AHCH’s operating definition of homelessness includes persons who spent the previous night 1) in an emergency shelter, 2) “on the streets” (in sites not intended for human habitation, including abondoned or public buildings, vehicles, or elsewhere out-of-doors, 3) in a motel, 4) “doubled up” temporarily in homes of friends or family members.

Albuquerque Health Care for the Homeless, Inc. marks 30 years of continued commitment to serving healthcare needs of people experiencing homelessness.

PROGRAMS & SERVICES AHCH’s extensive and assertive outreach and free site-based services reduce or eliminate barriers to care and address the health-related causes and consequences of homelessness. AHCH serves a diverse range of men, women, unaccompanied youth, children and families.

Organization-wide services include: • primary medical, dental, medication-assisted treatment, and specialty care; medical outreach services for children and their families; • ArtStreet, a free community-based arts and art therapy studio project; • harm reduction street outreach, including syringe exchange, auricular acupuncture detox, overdose prevention and training on administration of intranasal Narcan, and a peer education/ prevention project; • social and behavioral health continuum; o Social Transition and Resource Services (STARS) Resource Center & social services (comprehensive case management, social work and client advocacy) o integrated behavioral health program, including crisis, individual and group counseling and therapy & brief interventions o navigation, linkages, and follow-up to African Americans who are new patients that are most out-of-reach and most vulnerable to poor health and continued homelessness, due to lack of access to care via the Pathways to a Healthy Bernalillo County Program o continuous support services and care coordination at mixed-use, mixed income affordable housing projects, including set-asides for persons with serious mental illness or co-occurring substance abuse disorders who have experienced homelessness or near-homelessness o supportive case management services tied to permanent supportive housing for up to 80 people exiting Metropolitan Detention Center o intensive case management to 40 individuals identified as those most vulnerable via Albuquerque Heading Home Initiative o housing placement and rental assistance (80 units over a 12-month period) so that individuals can access affordable housing. People experiencing homelessness enter the AHCH continuum at any point or through any program and utilize a free package of services and resources according to their individual needs.

This report represents a new era for our work. In these pages you will find information extending over two fiscal years from FY2012-2013 and FY2013-2014. You may recall that this stretch of time ushered in the reality and implementation of the Affordable Care Act -- a monumental shift in health coverage resources that were never available historically to most people experiencing homelessness. We prepared to break ground on an expanded health center campus. The additional 8,600 square feet will allow us to serve at least 500 more people each year and create community space for our partners to work with us on-site and side-by-side in cross-organizational, integrated teams. We achieved Level II Patient-Centered Medical Home recognition from the National Committee on Quality Assurance, further codifying and solidifying our model of care. The Health Resources and Services Administration of the US Department of Health & Human Services conducted a stellar review of our Federally Qualified Health Center that scored us in the 95th percentile of health centers nationwide, validating our motto that we must be impeccable in order to remain bold and innovative. Also during this time, Jenny Metzler returned to the role of Executive Director and her work with us to build a strong and diverse board of directors, a committed and highly skilled staff, and strong alliances with both likely and less common partners to find solutions to homelessness. Among the most important indicators of our growth and successful navigation through a rapidly changing environment, are the unprecedented increases in our philanthropic support. You will find yourselves in these pages as well. With your belief in our mission and sustaining contributions to our work, we know that, together we do make a difference. Though a $3.65 million capital grant from the Affordable Care Act, Albuquerque Health Care for the Homeless completed construction of a 8,600 square foot Resource Center on the Northeast corner of 1st and Mountain. In addition, the existing campus houses our social services teams - client advocates, housing specialists, case managers – as well as Social Security Outreach and Access to Resources-trained staff, and space for medical and dental triage from clinicians working across the street in existing space.

ArtStreet, a program of Albuquerque Health Care for the Homeless, is an arts program which provides a continuum of art, art studio and art therapy services. ArtStreet allows artists who are experiencing homelessness a safe place to be creative and produce their work. Free of charge, the studio is open twice a week and artists can choose to sell their art at an AHCH art selling table or art exhibits that take place at various venues throughout the year.

LEGACY GIVING has made a lasting impact in the lives of people experiencing homelessness today. A long-time donor, who wishes to remain anonymous, succumbed to a chronic health condition. Danette Webb also left a legacy this year. Her life was tragically taken much too soon. Even in their deaths, they made a lasting mark in ending homelessness. Danette made giving back a large part of her life. She loved her dogs, ArtStreet/AHCH & Noon Day where she volunteered for more than 20 years of her life. “Dannette wanted the world to love others... she gives back and offers hope for a better life.”
