Thank you for the warm welcome that you have given me and your acceptance to hold this meeting with me after such a short notice of my desire to meet you provided you may have had some other commitments. As earlier stated in my notice sent to you, I would like to conduct an interview about your experiences in the workplace and any ethical dilemmas that you might have been involved in. Hope you are okay with that.

Judith: I is fine, I am ready for that.

Me: Thanks again Judith. Now, before we start, could you please give me a brief background information about who you are and your professional capacity.

Judith: Well, I am a nurse by profession, and I have been one since I was 15 when I used to care for my grandmother if I should say (laughs) that had a chronic illness I have never understood what it was until now. Professionally, I have been a nurse for about eight years or so and indeed have come across many ethical dilemmas, some easy to solve others too difficult to handle.

Me: Indeed it must have been an early calling for you into the profession. Kudos for that and I am sure your grandmother and family are pretty grateful for your recommendable effort.

How is she doing now?

Judith: She is doing great at the moment even though she is developing some dementia the reason I have never understood what chronic illness she had back then but I can tell you she is quite dear to me.

Do you still care for the elderly in the hospital?

Judith: Yes, I occasionally do especially during and emergency, but in most instances, I deal with pediatric patients since I also love interacting with the youngsters.

Me: talking of the youngsters, do you find making some decisions as challenging as when making with the adults especially about ethical dilemmas?

Judith: Indeed as per the experience I have had so far, I can say that handling the issues of the young people in the hospital set up is quite challenging. For instance, as you might already know, those admitted as pediatric patients have not attained legal age to make competent decisions and as much as that may sound simple, it has in many cases implicated in ethical dilemmas.

Me: You must indeed have the guts and unique skills to handle such matters. Now, tell me as a person, how do you go about that given that such circumstances also have some legal implications?

Judith: First and foremost, I can say that God has been my guide in all the decisions I make.

To be honest, since it dawned on me that there are such circumstances way back in my nursing school, I have always tried to do extensive research on various ethical dilemmas that have ever occurred elsewhere and the best-informed solutions provided to them. This move has helped me a lot in dealing with such matters. At the moment I feel confident to handle many of these issues as compared to my early years in the profession. Maybe as they say old is gold (smiles).

Me: Indeed, I also share your sentiments. I also believe so, because as earlier said I love socializing with the elderly to get a piece of their wisdom. Do you find your job too demanding when it comes to making such decisions?

Transcript Of An Interview Pertaining To An Ethical Dilemma 4 Judith: No, except for a few situations in which you are expected to make quick decisions that could save a life that is as well less controversial. That means, a lot of emphases is put on critical thinking otherwise you can be prone to making a blunder all the time and you know that is not good for personal and professional growth.

Me: Have you been involved in any single blunder over the eight years you have been in the profession?

Judith: Not at all. However, I have had some members of nursing staff make a few of them, but we always consult each other and work as a team to always provide the best for our clients to the best of our knowledge.

Me: Among those few ethical dilemmas, have you ever been challenged by any?

Judith: Yes indeed, not once but many times. I also depend on others to make collective decisions, and it works out in most instances.

Me: Which of those ethical dilemma cases that you know of is the most memorable and why?

Judith: Well, the most memorable ethical dilemma case that occurred while I was on duty recently is that involving a 12-year-old patient that urgently needed a blood transfusion. To take you through the family background of the patient, he comes from a family in which the mother and mother separated in an informal manner from each other three years ago. Since they have not legally separated, that means both are in custody of the child and the law requires that either of them has the authority to make informed decisions besides providing informed consent in critical hospital procedures as surgical operations and blood transfusion.

Upon informing the mother who accompanied the patient to the hospital of the need to transfuse the patient with blood and the existence on no other option, the mother declined citing it to being against her religious beliefs (Jehovah witness). Indeed after inquiring from the patient about it, he confirmed that it was against the religious teachings and his family beliefs. As it stands, the mother could not give consent to transfusion. Transcript Of An Interview Pertaining To An Ethical Dilemma 5 doing a further physical examination on the patient in private, the patient told me to go ahead with the transfusion since he wanted to live long despite their religious beliefs. For this reason, I found the need to make further consultation and decided to call his father to get his view on the matter through a recorded phone call. Contrary to the mother’s decision, the father provided consent for me to go ahead with the transfusion. At this point, I was left confused since I had two contradicting information from two individuals with the same capacity in providing consent that could determine the life of the patient. It was a tough decision to make, but I finally did since I had to.

Me: What exactly was the ethical dilemma here?

Judith: This case had some ethical aspects involved primarily patient autonomy, maleficence and beneficence. It was evident that considering the age of the patient; they could not make an informed decision on their own and the parent assumes that responsibility automatically as provided by the law. However, the decision made was contrary to the wishes of the minor.

Despite that, the patient was not autonomous on that, and decisions are to a large extent influenced by the person having the responsibility to provide consent. On the other hand, both parents provided contradicting directives, and it depended on me to make the decision that I deemed the best to the best of my judgment. Going ahead and transfusing the patient could go against the wishes of one parent and it could be a violation and could be termed as maleficence as it could be disregarding their religious beliefs and spiritual wellbeing but it could save a life. Not transfusing the patient could as well a violation of the other parent’s wishes and could be against the ethical principle of beneficence. At this point, it all depended on me to make a decision based on beneficence and beneficence ethical principles and which could be observed at the expense of the other but still bring out the best outcome.

Me: That must have posed a great challenge for you. Whose directive did you follow?

God forgive me, I was at one point tempted to recommend a referral of the matter to another facility but since I remembered the words of the patient I decided to be involved in every sense in the case to ensure it was solved.

Me: That must be so kind of you. So, what did happen eventually?

Judith: I finally had to transfuse the patient regardless of the conflicting directives, and luckily he recovered. It was my joy to see the youngster back to his feet.

Judith: how did you reach making that decision given that you were violating someone’s wishes?

Judith: as I told you earlier, I have gained a lot of experience and developed my knowledge over the years in making such difficult situations. As a matter of fact, I was left with no option but to do what could result in the best outcome and it was what I did, I believe.

Me: Do you mind if I suggest another approach that one could use in such a situation to solve the dilemma?

Judith: No, I could appreciate that.

Me: The approach is called PEPSI. This is an acronym that stands for problem, ethic, perspectives of parties involved, solution and impact. While using this approach, it demands that you identify the dilemma which is the problem and relate it with ethics. Afterward, you can examine the perspectives and views of other parties and identify all possible solutions based on these facts. Finally, you can explore and implement the solution with the least negative impacts. So far, find whichever approach you used resulting in similar results and that was the best decision I could have made if I was in your shoes.

Judith: Thank you. This is such a good approach, and I am sure to use it whenever I am in a similar situation.

Me: so at this point, what will be your inspiration and message to the professional fraternity?

However, it is imperative to understand that knowledge is power and one should strive to develop their knowledge pool every day as it is the key to making an informed decision.
