DISCIPLINE RECORDS AND NOTICE
The chair of the Hearing Board shall be designated to take notes at all hearings. These notes shall include a copy of the original complaint, a copy of all written documentation pertaining to the case, the date and location of the hearing, a list of all witnesses who appeared at the hearing with a brief summary of the testimony of each, a copy of all notices to appear that were sent out, and a copy of the decision.
Minutes of all Hearing Board hearings shall be maintained in Maxient, the University’s conduct record keeping system. These minutes will be the responsibility of the Assistant Director for Fraternity and Sorority Life or their designee. Notice for all hearings shall be sent to the chapter president and the chapter advisor in accordance with The Code.

An update of responsible violations and the approved outcomes will be shared at the first IFC, LMC, and PHC council meeting following the window of the approved appeal timeline.
