RESUMO Este artigo propõe uma Reflexão Teórica e Conceptual acerca da intervenção do enfermeiro na gestão do risco clínico nas Unidades de Cuidados Continuados Integrados (UCCI). Os objectivos prendem-se com a identificação e a fundamentação das intervenções do enfermeiro neste âmbito. No enquadramento teórico abordam-se o Modelo de Adaptação de Roy (MAR), de Callista Roy, o erro em enfermagem, a segurança do cliente, as Unidades de Cuidados Continuados Integrados e o processo de gestão do risco clínico. A possibilidade do enfermeiro assumir cargos de gestão nas Unidades de Cuidados Continuados Integrados – resposta recente do sistema de saúde português – é uma oportunidade para amplificar a sua área de intervenção, valorizando-se ainda mais a profissão. Utilizando o processo de gestão do risco clínico, o enfermeiro contribui para a prevenção de infecções, prevenção de quedas, prevenção de úlceras de pressão e de erros medicamentosos, assim como avaliação do risco de quedas e de úlceras de pressão. Concluiu-se que o papel do enfermeiro é fundamental na supervisão e monitorização de todas as etapas do processo de gestão do risco clínico, sendo um mediador da comunicação e um elo de ligação entre todos os intervenientes, promovendo a excelência da qualidade dos cuidados de enfermagem prestados nas UCCI. ABSTRACT This paper exposes a theoretical and conceptual reflecting about nurse care intervention and clinical risk management in Integrated Continuous Care Units. The aims were to identify and ground nurse interventions on risk management under the scope of the National Integrated Healthcare Network – Units (Portugal). Within the nursing care topic, the Roy Adaptation Model, by Callista Roy, was focused, as it systematically addresses the outcomes of care, subscribing with the objectives of clinical risk management. The topics of error in nursing and patient safety were developed to understand the importance of the theme under the nursing intervention sphere. The National Integrated Healthcare Network - Units and the process of clinical risk management were approached in order to understand their organization and the context in which these interventions take place. The possibility of a nurse to assume a management position in the Integrated Continuous Care Units, a recent development in the Portuguese national healthcare system, opens an area of opportunity and visibility for their intervention. The clinical risk management process can be applied in the various quality indicators, such as, prevention of falling or prevention of pressure ulcer, and their evaluation, as well as in the prevention of medication errors and prevention of infection. Concluding, the nurse role in the implementation, supervision and monitoring of all stages of the clinical risk management process assumes a crucial and unique importance. The nurse has the potential to be the link between all of the actors in this process, promoting excellence in the quality of healthcare on the Integrated Continuous Care Units.
