DISABILITY INSURANCE CAN HELP!

If sickness or injury prevents you from working for an extended period, you may be eligible for disability benefits.

SHORT-TERM DISABILITY PLAN

Short Term Disability (STD) benefits take away the worry of not being able to work and bring home a paycheck if a non-job-related disability prevents you from working. Elimination Period: 7 days for disability due to an injury or sickness Benefits begin the 8th day after the elimination period is completed Length of Payment Period: Up to 12 weeks

60% OF WEEKLY EARNINGS TO A MAXIMUM BENEFIT OF $1,000 PER WEEK PRE-EXISTING CONDITION LIMITATION

Any injury or sickness for which you received medical treatment or advice (including diagnostic measures) or had drugs or medicines prescribed in the three months prior to the day your STD coverage began. Benefits will not be paid for any disability that begins in the first six months you are covered by the STD plan, if that disability is due to or results from a pre-existing condition.

LONG-TERM DISABILITY PLAN

The financial consequences of a lengthy disability can be very serious. The LTD plan provides monthly income protection if you become unable to work due to a disabling condition that lasts beyond 90 days. Elimination Period: 90 days Length of Payment Period: For as long as you remain disabled or until you reach SSNRA whichever is sooner

60% OF MONTHLY EARNINGS TO A MAXIMUM BENEFIT OF $10,000 PER MONTH PRE-EXISTING CONDITION LIMITATION Any injury or sickness for which you received medical treatment or advice (including diagnostic measures) or had drugs or medicines prescribed in the three months prior to the day your LTD coverage began. Benefits will not be paid for any disability that begins in the first 12 months you are covered by the LTD plan, if that disability is due to or results from a pre-existing condition.

Benefits may be reduced by other disability income benefits, such as state disability insurance, sick leave, or unemployment insurance.
