all we did was talk to her and provide a bit of scaling and cleaning.”

When asked if the volunteering experience has made her a better practitioner, Lisa says it has broadened

“It’s such a fantastic organisation to be part of and the dentist is just one part of a big team making a difference,” she says. “The great thing is, they’re really flexible and you don’t have to commit to once a fortnight like I do, or even once a month. You might have a few weeks off work and want to go in for a couple of sessions or you might be changing jobs and want to go in and help. They don’t ask anything of you in terms of commitment, and whatever you can offer they are grateful for.

“I cannot think of any barriers that would put anyone off volunteering,” she adds. “It doesn’t matter what your experience level is, from a new graduate to a retiree, there is something everyone can offer to St Pat’s.

“Whenever I leave after a session at St Pat’s, I leave with such a sense of joy. Dentistry can be stressful, but this isn’t and that’s why I enjoy it so much.”

Dr John Whyte Dr John Whyte has been volunteering at St Patrick’s Community Support Centre since its oral health clinic opened in 2016.

“Dr Russell Gordon heard that I had sold my practice and was working part-time and asked if I would be happy to do a little bit of volunteer work,” John recalls. “I said to let me know when the clinic was ready and that I was happy to do a session a month. I have been volunteering ever since.”

For John, volunteering has been a way to give back. “I have been a dentist for 45 years and I have got a lot of professional satisfaction and financial renumeration from being a dentist, and I wanted to put a little bit back into the community,” he says.

“When I was working full-time at my practice there is no way I had the time to do that so when I was working part-time, I finally had time up my sleeve to volunteer, so it suited me perfectly.”

John says most of the treatment he does at St Pat’s includes simple fillings and extractions – and the patients are very appreciative. He adds it is satisfying to be able to get these patients, who are usually massively disadvantaged, out of pain.

If you have a spare five hours a month, go there because you will get plenty out of helping people.

“Often they come in with huge decay and abscess issues and they put off seeking treatment because they are scared of going to the dentist,” he says. “I had one guy yesterday with a lower wisdom tooth that was abscessed. He was trying to take Nurofen, and that was never going to fix the problem.”

As someone who has been with St Pat’s Oral Health Clinic from the start, John says although there were some teething problems early on, St Pat’s is an enjoyable place to volunteer, especially since the Australian Dental Health Foundation was awarded the Fremantle Foundation 2020 Impact100 grant last year – with $100,000 going towards the Oral Health Clinics.

For dentists considering volunteering at St Pat’s, John says to go with an open mind, expect to be seeing the disadvantaged people of Perth, and enjoy the journey.

St Pat’s is not the only organisation John has given his time to. Along with his colleague and friend, Dr John Moran, he has taken part in several trips to volunteer in East Timor with the Timor-Leste Dental Program. At 67, John will be retiring at the end of June, and encourages dentists approaching retirement to volunteer while they can.

“For dentists like myself, who are nearing the end of their career – if you have a spare five hours a month, go there because you will get plenty out of helping people,” he says. “Most dentists of my vintage have got a lot of out of dentistry in a financial sense, so put a bit back in for the people who cannot afford to go to a general dentist in normal circumstances.”

Remembering Dr Julia Kovesi

BDS 1958 5 February 1925 – 4 April 2021

Photograph and words by Meredith Arcus Julia was one of the earliest female graduates of UWA Dental School. She was born in Bicske, Hungary, in 1925 and was a medical student in Budapest (a prerequisite for dentistry) when she made the decision to leave Hungary due to the unrest in Europe. She fled with her future husband Paul and his family, having to leave her parents and two siblings behind.

Their journey took them to Austria for two years before boarding a ship bound for Fremantle, arriving in July 1950 as refugees.

Northam migrant camp provided a safe haven, and working as medical orderlies at Wooroloo Sanatorium, the opportunity to learn English.

Paul and Julia married in 1951 and they both enrolled in dentistry at UWA in 1952. Paul transferred early to a Bachelor of Arts degree and became a Professor of English Literature.

His work took them to live in both the UK in 1960 and then Melbourne, but Julia always thought of Perth as home.

Julia was a dentist at Cottesloe Dental for 16 years and also worked in the WA school dental service. She was a valued Senior Lecturer in paediatric dentistry in the 1990s and a working mother and bread winner when few women had professional education or roles.

Julia and Paul had two children, Paul and Suzanne. Julia was an active member of the Sally Joyston-Bechal Study Group and a peer of Sally, who visited her in December 2019.

Sally remembers Julia as a very kind and helpful person who was always optimistic, looking on the bright shade of things and the good side of everyone. In her later years she was an active member of the UWA Convocation and Friends of the Library.

When Julia retired, she learnt to swim and became a regular polar bear at Cottesloe Beach. I would often bump into her coming out of the ocean on my morning walk. She also learnt Italian to read Dante and loved classical music. She was a great cook and dedicated grandmother to her three grandchildren and great granddaughter Georgia. Julia’s motto was “to get on with it”. I remember Julia specifically did not like any divisions, including within the dental profession. On reflection, her experiences and choices in her youth would explain this. Julia will be remembered by the dental profession with great respect as a competent practitioner, valued contributor and remarkable role model.

Another glorious afternoon for golf was enjoyed by 25 enthusiastic WADA golfers. The course at Gosnells Golf Club was in pristine condition in preparation for the upcoming WA Senior Amateur Championship.

A light lunch overlooking the course provided an opportunity to contemplate the challenge ahead.

A midday start off two tees ensured the group would enjoy the best of conditions.

The challenging Gosnells layout proved a stern test for most. Not so for Gosnells member and WADA Golf regular Russel Redpath from Dentsply Sirona, who shot a superb 8 under his handicap to romp home and win the day with 44 stableford points.

In Russel’s wake were Kris Thyer in second place, shooting a very respectable 37 stableford points.

Jane McCarthy took third place on 35 points, on a countback from first-time player and sponsor Garry Bishop from Health Practice Brokers.

Another first-time player in Keenan Inderjeeth, controlled his nerves playing alongside his former UWA tutor Frank Welten, to post an unofficial 36 points and take a step towards establishing an official golf handicap. We welcome Keenan and Garry and hope that other interested golfers will take the opportunity to join us for a relaxed afternoon of golf.
