I hope everyone had a relaxing break over the Christmas and New Year shutdown.

This year promises to be an interesting time for all of us. The genie is out of the bottle: border opening or not, we are going to have to adapt in different ways with the spread of COVID-19 in Western Australia. But adapt and overcome we will! At ADAWA we have been collecting resources for all our members, which we are updating on our website regularly and sending out information to our members as and when we receive it, be it via email or social media.

Despite limited official information from the Government in regards to the practise of dentistry in our State, I have noticed that many of our members have been very proactive in comparison to our initial hit with COVID-19 two years ago. The awareness of the considerations within the "Risk Management Principles for Dentistry" document that we sent out and the uptake on the opportunity to perform fit testing for N95-P2 masks shows that our ADAWA community is ready to step up and overcome all the challenges that we are going to face in the future.

My advice to our members and especially practice owners is that every practice is different, hence using this time to prepare your staff protocols and formulate a customised framework for your practice on how to deal with different COVID-19 scenarios based on mild, moderate, and high community spread, would be a good idea.

We don’t have all the answers in this dynamically changing environment, but our ADAWA staff, CEO, Executive, Council, and especially our ADAWA Infection Control Committee have been working tirelessly to collect and filter through all the resources available to forward it to members. We are strengthening our relationship with our AMAWA colleagues too, as together we shall have a louder voice to be heard. Plus, hopefully by working together, we shall be able to source additional avenues for limited supplies that will be critical for our members.

On another note, COVID-19 will not stop the other constructive work performed at ADAWA. At the end of 2021, we welcomed our most recent graduated cohort of UWA dentists for 2021, by holding a Welcome to Profession event. This event, which was started a few years ago by ADAWA, is an opportunity to introduce new dentists to the stalwarts within our profession. This event is covered further in this issue of the Western Articulator.

In addition to all the above efforts at ADAWA, in early 2022 your Executive and Council are going to undergo training with the AICD (Australian Institute for Company Directors) to help ensure that the ADAWA leadership is taken to the next level and strengthened for future challenges.

We have also selected our branch Committee chairs for 2022 and we will be having a Strategy Day very soon to help formulate our action plan for 2022.

Uncertain but exciting times ahead, nevertheless, it is nothing that we can’t conquer together.

I joined ADAWA — or ADA (WA Branch) as it was then known — in 1975, shortly after my graduation from The University of Western Australia. Over 40 years later, when I was presented with my 40-year membership pin, I announced that, for me, the idea of not renewing my ADA membership was unthinkable.

As I see it, if we are to ensure that our professional and business interests remain protected, we must be an active part of an organised, strong, and well-connected association.

Over the years of my dental practice, I have seen the challenges our profession face become progressively more complex. Adding to the difficulty is the fact that in the outside world of politics and commerce, a highly competitive, adversarial mode of negotiation has subsumed the more problem-sharing/solving approaches of the past. As individual dentists, we have little hope of having our lone voices heard in the wider community. To go forward, we must be united. The voice that safeguards our professional ideals must come from the combined, consenting voices of a potent group.

Renovating a dental practice is no small feat; it is a significant commitment in time and cost to help improve the workspace for both patients and practitioners.

We spoke to two practice owners who recently renovated about their experiences.

DENTISTA DENTAL CARE NORANDA RENOVATED BY Henry Schein OUTCOME Addition of surgery room

WHAT THEY DID When Dr Carla Graneri purchased Dentista Dental Care in Noranda from Dr Brian Hurwitz she quickly started making plans to add another surgery room to the practice. “We just had one surgery room,” Carla says. “The other room was used by Brian's wife as a speech therapy room, so Brian kicked her out and we converted her office to another surgery,” she laughs.

Carla says she was lucky to be working with great tradespeople who knew what they were doing, adding she had the benefit of working with different chairs so knew exactly what she wanted when it came to choosing the chair.

She also had a good idea of what she wanted when it came to the flow of the room.

“I knew that I wanted the patient to be able to easily see their radiographs,” she says.

“We make videos for the practice, and we have heaps of patient information on our website, so I wanted our patients to be able to look at those things sitting up and with me holding the mouse as well. Otherwise, when you have things put on the ceiling on the TV, you are behind the patient controlling the mouse, which means you are behind them when talking.

There can also be quite a delay with the TV to connect to the computer.”

Inspiration also came from other practices.

“I noticed at a friend’s practice that he had a computer monitor to the right of the patient. I thought that was such a nice design feature because that is where you can communicate with the patient while they are sitting up. It is a much nicer interaction than if you are standing behind them on your computer.”

Although the turnaround of the room renovation was quick, it was many months in planning, with the main hold up being the arrival of the chair, due to shipping from Sydney being delayed a few times.

ADVICE TO OTHERS Carla recommends measuring out the flow of the room. “Spacing things out, marking them out on the floor and using props to see where you like to reach for things to work out is really helpful when working out where you want things to be placed,” she says.

“Sometimes too much space is a burden,” she adds. “Our existing surgery room has two sinks and two basins, but it was wasteful because everyone still uses the one sink, so that additional sink is unnecessary. Although our new room is smaller, it is very efficient.

"The biggest thing I would say to others planning these renovations is to keep notes in the planning meetings," she says. “For example, when Tom from Henry Schein was here and we were discussing the dimensions of the room, we thought everything would be perfect if the bench was 500mm deep. Unfortunately, I didn’t communicate that to the cabinet maker. Luckily it was still fine and worked out well, but I had completely forgotten about it until the preinstall for the chair and we realised it was going to be a very tight fit.”

WHAT THEY DID When Dr Gaurav Vasudeva decided to open his practice, he chose to renovate a 1940s house in Applecross. “This particular building was run by a chiropractor before I took it over, so from a council point of view it was approved as a medical premises but was not right for a dental practice,” he says. “There was a considerable amount of work we undertook.”

Gaurav wanted to retain the building’s old charm and features, so not much changed externally. His focus was on creating an ergonomic space for both patients and staff, so he was very involved in the design process.

“Conventional designs are not always very ergonomic, so I think a dentist must give input, because good ergonomics starts from design.

“There can be a lot of traffic created in one small space with patients and staff moving from space to space, so I wanted to have two exits or entries in both surgeries,” he explains. “I wanted to ensure I could enter the surgery from the front side where the patient can see me and not sneak up from behind, because most patients are nervous, and we want to make them feel really comfortable.”

The flow of the practice was also very important to allow for efficient movement between treatment spaces.

“For example, when we take a radiograph, the dentist and the assistant move out of the surgery,” Gaurav explains. “This is something in a general practice that happens at least 20 times a day, and every time someone gets up from the chair and moves out there is conflict, and it impacts the ergonomics. So, the moment I set up a patient to take a radiograph, my assistant has her own path to walk out, and I have my own path, so we don’t conflict and there is no interference.”

This “no-interference” flow of the practice continued with the creation of a separate consultation room, which has a CBCT unit in the same space. “The workflow goes: I have seen your teeth now, so now I need to take a CBCT. "The moment the CBCT is done, the patient sits down in the same room because that is also my consultation room, and it is a private area where I can discuss everything and show my patient my findings. We don’t go back and forth between areas; we just go forward in the one direction.”

A good dental stool and live-stream monitors were also important additions for ergonomics during treatment. “I use a dental stool that has hand rests – the reason why you need hand rests is to give you stability,” Gaurav says. “We see patients for six to

seven hours in an eight- or nine-hour day and if you are sitting on a stool with no support to your shoulders, neck or arm they are bound to fatigue.

“My assistant has the same dental stool, and access to a live stream on the side of her wall. I have access to another monitor live stream on my side of the wall, so we don’t have to twist and turn our necks here and there to see what is going on.”

ADVICE TO OTHERS The renovation went very smoothly and only took seven weeks. “There is an advantage of having people who know what they are doing – you need to leave certain things in the hands of the experts,” he says. “Have the right team around you, get commitment on paper and be really hands on, as it is your working space. I was lucky but I did do my homework and settled with a company I felt comfortable working with.”

Gaurav also recommends if someone is looking at a premises to contact the council first. “It is surprising that a Perth city council would be absolutely different from say Melville or Stirling but they all have different rules and regulations, which can be a hurdle,” he says. “Council approvals and parking issues are set out separately for different councils so look out for that if you are looking to lease or buy.”

INVEDENT Carla signed up to digital stock management and ordering system, Invedent (an ADAWA partner), to do a stocktake – but it quickly became a handy way to keep the practice organised year-round.

“Invedent was a great help for us because there was no stock control; it was just one team member having the whole responsibility. As the practice grows, everyone needs to know at a glance what the stock situation is.

“Invedent really allows that to happen,” she adds. “You just click, and you can see exactly what you have, and what you need to order. It is so helpful to us.”

Giving back – with gratitude One person can make a world of difference – as dentist Dr Tracey Gold beautifully demonstrates.

Dr Tracey Gold always wanted to volunteer. “My kids hate hearing it from me, but I just feel we are very lucky, and I feel very privileged,” she says. “I grew up in Perth, which is such a wonderful place, and in a household with a good, functioning family who loved each other. I always felt I had every opportunity to do anything I wanted to do, and be anything I wanted to be, and I was supported.

“I am conscious that not everyone has those opportunities and I always thought that I needed to do something to try to help people, as it feels like so much was given to me.”

Tracey fulfilled her calling to volunteer after reading the Western Articulator a couple of years ago.

“I had left my private practice and had started working at Curtin, where they encouraged community involvement,” she recalls. “I happened to be reading the Western Articulator and saw the volunteering opportunities and started thinking about Dental Rescue Days. Then Zara (Torre) mentioned Healing Smiles and I realised this was exactly what I wanted.”

DENTAL RESCUE DAYS In 2019, Tracey contacted ADHF state coordinator Andrea Paterson about the possibility of arranging a Dental Rescue Day at OHCWA, utilising Curtin’s OHT students.

“When I started working at Curtin, I saw a need amongst the students to get involved in volunteering,” she says.

“Students only see a certain demographic of patients, so I thought it would be very useful for our students to be introduced to volunteering, and to see the difference they could make as oral-health therapists.

“I called Andrea and asked if it would be possible to have a large-scale Dental Rescue Day. She said: ‘Of course, let’s do it,’ and it has been amazing.”

The first Dental Rescue Day, held at OHCWA, was in July 2019. The initial plan was to hold a Dental Rescue Day once a semester, and although COVID-19 restrictions stalled some of these plans, many Dental Rescue Days have gone ahead, usually treating around 30 grateful patients each time.

As well as being an amazing experience for the students, it has also been well received by the volunteering dentists.

“They’ve all absolutely loved it,” Tracey says. “Every person who has volunteered has said: ‘I want to come back. Please include me in the next one,’ because they’re helping patients who are in need.

"When you see the state of (the patients') mouths, compared to the patients you see on a day-to-day basis, there is so much need out there, because these people haven’t had access to care. Most of them don’t know where they can go and they can’t really afford even a copayment plan through a government service, so that part of it is very rewarding.

“Another thing the volunteer dentists say is they love the atmosphere and the vibe on the day and that is my thing as well,” she adds. “Every single person who volunteers is the person that you want to be around. With any job, it’s not what you do but also who you are working with, and this makes it such a rewarding experience.”

HEALING SMILES In 2020, Tracey joined the Healing Smiles committee, in an admin and organisational role, working closely with other members of the committee, Drs Jacinta Vu, Lida Sayadelmi and Shanash Bishnulall.

“With all of the committee members – all of them – nothing ever seems to be too much,” Tracey says. “They are the most accommodating people you will find, and their willingness to do whatever they can within their means is amazing. It is such a nice team to be a part of.”

Although Tracey's involvement with Healing Smiles has not involved treating patients, she says the screening days have been particularly satisfying.

“We have done a couple of screening days for Healing Smiles – during which Lida and I have seen people on the waitlist, so we can allocate their care to some of our volunteers,” she says. “These people get signed up by their case worker and at that time, we are just a name,

and they are a name on a list. Finally, when they come to that screening day, we say: ‘I see these problems and now we are going to fix it’. Not: ‘We’re going to put your name on another list.’ You see the relief of these women’s faces that they are finally at that point where someone is going to take care of them.”

You will never regret that moment and think, ‘I wish I was at the beach or out for coffee.̓ It is the best thing you can do on any given day and if everyone did a little bit, we could take care of a big need.

ST PAT’S Tracey also volunteers at St Pat’s whenever she is able.

"Whilst working at Curtin for the past few years, I've had the opportunity to volunteer at St Pat's during nonsemester times," she says. “I don’t have access to my own surgery, and I really wanted to treat some patients myself, because my other volunteer work is largely administration and enabling, which is great, but I also wanted the chance to treat patients.

“Volunteering at St Pat’s is such a humbling experience,” she adds. “I have never been thanked so deeply and profoundly as I get thanked by every patient there.

When you go to work you get thanked by money, but when you get thanked by someone who has had a broken-down tooth for the last 10 years and you take that tooth out, they are so happy that you really feel like you have done something that has made a difference.

“It is a very well-run clinic, and it is a very easy place to work,” she says. “It’s a very flexible and easy-going workplace, which has so many different clinicians coming through. The practice manager there and the nurse are very understanding, so it works so well.”

MESSAGE TO OTHER DENTISTS Tracey says taking the step into volunteering has opened a world of new colleagues and people who are likeminded and inspiring.

She recommends volunteering to one and all. “There are lots of ways to volunteer and none of them are more important than the other,” she says. “Find the one that works for you. If you have your own clinic and you have some time, see someone there. If not, get involved in a Dental Rescue Day. If you have a day, even once a month, then you can go to St Pat’s, they will be very thankful to have you.

“You will never regret that moment and think, ‘I wish I was at the beach or out for coffee.’ It is the best thing you can do on any given day and if everyone did a little bit, we could take care of a big need.

“I think there are many people who do want to volunteer but feel there are certain barriers,” she adds. “Andrea (from ADHF) is a beautiful person to talk to – and has her finger on the pulse of all the different ways someone can volunteer. You can find a way that works no matter how your hectic life is. Some people cannot do a regular commitment and that’s fine, there are so many options and people will be so thankful, whatever you can offer.”

Women in Dentistry lead the way What started as a society to bring female dentists together has grown into an important and influential West Australian society. New President, Dr Amanda Phoon Nguyen, shares the society’s story and its future plans.

Women in Dentistry was established in 2014 by prosthodontist Dr Janice Kan, with the aim of promoting mentorship and collegiality amongst female dental practitioners in WA, via regular social and educational events.

In recent years, Women in Dentistry has becoming increasingly focused on its Healing Smiles program, which provides pro bono dental work to female victims of domestic and family violence. With funding of Healing Smiles now managed by ADAWA’s Uniting Smiles charity, the program will be able to help more women than ever before.

New Women in Dentistry President, Dr Amanda Phoon Nguyen, looks forward to helping more people in the community through Healing Smiles. She also plans on organising more of the events that were such a big part of Women in Dentistry’s original purpose.

PLANS FOR 2022 “We want to bring back a lot of the social events,” Amanda says. “Personally, that is what I always enjoyed. The current committee includes Vice President Nandika Manchanda, Secretary Lalima Tiwari, Past President Jacinta Vu as well as Serene Ong, Suanne Lam and Shanash Bishnulall.

Females are far more likely to undersell themselves and that is something I would very much like to change.

“We hope to do what the past committees did very well and what people tell us they want – creating a safe social space, and organising informative events with speakers beyond dentistry, such as at Women in Dentistry’s End of Year AGM and breakfast where we had a paediatrician come to speak to us about COVID FAQs. “It’s important to note that although the society is called Women in Dentistry and obviously the focus is on female dentists, we’re not anti-men at all, and men are very welcome to come to our events,” she adds. “It’s all about building community.”

SUPPORT FOR FEMALE PRACTITIONERS

FINDING BALANCE

Previously, Amanda’s involvement with Women in Dentistry focused on attending the social events, but she realises the support the society provides is also very important.

As the new President, Amanda brings a wealth of experience to the role, through her involvement with many professional committees, and the time she dedicates to many community volunteer causes and online oral medicine educational/special interest groups.

“It was the social aspect that I liked; it was wonderful to get to know other dentists, but then it was clear how important it was for practitioners to have support. I think Women in Dentistry fills this unique space.

“Sometimes, there are questions that you are more comfortable asking someone who has been there.

For example, when to tell your boss that you’re pregnant. Or women who have come from interstate and want to know about mother’s groups or schools for their children. Many people have these life questions, in addition to being a practice owner or associate, that they want to discuss with people who understand.

Having experienced issues in the past myself, I realised these feelings weren’t uncommon amongst dentists, which is why I am so passionate about this space in the WA dental community.

“It’s also nice how Women in Dentistry is tied to the female pioneers, like Sally Joyston-Bechal, Jenny Ball, Meredith Arcus and Bernadette Pilkington – all of these wonderful ladies who I don’t think I would have heard about or got to know as well if I had not attended these events.”

Amanda adds she would like there to be more synergy between Women in Dentistry and ADAWA to mutually support dentists coming back from maternity leave, for example.

She would also like to see the introduction of a Women in Dentistry award for dental students in the future.

“I know UWA has a Sally Joyston-Bechal award (the Sally Joyston-Bechal Study Group Prize), and I would like to see if there could be one specially for female dentists who show leadership,” she says.

“Females are far more likely to undersell themselves and that is something I would very much like to change.”

As someone with so much on her plate, she is a great person to share advice on how other practitioners can find balance, although she says her idea of balance is not perfect.

If you take on something that you are not passionate about, it is always going to be an uphill battle.

“Do the things that you enjoy and that you want to do,” she says. “Sometimes I think people, not just women but all people, struggle with saying no. If you take on something that you are not passionate about, it is always going to be an uphill battle.

“For me, I do not say yes to all the committees I get asked to be on, although I wish I could. It is also important to know our limits,” she adds. “Do not feel bad saying no or feel like you are missing out. The roles and the positions that are meant to be yours, will always be there if it was meant to be.”

JOINING WOMEN IN DENTISTRY Amanda encourages all female practitioners to join Women in Dentistry.

“We are a friendly and supportive group,” she says.

“Dentistry, motherhood and life in general is hard enough, so having that support and community is so important.

I feel energised when I go to Women in Dentistry events and there is something special about being part of a community or group that vibes with you. Please join us!”

Janina is an oral-medicine specialist and a recent member to the ADAWA council, commencing in 2020. “I nominated for the Federal Councillor position to understand the challenges faced when trying to make changes to improve dental care and oral health awareness in the community at a national level,” she says.

Peter graduated from The University of NewcastleUpon-Tyne in 1991. He has held positions at Perth Dental Hospital and as an area dental officer responsible for Geraldton school and adult services, and has been the principal dentist at Western Smiles Dental Care in Mirrabooka for 15 years.

Janina works in private practice, has public placements at Perth Children’s Hospital and Sir Charles Gairdner Hospital, and is an adjunct senior lecturer at UWA. She also volunteers at the St Patrick’s clinic and for Healing Smiles.

Peter is no stranger to the ADAWA council – he has served as an ADAWA councillor since 2011 and was chair of the ADAWA Infection Control Committee.

She says she is passionate about advocating for the provision of dental care to all sectors of the community and to further the promotion of oral health awareness.

“Both these factors will ultimately result in the prevention of disease and improvement of quality of life for our community,” she says.

As an ADAWA council member, Janina hopes to continue the advocacy for the promotion and access of quality dental healthcare to all community sectors.

“The aging population are one of those sectors which will require increased support,” she says. “I would like to help ADA continue to support oral health campaigns, submissions and research, since this will increase community awareness of oral health issues and their associated impacts.”

Peter says he nominated for council because he wanted to bring an element of business understanding from the perspective of a principal dentist to council.

He is also committed to helping council navigate practising dentistry in 2022.

“My focus this coming year is to assist council through the process of allowing dentistry to practice smoothly during the transition of COVID in the community when vaccination rates reach 90% and the State borders open,” he says.

“I would also like to continue to work in the area of building community within general membership and the broader community,” he adds.

Dr Ahmed Saleh graduated from Dental School in 2002.

He practiced general dentistry in Cairo for seven years and in Australia for another three years. He spent three years as a Periodontics Registrar at UWA and has been a practising periodontist for seven years.

He has served as a volunteer in numerous dental communities including RACDS (Regional Committee Member), Australian Society of Periodontology (Federal Treasurer) and ASP WA (President, Treasurer and Committee Member). He is also an ADC examiner.

Ahmed says he is looking forward to using his previous experiences to help provide the best support to our dental community.

“I am passionate about any avenue that would allow our WA Dental community to excel in patient care,” he says. “Our physical and mental wellbeing is vital to help us carry out our duties with most efficiency and least stress. I am also interested in continuing education to ensure that we are up-to-date and can provide the finest service to patients in our community.”

Dr Tim Clair has worked as a general dentist since 2010, after completing a Bachelor of Dental Science (Honours) at The University of Western Australia in 2009.

Tim has volunteered with the Kimberley Dental Team since 2012 and has served as an Ordinary Member of Council for ADAWA since November 2019.

He made the decision to nominate for ADAWA council to broaden his involvement in the profession and make a contribution to the work and direction of ADAWA in advocating for dental health and the dental profession. “I am interested in ADAWA’s provision of services to members including medical indemnity insurance and CPD opportunities, the association’s representation on issues such as vertical integration of health funds and oral health initiatives, as well as its role in advocating for volunteerism within the WA dental community,” he says.

“I hope to contribute positively to ADAWA endeavours where the ultimate outcome is ensuring provision of dental care for patients across the community in WA.”

Dr Gwen Chin graduated from UWA in 1999 and has been working in private practice since. She is a member of Healing Smiles and is the current UWA Dental Alumni Society President.

“I am passionate about fostering good relations between the students and our alumni and within our entire dental community,” she says.

This is Gwen’s second year on ADAWA council, and she originally nominated because she wanted to be more involved with helping the community.

Dr Sahara Saeedi graduated in 2013 and has since worked in private practice. She is the ADAWA Chair of Oral Health Education and an ADAWA council member.

“I nominated for ADAWA council as a voice for the younger cohort of dentists,” she says. “I am passionate about providing opportunities for dentists to increase oral health awareness in the community, in collaboration with the ADAWA media team.”

“My goal is to raise the profile and awareness in our community of OUR community,” she says. “To facilitate the creation of better relationships between our members and increase collegial links between our members.”

Iris has a history as a qualified dental nurse and dental therapist before graduating as a dentist from UWA in 1997. Additionally, she has since completed a Post Grad Diploma in Forensic Dentistry.

Iris is the newly elected Country Councillor, and as someone who has worked in Mount Barker as a single operator in her own surgery for almost 24 years, it is clear she will have a great understanding of the issues that are close to our country members.

“I thought it was time to become involved and try to give back to the ADAWA after the many years of support received,” she says.

As a new ADAWA councillor, Iris would like to provide support to country members and make them feel more connected to the ADA, even though they are remotely located.

She would also like to help direct and educate the way general dentists transfer records, so legal documents can be admissible in court when required. “My biggest bug bear is the overall poor record keeping and poor transfer of records, as well as severe shortfalls in computer dental programs that result in inadvertently falsifying dental records,” she says.

Iris is also a Dental Officer with DHS, providing subsidised dental treatment to the aged and disadvantaged. Additionally, she has volunteered for the past 23 years as a board member for a local board that provides independent housing for the aged. “I would love to teach/volunteer, however, I am a little bit far away for regular stints,” she adds. “I am very passionate about providing affordable, sensible and high quality general dental treatment in my community, strongly opposed to the corporate dental surgeries and would love to see under-utilised local hospitals as a facility providing dental treatment under sedation/GA for children in need of extensive work.”

Dr Bec Penco graduated from UWA in 2016; she was awarded the KJG Sutherland prize for most outstanding achievement during the course of the degree and graduated with Distinction. She was the President of the University of Dental Students’ Society and is currently a member of ADAWA’s CPD Committee, the Federal ADA’s Recent Graduate Advisory Panel and the Federal ADA’s Employed Dentist Working Party. She is the Chair of ADAWA’s Recent Graduates committee and helps coordinate ADAWA’s New Practitioner Program (NPP).

Bec is currently working full-time in private practice at Lifetime Dental in Como and is still involved in tutoring dental students. She has always enjoyed being heavily involved in the dental community, and hopes to continue to represent the recent dental graduate cohort on the ADAWA council.

Bec has been on ADAWA’s council for three years, and says it was soon after she graduated that she realised she wanted to help the dental community. “For my first year out of Dental School I was based at OHCWA, so I got to be involved in various committees, including the CPD committee with Jenny Ball,” she recalls. “When I left OHCWA and went into private practice I felt I did not really have the opportunities to mingle, to have a say in things and to be engaged in what was happening in the dental community, so I thought joining ADAWA council would be a good way to do that.”

As someone who is only five years out of Dental School herself, Bec’s focus is to help recent graduates. “I want to support the recent graduates, particularly in the CPD aspects,” she says. Bec explains that the New Practitioner Program (the brainchild of Drs Sean Archibald, Amanda Phoon Nguyen, Greg Crane and Bec) was created in order to provide affordable and relevant CPD to newly graduated dentists.

DR PATRICIA ELDER ORDINARY MEMBER

Dr Patricia Elder is a proud Aboriginal Yindjibarndi/ Yamatji woman.

Her professional career in healthcare began in 2012 as a registered nurse, before going on to study a Doctor of Dental Medicine – graduating in 2019.

“I always wanted to return to country and provide dental treatment for my people and the wider Aboriginal community,” she says. “Until recently I lived and worked in Kununurra for Dental Health Services as a remote outreach dentist, which incorporated providing community outreach service delivery to Balgo, Kalumburu, Yuri Yungi and the Aboriginal Medical Services at the Ord Valley Aboriginal Health Service. I have since returned to Perth and taken up the role as Acting Director Aboriginal Health for North Metropolitan Health Service.”

Patricia is part of the Alumni for The Follow the Dream Program, Kurongkurl Katijin, Centre for Indigenous Australian Education and Research at Edith Cowan University and the School of Indigenous Studies at The University of Western Australia.

Patricia is a newly elected ADAWA councillor and decided to nominate for council because of her passion for Aboriginal health and wellbeing. “I strive to achieve a culturally respectful and non-discriminatory health

system with equity and equality in the access and delivery of services where my people feel safe, heard, important and their individual needs are met,” she says.

Being a council member, she hopes to help ADAWA: • Educate and inspire other people to aspire to improving health outcomes for Aboriginal people, including creating meaningful and lasting partnerships with individuals, communities and organisations.

• Provide appropriate education that is culturally appropriate.

• Expand opportunities to strengthen the Aboriginal and Torres Strait Islander health workforce.

• Enhance and strengthen the capacity, capability and leadership of Aboriginal and Torres Strait Islander Health Professional Organisations.

• Create a safe place for new Aboriginal health professionals and graduates to develop skills and knowledge to advocate for and deliver culturally safe healthcare.

• Implement strategies in community-based areas, which would see investment and infrastructural works and see community networking and community led initiatives in the planning and design process at state, regional and local levels.

Dr Jenny Ball is an ADAWA Federal Councillor and the Director of Continuing Professional Development at ADAWA – working to develop a world-class WA Dental CPD program. She has been involved with dentistry for nearly 40 years. She has worked in private practice, and has dedicated her time to volunteering and tutoring at the Dental School. Jenny has been on ADAWA council for 13 years, and in that time has served on executive, including as President.

She hopes to provide ongoing support to ADAWA council to support ADAWA members to carry out their profession.

DR NICHOLAS ALBATIS ORDINARY MEMBER

Nicholas has returned to ADAWA council after a 17-year hiatus. Nicholas graduated from UWA in 1988 and has spent most of his career practising in Oxford Street, Leederville. He has had a long relationship with the dental faculty at UWA, where he has been a clinical tutor for over 30 years.

He held the position of Secretary of the UWA Dental Alumni Society for 15 years and is the Immediate Past President of the Dental Study Group of WA. He says his reason for electing for ADAWA council is simple, if a little clichéd. “I wish to represent the membership and act in the best interests of ADA members,” he says. “I would also like to feel that I’m accessible to all members and would encourage members to contact me at nickalbatis@gmail.com or on 0417884841 should they have anything to discuss regarding the association, be it positive or something they disagree with.”

Dr Martin Glick graduated from UWA in 1982. He has also completed a Graduate Diploma in Public Sector Management from Curtin University.

Martin has been an ADAWA member for a number of years and has been on the ADAWA council as the DHS representative since 2009.

Martin’s formal qualifications, along with public sector experience, bring valuable information to council.

He is also involved in service provision, as part of his role in working for Dental Health Services (DHS), for such population groups as those with special needs, non-ambulatory, aged care and prisoners.

Martin says it is important that there is a representative from DHS on council. “It is vital to have that connection and ability to discuss issues between public and private sectors, given that we are involved or are aware of changes to the local environment in regard to public policy. It is important to have that ability to discuss those issues,” he says.

“The relationship (between DHS and ADAWA) has been one that is beneficial, collegiate and supportive on both sides for many years and certainly pre-dates the time since I have been a part of the ADA(WA) council.”

Our council also includes Dr Peter Wilkins (Salaried Dental Councillor) and Professor Hien Ngo (Head of School, Dean of UWA Dental School and Director of the Oral Health Centre of Western Australia).

Professor Lisa JA Heitz-Mayfield received a Master’s Degree in Periodontology (1996) and Odont Dr (PhD) (1998) from Lund University, Sweden. Lisa was Head of the Oral Microbiology Research Laboratory from 1999-2003 at the University of Berne, Switzerland. She was Chair of the International Team for Implantology (ITI) Research Committee, and member of the ITI Board of Directors from 2013-2021. She is a Board member of the Osteology Foundation. Her current positions include Adjunct Professor at the International Research Collaborative, The University of Western Australia, Professor at The University of Sydney, and Guest Professor at the University of Berne. She is the Editor-in-Chief of Clinical Oral Implants Research.

“We have the return of some of our favourites and lots of hands-on courses that are very popular,” Jenny says.

Regional members have not been forgotten, with two additional country courses on the cards. There will be two courses in Albany, a return to Darwin and, for the first time, a course in Karratha.

Are you on track to clock up your required CPD hours? “This year will be the end of a three-year cycle, so you need to have your 60 hours,” Jenny says. “The cycle finishes on November 30, and you need to have accumulated your 60 hours by then.

The Dental Board of Australia does conduct audits.

Practitioners are also spoiled for choice when it comes to dinner courses. “There are several dinners planned and detailed in the CPD booklet, but I have a few more dinner courses up my sleeve, so keep an eye out for them,” Jenny reveals. “Dinner courses are always popular – they are short and sharp, and you get your CPD, socialising and networking in.”

It is recommended you book your CPD early, especially when it comes to some of the most popular courses. “The course by Asheen Bahari and Paul Gorgolis (Biomimetic Minimal Prep Restorations in June) is always popular,” Jenny says. “Last year, the Primary Dentition course (which will be presented by Vanessa William in April) was so popular we had to run it twice.

“Endodontic courses have also been quite popular,” she adds. “We have four endo courses this year in a variety of formats by a variety of presenters.

“We will also be adding to the CPD program over the year as we always do, so you need to keep an eye out for your EDMs and on the website.”

Janice is the Past President of the Academy of Australian and New Zealand Prosthodontists (AANZP), a Fellow and Registered Lecturer of the International Team of Implantology (ITI), and a Fellow of Academy of Dentistry International (ADI) and International College of Dentists (ICD), lecturing widely at various local, national and international dental events.

She has published in respected international dental journals and clinical textbooks, including the Clinical Oral Implants Research and ITI Treatment Guide, and is hoping to undertake a PhD on Implant Dentistry Biomechanics in the future.

Supporting our new practitioners The NPP program was hugely successful in 2021, with plans to make the program bigger and even better in 2022.

ADAWA’s New Practitioner Program was launched in 2020, with the goal of assisting new and recent graduates to transition from a big cohort in a predicted environment to part of a network of peers.

Comprising five study clubs, held throughout the year – culminating in November’s Young Dentists’ Conference – NPP allows new practitioners to hear from some of the best in the industry and have the chance to ask questions in a safe, supportive setting.

“We always tell attendees of the study groups to let us know what they want and what they find interesting,” Amanda explains. “We also encourage them to let us know if they run into any cases or have any questions, so it is quite organic; we craft the program based on what people tell us they want to know.

“For example, Leon Smith’s talk at the latest Young Dentists’ Conference was: ‘What goes wrong at 5pm on a Friday and how to prevent it’. That idea was born because something did go wrong with a patient at 5pm on a Friday, so Leon asked the registrars at the hospital what they most commonly see after hours – and the talk was based on that.”

“As a result, we targeted the talks around what people wanted to know,” she adds. ‘Biomimetic dentistry is something our young dentists want to know more about, so we had two amazing speakers for that. We tried to keep the focus of the program on very practical tips, so whoever attended and listened to the talks could hopefully start applying things the next day or the next week. We aimed to keep things interactive, so we had panel discussions, several quizzes and prizes sponsored by DPL and WA Dental CPD.” The social event afterwards was a hit, with great music and tacos for lunch, CARAD Food Truck (a social enterprise staffed by refugees and asylum seekers) in attendance, and a live paella station.

Together, the group is excited about what 2022 will bring, with the study club program lined up for the year.

“We have speakers who are passionate in their field and who are very approachable,” she adds. “One of the best things about the program is that we encourage attendees to ask questions throughout the presentations. After all, it is not a lecture but a study group. We want people to feel comfortable asking any question that comes to mind so they get the most out of the sessions.”

WHY NEW GRADUATES SHOULD COME ALONG “Come along because I feel that we have a really good program that we are proud of and we want this to be something that will help dentists,” Amanda says.

“Everyone has a lot in common, so if you feel like you don’t know much about something or you want to learn more, don’t feel shy or embarrassed to come along because nearly everyone is in the same boat.

No one graduates knowing everything and even experienced dentists don’t know everything – it is always a learning process, and this is why the study clubs are so important.”

“When you graduate it is very easy to become isolated because you work in a silo,” Bec adds. “You have your practice, and you mingle with the other dentists there if you are lucky, but so often if you have a chat to people outside of where you work you learn so much so quickly.

“We are trying to grow a community because we can all learn from each other.”

The NPP is going from strength to strength, thanks to the work of the committee and the enthusiasm of the attendees. There are plans to tailor a CPD roadmap for new grads and hopes to one day take the program on the road to rural practitioners. Watch this space.

HISTORY

CLINICAL FINDINGS

A 72-year-old male was referred to me from his general dentist due to a "painless swelling on the left jaw”, that had been enlarging over the preceding six months. He reported that the swelling was initially soft and painless, and over time had increased in size and become firm.

He also noted his saliva had gradually “thickened” in consistency in the last 12 months. Upon further questioning, the patient also reported dry eyes for the past six months. His medical history is significant for stable immune thrombocytopenia.

My examination revealed a firm, non-tender, 5cm swelling associated with the left parotid gland along the ramus of the mandible (Figure 1). No regional lymphadenopathy was palpable extra-orally.

Temporomandibular joint and muscles of mastication were within normal limits. Normal mouth opening and range of motion was noted. No oral mucosal swellings or lesions were noted. There was a lack of salivary secretion noted upon the palpation and milking of the left parotid gland. Furthermore, ropey saliva consistency was noted.

How to Support an Employee Returning from Parental Leave The Fair Work Act 2009 (FW Act) provides that employees who take parental leave have a return-to-work guarantee, which means they have the right to return to their pre-parental leave position.

However, many employers do not consider the impact that an extended period of leave may have and expect employees to return to work with any little guidance or support. Employers will often assume that employees will remember the “lay of the land” and therefore will not require any assistance in starting work again.

However, much can change in a short period of time, and many employees may benefit from being “reboarded” upon their return to work.

WHAT IS REBOARDING? Reboarding essentially refers to giving an employee a short onboarding upon their return to work after taking leave. This does not mean a practice needs to undertake a full induction process, rather, the practice should take the time to engage with the employee and bring them up to speed with what has happened in the workplace during their absence, such as policy and procedure changes. This is particularly important given the nature of the ongoing Coronavirus pandemic, which often results in shifting infection control requirements, density limits and general operational changes.

Reboarding assists to promote productivity as it can help bring the employee up to speed faster and more effectively reintegrate them back within the workplace culture.

Reboarding also ensures employers develop an appropriate plan to manage and structure the employee’s return, offer support to them, and reduce any stressors associated with returning to work.

The concept of reboarding can also be applied to other employees returning from long-term absences (generally eight weeks or more).

This may be due to long-service leave or an illness or injury.

KEEPING-IN-TOUCH DAYS In the context of parental leave, employees may benefit from using their “keeping-in-touch days” in the build up to their return to work.

The FW Act provides that an employee on unpaid parental leave Is entitled to 10 keeping-in-touch days which are designed to enable the employee to keep in touch with his or her employment in order to facilitate a return to that employment after the end of the period of leave. Keeping-in-touch days are paid and can be worked as: • a part day

• a few days at a time, or

• 1 day at a time

• all at once.

A keeping-in-touch day could include any of the following: • taking part in a planning meeting • doing on-the-job training • doing work to become familiar with the workplace or role before returning to work.

For example, if there has been a change in the way your practice performs a specific type of procedure, an employee could use a keeping in touch day to familiarise themselves with the process.

Keeping in touch days do not break the employee’s period of parental leave.

ENSURING REBOARDING SUCCESS

In November 2020, the FW Act was amended and introduced the concept of flexible unpaid parental leave.

This type of leave enables the employee to take up to 30 days of their maximum 12 months unpaid parental leave period on a flexible basis. It may be taken as a single continuous period of one or more days, or separate periods of one or more days. Flexible unpaid parental leave can be taken for a variety of reasons, including to assist with gradually returning to work.

HOW TO REBOARD AN EMPLOYEE The reboarding process should commence prior to the employee’s return. This may involve the Practice making contact and providing updates to the employee about any changes in the workplace. Practices may wish to engage with the employee to understand whether they wish to take any flexible unpaid parental leave (subject to the notice and evidence requirements under the FW Act) or whether any other temporary arrangements to assist the employee are necessary.

Employees should be provided with the opportunity to review any new processes and procedures in the workplace, read through any new policies and have time to digest this material, and ask questions if necessary.

Practices must understand that the needs, priorities and perspectives of the employee may change during their time off and these should be reasonably considered by the employer when reboarding them.

• Seek Advice: Leverage the learnings from the latest employees who have returned after an extended period off. Discuss the things that worked well for these individuals and how things could be improved in the future.

• Be Emotionally Aware: Employees may return to work with a different perspective, it is important for practices to be aware of the employee’s emotional state and offer employees regular check-ins and a comfortable environment to discuss how they’re feeling.

• Acknowledge Workplace Changes: Returning employees may feel overwhelmed when realising that their colleagues or former subordinates have moved into a position with greater responsibility, often above the returning employee. In these instances, this information should typically be shared with the employee prior to their return to manage their emotional state and ensure they are not caught off guard.

• Create Social Opportunities: Returning employees should be provided the opportunity to reconnect with their colleagues and engage with them in a casual social environment. This will ensure ease during the employee’s transition back into the office and provide them with support and reassurance.

WHAT TO FOCUS ON DURING THE REBOARDING PROCESS: Returning to the workplace after parental leave can be daunting for an employee, but by ensuring that a structure is in place to effectively manage their return, the process will become much smoother.

Ultimately, practices should consider: • How have the employee’s priorities changed?

• Which projects have been completed and which have been started since the employee left?

• Has the company’s objectives or priorities changed?

• What changes and upgrades have occurred during the employee’s time off? adawa.com.au

I organised a CT head and neck with contrast for the patient. The CT demonstrated a large (48mm) slightly ill-defined mass arising within the anterior aspect of the superficial lobe of the left parotid gland (Figure 2). There was generalised atrophy of the lacrimal glands, right parotid gland and both submandibular salivary glands that are strongly suggestive of Sjogren’s Syndrome. Blood investigations were also strongly positive for SS-A, SS-B and SSA Ro52 extractable nuclear antigens. Rheumatoid factor (RF) was borderline positive. The patient underwent a core biopsy of the left parotid mass, that confirmed a diffuse large B-cell lymphoma.

The final diagnosis was confirmed as Stage II E diffuse large B cell lymphoma (DLBCL) (AJCC 8th edition) associated with primary Sjogren’s Syndrome. I subsequently referred the patient to haematology colleagues where he underwent chemotherapy for the management of DLBCL with R-CHOP (rituximab, cyclophosphamide, vincristine, prednisolone).

Of all the systemic autoimmune diseases, Sjogren’s Syndrome (SS) has the highest risk of lymphoma development. Approximately 5-10% of SS patients develop lymphoma, with risk of development increasing with duration of SS. Salivary gland enlargement, lymphadenopathy, Raynaud’s phenomenon, Anti-SSA/Ro and/or anti-SSB/La positivity, rheumatoid factor positivity, monoclonal gammopathy and C4 hypocomplementemia are independent risk factors for lymphoma development among SS patients. The risk increased to almost 40% in patients displaying three to six risk factors and increases to 100% if all seven risk factors are present.

The above case highlights the complications that can occur with Sjogren’s Syndrome. It is essential to recognise and follow-up on independent risk factors for lymphoma development from the initial diagnosis of SS, in order to recognise high-risk patients.

Back in Full Swing ROYAL FREMANTLE GOLF CLUB The first fixture of the year was held at Royal Fremantle Golf Club under challenging conditions, with the temperature reaching 39 degrees as the group teed off. Sixteen brave golfers took to the course for our traditional opening game at the superb Royal Fremantle course.

Michael Welten managed the difficult conditions superbly, finishing with a terrific score of 37 stableford points, including an eagle three on the closing par 5 eighteenth hole. This was enough to hold off Hugh Gillespie on 36 points and John Scully in third place with 34 stableford points. Congratulations to those players.

HEALING SMILES AND WOMEN IN DENTISTRY BREAKFAST

Launched by the Australian Medical Association, the #SicklySweet campaign turns the tables on the many sophisticated soft drink ads directed at young people each summer.

The Healing Smiles and Women in Dentistry Society breakfast in December was a wonderful event, with volunteers celebrated.

There was also an informative talk by paediatrician and infectious diseases physician Dr Daniel Yeoh on COVID-19 vaccination.

RISE OF CDBS REBATES From 1 January 2022, the amount of dental benefits payable for dental services provided under the CDBS increased. The amount increased from $1000 to $1013, with a further rise for the 2022-2023 period to $1026.
