it’s been At leAst A few decAdes since All of A PAtient’s heAlthcAre needs were met by A single ProfessionAl. gone are the days when the family doctor drove from clinic to clinic to check up on their patients. driving around town during busy office hours making “rounds” to follow up in person on hospitalized patients went out with the invention of “hospitalists”. Physicians who provided home health care, Palliative care, hospice and the use of a caregiver service collaborative resulted in not only more precise care tailored to specific patient needs, but it also increased greatly the prospect of patients receiving a more practical, and seamless continuity of care. home health care often follows hospitalization or transition to home from a rehabilitative stay in a skilled nursing facility. it’s usually less expensive, more convenient, and an appropriate level of care in the cycle of continuity. the goal of home health care is to treat an illness or injury. home health care helps patients get better, regain their independence, and become as self-sufficient as possible. in general, home health care includes part-time or intermittent skilled nursing care, and other skilled care services such as physical occupational and/or speech therapy. services may also include medical social services or

assistance from a home health aide. most often, home health care agencies coordinate their plan of care based on what a patient’s doctor orders. home health can include such services as wound care for pressure sores or surgical wounds; injections, intravenous or nutritional therapy; patient and caregiver education; and the monitoring of serious illness or unstable health status. even with this level of care, some patients require additional services specifically directed at improving the quality of life. Palliative care as it’s called focuses on pain and symptom management, and depending on a patient’s illness or stage of illness, can include bereavement counseling and monthly health assessments. Although the concept of hospice is an ancient one dating back to the middles ages, the hospice philosophy is a major component of the modern day attempt at providing a seamless continuity of care cycle. the hospice approach is based on the idea that all people approaching the end of their life should have access to services to support their physical, emotional and spiritual needs. hospice care is provided in a patient’s home, skilled nursing facility, hospital, residential care facility or an assisted living environment. under the fully covered hospice medicare benefit, hospice and palliative care services are restricted to patients who have a general prognosis of six months or fewer left of life if their disease follows its natural course and those who have chosen to forego therapies with a curative intent. hospice is designed to provide comprehensive interdisciplinary team-based palliative or comfort care including relief from pain. hospice palliative care becomes appropriate when patients and their families decide to forego the therapies intended to cure in order to focus on maximizing comfort and quality of remaining life. All levels of care are provided by a team that supports the patient and family throughout the process and includes such professional services as spiritual and bereavement counseling. Although aging is inevitable, most of us have a choice in how we age which also impacts how our skin will age. As a holistic practitioner, aging well and having an age-management program for my skin has been a personal goal, so i am delighted to have an opportunity to share what i’ve learned with you!

dO YOu TAkE YOuR SkiN FOR gRANTEd? it’s probably a truism that most of us don’t give much thought to our skin until it starts showing signs of aging or if we have sustained an injury to the skin. yet the skin plays such a vital role in influencing the quality of our life. for example, it literally separates our insides from the outside world. the skin is also the body’s largest organ, acting as a protective barrier that defends us from harmful environmental factors that accelerate aging such as pollution, overexposure to dnA-damaging uV rays and infection. the skin also informs us about our world by means of its embedded sensory receptors. so what can we do to support our skin? let’s begin by reviewing how and why the skin ages.

AgiNg iS A PROCESS OF dEhYdRATiON Apparently after age 25 the processes associated with skin renewal begin to slow down. this slow-down gradually reduces circulation of body fluids like lymph and blood which are vital for tissue function and repair. research has shown that one of the primary culprits in aging skin is dehydration at the cellular level. dehydration leads to fine lines and wrinkles; loss of elasticity, luster and plumpness; and a reduction in collagen formation. According to skin care expert, rhonda Allison, the following factors can cause the skin to become dehydrated: intrinsic Factors • genetics. • Antihistamines. • medications. • diet/smoking. • Antibiotics. • hormones. • illness. Extrinsic Factors • sun exposure. • Alcohol-based cosmetics. • daily skin regimen (under-cleansing, overuse of scrubs, over-moisturizing). • clay-based masks.

RESTORiNg ThE SkiN fortunately much can be done to prevent and even reverse damaged skin. begin by eliminating the extrinsic factors in the list above. then consider the intrinsic factors you can change such as diet and smoking. sometimes improved diet and lifestyle changes can even help a person reduce their dependence on medication (speak to your doctor about this). Adequate sleep and stress management are also important. many non-invasive, corrective and restorative skin treatments are available today which address hyperpigmentation, inflammatory skin conditions, dehydrated skin, texture, and fine lines and wrinkles. do your research to find the procedure that makes you feel most comfortable. in addition to skin treatments there are some amazing topical cosmeceuticals rich in antioxidants and botanicals which can have a remarkable effect on the skin, diminishing and sometimes reversing damage. these include peptides, ferulic acid, vitamins c and e, curcumin, pumpkin, papaya, resveratrol, bromelain, green tea, growth factors, and l-lactic acid and a-lipoic acid (AlA). dr. doris day, a dermatologist at lenox hill hospital in new york city, recommends including the following high-antioxidant foods in your diet: fruits, vegetables, greens, nuts, seeds and avocados, while reducing your intake of red meat and dairy. she said, “We are really what we eat and it shows in your skin—and there are no shortcuts. … Your skin is a reflection of your overall general health. ...The healthier your skin is, the better it functions, the more it can help the rest of your body function; the healthier the rest of your body is, the healthier your skin is as well.” excellent words to live by! whAT iS ShAmE? to understand what shame is, it’s important to delineate between shame and guilt. guilt is when you feel you’ve done something wrong. shame is when you feel there’s something wrong with you!

Paul as a kid Paul’s parents split up when he was eight. His father seemed to quit everything in his life— work, family and himself. Paul’s mom found herself on welfare while Paul found himself on the wrong side of the tracks in his Midwestern town. Dad disappeared. Without his father’s guidance, Paul and his pals determined for themselves what was right and what was wrong. In her desperation to survive, Paul’s mom recklessly introduced a series of men into Paul’s life, causing him to witness his mother engaging in drink, drugs and sex. Paul also learned about abandonment, loss, and sexual abuse at the hands of one of these men.

where does shame come from? shame comes from the lessons we learn while growing up. shame develops over time as we’re told that what we’re doing, what we’re feeling, how we’re expressing ourselves or what our dreams are, is somehow wrong, unacceptable, “not what’s done in this house,” or “not what good little boys do.” but when we, as boys, continue to have those feelings—continue to want to behave like boys—express ourselves as we see fit or dream those unacceptable dreams, we eventually come to believe there must be something wrong with us. the boys then grow up.

Paul as an Adult The moment of his attack has haunted Paul for 50 years. He’s always felt he was somehow to blame—that something about him caused it to happen. That’s how boys process traumatic events like these. This shame caused him to quit on himself again and again, just like dad. He continually sabotaged relationships and wondered why anyone would want to be with such a worthless fraud. As a result of certain life experiences, we become men who believe there’s something wrong with us. we can become angry, depressed and lonely. we can become over-achievers, absent fathers, and unhappy men. tragically, all this pain is suffered because of the misguided belief, “there is something wrong with me.” All of this pain is caused by shame. by the time most men find their way to counseling or other forms of support, shame has taken a devastating toll on their lives and on the lives of the people they love and who love them. the good news is, men can shed the shame they’ve been carrying with the support of other men.

Paul with the men Paul kept his mask on for quite a while after joining his men’s group. He was in no hurry to let the men in on his dirty little secret—that he was a piece of crap as a man. The truth is, he suspected everyone could tell. One evening at a group session the men were asked to tell each other the one thing they did not want to reveal about themselves. The jig was up. It was time to tell the truth; so he finally shared his shame with the men. the fathering energy that comes from a coach or from other committed men participating in a men’s group has the power to help men make important changes in their lives— changes to become the best father, husband, son, brother, friend, co-worker, boss or mentor they can be.

that means wives, children, co-workers and neighbors are all positively affected by the work men do to shed the shame and to become the best men they can be.

Paul Today That terribly difficult moment during group proved to be a turning point for Paul in his journey to heal, to let go of his shame, and to start showing up as the man, father and husband he never thought he could be. Paul began to realize others feel shame just as he did, but for completely different reasons. Yet despite each man’s differing life details, the pain is the same—as is their need for the support of good and caring men, just like Paul.

how the Process works in individual coaching, in men’s groups and through experiences like men’s retreats, men’s lives can change dramatically. men discover that not only are they not alone in carrying their shame, but through strong and trusting relationships with other men they learn there’s actually nothing wrong with who they are and how they feel. in time, self-esteem increases and relationships throughout the man’s life improve. maybe you’re one of these men. maybe you have a man like this in your life. Perhaps it’s time to introduce that man to possibilities he may never have imagined—the possibilities that arise when men reach out and get the support of other men. Wayne M. you have seen the radiant, beautiful skin of a healthy infant or a child. his eyes sparkle and her skin is glowing. you know the pale face of someone who is ill, the wrinkles and dry skin of someone who smokes or has a body filled with toxins and poor nutrition. you have seen the face of someone who is stressed, tired or depressed.

every half inch square of skin contains 500,000 cells, 32 inches of blood vessels, 100 sweat glands, 21 inches of nerves, 15 oil glands and 230 sensory receptors.

ThE CRiTiCAL ROLE OF NuTRiTiON

the average person eats 29 pounds of french fries, 23 pounds of pizza, 24 pounds of ice cream and consumes 53 gallons of soda, 24 pounds of artificial sweeteners, 2.736 pounds of salt and 90,700 milligrams of caffeine per year. do we really think we can create health in such a toxic environment? dr. mark hyman, who has dedicated his career to identifying and addressing the root causes of chronic illness through an approach known as functional medicine, shares other average annual intake data: 632 pounds of dairy, 85 pounds of oil and 63 pounds of beef alone. do we really think we can have beautiful skin and hair while eating processed food, saturated fat and preservatives? many women experience hair loss. WHAT IS A PROSTHODONTIST?

Experts in the field undergo clinical training in aesthetics and cosmetics, crowns, bridges, veneers, inlays, complete and removable partial dentures, dental implants, temporomandibular (TMD) jaw joint problems, traumatic injuries to the mouth’s structures, congenital or birth anomalies to teeth, snoring, sleep disorders, oral cancer reconstruction and continuing care. “Prosthodontists are masters of complete oral rehabilitation and are dedicated to the highest standards of care in the restoration and replacement of teeth,” explained Jivraj, who completed his dental degree at the University of Manchester in England in 1993 and his advanced prosthodontic training at the University of Southern California. He completed a Master of Science in Education with a concentration in Medical Education at the Rossier School of Education at the University of Southern California. Jivraj is the co-author of Treatment Planning in Implant Dentistry, a book used worldwide by dental students, dentists and other specialists. He is also the former chairman of fixed prosthodontics and operative dentistry at USC School of Dentistry. “The American Dental Association recognizes nine dental specialties, and the ADA does not include cosmetic dentistry as a specialty,” Jivraj said. “Prosthodontists receive extensive training and experience in dental aesthetics and cosmetics during their graduate programs which currently last three years. The difference between a Prosthodontist and a Cosmetic dentist is the amount and time of training received. Cosmetic dentists receive training during seminars or a series of courses, but this is not a University accredited specialty program. Ultimately, a prosthodontist is best viewed as the architect of your dental project who has the vision of your final outcome—both the aesthetics of your smile and the improved function of your bite.

I enjoy the response after helping them regain their confidence.” He is currently working to establish a new facility in the Ventura County area. “This will be a state-of-the-art facility where there will be no compromise in technology so the patient can get treated efficiently and predictably.” In other efforts, “We are building educational programs for dentists that will allow us to train thousands of dentists around the world via the worldwide web.” Most importantly, “I am organizing my time so I can be there for the most important people in my life—my family,” said Jivraj, who has been married to his wife, Dee, for 15 years. They have two young children, Sara and Zain. “I have to thank them for allowing me to do what I do. It involves countless hours away from family and ultimately they are the ones who make the sacrifice.”

“the reality of having diabetes is that you need to live a disciplined life, a lifestyle that all people should be living who don’t even have diabetes,” said Amanda, who was diagnosed with type i diabetes 16 years ago at the age of 14. “i was born with the disease; i had all the signs of it growing up,” recalled Amanda, who was lethargic, had eyesight problems, always hungry, and looked anorexic. “it took 14 years to discover what was wrong.” today Amanda is a diabetic advocate and spokesperson as well as a professional performer. “i have been dancing since the age of two. i started with ballet, tap and jazz,” said Amanda, who was the youngest professional ballroom dance competitor in california—by the time she was 16. “i dance everything from hip hop to international dances from all over the world.” she is recognized mostly for her brazilian samba and the fine art of burlesque. “i combine burlesque with the art of international latin ballroom and Afro-latin dance which sets me apart from most dancers in the burlesque world,” said Amanda, who makes all her own stage costumes and is inspired by the styles and materials of Vaudeville, musicals and Ziegfeld follies. “having diabetes doesn’t need to be difficult or negative,” she said. “the hardest thing about it is change, and admitting to yourself and everyone around you that you have this disease. once you get past that, your diet will have to change and you will live happy and healthy.”

the difference between type i and type ii diabetes is type i is a virus. type ii is brought on by poor diet and lack of exercise. “with type ii the pancreas cannot produce enough insulin to filter out the amount of sugar consumed in the body. if left untreated it can lead to major complications and even death,” Amanda further explained. “most people today don’t know that they have diabetes or that they are pre-diabetic. it is so important to get tested twice a year and to take care of yourself. we have a fast-growing epidemic of type i and type ii in this country. type i can be caused by genetics—even more of a reason to take care of yourself if you want to have children.”

Amanda attributes her clean living lifestyle to her dietary intakes. “clean living and diet is something that will benefit everyone.” for instance, “i stay away from all processed foods with gmo. i try to eat organic. i mostly eat east indian food because of the spices that help regulate blood sugars. i also eat a mediterranean diet. i stay away from bread and rice because of the starches. i also eat spicy chilies because it helps to cut carbs and aids with digestion.” “Alcohol is off limits,” she further emphasized. “i have never consumed alcohol. i believe you don’t need to consume in moderation if you are diabetic—you just don’t need it.” Additionally, “it complicates blood sugar levels and aids in aging your body. in an effort to help others through her own experience, Amanda continues to use dance, modeling, fashion and the flashy costumes she creates to bring attention to the subject of diabetes. “i want to inspire and be an example to others showing that you can pursue your dreams and ambitions and live a healthy lifestyle no matter what ailments you have.” she credits her family and friends for her success so far in life. “i have always had a wonderful support system around me. i am blessed to have a wonderful mother who has always supported my dreams. i had amazing grandparents who introduced me to classic films and musicals which has been the inspiration to create my costumes and choreograph my shows. i have great friends that give lots of love.” most recently Amanda has embraced social networking on a grand level. Amanda spreads her message, with respect to her daily views on diabetes. “facebook has been a wonderful marketing tool to connect with people worldwide. i have met the

most amazing people from around the globe that i can truly consider friends. it is the loving messages they post every day that make me feel loved.” her mother, Venita Van slooten, said, “Amanda is a very resourceful young woman. she continually educates herself and is always researching different subject matters. As busy as Amanda can be, she still finds the time to help people with various issues, whether they be health-related or otherwise.” since her daughter was diagnosed at the age of 14, “she has and still faces difficult challenges with this disease, but it doesn’t stop her from living her life to the fullest,” her mom added. “the key for her has been the desire to use good common sense and keep things in the proper perspective—it is a balance of discipline and creativity. in short, she says you have to be true to yourself.” laughter is also a huge part of their lives. “Amanda has great comedic timing; she gets that from my dad. each day has been a pleasure to watch something new and exciting unfold in her life.”

The dance Continues

As far as her future is concerned, “i will continue to model, dance and speak out about diabetes,” Amanda said. she is also involved in an upcoming independent film, “so i would like to continue acting in the future.” in other endeavors, she is working with world-renowned sculptor david richardson on his first burlesque-inspired sculpture—in full bloom—which will appear in upcoming art shows around the country. “my dream is also to continue producing shows with my entertainment company, sugar free entertainment. that i may bring back the era of the old Ziegfeld follies sprinkled with the glitz and glamour of the mgm musicals—shows with beautiful costumes and talented entertainment that makes everyone happy.” Above all, “it’s not just enough to entertain. if you can inspire people and set an example, that’s what makes a difference.”

NOYER-EREZ: i studied in china for a few weeks at the international Academy of orthopedic Acupuncture. i was fortunate enough to go with one of my mentors, dr. hua gu, Phd, lAc, a highly experienced practitioner of eastern and western integrative medicine. our trip consisted of observing and practicing at a variety of chinese medical clinics and western hospitals that incorporated chinese medicine. one of the most amazing procedures i saw was a cesarean section performed using acupuncture and no anesthetic. it was great to know that my four years of training at emperor’s college in santa monica was right up to par with the training in beijing. now, after practicing on my own for more than 10 years, i still remember and appreciate my chinese training.
