WELCOME Dear attendees, speakers and sponsors: Welcome to the sixth annual IHC FORUM & Expo produced by The Institute for HealthCare Consumerism. As the flagship event of The Institute for HealthCare Consumerism, FORUM & Expo has steadfastly tracked and promoted the consumer-centric changes in the health and benefits industry over the past six years. With the help of numerous speakers, sponsors, exhibitors and advisors, IHC FORUM & Expo has become the main event where industry stakeholders can access the most current insights, innovations and industry leaders around health care consumerism in one place. As health care consumerism grows, so has the IHC FORUM. This year’s conference continues its focus on account-based plans, health savings accounts and corporate wellness – all cornerstones of health care consumerism – but also turns its focus to the numerous technologydriven innovations that are empowering employee-consumers, including telehealth, private exchanges, digital health, wearables and more. These technology solutions are driving the future generations of health care consumerism. Change is the new constant in employee health and benefits. But today’s chaos represents the possibility for real change in not just employee benefits – but our health care system universally. It is my hope that the best practices, innovation and expert outlooks included in this week’s program guide attendees in their health care consumerism journey. In closing, I would like to thank everyone that helped with the production of our sixth annual FORUM & Expo – from my staff to our attendees to each and every sponsor. A special thanks is reserved for Aetna, our sole platinum-level sponsor, and our seven gold sponsors: ConnectYourCare, Quantum Health, Cigna, Teladoc, WageWorks, Quadrant 4 and Softheon. On behalf of all of us at The Institute for HealthCare Consumerism, we sincerely appreciate your participation and look forward to your feedback at the end of the event. Sincerely,

DOUG FIELD

PRE-CONFERENCE EVENT #2 PRIVATE EXCHANGE FUNDAMENTALS Tuesday, June 23 9:00 a.m. – 1:00 p.m.

Helping Employers Understand Defined Contribution and Private Exchanges This hands-on, pre-conference course will help employers and those working with them understand the various exchange models and key points in determining whether a private exchange is right for your company, your employees and/or your clients. More and more, employers are responding to industry surveys similarly: they see private exchanges as a real option and perhaps the clearest path and means to accomplish their primary goals as it relates to health and benefits: cut health care costs; reduce administrative burden; improve choices provided to employees covered under the company health plan(s); increase access to coverage for those employees not currently under a company plan; and maintain valid options for active and retired employees. Yet, they state that the key questions remain unanswered. With so many options in the market from parties with vested interests – such as brokers, consultants and even the insurance companies themselves – who can be trusted to deliver the right fit solution for us, an employer with so many unique benefits requirements and distinct employee situations? This course is designed to answer many of your questions as employers and to support you in the development of a singular strategy related to private exchanges that works in your company’s sole case. FACULTY: Barbara Gniewek, Principal, GHRS, PriceWaterhouseCoopers Donald Weber, Managing Director, PriceWaterhouseCoopers’ Human Resource Services Group Jonathan Har-Even, Consultant, GHRS, PriceWaterhouseCoopers

GENERAL SESSIONS OPENING KEYNOTE ADDRESS Tuesday, June 23 2:30 – 4:30 p.m.

C-Level Executives Share Their Health Care Strategies, Including the Shift to Consumerism and Their Look Ahead Health care consumerism is gaining widespread acceptance across the health care landscape and inside the C-suite of many major employers. Those executives that are embracing health care consumerism as the solution for health reform are doing so by empowering their employees in their health care decision-making. According to a survey released in March 2014 by Towers Watson, in which nearly 600 major employers representing nearly 12 million employees responded, a half of all employers plan to make significant to transforming changes in their health benefits plans during the period between 2015 and 2018. Nearly 95 percent plan to make modest change. In this opening keynote panel to the sixth annual FORUM & Expo, learn about the specific ways leading executives are promoting and achieving change inside their own organizations or in those of their clients. Learn what they have already implemented, where they plan to make additional changes and what experiences they have gained along their way that can be beneficial to your organization. AFTERNOON GENERAL SESSION Wednesday, June 24 1:30 – 2:45 p.m.

Health Care Reform and Compliance Issues The Patient Protection and Affordable Care Act (PPACA) is the law of the land, and regulations are being issued at a record pace. What impact do these regulations have on consumer-directed health plans? What’s the future of HDHPs and HSAs or HRAs in the next three to five years? What design changes will be necessary to comply with new regulations? Equally important, with the changes in the electorate within the legislative branch in Washington as of November 2014, what changes can we anticipate and even expect? These questions and more will be answered during this cutting-edge general session. Panelists will give the audience an inside look at the implications of the PPACA and its impact on consumer-directed health care. They will offer practical, actionable strategies your company can use immediately to be alert to the upcoming requirements and be prepared.

GENERAL SESSIONS CLOSING GENERAL SESSION Wednesday, June 24 4:15 – 5:45 p.m.

Defined Contribution and Exchanges: A Panel Discussion with Leading Experts The health care law has changed the way that brokers solicit health insurance and how employers provide health insurance to their employee population. The emergence of public and private exchanges and the concept of defined contribution, which encourages the philosophy of health care consumerism, is becoming a game-changer. Exchanges are meant to increase accessibility to affordable health care. Yet, employers and brokers need to examine how the different exchange models (whether private or public) best fit within their health benefit management strategies. For those employers, brokers and consultants who are trying to remain relevant in an ever-changing, dynamic world of benefits, they should understand that exchanges are becoming the “catalyst for health care consumerism” and will change the way benefits are delivered and acquired by the employee-consumer. In this general session, panelists will explain the different exchange models, the benefits of these different models, the timeline for implementation, compliance issues and how the concept of defined contribution fits into the exchanges. Equally important, the discussion will include how exchanges are being received. Come learn what is working, what is not and where we go from here. MODERATOR: John Young, CHCC, CEO, Consumerdriven, LLC PANELISTS: Nancy Vasta, Business Lead, Proprietary Retail Exchange, Cigna David Firestone, VP of Exchange Sales, Aetna, Inc. WORKSHOP 107 TRACK ONE WORKSHOPS Tuesday, June 23 4:45 – 5:45 p.m.

Health Care Transparency: What You Don’t Know CAN Hurt You Today’s rapidly growing movement for health care price and quality transparency is fueled by the increasing adoption of consumer-driven health plans by employers and other purchasers across the country. The movement is not new to purchasers, however, who have long advocated for public access to good data, beginning with the founding of the nonprofit Leapfrog Group in 2000, a purchaser-driven initiative to collect hospital safety and quality data and publicly report it to drive a market for the best care. Today, that movement is accelerating, and Leapfrog purchasers are active as never before in demanding information their employees need to make good decisions about selecting doctors, hospitals and other services. In this workshop, meet Leapfrog’s president and two of the organization’s leading employer members – Michelle Martin from CBS and Frank Johnson from Maine Health Management Coalition – and hear how transparency has already changed health care, with more disruption ahead. How can employers effectively support employees’ informed decision-making, and advocate for more and better information publicly available? What are the emerging best practices for translating complex data into actionable consumer information? How can industry leaders, transparency vendors, payors and purchasers work together in the spirit of transparency to support consumers in navigating the new realities? Top 10 Private Exchange Statistics That Matter (and Some That Don’t) Now is the time to look at cold, hard facts. Predictions have been made, and now there is raw data to fact check these predictions. Living in a post-ACA world has generated plenty of stats, speculations, theories and confusion on the employee benefits industry transformation. Brokers are faced with new competition; CEO/CFOs are forced to rethink their benefits expense and efficacy; HR administrators are up to their ears in paperwork while employees struggle to understand what this all means for their benefits options. In this workshop, Jeff Yaniga cuts through the clutter, sharing facts that matter. Presenting a curated selection of statistics and insights, Yaniga will fact-check information presented by benefits pundits and provide thought-provoking ideas on how to optimize the way we all do business. The attendees of this session will take away these learning points: • Signal versus Noise: Numbers that are irrefutable that you can take to the C-suite. • Which analysts got their predictions right? What are their predictions for the next three years? • Why these stats matter and immediate actions to consider.

Can Price Transparency Promote High-Value Care? Most Americans still don’t have access to meaningful information about the price of their health care. During this workshop with key transparency experts, the audience will learn: • Several website resources (by health plans and independents) to use for health care pricing • How private sources are making marked improvements in showing value (cost and quality) • Why surveys say less than two percent of employees use these tools • Why the public domain lags behind for consumers without access to price through their employer MODERATOR: Doug Field, CEO, The Institute for HealthCare Consumerism PANELISTS: Donna Smith, Executive Director of Business Development, FAIR Health Eric Bricker, MD, CMO, Compass Professional Health Services J. What’s Working, What’s Not and What Lies Ahead: Leading Brokers/Consultants Discuss Current State of Private Exchanges Some advisors predict enrollment on private exchanges could outpace enrollment on public exchanges by 2018. Other experts are more cautious, saying private exchanges for active employees have yet to prove their bottom-line value to employers. With only a few years since private exchanges shifted to the servicing of active employees, there is still quite a bit of uncertainty about the future of private exchanges. What lies ahead depends on how exchanges – both private and public – perform. In this workshop session, a panel of private exchange experts representing leading brokerage and consulting firms will divulge their thoughts on: • Private exchange models and how they “fit” for different workforces • Market trends, such as the shift to defined contribution • Particular recommendations on defined contribution and exchange solutions they are offering their clients MODERATOR: Doug Field, CEO, The Institute for HealthCare Consumerism PANELISTS: Sanjiv Luthra, Private Healthcare Exchanges Practioner, Deloitte Consulting LLP Rob Harkins, Practice Leader, Private Exchanges, Willis Rick A. Consumer-Driven Treatment Choice: Finally... The New Standard of Care The increasing role of consumers in the post-ACA and high-deductible world is well documented, especially with high health care costs and up to 30 percent of health care documented as inappropriate, wasteful or harmful. Consumers and providers need to routinely employ a consistent process of Shared Decision-Making (SDM) with a common language to have the consumer choose the best possible treatment based on medical evidence and patient values and preferences. Explore the compelling value of SDM, learn how big data is driving technology-enabled tools designed to bring evidence, patient-reported outcomes and individual preferences together to propel patients and providers onto a path of higher quality care at a lower cost. Employers, health plans and health management vendors will all benefit from learning about the necessity of Shared Decision-Making (SDM): • The historical evidence that shows why and how SDM, in both current and emerging models, has become a national standard of health care • How to identify key data that target opportunities and estimates the value of SDM for any population • How SDM fits in your consumerism strategy to engage members, reduce inappropriate care, improve health and reduce costs SPEAKERS: Barbara Flitsch, VP, Sales and Marketing, WiserTogether Inc. Key takeaways from this session will be: • The ROI of a financial wellness program includes reduced absenteeism, lower health care costs, improved employee morale and increased retirement plan participation. • There are key generational differences regarding the obstacles to financial wellbeing and financial priorities when comparing the Millennials to Gen X and the Baby Boomers. • Adding a financial component to an overall wellness program can be more beneficial than providing financial education strictly through the lens of the retirement plan.

In this panel, we’ll hear from experts representing different facets of the health care ecosystem. With health care continuing to become more consumer-centric, engagement strategy has risen in prominence in helping to support individuals with their healthcare decisions and health behavior choices. What messages do they deliver to successfully encourage and empower consumerism skills? In this workshop, panelists will share their experiences and provide actionable ideas and recommendations on ways to improve engagement from different perspectives.
