Launching CCCM into the Patient Context from PMS HealthLink

To access the COVID Clinical Care Module (CCCM) from within your Health Provider Patient Management System, please follow the steps as outlined below: 1.

Open the patient profile in your PMS and click on the HealthLink Forms icon.

Select the COVID Clinical Care Module System link under the Referred Services section.

If the person has an active CCCM record, this link will open that person’s record. You are then be able to navigate their CCCM information from within this page.

If you are ready to exit, and “complete session” is selected, it will write a PDF of what has been done in CCCM up until this point. This PDF will go to the PMS Patient Outbox.

Viewing the Vital and Note Timelines 1.

The summary tab will default to display the patient Vital Summary on the Vital Timeline.

By selecting the Note Timeline on the lefthand side of the screen you can view Patient notes that have been recorded for a person (these will display in chronological order).

Here you can identify all notes that have been recorded in the CCCM.

From the Note Timeline you can add a Note by selecting the Note icon. To save your note, select Save.

To filter notes by Type, select the Note type on the right-hand side.

Navigating Person Information cont.

The Person Information screen will display all key information related to the person and their bubble (where applicable).

This information includes:

Name, Age & Gender Flags i.e. Mental Health Issues, Require Welfare, Pregnant.

Next test (coloured dot outlines latest result – black = negative, red = positive, orange = repeat needed) Assessment status will display if they require an initial assessment, regular health check or if they have been completed for the day. The red tick means they have COVID-19 Symptoms on their Health Check – the Done button indicates that a test has been done today.

Here you can see how many people are in the bubble – the relative number stays constant, and the grey highlight shows which record you are in currently.

Patient information icons for all bubble members. By hovering over these icons key patient information can be viewed without the need to fully open each bubble members profile.

Use the arrow keys at the bottom right of the screen to view further detail about a patient such as; •

Demographics, Linked General Practitioners and Next of Kin

Tip: As this writes back to the patient record, you are unable to navigate to the next bubble member from here – however, you can open them in the PMS

Navigating Person Information 1

Complete this as part of an Initial Health Assessment – this can be updated as required: 1.

On the Person Information page, fill in all fields that are relevant for the person.

To the Person Information data you have entered, select Save.

Use the navigation arrows on the bottom right-hand side to see all the pages and complete the assessment.

Please note, there are no compulsory fields on CCCM.

Adding a Whiteboard Note Adding a Whiteboard Note allows you to leave a note that is easily visible.

1.

To add a whiteboard, select the whiteboard icon. This will enable you to write a whiteboard note which will be visible to anyone reviewing the patient record.

Note: The note will appear on the Task Bar and Note Timeline. Even if the Whiteboard note is replaced on the Task Bar, it will still be visible in the Note Timeline.

Triage Score The NCTS Triage Score of a person is visible on the CCCM next to the CCCM Acuity Rating: 1.

You can view the NCTS Triage Score the Dashboard, the Person Context Banner, and the Bubble List screen.

If an update is made to the Triage Score in NCTS, this will pull through to the CCCM immediately. To view the history of the Triage Score, select the Triage Score icon and a pop up will present to display the Triage Score History.

Initial Assessment

Use this page to complete the Initial Assessment and any welfare referrals: 1.

On the Initial Assessment page, complete as much information as relevant. Use the navigation arrows in the bottom right to move between pages.

Information key to the Initial Assessment will pull through from the National Contact Tracing Solution (NCTS) if an assessment has already been completed.

This information will be included as a note available by clicking the Underlying Condition Noted in NCTS button, which is visible on all pages of the Initial Assessment.

On some pages you can select Mark all above as No to save time. Click Complete when done to Save it.

On page 5 of the Initial Assessment, you can enter an Individual Assessment Plan/Guideline. This is accessible from the Summary Page after completion, where you can edit the Plan/Guideline as required.

Doing this will write a note on the Note Timeline.

Reg. Health Check Use this page to complete a Daily Health Check: 1.

Complete as much of the regular health check information on all pages as relevant by selecting the text box to enter data or selecting Yes / No answers. Use the navigation arrows in the bottom right to move between pages.

To save, click Complete.

Note: For positive cases, important symptoms are highlighted in red.

After Reg. Health Check Completion 1.

Once the Reg Health Check is completed, you will land on the Vital Timeline.

Symptoms of concern will be highlighted in Red.

If you have completed more than one Reg Health Check in a day, clicking on any of the symptom indicators for that day will display all the checks that have been done.

To see any Other Symptoms that were recorded, you can select the Y in the Other Symptoms field to see a pop up of what was recorded.

To see the Obs recorded in a graph, click the Day count at the top of the day e.g.

D4 or D5. Here you can see an overview for all Health Checks or today’s Health Checks.

Acuity Rating You can update the Acuity Rating during a Regular Health Check, or without completing a health check via the following method:

Update from Person Information by clicking the current acuity rating (highlighted in purple)

2.

Select the appropriate Acuity Rating after the clinical assessment. Enter your reason for the change. This will write a note on the Note Timeline.

To save your changes, select Save.

Note: Acuity rating is used to measure how much time someone might need from staff to help plan resourcing, rather than ‘how sick’ they are.

Please refer to HealthPathways for advice on how to use the Acuity rating to match baseline clinical risk and current symptoms.

Exit Health Checks 1.

If a person is approved for release from your care, complete Reg Health Check as normal.

On the final page, select Yes to Is this the Final Health Check?

Instead of a pop up asking about Quarantine/Isolation/Blue band, you will get a pop up called Final Health Check. Click either: • No – to just save the health check: this will bring up the quarantine/isolation/ blue band popup and will save as a Regular Health Check • Yes – to Complete and send to NBS: This will provisionally release the guest in other systems and mark their Final Health Check as complete.

Tasks 1.

From the tasks tab, you can add your own Tasks to the Person Information Profile.

Enter the relevant information and when complete, select Add Task.

View and update the task status by selecting the Status column in the relevant row.

We also run CCCM Training and Drop-in sessions. In these sessions we demonstrate new features from each release, provide general support, and answer any questions your team might have.
