WELCOME LETTER Welcome to the second Private Exchange FORUM produced by The Institute for HealthCare Consumerism. In March, we held our first private exchange-specific event in Dallas and witnessed a sell-out crowd of nearly 600 attendees gather to discuss the present and future of private exchanges. Due to the collaborative energy and excitement at that event, we are excited to host our second Private Exchange FORUM of 2015 in Baltimore. Over these two days, various experts representing all industry stakeholders will attempt to unravel the confusing tangle of models, choices and offerings that is the current private exchange market and attempt to construct what the private exchange future will look like for all stakeholders. There is a real hunger for information right now as each industry stakeholder envisions how its business will change with the arrival of private exchanges. With that in mind, the Private Exchange FORUM agenda has been designed to take attendees on a journey through the process of evaluating and selecting a private exchange model. The Fundamentals of Private Exchanges This hands-on, pre-conference course will help employers and those working with them understand the various exchange models and key points in determining whether a private exchange is right for your company, your employees and/or your clients. More and more employers are responding to industry surveys similarly: they see private exchanges as a real option and perhaps the clearest path and means to accomplish their primary goals as it relates to health and benefits: cut health care costs; reduce administrative burden; improve choices provided to employees covered under the company health plan(s); increase access to coverage for those employees not currently under a company plan; and maintain valid options for active and retired employees. Yet, they state that the key questions remain unanswered. This course is designed to answer many of your questions as employers and to support you in the development of a singular strategy related to private exchanges that works in your company’s sole case. We will start by ensuring your goals, objectives and strategies fit with the private exchange model. We will then together evaluate various potential choices among the many types of exchange vendors and attempt to pinpoint credible sources for your business. We will explore the challenges present when considering exchange solutions and review critical features and capabilities to look for and weigh while on your search.

PRE-CONFERENCE CERTIFICATION COURSE Tuesday, Sept. 1 8:30 a.m. to 12:30 p.m.

Special Certification Program for Brokers on Self-Funding Developed by experts in the market space, this four-hour course instruction will ensure that students understand the key technical components of self-funding and that brokers are better prepared to counsel their clients on the various benefits of self-funding, including eliminating most premium tax, lowering the cost of administration, lowering the cost of claims/administration and lowering the cost of customer service to employees. Additionally, this session has been extended by one hour to now include a primer on captive insurance products for health benefits and their relationship to self-funding, including how private exchanges may be used to manage this offering to employees and dependents. While anyone is welcomed to attend this course, the program is especially suited to brokers who want to gain a better understanding of self-funding options for their clients. CEU credit of four hours is available for this course in all 50 states. This course is being offered in partnership with the National Association of Health Underwriters (NAHU).

Private Exchanges: What’s Working, What’s Not and What Lies Ahead An expert panel of objective consultants deliberate the current state of private exchanges, predict where the market is headed and identify the pain points for employers using current exchange models. The primary focus of private exchanges in the last several years has been on retirees, those covered by COBRA, part-time and other seasonal workers. However, the transition of active employees to private exchanges in 2014 and 2015 has infused the market with new energy, causing some to take a step back and examine whether implementing a private exchange for active employees makes sense. According to Forbes, many advisors – including Accenture, Goldman Sachs and William Blair – predict enrollment on private exchanges to outpace enrollment on public exchanges by 2018. On the other hand, some experts have adopted more cautious posturing, warning that private exchanges for active employees have yet to prove their bottomline value to employers. Come hear this keynote panel discuss what’s working, what’s not and what lies ahead. Understanding Private Exchange Models and Implementation Options The private exchange provider’s perspective: innovators explain technology behind private exchanges. With so many private exchange options available in the market, finding the right fit for each employer – taking into account an employer’s workforce and unique benefits requirements – can be challenging. Employers can, however, use this technology to deliver an online environment that allows consumers the opportunity to shop for health care much like they do for other consumer goods, including comparing costs and making value determinations. With these advances in technology, a single platform may be used for different purposes by different parties, including health plan insurers, health plan sponsors, third party administrators and employees. If your company is one of the forty-five percent of employers who are considering implementing private exchanges for their full-time employees before 2018, you don’t want to miss this session. What Defined Contribution and Private Exchange Solutions Are Leading Brokers/Consultants Offering Their Clients? A panel of experts from the nation’s four largest private exchange providers share lessons learned based on hard data derived from multiple enrollment periods. In this session, Aon Hewitt, Mercer, Towers Watson and Buck Consultants discuss how the combination of private exchanges and employer-funded defined contribution accounts has resulted in better health care cost management. These “big 4” private exchange carriers command more than 75 percent of the private exchange market, with more than five million covered lives. These companies serve many large companies, including Walgreens, Time, Sears and Darden. Employers that completed a second-year renewal had average annual cost increases of 2.6 percent over the two years they were on the exchange. By comparison, the average cost increase during that same time for large employers with similar benefit structures is projected to be 6.5 to eight percent this year. This expert panel will share thoughts on the evolution of defined contribution as used in connection with a private exchange solution and trends they are observing regarding costs of care, enrollment levels, etc.

Defined Contribution Health Care — Why Do It, How to Do It and What to Do in Renewal Years The audience will hear from four different industry stakeholders – all with extensive experience with defined contribution health care and private exchanges. The defined contribution concept remains confusing to many employee benefit professionals. Yet, it is critically important in the context of employee benefits.
