After work, and sometimes during lunch breaks, she runs to the home of her aging parents to see if they need anything from the pharmacy or the store, or to take them to doctor appointments. The husband’s parents live in another county several hours away. They are in an assisted living facility and a call has just come in from the administrator that changes have occurred and dad is showing signs of Alzheimer’s.

The husband’s work does not allow him to drop everything, pack and be present for the imminent evaluation and conference. More money and more coverage will be needed for dad. Funds and benefits are limited. What happens next? Her local parents feel they can still handle things at home with little or no help, but it is apparent that some assistance is now needed. Tension mounts as a conversation about more care ends in a stern, “No, we are fine.” Does this sound familiar? Are you experiencing this personally or know of someone who is? This scenario is more common than you think and the ones in the middle are the Baby Boomers—also known as the Sandwich Generation. Taking care of parents while running a household, dealing with certain progressing ailments, finding the right supplementary caregiving, and the funds and programs to implement the plans is a stressful balancing act. Why did you choose this career in path? I was drawn to the field of Urology in large part due to that there were no limitations in the ability to treat women, men, and children. With offices in Camarillo, Westlake Village and Hancock Park in Los Angeles, Dougherty has performed more than 30,000 lens and laserbased vision-correction surgeries since 1993—more than any other surgeon in the area. This includes more than 20,000 laser procedures. Unlike most surgeons who have taken a weekend course in LASIK, Dougherty spent one year training as a cornea and refractive surgeon under the tutelage of Richard Lindstrom, MD, who is credited with popularizing LASIK in the United States.

Dougherty was among the first laser vision surgeons in the U.S. to perform LASIK on an investigational basis before it was approved by the FDA in 1995. “I was also one of the first ophthalmologists in the world to have LASIK on my own eyes in January 1997,” he said.

say, ‘doctor heal thyself’.” The greatest satisfaction from his vocation is to gift vision to those who cannot afford it or have access to it. “I love the feeling of making the blind see—I get goose bumps every time I think about it,” Dougherty said. “My goal is to improve lifestyle through quality vision—to essentially have a positive impact on every life touched.” Andrew Dougherty Vision Foundation The Andrew Dougherty Vision Foundation is dedicated to improving the quality of life for people in the U.S. and around the world, mainly in war-torn and disaster-struck areas, by providing pro bono vision correction surgeries to those in need. “I work hard at my practice to fund my other passion, the Andrew Dougherty Vision Foundation, whose tagline is, ‘So the world may see’.” Established in 2010, the nonprofit effort was inspired by his son Andrew’s premature and unexpected death before his sixth birthday. “I was granted clarity into the reason I came to this earth: to gift vision to those who cannot afford it,” said Dougherty, who routinely gifts vision-correction and cataract surgery to homeless and wounded U.S. veterans as well as internationally to those unable to pay. For instance, the foundation has gifted seven successful bilateral LASIK procedures to wounded veterans from the warrior transition facility in Ft. Hood Texas; it also gifted cataract surgery to six homeless or destitute patients here in the U.S. “With the help of the United Nations, we gifted 35 successful cataract surgeries to destitute Palestinian refugees in Hebron, West Bank in 2010 and successful cataract surgeries to 52 refugees in Beirut, Lebanon in 2011,” Dougherty said. The Gift of Vision During the last mission to Portau-Prince, Haiti, which was featured nationwide on ABC’s Nightline, Dougherty’s foundation gifted cataract surgery to 38 Haitians and saw hundreds of patients.

Two of the patients he saw were permanently blind in one eye from glaucoma and subsequently blind in the other from cataract. “One of these patients used to run a grocery store, but lost her business after the earthquake when people stole all of her goods because she could not see,” Dougherty recalled. The last patient lived in a mud home with a tin roof on a farm outside a village called Croix des Bouquets— he had no electricity, running water or sewage. “He was blind in both eyes and had to stop farming…he is the head of his family with seven children and 21 grandchildren.” “Overall, it was a very successful trip,” Dougherty further emphasized. “We made the blind see. All total, we performed cataract surgery on 35 patients—not bad for working out of one room in a third-world hospital which had never done eye surgery before.” With the next mission trip planned for 2014 to Vietnam, Dougherty is planning fundraisers including a night of magic, a golf tournament and a celebrity poker tournament to help raise funds for the foundation’s work. “My goal is to gift as many people as I can with vision-correction surgery or cataract surgery, which is the leading cause of treatable blindness in the world.” For more information, visit www.doughertylaservision.com or www.advfoundation.com.

Dr. Albaugh, who spent 16 years in the United States Navy mostly on active duty, completed his training during active duty at Portsmouth Naval Hospital and later at University of Medicine and Dentistry of New Jersey. He completed his vascular surgery fellowship at Medical College of Virginia, and was the first DO ever accepted into each of these training programs. Sometimes when things are in the process of change, the very people in the midst of it miss it. As 78 million baby boomers (born between 1946 and 1964) move into the second half of their lives—the notions of aging, work, retirement, healthcare, long-term care, caregiving and the stereotypes of growing older are all undergoing transformation. According to many experts, we are about to witness the biggest fundamental shift in aging at any time in our history.

Boomers today are far from worry-free about their future healthcare needs, but most may not realize the magnitude of the challenge they face.

In particular, three major themes will dominate the landscape for this huge generation of baby boomers as they age—care for themselves, care for their aging parents and facing the emotional challenges of getting older.

The American Hospital Association (AHA) states that boomers will have a greater impact than ever on the United States healthcare system because more patients will be added to an already underfunded system. But the system is not just underfunded, it is under-staffed. As our senior population doubles during the coming years, according to the Institute of Medicine, less than 3 percent of students in medical schools have chosen to take even one course in geriatrics. Geriatrics focuses on the unique medical needs of the elderly. Today in California we have one geriatrician for every 4,000 Californians aged 65 or older.

But the shortage does not stop there. According to an Institute of Medicine Report, less than half of our pharmacy schools have a distinct course in geriatrics despite the fact that per capita prescription drug use by people 65 and older is triple that of younger individuals. According to the same report, in 2005 there was a shortage in the United States of about 220,000 registered nurses. By 2020 that gap is predicted to be more than 1 million. While a significant gap in healthcare workers exists, the need for them will expand exponentially. According to the AHA, as boomers age, the number of people with multiple chronic conditions will quadruple from 8.6 million today to almost 37 million by 2030. By 2030, six out of every 10 Boomers will have more than one chronic condition to manage. The most prevalent chronic conditions include diabetes, arthritis, obesity, heart disease, cancer, hypertension and high cholesterol. In spite of their needs, boomers probably won’t settle for the chaotic ways of our current healthcare system. According to David Cutler, professor of economics at Harvard University, “Baby boomers will soon discover how haphazard the U.S. healthcare system really is when they need it, from coordinating care, to seeing different specialists to obtaining prescription drugs. They are already finding this while helping their parents and they will not put up with it for themselves.”

Caring for Mom or Dad Almost every one of us knows a boomer-age friend caring for an aging loved one, most often a parent. In fact 80 percent of all long-term care in this country is provided by a family or friend caregiver. Statistics show that one in every four individuals is caring for an elderly person, and the average caregiver spent 35 hours per week and $462 a month from their own funds caring for the aging family member. The emotional, financial and physical effects of caregiving can be devastating. There are legions of family caregivers who embarked on a caregiving journey that will eventually take them down a road they never expected or prepared for. More than three-quarters of all caregivers say they need more help or information related to caregiving. Caregivers are often doing tasks they are not trained or prepared to do. Nearly one in five informal (family or friend) caregivers who assisted with medication management and one in three who assisted with changing dressings or bandages, received no instruction or training in performing these tasks. Caregiving is a new role millions of boomers will face. The Rosalynn Carter Institute for Caregiving believes that as our population lives longer, we face a caregiving tsunami and that caring for the elderly will be the most challenging issue our nation faces.

Boomers are at the forefront of that tsunami. In massive numbers they are actively searching for resources and information that can help them make better decisions and advocate on behalf of their loved ones.

Dealing with the Emotional Challenges of Growing Older Bette Davis, in one of her last public appearances on the Tonight Show said, “Getting old isn’t for sissies.” Aging life transitions can be a difficult journey with disappointment, illness and loss. Arguably, old age can be the one of the most emotionally challenging times of a person’s life. We know from research, dealing with old age takes resilience. Resilience, as it relates to aging, is the process of adapting to difficult or challenging life experiences. Where does resiliency come from and why is it that some people seem to bounce back from adversity with relative ease—even thriving after negative events—while others crumble? Psychologists agree that some people seem to be born with more resilience than others. But they also assert that it’s possible to cultivate more of it. One key is adjusting how one thinks about adversity. A long-term study of 99 Harvard men showed that the way people view negative life events (as fixed and unchangeable versus temporary and subject to influence) predicted their physical health even 35 years later. How boomers experience aging will in part be due to their ability to practice resilience to triumph over the challenges of growing old and possibly become a generation no longer afraid of aging.

A staggering 35 percent of Ventura County’s population will be 85 or older by 2033. The aged are all too frequently perceived as a burden and obstacle to personal agendas. As a result, some elders are cast aside and neglected – others endure unthinkable abuse.

The problem has become so pervasive that the United Nations acted in 2012 by declaring every June 5 to be World Elder Abuse Awareness Day. According to the American Psychological Association, roughly 2.1 million Americans currently are being victimized. An estimated five suffer in silence for every reported case of abuse.

Warning Signs Victimization can result in visible physical afflictions or signs of neglect, as well as perpetuate depression, unusual mood swings or anger, or fear of being touched or approached by others. It can be triggered by isolation and involuntary disassociation with parties who had previously been a part of the person’s inner circle. John Hancock recently completed a nation-wide study, showing that the country’s populace is living longer in increasing numbers, but not aging well. Longevity ensures an increased risk of dementia. Without early

detection and intervention, cognitive compromise will cause more individuals to make irrational monetary and lifestyle decisions and increasingly challenge the court’s ability to help them. Financial abuse will mushroom with computer-literate baby boomers who can be victimized within the confines of their home without the knowledge of even the most attentive familial members and caregivers.

The Offenders Family members and caregivers are the most common violators of the rights of elders to be protected from harm, have medical and emotional and day-in, day-out needs met, and assurance their personal effects and assets are kept safe. Ventura County District Attorney Greg Totten has been quoted as saying 90 percent of the elder abuse cases prosecuted by his office involve alleged crimes by relatives. Some prominent organizations and religious groups prey on the defenseless. Sixty percent of county nursing home residents never have a visitor and are particularly vulnerable to devious care providers. Unscrupulous culprits know no socio-economic or ethnic boundaries or moral order. Movie icon Mickey Rooney became an advocate for the rights of the elderly after a 2011 court battle to disallow his stepson, a Ventura County resident, any involvement in his care and financial matters on the heels of allegations of abuse.

Ventura County Elder Abuse Court Ventura County’s Superior Court is one of few in California and across the nation to establish an Elder Abuse Court within its system. Whereas county residents are fortunate to have this service for its abused elders, it is a disconcerting testament to the prevalence of these horrific crimes in our own backyard and scope of need for legal intervention. The Elder Abuse Court was launched in 2009 after funds were earmarked to sustain it, modifications to the courtroom implemented to facilitate the physical challenges of victims, and Superior Court Judge Colleen Toy-White agreed to dedicate her services to that bench. The primary objective of the court is to streamline the judicial process and achieve restitution from, and/or conviction for, offenders expeditiously. Time is of the essence for victims in their life’s end journey.

Reporting Neglect, Abuse Anyone who suspects abuse should contact authorities. Any party who has full- or part-time custody or care of an elder or dependent adult is legally required to report suspicious activity, as are social workers, clergy, health care practitioners,


advocacy groups, fire department personnel, and financial institution staff. Banks now train staff to recognize potential scam or other fraudulent or suspicious activity involving aging customers, and contact law enforcement to intervene and investigate.

Every day I have the opportunity to speak with people who have taken all the right proactive steps to benefit their health; they’ve incorporated a healthy diet and a regular exercise regimen, yet still find themselves feeling tired, unable to lose excess weight and lacking in libido and a zest for life.


The general consensus is that it’s normal for most of our physical and mental attributes to decline, sometimes dramatically. From a health and well-being perspective, it is not normal to endure a significant loss of mental sharpness, increasing fatigue, a sudden drop in libido, or other difficult symptoms normally associated with the aging process. Through my practice, I encounter many patients suffering from these symptoms and find that the underlying reason is often due to a lack of hormones or a hormonal imbalance. You can take two approaches with anti-aging—one is the external approach using skincare products or plastic surgery. This may temporarily satisfy your desire to look more youthful without changing anything internally. The external approach is endemic in our society, promoted by massive companies using aggressive marketing ads to make youth is beauty the new mantra. The external approach is mostly smoke and mirrors. Sure, it may feed your ego, but it doesn’t really do much for the real causes of your aging body and all the important vital functions. At the office of Gary C. Bellman, MD, we strive to take a systematic approach and find direct reasons for symptoms that affect the quality of our day-to-day lives; using each opportunity to make an improvement in one’s internal health. As we start to age, testosterone and estrogen levels can decrease and cause health issues in both men and women. Many people may experience fatigue, weight gain, osteoporosis, a lack of muscle definition, decreased motivation for exercise and sports, as well as a loss of interest in the bedroom.

Testing Your Hormone Levels Your hormone levels can be checked with a simple blood test. For men, we recommend to have blood drawn

early in the morning when testosterone levels are at their highest. Your body has more testosterone than can be easily measured in your blood work, so we may request that your blood also be analyzed for other hormones such as prolactin, estradiol, luteinizing hormone, and DHEA, to get a better sense of how your various hormone levels compare to one another. Many women are surprised to learn the symptoms they’ve been dealing with for months or years are really signs of hormonal imbalance. A blood test also evaluates hormonal balance in both pre- and post-menopausal women.

Is This a Common Problem? Yes—30 percent of 30-year-olds, 40 percent of 40-year-olds, 50 percent of 50-year-olds and onward have hormonal deficiencies which keep them from feeling their best and being as healthy as they can be.

Why is my Body not Making Enough Hormones? Once we’re past our reproductive prime our hormone levels drop. Our bodies are programmed to reproduce when hormones are peaking throughout our 20s and 30s. Aging men lose their testosterone while women lose their estrogen and experience menopause. There are many reasons estrogen levels fall, including: • Hypogonadism • Hypopituitarism • Pregnancy failure (estriol) • Perimenopause and menopause (estradiol) • Polycystic ovarian syndrome (PCOS) • Anorexia nervosa (eating disorder) • Extreme exercise or training • Also, women experience low levels of estrogen immediately after child birth and during breastfeeding. Testosterone is a principal male hormone or androgen. This hormone

does appear in both genders but levels are much lower in women. Testosterone is secreted by the testes in men and the ovaries in women. After age 30, men begin to experience lowered testosterone levels by about 1 to 2 percent per year—which is considered in the normal range. Some men experience a dramatic drop in testosterone sooner, or lose a higher percentage per year. This decline in testosterone, not related to natural aging, is called male hypogonadism. This can be caused by various conditions, including injury to the testes, disorders of the hypothalamus or pituitary gland, cancer treatment, certain drugs, obesity and HIV/AIDS.

What are the Health Consequences of Hormone Problems? Hormone deficiencies can lead to thinning of the bones, an increased risk of hip fractures, heart problems, diabetes, weight gain and depression. Some studies have even shown that one’s life expectancy is longer once hormones have been maximized or replaced.

How Can Hormone Imbalances be Fixed? Usually the solution is hormone replacement, where we replenish the body with the lacking vital hormones. People often wonder if the november 2013

body can be jump-started to begin producing these hormones on their own. The usual long-term solution is hormone replacement.

What is the Best Method of Hormone Replacement? In the case of testosterone, any oral method of replacement is usually unsafe for the liver. Many men take oral androgens or testosterone products or pro-hormones and should be discouraged from using these products. The only safe methods of testosterone replacement are topical gels, creams or patches, injections and pellets. Generally, topical products are not very effective in men as they don’t absorb well. Additionally, they can be messy, sticky and risk transference to women or children. Injections are effective but must be given weekly. Currently, we are placing small pellets under the skin (Testopel) that last for three months and slowly dissolve in a sustainedrelease fashion. Topical or bioidentical creams are very effective in women.

Is Hormone Replacement Safe? Yes, hormone replacement is safe. There have been concerns in the past about the risk of prostate cancer in men receiving testosterone replacement and those concerns are largely unfounded. In the hands of an experienced anti-aging physician, hormone replacement is very safe.

Do I Need to Continue this Forever? People worry that if they start on a program of hormone replacement they will be committed to stay on it and their bodies will become dependent. This is not the case; most people on hormone replacement stay on it because it makes them feel good. Because patients don’t want to lose improvements, they want to continue therapy to maintain overall health. If, however, they do stop with therapy, they simply return to the way they were before.

What are the Side effects of Testosterone Replacement for Men? Fortunately, the side effects are few for men receiving testosterone replacement. An important one is the

impact on fertility. If a man wants to conceive a child, he should not be placed on testosterone replacement as it can decrease the sperm function.

How will Testosterone Replacement Help my Workouts? Many men and women lack the motivation to exercise and when they do, they feel pain in muscles and joints for days. Hormone replacement helps with the motivation to exercise, and aids in the recovery process, decreasing pain and soreness.

Will Hormone Replacement Help with my Sex Life? An emphatic yes! Testosterone levels are directly related to libido in both men and women. As we age, the desire for sexual activity often decreases and a lack or loss of sexual desire can adversely affect relationships. Testosterone replacement can play a key role in restoring intimacy with your partner.

Estrogen-dominance Estrogen is a primary hormone found in men and women. Listen to Music According to a study conducted at Boston University School of Medicine, Alzheimer patients can remember verbal information significantly better when it is set to music. Alzheimer patients and healthy older adults were recruited to listen to spoken words or words set to music. The results indicated Alzheimer patients were more accurate on memory tests when the words were set to music.

Drink Blueberry Juice Blueberries are loaded with health-promoting phytochemicals which have antioxidant and anti-inflammatory properties. Polyphenols and anthocyanins can slow decline in memory function by increasing cognitive signals and assisting glucose metabolism in the brain. Researchers found that individuals with memory decline showed significant improvement in learning and memory tests when they consumed 2 ½ cups of blueberry juice every day for 12 weeks. Polyphenols found in berries (especially the berries’ flavonoid content) have been associated with improvement in neuronal functioning with aging and cognition. Sexless marriage is on the rise; 50 percent of all marriages end in divorce and of the remaining couples who do stay together, more than half live in a sexless marriage.

What is a Sexless Marriage? A sexless marriage can be defined as one in which the couple has little to no sex every month or during a span of several months. This situation is not due to an illness or sudden event but a natural rhythm that happens over time in a marriage. There are a few main causes for sexless marriage, and some proactive solutions to prevent it. Men and women have equal sex drives, but at different times of the day and the month. A woman’s sex drive varies throughout the month with highs and lows depending on ovulation and menstruation. A man’s sex drive varies throughout the day with the highest testosterone levels in the morning and then decreasing toward evening. As men age they also go through a gradual decline in testosterone and naturally move into the quality of the sexual experience rather than the frequency.

Men and Women Approach Sex in Different Ways For women, sexual intimacy starts outside the bedroom; whereas most men can move right into sex when turned on. Sex in a marriage is more than the act of intercourse. It is an act of sexual intimacy—with two people sharing a special bond. Sexual intimacy is a feeling that accompanies sex. When the partners are sexually satisfied with each other they will also feel sexual intimacy. A woman goes through times during her monthly cycle when she is more sexually attracted to her mate. This is when a woman should approach her husband and initiate sex. Part of the problem for many couples is that

men are expected to initiate sex, but initiation should be more equal. Because the level of a man’s testosterone is at its peak in the morning, sex in the morning instead of later at night may be a better time for couples. Timing is where couples often make a mistake in love making. Sometimes morning sex can be best, especially when both are in need of sleep after a long day. Once recharged in the morning, the couple may find it the perfect time for making love. Romance at night can mean hot sex in the morning. Have a sexy date night, a good night’s rest and then wake up to morning passion. One major reason for sexless marriage is the thought that once married, sex is a given and no longer takes work. This is a sad mistake. Keeping sex exciting rather than boring in marriage may take effort—but try making it fun, sexy, and erotic. Couples with great sexual relationships have less to argue about—minor things tend to remain minor. If you are in a sexless marriage, get help. Sex is natural, normal, healthy, and should be enjoyable! All the wonderful hormones released through mutual orgasm can be the glue that keeps the marriage together. Being a baby boomer is a state of mind and embodies amazing experiences in the areas of technology, medicine and more. We are the cohort having televisions and computers in almost every room and a group for whom time is passing at incredible speed. I am the product of two obese parents. I have struggled with my weight for as long as I can remember, having lost more than 50 pounds five times. So the concept of weight loss wasn’t new to me—the concept of keeping the weight off was. I hated exercise. I’d heard it said, “If God wanted us to bend down, he would have put diamonds on the floor.” For me, he’d have to lay down some serious ice cream sundaes! A colleague of mine recently informed me that the fitness industry now refers to baby boomers as the actively aging population. Actively aging? When I began my fitness quest, I was inactively aging and eternally agitated. My change of life came complete with a broom on which to fly and I became adept at doing so. So what changed?

After being diagnosed with an auto-immune condition, I decided it was time to work on my health and get my excess weight off (it has now been off for years). Once I learned the practical steps required to improve balance, flexibility, endurance and strength, it occurred to me that the vital missing puzzle piece was my ability to wrap my head around a long-term lifestyle change. After some very important counsel, I came up with a list of action items I now apply to every task I undertake. I’d like to pass my list on to you.

Find Your Team. Everyone who has ever achieved greatness has had a team of professionals to assist. I achieved success when I asked for help. They empower me to do what, for some reason, I don’t always do on my own and help me to inspire others. If I had given up at any time on the journey of regaining my health, my fate would have been sealed. Had I not engaged the help of a caring, wellinformed team, I would still be executing old, bad habits, expecting new and different results. If that isn’t insanity, I don’t know what is.

Be the Caretaker of Your Thoughts and Words. Inspiration in life is critical. Who or what motivates us? The most powerful input is what we tell ourselves. The silent programming that occurs inside our heads is decidedly influential. How many times have you said less than favorable things about yourself when looking in the mirror? I became careful about what I was telling myself. Words can become self-fulfilling prophecies.

Stop Looking for Quick Fixes. I spent years looking for shortcuts to get my weight off and I found them. I did. I found fast, quick-fix weight loss plans four or five times. And I took the weight off quickly four or five times. And guess what? I put it back quickly—four or five times! Now that’s painful! So the last time around, I decided slower (perhaps more difficult) was better. The bottom line is to eat less and move more. We can find ways to move, even when we hurt. Seek the help of a professional when starting out.

Start Fidgeting! One pound = 3,500 calories. Genetics aside, in order to lose one pound per week, you’ll need to burn off 500 more calories each day than you take in. Scientists recruited 20 self-described couch potatoes, 10 of whom were mildly obese and 10 of whom were lean. They discovered that the lean ones, though they loved lazing about on the sofa, did in fact move a lot more throughout the day—standing, pacing, wriggling, walking, cleaning, shifting from one foot to the other, etc.

On average, they sat two hours less every day than their chubby counterparts. Those extra two hours of activity made all the difference in whether subjects stayed lean or got fat. They calculated that if obese participants were as active as lean participants, they would burn an extra 350 calories per day, which would translate into at least 30 pounds lost over the course of a year.

Take Baby Steps. Don’t Quit. Balance is the Key. I completed two marathons around the age of 50. Hooray for me! But that’s not why I’m telling you this. When I first went back to regular exercising and started working with a personal trainer, I was huffing and puffing after one minute on an elliptical machine. I was so afraid of being hurt. These patents would prevent this use of the fennel flower by any other company, allowing Nestlé alone to profit from it. The problem is, this traditional use of fennel has been known for thousands of years. In recent years, scientific studies in Egypt, Pakistan, Turkey and Saudi Arabia have all addressed this use of fennel. Nestlé did not discover this compound or this use of it.

The Down Side if we Allow This What could happen if we sit back and let these patents go through? Nestlé would have the right to sue any other company who processed fennel for this use. They could gain control of the price and availability of fennel supplements for food allergies, making it illegal for herbalists to create and sell herbal extracts of the plant. They could price their own supplements at any price point they desire, making it more expensive for us to access this widely available, natural remedy.
