Checkboxes
Create a detailed list of common hazards found at each work site, then have employees check off those that were present during the FLHA. You can program notifications to supervisors when certain checkboxes are selected, depending on the severity of each hazard.

Open Text Fields Dropdown Lists
Allow employees to create their own lists of hazards discovered, then automatically assign the assessment to a supervisor for review. Alternatively, leave text fields open for workers to make notes on what caused the hazard and the hazard controls used.
Using customized dropdown lists, evaluate multiple tasks on the same form. When workers select a task, a customized hazard checklist appears for each task. To keep data consistent, allow the form to automatically duplicate each checklist

Image / Video Integration
Allow workers to upload images and videos to their hazard assessments, giving you a better picture of what's being discovered on the job site.
You can also embed videos directly into the form for employees to view, allowing you to provide detailed instructions without causing confusion or misinterpretation
Calculation Fields Document Uploader
Streamline your risk analysis process by including calculations for determining the likelihood, severity, and frequency of hazards. Workers rate the three risk categories on a scale of 1-3, and the hazard assessment software generates a comprehensive automated report in real time to assist in identifying risk levels and hazard prioritization.
Allow employees to upload any necessary Permits/confirmations directly into the form using any smartphone or tablet with an integrated webcam.

Required Fields
Specify which fields are required to ensure the safety of your employees. You can even specify the conditions under which a specific field is required. If this box is not checked, the form will be flagged for supervisor review.

Electronic Signatures Date and Time Stamps
Use the software's electronic signature feature to ensure that workers are signing off on their hazard assessments. The assessment should then be programmed to be assigned to supervisors upon completion, and they should provide electronic signoff as well.
Keep track of the frequency and consistency of your FLHAs by noting the date and time of each assessment.
