ACC’s Admissions Coordinator – Providing Care and Compassion in a Time of Need

How do you take the fear and apprehension out of choosing a nursing home after hospitalization?

Meet Kory Ishisaka - the Admissions Coordinator at ACC Care Center. Her job is to provide compassionate assistance to families needing help with finding care following an acute hospital stay. She provides education about what to expect at a skilled nursing facility and how to navigate the complex post-acute healthcare system. Kory is a Licensed Vocational Nurse with over 25 years of long term care experience who has been with ACC for almost four years. Her caring style and ability to share her knowledge in an understandable way helps smooth the transition from hospital to rehabilitation and back to home. We appreciate hearing from thankful families about Kory’s ability to help them through a stressful transition from the acute hospital back into the community. Kory is a shining example of the exceptional staff at our Care Center.

ACC Care Center has a long-standing reputation for being one of the best facilities in California. This has come about through the efforts of our compassionate and caring staff that provide culturally sensitive, personcentered care to our residents. Our attention to family gathering spaces and creating a home-like environment distinguishes us from our competitors.

Wall-to-wall Wi-Fi is enjoyed throughout the Care Center along with smart HDTVs at each bedside. With park-like outdoor areas and updated interior spaces, visitors to ACC Care Center continually comment that it does not look or feel like a typical nursing home.

Get Ready for the 2016 BIG DAY OF GIVING!

Tuesday, May 3, 2016 is the Big Day of Giving, a local initiative spearheaded by the Sacramento Region Community Foundation to increase philanthropy in the Sacramento region, with a focus on online giving. ACC is participating for the second year in a row—our goal is to continue to raise funds to support our community-based programs—ACC Rides, Lifelong Learning & Wellness, Bridge to Healthy Families Caregiver Support, and Utilities Education & Assistance. Last year, 123 donors contributed $9,640 to ACC Programs on the Big Day! Where did the donations go? Check out our infographic showing the 2015 accomplishments of ACC Programs:

President’s Message

Community leadership is the foundation that leads to the effectiveness and achievements for ACC Senior Services to succeed in its vision to create an array of services that help older adults sustain their independence and life styles. That leadership was best demonstrated by the attendance of over 70 past and present ACC directors and committee members at the Annual Board Event held on January 25, 2016. Included in this leadership group were major ACC donors, Visionaries and supporters of the Capital Campaign.

The event was hosted by the ACC Board as a celebration of our first anniversary at the ACC Campus and occasion to recognize outgoing ACC leaders and introduce new board directors.The gathering allowed the community leadership members to socialize and share one’s connection to ACC in their respective leadership role.

As a nonprofit corporation, ACC is governed by a voluntary Board of Directors who directs and guides ACC’s operations and focuses on strategies and policies to assure the organization is meeting its goals and achieving its mission. The Board is assisted by more than 100 volunteer committee members who serve on the Board’s eight standing committees and the various numbers of event and advisory committees.

To understand the important work and achievements of the standing committees, reports were presented. The Finance and Audit Committees reported on the strength of the organization’s financial condition.

The Greenhaven Terrace and Care Center Committees reported on the quality services delivered to our residents. The Program and Volunteer Development Committees produced video presentations about our community service programs and the ongoing efforts to recruit and recognize volunteers.

The Fund Development Committee provided a review of our various fundraising events and asked the community leadership to share the message to support the ACC Capital Campaign.

As we begin another year, ACC welcomed 3 new directors to the Board. Diane Van Maren is a first time director and current member of the Program Committee. Joyce Iseri is also a first time director and current member of the Greenhaven Terrace Committee, Program Committee and the Village Advisory Committee. Chiang Wang is a returning director and Co-chair of the Care Center Committee as well as a member of the Greenhaven Terrance Committee.

Outgoing directors Ralph Sugimoto and Howard Harris were recognized for their many years of service on the Board and for their leadership. Over a span of 15 years, Howard has provided leadership as a director, Co-chair of the Care Center Committee and Chair of the Planning/Development SubCommittee. Howard’s oversight and project management skills, with his keen eye for cost controls, has culminated in a number of capital projects including the renovation of the Care Center, purchase and opening of Greenhaven Terrace and ACC Assisted Living and major remodeling of the ACC Campus to house ACC Administration and ACC Programs.

Ralph has performed many leadership roles during the last 25 years and has served as President, Treasurer/Chief Financial Officer and Chair of the Finance Committee. As a CPA, Ralph has assisted the Board in carrying out its fiduciary duties by providing critical perspectives to help ACC maintain a strong financial foundation to fund programs, property acquisitions, and all aspects of facility development and growth. As a director, Ralph has embodied the spirit of giving and attitude of compassion for the many seniors we serve.

As directors and committee members continue to work to maintain and expand an array of programs and services to benefit all older persons and their family caregivers we ask you to continue with your generous support to ACC Senior Services and the ACC Capital Campaign.

WHEREAS, over the span of 15 years, he provided valued leadership and oversight of all phases of ACC capital projects and facilities, including planning and development, building acquisition, tenant improvements, design and construction, project close-out activities, and establishment of a 15-year preventive maintenance and replacement schedule for ACC’s capital assets; and, WHEREAS, he was a member of the Acquisition Team involved in the purchase of Greenhaven Terrace, Maple Tree property, Bowling Green property, and the Campus; and, WHEREAS, Howard Harris’ activities culminated in the renovation of the Care Center, opening of Greenhaven Terrace and ACC Assisted Living and the relocation of ACC Admin, ACC Senior Services, and ACC Rides to ACC Campus and the relocation of Meals on Wheels by ACC to Park City location; and, WHEREAS, Howard Harris is being honored by the ACC Board of Directors and staff in appreciation of his 11 years of service on the Board of Directors; and, NOW THEREFORE, be it resolved that the ACC Board of Directors presents this resolution to Howard Harris upon the completion of his 11 years on the Board of Directors.

With both a Medical Director for Postacute (rehabilitative) Service and another Medical Director for Continuing Care, we are able to provide more clinically complex care and more individualized attention than many other skilled nursing facilities. Our medical directors and staff continually evaluate and employ the latest in advanced clinical technology and practices. For example, we use an electronic health record that enables the primary physician to review their patient’s records remotely. Your Healthy Heart Hypertension is the medical term used to describe high blood pressure. It is a common but very serious condition; one-third of American adults and two-thirds of American seniors (those over the age of 65) are affected by hypertension. The human body requires a certain level of blood pressure in order to pump blood throughout the arteries. When the arteries become narrowed, the heart must pump faster in order to carry out the necessary functions of the body. Your blood vessels respond more slowly to a change in body position. Talk to your Nurse, Therapy Specialists’ therapist and your physician if you are feeling lightheaded or off balance when you stand up.

Although high blood pressure usually cannot be cured, in most cases it can be prevented and controlled. Your doctor will work with you to find the proper diagnosis and treatment plan. Treating and preventing high blood pressure rely on the adoption of a healthy lifestyle and the judicious use of anti-hypertensive medications.

For patients with hypertension, treating the underlying cause of hypertension is often the best option. For long term success.

This can be accomplished with changes in diet and lifestyle Page 4

(quitting smoking, for example), as well as with healthy techniques such as physical therapy.

There are a number of physical therapy techniques that can help to lower blood pressure in conjunction with healthy, positive lifestyle changes. Many hypertension patients struggle with weight-related issues or injuries such as neck pain or back pain. Physical therapy can help patients with a wide range of medical conditions to begin an exercise regimen that is safe and effective.

Stress relief is another major factor in reducing hypertension.

For hypertensive patients who are suffering from anxiety, massage can be a powerful weapon in lowering blood pressure readings.

One reason massage is so helpful is that is works to reduce steroid hormones, a good indicator of stress levels.

ACC Rides Transportation Services Awarded 2015 Rosa Parks Diversity Leadership Award

ACC Senior Services is proud to announce that ACC Rides Transportation Services has been awarded the 2015 Rosa Parks Diversity Leadership Award by the Sacramento Chapter of the Women’s Transportation Seminar (WTS). The WTS is an international organization dedicated to building the future of transportation through the global advancement of women, and this award honors an individual, group, or organization that has made significant contributions in promoting diversity and cultural awareness within their organization, the transportation industry, or in a project or activity that supports the goals and mission of the WTS.

Spring Brings New Activities to the Welcome Center

Spring has sprung and the Welcome Center Team is busy with new activities. We are looking for bridge and chess players for our new activities! Currently we have 2 bridge players who are looking for others to join them in a game on Thursday afternoons. On Fridays, we have 2 chess players who encourage others to join them. We are also planning a cribbage session in March for those who are interested in learning about cribbage or would like to play.

More information about the cribbage session will become available on our website, Facebook and at the Welcome Center when they are finalized. Stop by the Welcome Center to check on these new activities! Or just stop by the Welcome Center to say “hi” and enjoy a nice beverage.

Instructor: Pat Pinkerton We will be cooking this delicious, classic family chow mein recipe from the original Hong Kong Cafe. Some of the simple ingredients included are the Hong Kong style pan-fried noodles, dried black mushrooms, fresh mushrooms, char sui, carrots, bok choy, bean sprouts, green onions with a special sauce (soy sauce, oyster sauce, etc). The Joys of Being an Instructor

Teachers – they share a skill, an attribute, knowledge with others. Some do it as a career, others as a volunteer activity. I chose the latter. My teaching experience began at the 2012 ACC Volunteer luncheon. Joaquin Ngarangad (ACC’s certified fitness trainer for the last ten years!) asked if I would be willing to teach a beginner’s swimming class. He mentioned that there were a few students in his aqua aerobics class who expressed interest in learning how to swim. Joaquin knew I had a love for swimming since childhood. Since I had retired, I had the free time. So, I began offering beginner swimming classes in the Summer of 2012 at Greenhaven Terrace, and have continued to offer them each summer since then.

Last summer, I had a class, with students ranging in age from 60+ to 80+. As part of the introductions, two of the students shared they had water fears since childhood -- one after being thrown into the deep end of a pool during a swimming lesson, the other being thrown into the ocean. I knew that I had a challenge, but felt that I would try my best to help them learn some basic skills.

shadow her all along the lap, that she would swim along the edge of the pool, so that if she needed to stop, she could reach up or I would catch her. She completed the lap, and was ecstatic with her accomplishment. We all yelled and hugged and applauded her accomplishment!

Then the other two students were ready to do their “final exam”. They wanted to swim the length of the pool together. I was very concerned about that, since I wanted to make sure I could reach them if they had any difficulties swimming the lap. Well, they pushed off, and swam like Esther Williams! More yelling and hugs and cheers (sorry GT residents!).

As we all celebrated their accomplishments, they “fessed up” --- in preparation for the last session, they figured I would have a “final exam” and had been practicing swimming longer lengths than what we were doing in class. They did that as a surprise to me, for them to show me how much they had learned in class.

I was so proud of them.

They told me that even though the gruff exterior of “The General” might frighten some, they knew that they would be in a safe and supportive environment in the pool…and they left the class knowing that they could swim…something they could use for the rest of their lives.

As each session ended, I felt that all of the students had improved significantly from the swimming skills they had when the class began. On the last day, I told them the “final exam” would consist of swimming one lap of the pool – the length of the pool. The entire class is conducted in the shallow end of the pool, doing exercises and laps along the width of the pool.

They told me that I gave them something they would never forget; what I didn’t share with them, is that THEY Three students participated in the “final exam”. The first gave me something I would never forget. Teaching is a student was very concerned and didn’t think she would rewarding experience. That’s why I teach at ACC – you be able to complete one lap. I told her that I would can too!

New Art Gallery at ACC Senior Services

Do you yearn to see some fresh and original art works? The new Art Gallery inside the ACC Senior Services Programs and Administration building may be just the ticket. The gallery, which is located in the hallway on the way to ACC’s Programs, is showing off some of the favorite works by participants of ACC Programs and Clubs. One of our goals is to encourage involvement in the visual arts via the gallery-- with plenty of volunteer input. Art representing a theme or subject matter will be on display for about a month or so, and then will be replaced by art representing another theme, so as to keep the exhibitions interesting. Currently, we have a couple of walls for use as the Gallery, but we have plans to increase the display space.

The Gallery will be an integral part of the Big Day of Giving (May 3) events at ACC, with a special exhibition, perhaps in collaboration with the other community-minded artists in our area. This dynamic blend of different social and artistic values should provide for some interesting and stimulating points and counter-points.

ACC Rides provides door-to-door transportation service to older adults residing in the zip code areas: 95832, 95831, 95828, 95824, 95823, 95822, 95820, 95818, and 95814 • Priority is given to seniors, who have no other means of transportation to Meals on Wheels café sites, medical appointments, grocery shopping, attend programs and classes at ACC Senior Services and other senior centers.

Elilgibility: At least 17 years of age, U.S. Citizen or Permanent Resident, HS graduate Volunteer Benefit s: Training opportunities, Monthly stipend and $1,527.78 Education Award (upon completion of 450 hours) • Contribute 450 hours of volunteer service from Nov. 1, 2015 to Oct. 31, 2016.

• Engage seniors in arts & crafts, light exercises, and other social activities.

• Provide much needed rest to family caregivers.
