WELD LEGACY FOUNDATION AWARDED $887,558 IN GRANTS AND SCHOLARSHIPS THROUGHOUT 2022.
MONFORT CHILDREN’S CLINIC - $82,993

In 1995, Weld Legacy Foundation responded to the overwhelming need for primary healthcare among children from Weld County’s low-income families and opened a pediatric clinic. The clinic was designed to provide complete medical care for children who are Medicaid eligible or uninsured.
The Monfort Children’s Clinic occupies a 15,000-square-foot building on North 11th Avenue in Greeley built specifically to house a pediatric clinic. In 2001, Weld Legacy Foundation partnered with Sunrise Community Health for the ongoing operations. The clinic is owned and supported by Weld Legacy Foundation and operated by Sunrise Community Health.
Today, the clinic provides primary care, dental, behavioral health, family planning, telehealth, and WIC (Women, Infants and Children) services.
This past year, Weld Legacy Foundation awarded $82,993 to the Monfort Children’s Clinic for building repairs and upgrades.
CURTIS STRONG CENTER FOR THE VISUALLY IMPAIRED - $95,000
Curtis Strong was a local business owner, avid pilot, and outdoor enthusiast. During the later years of his life, he suffered from an eye disease known as macular degeneration. Upon his passing, he left a generous bequest to Weld Legacy Foundation to establish the Curtis Strong Center for the Visually Impaired fund.
Weld Legacy Foundation has partnered with Ensight Skills Center to operate the Curtis Strong Center in Greeley to provide a full range of services for low vision patients. In 2022, Weld Legacy Foundation granted $95,000 to support the operations of the Curtis Strong Center.
NORTH COLORADO MEDICAL CENTER - $450,059

Weld Legacy Foundation was originally founded in 1975 to support the needs of North Colorado Medical Center (NCMC), one of Greeley’s hospitals. Through the years, Weld Legacy Foundation has funded many projects at NCMC including a hospital expansion, medical equipment, and various department needs.
In 2022, Weld Legacy Foundation provided $450,059 to NCMC. This funding provided support for the following projects:
• NCMC Organ Donor Honor Wall – to create a Gift of Life memorial wall at NCMC educating and honoring organ and tissue donors as well as donor recipients
• Hospitality House – to remodel the space which provides lodging for patients who are undergoing outpatient treatments and for families to remain close to patients undergoing care at NCMC
• Neonatal Intensive Care Unit – to purchase three isolettes and ventilators. An isolette is a specially designed piece of equipment to safely hold and transport an infant while controlling the temperature for the baby. The ventilators assist the infants with breathing.
• Hospice – equipment for their patients, pavers for their memorial gardens, and patient information
• Endoscopy Services – upgraded equipment to enable physicians to diagnose and treat lung cancer in earlier stages
• CardioVascular Catheterization Lab – equipment to expand the CardioVascular Insitute’s structural heart program
• Chapel – updated decor for employees, patients, and families to enjoy while seeking respite in the Chapel
• Cancer Center – courtyard improvements for patients and families to visit while undergoing treatment
• Staff Continuing Education – seminars and conferences for healthcare professionals at NCMC
• Employee Gym – equipment upgrades
MED-AID PRESCRIPTION ASSISTANCE - $5,720
This prescription assistance program helps individuals with one-time prescription assistance in Weld County who have an urgent medication need with no way to cover the cost.
PATIENT SUPPORT - $111,521

Weld Legacy Foundation provides grants to support patients and their families while they are undergoing treatments for serious illness at North Colorado Medical Center. In 2022, Weld Legacy Foundation granted $111,521 toward patient needs.

Those grants covered a variety of needs including:
• Living expenses
• Medical devices
• Mammograms / diagnostics
• Cardiac and Pulmonary Rehabilitation
HEART SAFE CITY - $38,573

• Lifelines
• Hospitality House stays
• Burn garments
• Travel expenses
The Heart Safe City program is a community-wide effort to educate the citizens of Weld County on how to administer cardiopulmonary resuscitation (CPR) and use automated external defibrillators (AEDs). Weld Legacy Foundation helped cover the costs associated with maintenance of the AED devices and training programs for community members in CPR and AED use.
SCHOLARSHIPS - $101,192
Weld Legacy Foundation has 15 active scholarships for students seeking a career in the healthcare field. Over 215 students have received a Weld Legacy Foundation scholarship to attain a degree or certification in the medical field. Last year, 25 students received scholarships totaling $101,192.
GREELEY CHAMBER OF COMMERCE - $2,500
Weld Legacy Foundation provided a grant for the Young Professionals group for capacity building services. The group held an event aimed to match volunteers in the community with nonprofit boards to serve on.
