Appendix 10 Acknowledgement Form
The following guidelines should be honored at all times in the weight room:
1. There must be a TAS Athletic Department team member present for any workout performed by students and other faculty. 2. The weight room is primarily for use by upper school students and faculty. Classes who wish to access the weight room during daytime hours must work around those already utilizing the facility.
Parents and friends of faculty are NOT permitted to use the Tiger Health and Wellness Center without express written consent. 3. Middle School students are prohibited from using the Tiger Health and Wellness Center. 4. TAS Alum are not permitted to use the Tiger Health and Wellness Center 5. Shirts, athletic shoes, and proper athletic attire must be worn at all times. 6. Lower Body Wear: Athletic shorts, pants, tights, etc. No jeans or non-athletic wear. 7. Upper Body Wear: T-shirts, tanks, and sweatshirts. 8. Footwear: Athletic shoes only. No open toe shoes allowed. 9. Users are responsible for all valuables and personal items. Personal belongings such as backpacks, wallets, etc., may not be placed anywhere in the Tiger Health and Wellness Center. There is storage located outside the weight room for personal belongings. 10. Users should not attempt to use equipment on which they have not received instruction. User should ask the supervisor on duty if instruction is needed. 11. Covering the padded equipment with a towel helps prolong the life of the equipment and keeps everything clean for the next user. 12. Users should immediately report any injury or facility/equipment problems to the supervisor on duty. 13. Users must remove and rack all weights after using a piece of equipment. Dumbbells must be returned to the proper storage station. 14. Collars are mandatory on all Olympic bars when in use. 15. Users should not move equipment around or modify equipment in any way. 16. Use exercise machines and equipment only for their intended use. 17. Slamming or dropping of free weights is prohibited. Weights should not be leaned against walls, glass, or equipment. 18. Spotters are encouraged when appropriate. 19. Screaming or use of offensive language will not be tolerated in the facility. 20. Equipment and towels shall not be removed from the area for any reason. 21. Medical check-up and approval by a personal physician before participation in any physical activity is highly recommended especially for men over 40, women over 50, and persons with any potentially limiting physical condition (pregnancy, back or heart problems, diabetes, etc.).
FAILURE TO FOLLOWANY OF THESE RULES AND POLICIES COULD RESULT IN LOSS OF TIGER HEALTH & WELLNESS CENTER PRIVILEGES.
Appendix 10 – Acknowledgement/Participation Form Signature Sheet
This Student-Athlete/Parent Handbook is a supplement to the Upper School Student Handbook. Though not exhaustive, it contains important general information and expectations for TaipeiAmerican School (TAS) student-athletes and parents. In order to be properly informed of the policies of TAS athletics, both student-athlete and parent should review each section of this handbook. This also serves as a parent permission form for your child to participate on TAS sports teams. Parents and student-athletes both must read and sign this form, and student-athletes must return this page to the athletics office prior to the first tryout date [a digital Signature for this form is acceptable through the parent portal (for parents) and US sign-ups (for student-athletes just select “YES” in the sign up form to acknowledge you’ve read and reviewed the handbook and this form)].

Athletic Responsibilities Acknowledgment
1. Prior to participating in any practice or tryout sessions for any TAS sports offering, each athlete must successfully pass a physical examination by a registered physician.Acopy of the Physical Examination form must be on file in the TAS health office. 2. The student-athlete and parent must acknowledge and agree to the following statements:
As a TAS student-athlete participating voluntarily in interscholastic athletics, I understand that:
1. I understand that as a TAS student-athlete I am required to house student-athletes from other schools when
TAS hosts athletic events that involve traveling teams. The minimum requirement is to house once per upper school sport that I am involved in. I understand that if I am unable to house I will find a replacement. 2. I will abide by TAS’s rules as outlined in the TAS Student-Athlete Parent Handbook, the coach’s team rules, and/or IASAS conference rules (outlined inAppendix 3). Violations may result in suspension or dismissal from the team and further disciplinary action from the US administration orAthletic Director. 3. I will conduct myself in an exemplary manner at all times, in accordance with TAS philosophy of athletics. 4. I will be responsible for all athletic uniforms and equipment issued to me throughout the season. I will return such equipment at the conclusion of the season, and I will cover any damage or loss to such equipment by paying the current replacement cost for any of the equipment and NT$4000 for uniforms not accounted for by me at the end of the season. 5. I will not use or be in possession of tobacco, alcohol or other drugs at any point in the season. If I do use any of these substances, am in possession of such substances, or am suspended from school for use or possession of these substances, I will be subject to disciplinary action and possible dismissal from the team. 6. I acknowledge that I have been properly advised, cautioned and warned by administrative and coaching personnel of TAS that I am exposing myself to risk of injury, including but not limited to, the risk of sprains, fractures, and ligament and/or cartilage damage which could result in a temporary or permanent, partial or complete impairment in the use of my limbs, brain damage, paralysis; or even death. Having been so cautioned
and warned, it is still my desire to participate in sports and to do so with full knowledge and understanding of the risk of injury. 7. I, along with my parents, certify that I have read and understand all the policies of the TAS Student-Athlete
Parent Handbook and in order to be eligible for participation I must comply with all the requirements listed. 8. I acknowledge that if I am participating in a varsity team, I will be required and will be able to travel internationally to exchanges and IASAS events, which will require a financial commitment, a current foreign
passport (non-Taiwanese) with six months validity for the dates of travel and where necessary, an eligible visa in order to travel.
9. During these uncertain times due to COVID-19, I understand and will abide by all current Taiwan CDC mandates as it relates to travel and quarantine restrictions upon arrival into Taiwan. Furthermore, I will comply with all host country travel mandates.
As a parent of a TAS student-athlete participating voluntarily in interscholastic athletics, I understand that:
1. I hereby give my consent for my child to represent his/her school in athletic activities and associated travel. I
understand that when departing from and returning to Taiwan on school-sponsored trips, my child must use a foreign passport with at least six months’validity and a validAlien Resident Certificate (ARC) (where applicable). 2. I understand that as a parent of a TAS student-athlete I am required to house student-athletes from other schools when TAS hosts athletic events that involve traveling teams. The minimum requirement is to house once per US sport that my child is involved in. I understand that if we are unable to house my child will find a suitable replacement.At least one parent will be present when hosting guests.
3. I authorize the school to obtain, through a physician of its choice, any emergency medical care that may become reasonably necessary for the student-athlete in the course of such athletic activities. (NOTE: The school will make every effort to make use of your child’s health and injury information). 4. I agree we/I will not hold the school or anyone acting on its behalf responsible for any injury occurring to the above-named student-athlete in the course of such athletic activities or such travel. 5. I have read the eligibility rules and regulations of student-athlete participation listed above (Student-Athlete
Agreement, bullets 1-8) and fully understand and support them. I further understand that should there be a time whereby either my son/daughter or the school is not in accordance with these guidelines, I may request a conference within three calendar days of such time with the school personnel—Athletic Director and/or coach—to discuss the matter further. 6. I give my permission for the student to have a baseline and post-concussion ImPACT (Immediate
Post-ConcussionAssessment and Cognitive Testing) administered by TaipeiAmerican School. I understand that my child may need to be tested more than once, depending on the results of the test. I understand that my student-athlete should be cleared by a physician and a TaipeiAmerican SchoolAthletic Trainer before returning to athletics or any activity following a concussion. 7. During these uncertain times due to COVID-19, I understand and will abide by all current Taiwan CDC mandates as it relates to travel and quarantine restrictions upon arrival into Taiwan. Furthermore, I will comply with all host country travel mandates.
The signature(s) below confirm our acknowledgement and understanding of the information in the TAS Athletics Student-Athlete/Parent Handbook and serves as parental consent for my child to participate on TAS competitive sports teams. We acknowledge the importance of complying with both the letter and the spirit of the rules, regulations, and guidelines contained in the handbook to include all IASAS rules and guidelines as outlined in Appendix 3. We further acknowledge that these policies are not exhaustive and do not bind TAS, and that TAS may modify, depart from, or replace these policies without notice.
