ACC’s Admissions Coordinator – Providing Care and Compassion in a Time of Need By Melanie Segar, Administrator, ACC Care Center
How do you take the fear and apprehension out of choosing a nursing home after hospitalization? Meet Kory Ishisaka - the Admissions Coordinator at ACC Care Center. Her job is to provide compassionate assistance to families needing help with finding care following an acute hospital stay. She provides education about what to expect at a skilled nursing facility and how to navigate the complex post-acute healthcare system. Kory is a Licensed Vocational Nurse with over 25 years of long term care experience who has been with ACC for almost four years. Her caring style and ability to share her knowledge in an understandable way helps smooth the transition from hospital to rehabilitation and back to home. We appreciate hearing from thankful families about Kory’s ability to help them through a stressful transition from the acute hospital back into the community. Kory is a shining example of the exceptional staff at our Care Center. ACC Care Center has a long-standing reputation for being one of the best facilities in California. This has come about through the efforts of our compassionate and caring staff that provide culturally sensitive, personcentered care to our residents. Our attention to family gathering spaces and creating a home-like environment distinguishes us from our competitors. Wall-to-wall Wi-Fi is enjoyed throughout the Care Center along with smart HDTVs at each bedside. With park-like outdoor areas and updated interior spaces, visitors to ACC Care Center continually comment that it does not look or feel like a typical nursing home. Continued on page 4
Get Ready for the 2016 BIG DAY OF GIVING! By Linda Revilla, Program Director
Tuesday, May 3, 2016 is the Big Day of Giving, a local initiative spearheaded by the Sacramento Region Community Foundation to increase philanthropy in the Sacramento region, with a focus on online giving. ACC is participating for the second year in a row—our goal is to continue to raise funds to support our community-based programs—ACC Rides, Lifelong Learning & Wellness, Bridge to Healthy Families Caregiver Support, and Utilities Education & Assistance. Last year, 123 donors contributed $9,640 to ACC Programs on the Big Day! Where did the donations go? Check out our infographic showing the 2015 accomplishments of ACC Programs:
