O

n July 5, the National Health Service entered its 60th year and set the pundits mulling over its past and pondering its future.

That day in 1948 was the pinnacle of a hugely ambitious plan, set out by health secretary Aneurin Bevan, to bring good healthcare to all. It was presented as a national treasure, the great white hope and a model to which every nation should aspire. For the first time, hospitals, doctors, nurses, pharmacists, opticians and dentists were brought together under one umbrella organisation to provide services free for all at the point of delivery, based on clinical need and not on the ability to pay. But, as the NHS approaches 60, what now for an organisation that is now the fifth largest employer in the world? We consider on page 45 the future of progressive healthcare and the changes the NHS will have to make if it is to stay true to its core principles. As the July 31 deadline looms for payment of the second tax instalment of the calendar year, we talk businesses through the important dates to remember in the tax timetable. See page 21. ndrew Warbrick has been appointed thyssenkrupp Access’s managing director. Andrew joined the firm in 2015 as Regional Sales Manager for the UK and Ireland and took on additional responsibility for the business in the Middle East and South East Asia in July 2016. His career has covered a number of different sectors, including a significant amount of time working in the elevator industry.

Josh Boggi, who bought a wheelchairfriendly Stiltz Trio Home Lift in 2015, will act as a spokesperson for the domestic lift manufacturer and represent the business at both trade and consumer events. Josh served in Afghanistan and was injured in a bomb blast in 2010 which resulted in him losing both legs and his right arm. He was later fitted with life-changing prosthetic limbs and is now a professional sportsman. Josh, whose motto is ‘Life Goes On’, was presented with the Overcoming Adversity award by cycling legend Sir Bradley Wiggins as part of The Sun newspaper’s Annual Military Awards in 2013 after cycling 420 miles on a hand bike from Paris to London to raise money for injured comrades. One year later, he represented the United Kingdom Armed Forces Team in the first Invictus Games in London - an international Paralympic-style, multi-sport event devised by Prince Harry for wounded, injured or sick armed services personnel. Josh won bronze in cycling. At the 2016 Invictus Games, he won two gold medals and two silver medals in cycling and indoor rowing. Josh has been training for the Race Across America (RAM) – which has been dubbed the world’s most grueling bicycle race – where he as well as seven others will represent the Help for Heroes Cycling team. Racers must cycle 3,089 miles across 12 states and climb over 180,000 vertical feet. Josh said: “I am delighted to have become a brand ambassador for Stiltz Lifts. obility start-up Staels Design won £100,000 in the recent 10th Scottish EDGE final for their design, the WheelAir® cooling wheelchair cushion.

Glasgow-based Staels Design was one of only two companies that were given the huge sum of £100,000 from the Scottish Edge panel within the main Edge category. Impressed by founder Corien Staels’ pitch, which they described as ‘exceptional’, judges awarded her £3,000 more than she applied for. The company originally asked for £97,000 to start the manufacturing of WheelAir® and expand the team.

Presenteeism is the new absenteeism Global office solutions brand, Fellowes, has launched new research urging business leaders to tackle the growing problem of ‘presenteeism’ across Britain.

T

he study found more than half (52%) of UK workers are currently going to work when their performance is negatively affected by work-related health issues – and a third (34%) of workers have admitted they have even considered moving jobs due to the negative impact of their work environment on their health – the highest percentage across Europe. Despite their best intentions, employees coming into work when sick are contributing to a rising trend of ‘presenteeism’ across the UK. When a worker is present but not able to perform their function properly, it compromises their productivity. With most employees continuing to work at sub-par levels rather than taking days off to recover, this also prolongs the effect of illness. Subsequently, businesses are experiencing a detrimental knock-on impact on the quality and volume of work produced, with a further impact on overall business performance. The research shows that Brits are amongst the worst affected in Europe, with UK employees suffering regularly from backache (34%), neck

GROW YOUR BUSINESS: APPRENTICESHIPS With so much political and financial uncertainty across the world, businesses are wondering how best to invest in their future. For years, traditional apprenticeships have been associated with trades such as mechanics, engineering and plumbing, and while modern notions have redefined these apprenticeships, opportunities for apprenticeships in other industries and sectors have emerged.

C

ompanies who offer modern apprenticeships are becoming increasingly attractive to the young workforce who may well be the future leaders in industry, so what are some of the incentives in offering these roles? Apprenticeships can tackle skill shortages, provide learning in the workplace (minimising disruption and maximising impact) and can help build the future workforce with the required skills. The government are keen to target and solve the issues of unemployment and skill shortages through apprenticeships – having promised three million apprentices by 2020. To fund this, a new apprenticeship levy has been

introduced. The annual bill of above £3m is footed by English businesses and is defined as employee earnings subject to Class 1 secondary NICs. Making up 0.5% of the annual pay bill, all employers receive £15,000 annually to be offset against the bill, meaning an annual pay bill for employers of £3m or less will pay no levy. Collected monthly by HM Revenue and Customs through Pay as You Earn (PAYE), English employers can then use the digital site to pay for apprenticeship training for apprentices who work for at least 50% of their time, which will be limited in funding bands. The monies will then be taken from the account. Non-levy paying businesses are entitled to other support.

However, while this is a great move forward in developing a new culture and generation of apprentices, it has hit a bump in the road. While still committed, the government has admitted it will revisit the distribution of funding meaning some companies will not receive money until 2018. Despite this, the original initiative still has its place in bringing new apprentices through, and should not dissuade you from taking on a new member of staff in the shape of an apprentice. The cost advantage for both employer and apprentice is still high for both parties. At its simplest, employers can train highly skilled employees-onthe job investing in the company’s future, while apprentices receive job training whilst on a wage and avoid racking up huge university or college debts. Wannabe apprentices see the opportunity as a good route into 

Qualified professionals can come at a costly price too, meaning apprenticeships can often be better value for an employer firstly in the short term, and in turn the long term. In England, the government will provide 100% of the funding for apprentices aged 16-18 for nonlevy paying companies, requiring employers to only pay at least the minimum wage, and in cases of apprentices aged 19 or over, employers contribute 10% and pay the minimum wage by law. For companies who employ apprentices between 16-24 and have fewer than 50 employees, they may be applicable for a National Apprenticeship Service grant of £1000 to be used for up to five apprentices. If you are providing formal educational training as part of the apprenticeship, additional funding may be available. By receiving funding for each apprentice you employ, further money can be saved in the recruitment process. Evidence has been found to suggest that your financial returns can spike with the presence of an apprentice. Companies who employ apprentices are found to be favoured by consumers according to researched gathered by the National Apprenticeship Service, leading to increased sales of service or product and in turn bolstering the books. The benefits of apprenticeships go beyond the apprentice and the employer though. The associated advantages of upskilling and retraining available to existing employees off the back of hiring an apprentice can be great for them and the business overall. On-the-

job training may also be available to current staff. apprenticeship, staff turnover can be reduced as loyalty and appreciation to the company grows. The new generation’s new ideas are not only useful to staff facing issues, but the company overall. The world is moving faster than ever, and with millennials keeping on top of the constant change better than anyone else, it would make sense to harness that knowhow and utilise new skills. On top of everything else, apprentices tend to stay within and progress in a company, but perhaps most important of all, New Apprenticeship Standards are more job related than any other type of learning. While the benefits of on-the-job training may seem obvious, compared to classroom based learning, employers generally look on experience more favourably. So even if it is a modern apprenticeship that splits time between the company floor and additional course work, apprenticeship graduates may well end up becoming more employable than the university graduate whose CV is sitting on your desk. 

Mencap backs apprentices

O

n top of ensuring the younger generation have a valuable route into industry and employment, leading learning disability charity Mencap are calling for the government and businesses to employ the use of apprenticeships to help people with learning disabilities into work. Mencap point to recent research published by the Health and Social Care Information that reveals fewer than 6% of adults with a known learning disability are in paid employment to highlight the need to find avenues for people with learning disabilities to become employed. In 2016, the government committed to lowering the minimum standards required in maths and English for people with learning disabilities wishing

to undertake an apprenticeship, but Mencap state there are still barriers to overcome. The process of gaining employment can be daunting for those with learning disabilities, from the beginning with complicated application forms, to negative attitudes from employers to regimented interviews. And if they do get the job, they may then be faced with a lack of on-the-job training and opportunities to climb the ladder. Mencap say, “by ensuring people with a learning disability are able to access apprenticeships, it will provide a route into work better suited to people with a learning disability where they can demonstrate their skills”. According to government data, in 2014/15 fewer than 1% of apprentices declared a learning disability.

 INCOME TAX

Starting with income tax, it’s clear that the tax you pay is going to depend on the profits made by your business, and for established businesses will be made in two estimated instalments: January 31 of the tax year you’re in (ie up to 5 April) and July 31 immediately following, with a balancing payment on 31 January after the tax year, at the same time as the return is due. The instalments are based on your previous year’s return and set at half of last year’s liability. If you know that’s not going to be appropriate (for example, your income has risen or fallen) then you should contact HMRC using a Form SA303 to revise the amount up or down – that won’t affect the interest due/payable on over or underpayments once the position’s finalised, but it will stop HMRC chasing in the meantime. Bear in mind that HMRC don’t send out payment reminders any more either, so you need to make sure it’s in your diary.

 CORPORATION TAX If you’ve incorporated your business then the first element of tax will be on any profits retained in the business. The rate for small businesses (and since April 2015, larger businesses too) is 20%. The tax return is due within a year of your accounting period end (accounts date), but the tax is due before that, nine months and one day from the accounts date. However, since the Companies House annual return filing deadline is 9 months from the accounts date, you should have all the information to hand by then, and there’s no reason not to finalise your tax return around the same time.

HMRC only have 12 months from the date they receive a return to enquire into it, so the sooner you submit the sooner you can be sure they won’t investigate it. There’s no advantage to paying the tax early though, as HMRC’s paid interest rate isn’t particularly high.

 VALUE ADDED TAX (VAT) If you’re registered for VAT then you’ll need to keep up to date with the returns for whichever scheme (say Cash, Flat Rate, Retail etc.) you use. Remember you’ll have to register if your ‘taxable supplies’ exceeded £83,000 in the year to date, or will in the next 30 days alone. Taxable supplies are broadly turnover (not profit). VAT returns and payments all need to be made online. VAT returns are usually made quarterly based on the months HMRC will have given you, although if your turnover is under £1.35m you may be able to use the annual accounting scheme, which requires only one return per year. Whichever timetable you are under, payments will need to be made at least quarterly, to the precise amount if you’ve submitted a return, or as an estimate if you only do one return a year. Any refunds can only be triggered by a return, so if your business mostly makes zero-rated supplies and is in an overall net repayment position then the annual scheme (triggering only one refund a year) won’t be for you. The normal quarterly accounting for VAT needs your return and VAT payment to be with HMRC within one month and seven days of your quarter end. What you put into the return will depend on whether you account for VAT under the standard scheme

or using the Flat Rate Scheme, for businesses turning over less than £150,000 per year. Under full accounting, you will need to document all taxable (VATable) supplies and all purchases and net off the VAT before paying the difference over to HMRC. Under the Flat Rate scheme, you charge VAT normally to your customers, but then have to pass only a percentage of that to HMRC (see http://bit.ly/1ksxRzT) as there’s no deduction for tax suffered on purchases (the only exception being for assets costing more than £2,000 which you can account for on a case by case basis). For retailing pharmaceuticals, medical goods, cosmetics or toiletries the flat rate is 8%. Working out if the flat rate scheme is right for you will involve finding out whether there is a scheme for your business activity, and if so whether the rate actually makes economic sense.

 PAYE If you have employees then you’ll need to apply Real Time Information (RTI) reporting. RTI is in principle a great way to simplify and streamline PAYE accounting, and for large undertakings with fully computerised payroll and payments processes it may well work just fine. But for smaller concerns, you need to be properly up to speed on the possible pitfalls and the filing deadlines which apply. It may well make sense to appoint a payroll agent to do the actual filing using a powerful payroll package that they can justify buying, but remember you’ll still need to get all the payroll information to them in line with their timetables for processing. RTI works on the basis of FPS (Full Payment Submissions) which have to be made every time you pay employees, and include details of your entire workforce and every change since the last FPS (ie the latest payroll) and EPS (Employers Payment Summary) filings, which cover claims for statutory payments such as sick pay or maternity pay, and any ‘nil return’ months. The basic rule is that an FPS has to be submitted every time some or all employees are paid. If you haven’t made any salary payments at all from sixth of one month to fifth of the next then you’ll need to file an EPS ‘nil return’ to keep the system ticking over.

 TO CONCLUDE HMRC is changing from paper to electronic communication and returns. It’s sending fewer reminders which makes it even more important to diarise key dates. Remember – missing a deadline triggers automatic penalties and interest. Further, it could lead to an investigation.

How long has Karma been established? Since 1999.

What does your job involve? A bit of everything as we are a relatively small company. One day I may be helping our warehouse team unload a container of wheelchairs and powerchairs or answering queries from retailers then the following day I can be on the road visiting some of our retailers and demonstrating our latest products. How has the company grown? Since Karma Mobility was founded by my father Fred in 1999, we have grown gradually as a result of providing great

quality products and providing excellent customer service. We currently have an extensive network of retailers across the UK where Karma Mobility is recognised as one of the leading manufacturers and suppliers of lightweight wheelchairs and powerchairs. I believe in supporting our retailers as much as possible as without them we would not have a business and this support includes point of sale material, brochures and next day delivery on most orders. What do you enjoy most about your role? The variety. I get involved in many aspects of the business which ensures no two days are ever the same. Over the years, we have built great relationships with many of our retailers and I certainly consider many of them to be friends. It may sound like a cliché, but seeing what a difference our products can make to someone’s day to

They have always invested extremely heavily in research and development and there are always new products or enhancements to existing products in the pipeline which is great for us and our retailers as it ensures they always have new solutions to offer their customers. We prefer to announce them when the products are available and not too far in advance to ensure there is never a delay in supplying the latest products launched to our retailers. Outside of work, what are your personal interests? I enjoy spending time with my wife and children. I try to enjoy a round of golf (it’s a cruel game) now and then and walking my wife’s dog which wasn’t supposed to be anything to do with me.

Research reveals need for more inclusive working culture Research has revealed that 39% of workers admit that they have experienced a form of bias in the workplace or when applying for a job.

T

he Badenoch & Clark research report, ‘Inspiring Inclusion in the Workplace,’ also found that one in five (20%) employees have taken action to hide their age, disability, social background or sexuality in the workplace or when applying for a job.

“Businesses need to commit to living and breathing diversity and inclusion throughout the entire employee lifecycle and in everything they do – every strategy, every hire, every decision. Ultimately, they should become inclusive by instinct.”

In addition, 22% think that their company does not embrace diversity and inclusion at any level, 29% have never read their company’s diversity and inclusion policy and a worrying 11% said that their company does not even have one.

Commenting on the research findings, James Nazroo, Professor of Sociology, University of Manchester and Director of the ESRC Centre on Dynamics of Ethnicity, added: “This report is extremely valuable in highlighting ongoing problems in making workplaces truly inclusive and in attempting to minimise the negative effects of discrimination on a diverse workforce.

This not only impacts on career progression, but also profoundly on the health and wellbeing of employees.

Despite this, some workers were positive about inclusion in the workplace. Almost half think that their organisation embraces diversity and inclusion at board level (46%) and at management level (43%). 86% also agree that their organisation employs a broad range of people from all social backgrounds.

Nicola Linkleter, President of Professional Staffing, said: “Whilst it’s great to see that employees are, in some cases, positive about the level of diversity and inclusivity in UK organisations, there is still a long way to go. Each worker that has experienced bias is one too many, and employees will only ever flourish if they feel they can truly be themselves at work.

“This not only impacts on career progression, but also profoundly on the health and wellbeing of employees. Employers in both the private and public sectors need to really engage in these issues and talk about the problems of discrimination in relation to age, gender, ethnicity and social background, not only to meet their legal responsibilities, but also to improve their workplace cultures and the experiences of their employees.”

When, where and by whom was All Terrain Wheelchairs established?

The company was established in September 2007 by myself and we will be celebrating our 10th anniversary this year. We are based in Telford, Shropshire, which is an ideal location as it enables us to reach the whole of the UK from the midlands.

What was the inspiration behind All Terrain Wheelchairs?

As soon as I saw the range of All Terrain Wheelchairs I knew it was a great product, so when the opportunity arose to take on the sole distribution in the UK I jumped at the chance. I knew there was a niche in the market for such a unique product and that people would be interested in such a bespoke solution. More and more people want to remain independent and be able to access the great outdoors. Children and young people in particular do not want to be confined to sanitised pavements, but instead want to enjoy life alongside their peers. Active people, such as ex-military, want to remain active despite having to use a powerchair and to have access to the places they have previously enjoyed. I wanted to be able to offer a solution that would enable them to regain their independence. People with an agricultural background also have a keen interest in our range of powerchairs as they allow them to keep working in a farming/outdoor environment.

How many people do you employ over which sites?

We currently employ three people (apart from myself), all

Being a smaller/family type of business, myself and the rest of the team remain very close to clients, often building relationships with clients and their families. We firmly believe in going the extra mile to ensure clients receive a bespoke solution that meets all their needs. We also pride ourselves on our after sales service, offering speedy turnaround on servicing etc. often being able to offer a next day service.

How would you describe your product range to trade customers?

The powerchairs we sell are a robust, bespoke built product, offering full customisation to the individual. The chairs are also stylish, colourful and up to date with the very latest technology. The products are constantly evolving to meet the demands of the market and the individual.

Have you recognised any specific turning points in the business in recent years?

We have seen a steady growth in sales throughout our time in business.

What sets All Terrain Wheelchairs apart from its competitors?

We believe our service stands out above other manufacturers and our order turnaround time is also much quicker. Our products are unique to us, as we are the sole importer for the UK.

Which exhibitions do you attend and which work best for you?

We attend Kidz to Adultz exhibitions, as there is a very specific interest from children and young people. We also attend The Mobility Roadshow and Naidex and are looking to do so again in 2018. We also get involved with charities, most recently Action Duchenne, sponsoring and attending their national conference.

What’s next on the horizon for All Terrain Wheelchairs?

We are currently looking into more sponsorship opportunities with national charities, particularly MS. On the exhibition front we are looking at focussing on agricultural shows in 2018, notably The Royal Cornwall Show and The Royal Welsh, as we have a strong agricultural client base. It is also our aim to increase our network of UK retailers.

Team building: The case against

rawing attention drawn to oneself through role play or other so-called teambuilding activities is a source of dread for team members who may lack the confidence to see it through without blushing and breaking sweat. For some employees, the embarrassment and humiliation associated with teambuilding activities actually drive a wedge between them and their colleagues and employer, rather than bringing them together and fostering engagement and team loyalty. When forced to take part in horseplay in the name of team building, bosses can be seen to be looking down on employees by suggesting that they need to participate in silly games in order to do their job to the best of their ability. For author Kate Mercer, co-founder of the Leaders Lab consultancy, the term ‘team building’ conjures up “ghastly visions of kneeling on the floor playing bongo drums in a conference room or building Lego constructions against the clock in competition with other departments.” According to Mercer in her book, A Buzz In the Building: How to Build and Lead a Brilliant Organisation, the act of team building simply doesn’t work. She writes: “There’s a very large learning gap between doing activities on Dartmoor with your colleagues and working with the same bunch of people back at the office. It’s possible to draw parallels and bring out useful learning points, but it takes skilled facilitation to do this effectively and it’s especially difficult to ensure that people take the learning back into the workplace.” It’s important to draw the distinction between socialising and entertainment designed to bond teams she insists. Forcing employees to socialise with people

FAILURES they would rather simply see as colleagues can create hostility and tensions that prevent teams working well for customers and stakeholders. “There’s nothing worse than to be ‘forced’ to be sociable, especially if your job is on the line,” says Kate Mercer. “You have an obligation to work effectively with your colleagues, but it should never be obligatory to play with them too.” The best team-building activities, she says, are the ones that firms should be working on anyway, such as creating strategy for the short, medium and long-term future or clarifying roles and accountability. According to a survey carried out by Vodafone UK and YouGov, most employees think that doing more company teambuilding events would not help improve how they work. The study suggests workers feel that some organised teambuilding activities can be a waste of time, and at worst, are toe-curlingly embarrassing. Workers would prefer being able to communicate with each other better at work rather than being forced to build rapport with their co-workers by sharing adrenaline experiences or performing ‘trust’ exercises. The research among more than 1,000 British employees uncovered some excruciating examples of awkward and silly team-building activities, including enduring massages from colleagues, holding lingerie parties, and eating crickets as part of a ‘bush tucker trial’ style event. While the majority of workers surveyed (66%) have been made to do some form of teambuilding activity, more than half (54%) don’t feel that doing more would help them work better with their colleagues. The research suggests that people place more value on open, collaborative and flexible

t was the pinnacle of a hugely ambitious plan to bring good healthcare to all. It was based on three core principles: that it met the needs of everyone, free at the point of delivery and based on clinical need, not the ability to pay. From its humble beginnings the NHS is now the fifth largest employer in the world behind major corporates like McDonalds and Wal-Mart. Interestingly these two are the only non state run companies in the top five. With a workforce of 1.7 million and varying standards around the country, it’s an easy target for criticism. In 2015 the Office for National Statistics stated

that the UK population would increase beyond 70 million in 12 years. To put this into context by 2039 the UK will have increased its population by the same number as the whole of Sweden - 9.7 million. Also, at this point, the number of people over 80 is projected to be one in 12. All very interesting I thought, until I started to look at what it looks like for me in 20 years’ time. Currently, the number of patients above the waiting time of 18 weeks for routine treatment in hospital is the highest it has been since 2008. 

Then there is the talk of rationing funds because some trusts repeatedly mismanage their budgets. This has been levelled at 14 areas of England to date but the details are unclear as to the amount of overspend. Every year the cost of the NHS goes up because of rising demand, and the answer each year is put more money in. If it were a Wal–Mart it would build another branch or online distribution to meet the demand. The alternative is to manage the demand by either withdrawing from sale or placing a premium on the product or service in question. However because the service has an emotional attachment the decision whatever the outcome will be an unpopular one. The fact remains that the NHS as it is now is not sustainable and cannot continue. The future of progressive healthcare lies with smaller businesses that can react to needs more quickly without the ties of red tape. Small businesses can grow more quickly with today’s technology and collaboration; funding and expertise doesn’t have to be in the same country. Also there are similar changes in population and demographics happening all over the world in countries without an NHS. An ageing population requires more medication, more equipment and more care. A local pharmacy may become the centre of the community as regards healthcare, so make sure you create a good working relationship together. Whether you are an occupational therapist, a small healthcare business or other specialist, ensure your local pharmacy is aware of you. Aware of exactly

what you can provide and of how much your costs are. By sharing vital information with each other, local healthcare and the community as a whole become more connected. At the same time, pressure is relieved where it is strongest – such as A&E departments and physiotherapy waiting lists. Most people believe in and support the NHS but when it comes to their own health, or that of their family or friends, views may change if something is available locally at a reasonable cost, especially when it avoids long waiting times, particularly when the person suffering is a close family member or friend. Despite the relatively small percentage

that use only private healthcare (between 8-10% depending on your source) the number of people dipping in and out of the private sector is rising. Pharmacies can work with local healthcare providers such as occupational health and other specialists by signposting and advising different treatments and care. To many people the thought of the ‘robot doctor’ is something out of a sci-fi movie, but it’s already here and forming the future of healthcare. With the numbers of doctors and nurses becoming less freely available there is no alternative other than to automate the health process. The only way in which to do this is to make use of the data that is collected everyday and if you want to be part of it review the way in which you collect and use data. Data and the way in which we use it will dramatically change the way in which healthcare is delivered. This data is collected in several different ways, but currently it’s held in silos of different health bodies: hospital, dentist, optician etc. As each generation passes the movable retirement finishing line, their ability to interact and use smart technology increases. This is increasingly evident in the baby boomers that are now more comfortable with technology than they were five years ago. Much of this is to do with the way in which we communicate with our children and within our businesses. This shift allows the use of more digital technology and more

preventative treatment being provided remotely. Smartphone apps and wristband technology already tell us how healthy the food we eat is and track your activity. Currently this technology enables the wearer to take better care of themselves using real time information on key indicators of health. The use of cloud technology cuts down the need for resource that would be involved with a hospital visit by just logging in and connecting to the local surgery. Early diagnosis via these means of data capture, allows certain conditions presenting themselves, to have earlier treatment and potentially results in fewer hospital admissions. If we step back and look at it from a data view, the lessons learned and practiced by businesses can be transferred to health. Amazon use terms and phrases such as ‘frequently bought together’ and ‘customers who bought this item also bought’. The same could be said of patients in the future having similar characteristics that are fed back via the wristband. The ‘Internet of Things’ was a phrase I heard recently about devices talking to one another. This science is no longer fiction but more real than I had imagined or even possibly

conceived. This is where the internet connects multiple devices which can talk to each other and perform different activities. Put another way, my future self with my wristband monitor could talk to my phone or TV to advise me to take my medication. In cases such as diabetes where trials are in progress, this application could prove life saving to people living alone with the condition. Today we see machines being used in the home such as Alexa, which is basically a device that responds to your voice. However it’s doing more than that it’s learning about you and the way in which you live. As it learns its responses are more informed and accuracy improves. This device in a different scenario of an elderly person at home alone could be a companion and an aid. Whilst this shouldn’t be looked upon as a replacement for human contact it does have the ability to connect the individual with the outside world. Today’s lifestyle is made up of much lower levels of physical activity, weight gain, poor sleep and eyestrain. Add to that the age to which people are living then the way in which healthcare is delivered has to change to meet the growing future demand. By having a smart wristband that is constantly updated or a means to plug in and upload

medical information remotely, highlights a level of possible risk. The data that is collected would build showing trends and patterns, which in turn builds predictive outcomes around certain conditions. My future self will have messages sent to my smart device regarding my state of health and the plan progress to date. There are more mobile devices in use than there are people on the planet making it the most accessible medium. Carers will be supported by an audible device, which will respond and update them as to how my activity has been, in their absence. I will see the doctor after I have been through online remote screening and a screen-to-screen conversation with the registered nurse. Annual health checks will be conducted remotely and dependent upon the results there may or may not be a need for a visit to the surgery or hospital. There will ultimately be a shift in the management of health where the client will be expected to take a more active role in the maintenance of their own health. The ‘Nanny State’ as the NHS was named, will change out of necessity if it is to maintain its core principles, but it can only do this through the use of technology and the help of small businesses. 
