That is a meaningful slogan to delve into on this year’s World Family Doctor Day! (FDD) For one thing, it implies a rarely mentioned truth about our profession: Our own hearts areamongthosethatarebeingnourishedby our work with Health Care Our job has a profoundimpactonusashumanbeings

Think what a privilege it is to know we’ve madeagenuinedifferenceinotherpeople`s lives Asweexperienceourpatients’trust,we growinourcapacity–andourdetermination –tobedeservingofit

The demanding and often lonely work of carrying other people’s pain takes a toll, making the human need for solidarity and companionship an inevitable aspect of our profession. It becomes particularly heartrending whenthatneedremainsunmet intimesofconflictanduncertainty

For me, receiving support from colleagues has a unique effect That’s the case with explicit support, spelled out in words, and the implicit support that underlies our professional brothers’ and sisters’ active assistance. Also, subtle support, such as in the heartfelt warmth wehearinacolleague’s voice, can remind us that we’re not alone in thismassiveandvitaltask

RemembertoshareourFDDwithourPrimary Care team partners Health Care is inclusive and collaborative teamwork Think a moment:whichofyourcolleaguescoulduse someone to ask them, now: “How are you doing?”Wecandaretoextendthe warmth of our hearts to our colleagues while they’re strivingto warm the hearts of their suffering communities I say “dare” because, unfortunately, just the thought of speaking from the heart may trigger obstructive shyness

It would be heartbreaking were we to recognize that too late We can use FDD to underline the urgency by highlighting the evidenceonwhichthefactisbased.

Let’s Celebrate this Family Doctors Day! Let our hearts sing out so the world hears, and we remember, just how much we value our profession – our work, our patients, and our colleagues. We’re glad to continue putting our hearts AND minds toworktoensurethat each and every person has unobstructed accesstoqualityhealthservices

The week following FDD, the WONCA delegation goes to Geneva for WHO’s World Health Assembly. We aim to bring Family Medicine to the foreground of the global health policy dialogue Because health is a fundamental need and everyone’s right

We carry with us the inspiring and heartfelt supportofourWONCAmembers!

We doctors are certainly well-educated and knowledgeable But we can’t be good clinicians unless our hearts are in our work Many or even most patients recognize quicklywhichdoctorshavetheir hearts in the right place; they react to that more than we mayrealize!

Just as our patients need their Family Doctors’ heartfelt care,sodothoseofuswho arecaringforthem AdedicatedFDDinspires ustoletour hearts go out alsotoeachother–our far-flung colleagues, living and working inastunningvarietyofcontexts.I’mthinking inparticularnowofthoseofuswhoconfront extremelydifficultconditions,suchasduring and after armed conflicts and natural catastrophes T

By uniting our hearts, FDD also strengthens the bonds at the core of our organization, further empowering us to fulfill WONCA`s mission:AFamilyDoctorforeveryindividual and every family, regardless of gender, age, orsexualorientation

We can benefit from using May 19th FDD as an opportunity to contact those outside our profession as well – such as politicians and policy makers (Don’t ever doubt that they too have heavy hearts at times!) Tell them loudandclear, from your heart to theirs,that the following is not just a nice idea of ours but rather a well-documented fact: Every country in the world will need more Family Doctors in the coming years if we hope to meet the global population’s health requirements

TheTaiwanFamilyMedicineResearchAward (TFMRA) was established by the Chinese Taipei Association of Family Medicine (CTAFM) in 2008. It is to support travel expense for three junior family physicians to attend each WONCA World Conference The purpose of TFMRA was to encourage junior family physicians to conduct research in the specialtyfieldoffamilymedicine

THE AWARD

The TFMRA is a prize of USD 1,500 to each youngfamilyphysiciansfortheexcellenceof theirresearch

CTAFMwillgiveatotalofUSD4,500tojunior family physicians for them to attend the WONCA World Conference in Sydney, Australia Three junior family physicians will bechosenbytheselectioncommittee

Winners should attend the WONCA World Conference2023inpersonorbyanofficially approved representative by the selection committee to receive the award from the PresidentoftheChineseTaipeiAssociationof FamilyMedicine.

ELIGIBILITY FOR APPLICANTS

1 The applicant must fulfill one of the following qualifications: a practicing family physi-cian; an attending physician at an academic institute, a resident in a formal training program, or a graduate student in therelatedspecialtyoffamilymedicine.

2.Theapplicantshouldbeunder40yearsof age and in the first three years of their researchcareer.

3 No more than one applicant from each WONCAMemberOrganisation

4 The applicant should submit a full paper that is unpublished and unpresented elsewhere

5 Thesubmittedpapershouldhavereceived nofundingfromanyresourcesoraward

6 The award paper should be presented at theWONCAWorldConference2023

CRITERIA FOR SELECTION

1. Relevance of the research topic to the familymedicineofthecountrywheretheresearchwasconducted.

2. The originality of the article and compliancetothecodesofresearchethics.

3 Rigoroftheresearchmethodology

4 Contribution of the research to clinical practice

5 QualityofEnglishwritingincludingbutnot limited to abstract, introduction/background,literaturereview,materials andmethods,results,anddiscussion.

OurWPfocusesonthehealthofwomenand theirparticularspectrumofrisksandneeds; and also on how to maximise the important contribution of women family doctors worldwide What this means for all of us as family doctors is – for patients, to recognise and design our services to ensure that womencanaccesspreventivescreeningand fertility related services, including maternal and child health, as well as other services, withoutculturalorfinancialbarriers.

We also need to highlight that, in many places, understanding the biopsychosocial needs of our female patients will often include recognising their role in caring for others. Caring for dependants or for sick friends and family often adds additional practicalandemotionalchallengesforthose who shoulder this burden; and, while social change may mean that this ‘work’ does not fall solely to women, this remains the culturalnormacrossmostoftheworld.

And for colleagues – our work as family doctors is very demanding, and we all need collegial support. Our WP has seen two commonstressorsthatcanimpactusall,but areoftencitedbyfemalecolleagues:one,as above, is that they are more likely to be juggling the complexities of caring for their partners and families as well as sustaining theirprofessionalactivities Andtheotheris thattheymaystillfacecareerbarrierssolely becauseoftheirgender–lowerincomes,less flexible work conditions, and fewer career opportunities So on WFDD 2023, we ask us alltohavealearningconversationwithyour colleaguesabout

1) How well you think your clinic is doing in meeting its female patients’ particular health needs? - and take any agreed priorities forwardtoenhanceyourcareevenfurther

Around the globe women are breaking barriers in a number of fields including medicine, education, science & technology, sports and leadership roles. The South Asia region is no different, today we see women participation and representation in a vast number of fields Air force female fighter pilots, Ayesha Farooq and Maryam Mukhtiyar, who have set an example of courage and bravery Likewise, Sania Mirza was a successful professional tennis player. Over years women have gained a lot of confidence and have dared to enter the horizons which were forbidden to them earlier Medicalandsurgicalfieldisthesame, we have seen competent female doctors in orthopedics,urology,cardiovascularsurgery and related specialties which were thought to be best served and managed by male surgeons only. CONFERENCE ON NONCOMMUNICABLE DISEASES: ROLE OF PRIMARY HEALTH CARE IN CHRONIC DISEASE MANAGEMENT

Khyber Medical University recently hosted a two-day conference on non-communicable diseases (NCDs) with the aim of highlighting the importance of primary healthcare in disease prevention and management. The conference, which took place in Peshawar, was attended by a range of healthcare professionals, policymakers, and researchers The president of Pakistan, Dr ArifAlvi,wasthechiefguest,underliningthe significance of the event for the healthcare sectorinthecountry.

The conference shed light on the high burdenofNCDsinPakistanandemphasized the need for a shift in the healthcare paradigm Instead of focusing primarily on hospitals,thespeakersadvocatedforgreater emphasis on prevention strategies at the primary care level One of the notable discussions during the conference was breaking the mannikins of NCDs. The speakers underscored the importance of family physicians in educating patients, promoting healthy lifestyles, and providing counselling services to manage and prevent NCDs The establishment of a proper structure and recognition of family physicians could lead to significant improvements in patient safety, disease prevention, and cost savings in the healthcaresector

The conference marks an important step towards a more holistic approach to healthcareinPakistan,emphasizingtherole of primary healthcare in managing and preventingNCDs

With the insights gained from the conference, policymakers and healthcare professionals can work towards implementingeffectivepreventionstrategies andreducingtheburdenofNCDsontertiary care

PRE-CONFERENCE WORKSHOPS CONDUCTED BY FAMILY MEDICINE DEPARTMENT

The Department of Family Medicine at Khyber Medical University also hosted preconference workshops on 21 February on a variety of important topics One such workshop focused on de-escalation of violence in healthcare, in collaboration with the International Committee of the Red Cross The ICRC's "Health Care in Danger" project focuses on training healthcare professionals in conflict and post-conflict zones, and the Department of Family Medicine is collaborating with the organization to provide such training in healthcare institutions across Khyber Pakhtunkhwa

According to recent data, around 496% of healthcare workers in Pakistan have experienced violence in the workplace, with a third of these incidents occurring within thelastsixmonths Healthcareworkersface arangeofviolentbehaviors,includingverbal abuse,physicalassault,andfalseclaims.The workshop provided training to healthcare professionals on how to recognize potentiallyviolentsituationsandhowtodeescalate them to promote safety in the workplace The workshop was attended by healthcare professionals from a range of institutions, and they provided positive feedback,statingthatsimilartrainingshould beconductedintheirinstitutionsaswell.

Anotherworkshopattheconferencefocused on medical and research ethics This workshop was designed to provide training on ethical issues and professionalism in medical research and practice. Attendees included doctors, dentists, public health professionals, and allied health science professionals

A third workshop focused on basic life support training, which is a critical skill for healthcare professionals The workshop covered topics such as cardiopulmonary resuscitation (CPR) and choking emergencies

The importance of these workshops cannot be overstated, as they provide healthcare professionals with essential skills and knowledge that can improve patient safety and care The Department of Family Medicine is committed to providing such training to healthcare professionals across the region and playing a role in improving safetyandprofessionalisminthehealthcare sector.

CommunityOrientedPrimaryCare(COPC):

In its most general definition, COPC is the “provision of primary care services to a definedcommunity,coupledwithsystematic efforts to identify and address the major healthproblemsofthatcommunitythrough effective modifications both of the primary care services and other appropriate community health programs” Towards this, theconceptualframeworkforCOPCisbased on 5 principles: (a) responsibility for comprehensivecareofadefinedpopulation;

(b) care based on health needs and its determinants;

(c) prioritization of those needs to implement health programs;

(d) programs that integrate promotion, prevention, and treatment; and

(e) communityparticipation

THE ROLE OF THE FAMILY PHYSICIAN IN RURAL PRACTICE:

Each AMRIT Clinic, managed by Basic HealthCareServices(BHS),providesprimary health care that is responsive, empathetic primaryhealth‘circleofcare’,thatisrooted in the community including provide preventive, promotive and primary curative services. to a cluster of about 3000 tribal families Most of these families own small unirrigated farms and have limited employment opportunities Such a situation forces many young men to migrate to cities for labour Food is scarce, and malnutrition levels among children and adults are high. AMRIT Clinics also utilize a range of innovations including HR Innovation, Partnership innovation, and Technology innovationstoaddressarangeofdaytoday situations that are highly challenging such as:Whatdoesamotherdowhenthechildis sick, father is away to a city for manual labour, and there is no health facility for 20 kilometers, and no transport? INTENT OF THE QUARTERLY RURAL HEALTH FEATURE SERIES:

This quarterly feature series on rural health intendstoinspirestudentsandpractitioners to engage in rural community practice, dive into a unique opportunity to observe from close, healthcare of adults, children, and whole families from remote communities, gain insights on delivering healthcare in places with limited resources, to learn and impart lessons on what changes in clinical approach, programs and policies might be required to improve the care for these neglectedandoftenforgotten,populations

In each article, we will provide a case study, try to locate it in a wider context, discuss learnings from clinical, epidemiological, health systems, and policy lenses, and propose a call for action. Each case study will illustrate the principles of family practice such as "deep generalism"; "person and family orientation"; "continuity of care"; "community based care"; "building a trustingrelationship";"counseling";and"an effective steward of resources" are highlighted.

Additionally,thesearticleswill:(1)reflecton the clinical insights, joy, challenges, and dilemmasofphysiciansinaddressinghealth needs of the rural communities; (2) contrast between a disease-oriented (specialist approach) and a person-oriented approach combined with COPC; and (3) suggest a course correction to the existing paradigms in medical and health sciences education of bothgeneralistsandspecialists

EVERY DOCTOR: HEALTHIER DOCTORS = HEALTHIER PATIENTS

My co-authors, Professor Michael Kidd, Dr Vihangi Abeygunawardana and I wrote the fully revised 2nd edition of Every Doctor because we believe that healthier doctors provide the highest standard of patient care –andfindmorejoyinlifeandwork

Family medicine is a fantastic career. It can be a privilege to witness the courage of our patients, gain extraordinary insight into ordinary life, and develop bonds with colleagues who become lifelong friends across international borders But we must also address the downside – general practitioners are continuing to confront and adapttoglobalandlocalpublichealthcrises, which are impacting on the mental and physical health of every community – and everydoctor

In Every Doctor, we are advocating for a transformational change in the way doctors protect their mental health; care for their colleagues, particularly early career doctors; and co-create a kinder, more humane work culture. Every doctor has the right to a safe workplace–ahealthyworkenvironmentthat protectsdoctorsagainstburnoutandmental injury, bullying, harassment, discrimination, racism, medicolegal issues and verbal and physical assaults This is every doctor’s responsibility

We all have more influence than realised to reform our health systems and workplaces through co-leadership with like-minded colleagues.Forexample,inthelastfewyears ofthepandemic,groupsofdedicatedfamily doctors banded together under the umbrellas of WONCA and National Colleges of General Practice and Family Medicine to drive health care innovation such as advances in digital technologies; expand telehealth provision; set up new services for isolated rural communities and war torn countries; rapidly establish vaccination centres;rolloutneweducationandtraining

programs;andprovidestrongadvocacyona very wide range of issues that impact on patient care This healthcare activism must continue

It’s also time for senior doctors to step up and provide more support to early career doctors Young family doctors at the front line of health care should not have to learn aboutnavigatingcareerchallenges“thehard way” through bitter experience. In recent times,thepandemicandotherpublichealth crises have resulted in loss of career progression, loss of certainty about the future, traumatic losses of patients, family members, friends and colleagues In this environment, workplace mental injury including direct and vicarious trauma in doctors is under reported, under recognised andundertreate

We can do so much more to care for our colleaguesinthesecircumstances
