The Advanced Dental Implant Institute and the Medical Sciences Campus, University of Puerto Rico (MSC-UPR) have the pleasure of announcing the The Puerto Rico MaxiCourseÂŽ Program and Clinical Residency in Implant Dentistry, which will consist in over 400 hours of continuing education, and will span from September 2020 up to June 2021. The course is presented in 10 sessions. However, due to the nature of the program, space availability is limited. Sessions will combine lectures with surgical procedures on live patients, cases discussions, lab exercises, anatomic dissections and IV Sedation training at no additional cost.

Dear Colleague: Thank you for registering in The Puerto Rico MaxiCourse® Program and Clinical Residency in Implant Dentistry, the most aggressive, challenging, innovative, cutting edge (and enjoyable!) dental implant program in America and the Caribbean! Participating in the Puerto Rico MaxiCourse® will most likely be one of the most valuable investments in your dental career. During the next ten months, immersion in this unique academic and clinical experience will transform your professional approach to Implant Dentistry. While supporting you in the quest for creating your own success story as an Oral Implantologist, you will learn the latest trends on medications, oral and intravenous sedation, surgical and restorative concepts, soft tissue and bone manipulation and the principles of NIRISAB. You will be exposed to hands-on applications and techniques on bone reconstruction (onlay bone grafting, bone expansion, interpositional segmental osteotomies) and nerve repositioning, among other procedures. Throughout the Program, you will have the opportunity to acquire unique concepts and techniques in scientific research, enabling you to sharpen skills in the development and journaling of your individual research projects and findings. Policies concerning the Puerto Rico MaxiCourse® Training Program I- Admissions 1. The Puerto Rico MaxiCourse® Program accepts no more than 20 participating doctors per Class. The limited spaces are due to our determination to guarantee a privileged mentor/student ratio. Also, our facilities (conference room, tables and chairs) accommodate comfortably 20 students. Two spaces are always available for Former Participating Doctors that request to extend didactic and clinical experience with Dr. Tatum and Dr. Pedroza. 2. The prerequisite requirements established for participating doctors are the following: a) Completion of the application process by filling out the application form provided. b) Proof that they have successfully completed a degree in Dental Medicine by providing an Official Transcript and Degree Certification Letter. Please note all of the previous must be completed by the first session. 3. Doctors interested in enrolling in the upcoming Class must to complete the following procedures: a) Submit a completed Application Form through either: Email - prmaxicourse@gmail.com Fax - (787-781-5030) Mail - The Advanced Dental Implant Institute P.O. Box 361357, San Juan, PR, 00936 b) Pay a $2,000 non-refundable deposit to reserve their space. c) Provide required documents to complete enrolling process. 4. The Board of Directors of the program is in its complete right to accept or deny any applications they see fit. 5. An early bird application will be accepted from December 1st to January 31th with a $300 tuition discount. The entire enrollment process must be completed by that time, including the Official Transcripts and Degree Certification Letter. 6. The established deadline for the regular application process is April 30th. 7. Late admission fee will be charged to those who apply from July 1 to August 31, upon availability. II- Participation 1. Participating doctors must follow the established requirements of the training program, including: a) Attendance to all sessions. b) Each doctor shall be responsible for one's conduct from the time of admission through the actual awarding of the degree. c) Doctors whose misconduct has a direct and distinct adverse impact on the Institution's community, its members and staff will be referred to the Board of Directors for evaluation. d) The program's Board of Directors is in full right to dismiss/terminate any doctor who they understand put at risk the integrity of the Institution. 2. The Puerto Rico AAID MaxiCourse® is a training program intended for Dentists that have completed a DMD or a DDS degree. No other professional will be accepted in the training program.


Policies concerning the Puerto Rico MaxiCourse® Training Program

III-Requirements to receive the completion certificate 1. Doctors must complete all ten (10) sessions of the training program in order to receive the certificate of completion. 2. In case of an extraordinary situation that hinders the participating doctor to complete the ten sessions, he or she is responsible of making up the missed session in agreement with the Directors. 3. Doctors must pay the entire tuition as established. 4. Doctors must follow all requirements for the training program, including participating during clinical sessions. 5. Doctors must successfully approve both written and clinical tests that will be administered during the training program. IV-Requirements to receive the IV Sedation Course certificate 1. Doctors must complete the totality of the training program (ten sessions), including the IV Sedation course offered by Dr. Daniel Becker and Dr. Shufflebarger. 2. Doctors must approve written, oral and clinical exams administered by IV Sedation instructors. Directors may require further instruction or practice to participating doctors in order to receive certification of completion on IV Sedation. 3. Participating doctors in need of further instruction or clinical practice are allowed to come back to the training program, upon space availability and by paying a $350 fee per session. 4. Doctors interested in acquiring the IV Sedation permit must submit the application issued by the State where they intend to practice, complete all necessary documentation, pay the established fees (which vary by State) and invest in all required instrumentation and equipment established by their State Board. The Advanced Dental Implant Institute's responsibility concerning IV Sedation is to offer at least 60 didactic hours and at least 20 patients to practice, as established by the American Dental Association guidelines, and to issue the IV Sedation certificate of completion including the IV Sedation charts from the required minimum of patients, as established by the ADA guidelines. V-Requirements of Former Participating Doctors interested in ongoing mentorship through the PR MaxiCourse® Program 1. Former Participating Doctors interested in coming back to participate in the academic and clinical sessions of the PR MaxiCourse® Program are allowed to continue developing their knowledge and surgical skills during the program's sessions, upon space availability and a $350 fee per session. 2. Former Participating Doctors must contact the Administrative Director, Dr. José E. Pedroza Rodríguez through prmaxicourse@gmail.com and request permission to participate in a specific session. 3. Former Participating Doctors are also allowed to bring patients for specific surgical procedures. These doctors are not required to pay the $350 fee. However, they must follow the protocols for bringing patients to the training program, including a case presentation to the class, participating in case study discussions, performing the procedure under the instructors' close supervision, and following-up with the patient. 4. Former Participating Doctors interested in enjoying a refreshing course with Dr. Becker and Dr. Shufflebarger, should contact the Administrative Director and pay a $450 fee, upon availability.


Policies concerning the Puerto Rico MaxiCourse® Training Program

5. Former Participating Doctors interested in enjoying a refreshing Anatomy course should contact the Administrative Director at least two months in advance and pay a required fee, upon availability. VI-Financial policies 1. Tuition for the Puerto Rico MaxiCourse® Program is $21,950. 2. A $2,000 non-refundable deposit is required to separate a space. 3. Total cost includes the following: a) Over 400 continuing education units, certified. b) An IV Sedation course that follows the ADA guidelines, also certified. c) $2,000 worth of Tatum Surgical instrumentation d) Lunch and assorted snacks. e) A one year membership with AAID f) Surgical gowns and instrumentation used during surgical sessions. g) Anatomy course with fresh specimens. h) The opportunity to bring patients and perform surgical procedures. i) The opportunity to return for follow-up sessions (with a $350 fee per-session) or for surgical procedures. 4. Total payment of tuition may be paid in full during the first session with a 7% discount. Payment methods of total tuition include personal checks, cashier checks or cash. 5. For the participating Doctors convenience, there is also a five equal payment installment option. Tuition balance (after completing the $2,000 non-refundable deposit) installments are due on the following sessions: September, October, November, January and March. 6. A $1,000 discount of $19,950.00 and a 10 months installment option as payment method is allowed for newly graduated dentists upon request. 7. Failure to comply with the financial policies may require the termination of the doctors' participation in the training program. VII-Reimbursement 1. In cases of extraordinary situations that oblige the participating doctor to interrupt his or her training program, the participating doctor should inform the Administrative Director as soon as possible. Payments will not be reimbursed. 2. If the participating Doctor has made the payment in full, reimbursement policies are as followed: a) Upon the termination of the program during or after the first session, a 72 % will be reimbursed. b) Upon the termination of the program during or after the second session, a 46 % will be reimbursed. c) Upon the termination of the program during or after the third session, a 36 % will be reimbursed. d) Upon the termination of the program during or after the fourth session, a 23% will be reimbursed. e) Upon the termination of the program during or after the fifth session, no reimbursement will be realized.


Policies concerning the Puerto Rico MaxiCourse® Training Program

VIII. Facilities 1. The Puerto Rico MaxiCourse® offers its sessions at the facilities of The Advanced Dental Implant Institute, in Parkside 6 St. San Patricio II Apts, Suite 1, Guaynabo, PR. The facilities include five dental operatories, two of them fully equipped with a Hospital-like setting. Also, a Conference Room with a fully equipped kitchen, CT scan machine, close circuit projection system, projector, conference tables and chairs that hold up to 18 Doctors. During the Anatomy session, participants meet at the Surgical Research Laboratory (10th floor), Surgery Department, School of Medicine, University of Puerto Rico. 2. The only equipment, supplies or instruments each student need to supply are their own scrubs and a USB memory drive or a hard disk. Electronic copies of all lectures are available when requested. Also, we can provide videos of some surgical procedures, IV Sedation techniques, patient preparation, asepsis during surgical procedures, etc. IX-Schedule 1. The schedules for each one of the sessions are established monthly. It is important to stress that each session schedule is created before the monthly session begins. This is so because The Puerto Rico MaxiCourse® Program focuses on live surgeries during most of its sessions. Patients interested in participating in the educational program are scheduled, and specific discussions are offered according to the surgeries scheduled. For that reason, every year the schedules change. However, there are specific topics that are discussed by particular instructors during each one of the sessions. Please see the upcoming yearly schedule.

REQUIREMENTS A-Participation Ÿ Tuition cost includes ten (10) academic and clinical sessions during the year. If

by any reason you cannot assist to one (1) of the ten (10) sessions, we offer a onetime chance to make it up during next year's Program, based upon availability. ŸWe encourage our Participating Doctors to bring as many surgical patients as

possible during the training program. Nevertheless, it is imperative that you follow the protocols of screening, selection, communication, overall management and surgical approach in regards to your patient, in order to guarantee the best service possible and to comply with the medical-legal parameters. Please refer to Protocols for Bringing Patients to PR. ŸYou are welcome to continue bringing patients to the Program, even after your

training has been completed, based upon the specific session's availability. It is important that you request your space with Dr. Pedroza, the Administrative Director and follow the protocols for bringing patients to Puerto Rico. ŸYou are welcome to attend specific sessions after you have completed your

training program, upon availability. IV Sedation Course and Anatomy require a particular fee. Please contact the Administrative Assistant. If you are willing to attend any other session, a fee of $350 per session is required. It would please us to see you again. B- Payments ŸWe provide the option of a payment plan that consists of five (5) equal

installments of $3,990 from the commencement of the Program (September) up to the Seventh Session (March), at no interest. ŸA credit card file is required to charge the agreed amount during the following

sessions: September, October, November, January and March. Ms. Kathiria Martínez, the Administrative Assistant will provide you with your credit card receipt; we also provide an updated statement of your balance after each payment. If by any reason you should request to change the payment method, we appreciate that you promptly inform the Administrative Director. ŸAn Authorization document will be filled out at the beginning of the first session

to facilitate the payments. ŸAll payment agreements should be responsibly fulfilled prior to the conclusion

of the training program. Your participation in this Program is privileged: admission is limited to only a small group of doctors per Class. Completion of the Program's requirements should be a priority.


REQUIREMENTS ŸAnother option we provide is to pay the total tuition during the first session. If

you agree to do so, you will receive a 7% discount off $19,950.00 that is $1,396.50. This payment should be made by check or manager check; please, no credit cards. ŸYou have been granted a privileged space to receive the best possible one on one

mentorship during your training process. It is our pleasure to help and guide you through the challenges of this extraordinary academic and clinical experience. If by any reason you are experiencing difficulties to comply with the payment agreement, the Participant Doctor should approach the Administrative Director to negotiate a new agreement that will guarantee the completion of the tuition fees. However, in the unexpected contingency of failing to fulfill new payment opportunities offered, TADII Directors will consider termination of your participation in the Program. Ÿ· Deposits and tuition payments are non-refundable. Please refer to Policies

Concerning the PR MaxiCourse. C- Other benefits ŸThe tuition cost includes a $2,000 credit for Tatum Surgical instrumentation.

You are allowed to choose among three different packages of instrumentation from Tatum Surgical during the 10-month Program. The Administrative Director will provide more details. ŸThe tuition cost includes an AAID one year membership. ŸThe academic and practical IV Sedation training offered as part of the Program,

and it complies with the ADA requirements (60 didactic hours plus at least 20 patients). Each Doctor interested in submitting the required documentation in his/her own State is responsible for handing in the application documents to the Administrative Director at the beginning of training. In addition, each Doctor is responsible for filling out the IV Sedation protocol after participating in each sedation procedure. More details available under “IV Sedation Course”. ŸUpon completing the over 400 CEU during the training program, the

participants are welcome to consider AAID and AGD professional credentials. ŸAn Anatomy Course with fresh specimens is included in the training program. ŸThe Puerto Rico Maxicourse® has the Educational Implant Patient Program,

where the patients are evaluated and selected for the surgical experiences.


Course Descrip on You are about to begin a most exciting educational journey. Chances are it will challenge you. It will enlighten some (perhaps) elusive concepts. It will definitely help you develop surgical skills, will increase your capacity to diagnose and treat complicated surgical cases, and will broaden the scope of implant dentistry. If you are a general dentistry practitioner, this investment will take you beyond the learning curve achieved by the average Dentist. Most of all, you will be able to expand the service you offer to your patients and incorporate life changing oral procedures. I have written some common questions and their answers, so you may become familiarize with the training program.

How does the Puerto Rico MaxiCourse® work? a) A small group (close to 20 Doctors registered in the one-year program!) gathers every month for 4 days to fully participate in ten dynamic sessions. b) The sessions emphasize in live surgeries, so most of the lectures serve to discuss the scientific background of the techniques you'll be learning during the live surgeries. Also, an outline of suggested readings from Carl E. Misch Contemporary Implant Dentistry 3rd Edition, will be provided. c) Dr. Tatum and Dr. Pedroza will develop lab exercises and mentorship sessions, depending on the need of the group, to strengthen surgical skills. d) We have organized the surgical tasks in different teams, and all the Doctors will switch responsibilities so as to offer equal opportunity to all. e) There is a strong emphasis on IV Sedation and the Doctors will be trained and evaluated accordingly. f) You will have the opportunity to bring your own cases discussions, to bring your own patients and do surgery on them while personally mentored by the instructors. g) It is important that you contact the Administrative Director, Dr. José Pedroza, so he may schedule your case discussions and the patients you are bringing to the program. 2. What are the benefits of registering in the Puerto Rico MaxiCourse®? a) Registering in the Puerto Rico MaxiCourse® will provide you the coverage you need to perform surgical procedures in your patients or to assist Dr. Tatum or Dr. Pedroza while they do the surgeries and teach.


Course Descrip on b) At the end of the training program you'll receive a CertiďŹ cate of over 400 CEU according to the AGD Program Approval for Continuing Education (PACE). c) At the end of the training program you will have received over 60 hours of IV Sedation instruction plus practiced in over 20 patients, thus allowing you to apply for the IV Sedation license, if your State Dental Board follows the ADA requirements. d) You will beneďŹ t for an ongoing (and true) mentorship relationship with the main instructors, even after your training has concluded. e) You will be allowed to come back to Puerto Rico and bring patients for surgical procedures, upon availability. You may also come back to continue enriching your learning journey. For most of the sessions you will only pay a reasonable fee for food expenses. IV Sedation Course and Anatomy Course do require a higher fee. Because of availability reasons, it is vital that you contact the Administrative Director beforehand. 3. Do I need to be experienced in implant dentistry? No, not at all. We have had participating Doctors with no experience at all, and we have had participating Doctors that have been placing implants for over 20 years. Some of our students have gone through a myriad of professional courses and training. Their names and phones are available for you to contact them and inquire about their experience. 4. Is it a requirement that I bring my own patients to the program? It is not a requirement; however, we encourage you to bring your own patients. This will be your opportunity to practice and face the challenges of doing surgery in a live scenario, with Dr. Tatum and Dr. Pedroza watching over your shoulder. 5. What happens if I don't bring my own patients to the program? a) You will do a lot of practice still. For instance: * watching surgeries (close to 40 during the 10 months sessions!) * assisting in some of them * becoming responsible for the pre and post-op patient management * participating in the Sedation protocol of the patient * participating in the treatment discussion and in the surgery discussion Remember, you'll become a trainee, not just a listener. There are Doctors that never brought a patient, and their names are available for you to inquire about their experience.


Course Descrip on 6. Are there any other options if I want to perform some procedures but cannot bring my own patients? Will it cost me? a) Yes. We update every year a list of patients willing to participate in the program, in need to undergo different oral surgical procedures but unable to pay for their treatment. Those patients are available if you are willing to pay for the procedure, and if Dr. Tatum and Dr. Pedroza understand that, you are ready to perform the procedure under their close supervision. Remember, the patient's wellbeing is always a priority. b) We have prepared a list of special fees according to the average costs of oral procedures in the United States. Some of our fees are close to 50% lower than the average costs. You will be provided a chart with our fees. 7. What is the protocol if I want to bring patients to the program? You will contact to schedule the case discussion during the next session. After your case has been discussed and approved by Dr. Pedroza, you will contact Kathiria Martínez, the Administrative Assistant, and she will hand you a registration form for your patient. You have the responsibility of presenting the case to your patient and discuss with him or her the details of the surgical procedure, including the financial arrangement, as well as following up after the surgery is performed. Your patient will contact Kathiria to make an appointment. The appointment will be made with a $500 deposit. Kathiria will follow up your patient, including emailing the instructions for oral surgery. We will provide you a document with all the details. 8. Do I need to do any kind of homework? a) If you choose to, you may want to prepare some of your challenging cases for discussion. The cases will be discussed in the group and there is no limit for you to bring cases discussions. Dr. Pedroza will provide a protocol for the case presentations, which include pictures, x-rays, study models, CT Scans, and any other necessary documents. b) There are some prerequisite readings for the IV Sedation course offered by Dr. Becker and Dr. Shufflebarger as well as for the Anatomy course. c) We encourage participating Doctors to have as a textbook Dr. Carl Mitch's Contemporary Implant Dentistry book. An outline of the specific chapters to read or review will be provided for your benefit. 9. Do I have to pay the whole program tuition before the program starts? a) No. You need to secure your space with a non-refundable deposit of $2,000. b) There is the option of either paying the tuition in full during the first session and receive a 7% discount of the amount $19,950.00 (no credit cards, please), or paying (with a credit card, check or money order) in five equal installments according to the info that appears in the pen drive and the webpage.


Course Descrip on 10. How do I dress? Very, very casual and with scrubs and comfortable shoes. However, bring a casual jacket for the air conditioned rooms. 11. When and at what time will I be leaving home? a) Check the schedule for every month. We begin at 8 a.m., so you'll need to be in PR at least the night before the session begins. b)For every session, we will end the day that appears in the schedule, so you will leave home the next day that shows in the schedule. For extraordinary situations out of our control, there may be slight changes in the schedule. We'll try to let you know ASAP. 12. Do I need to bring any equipment or material? Just bring your scrubs and comfortable shoes. You may also bring a laptop and loupes with light for your own use. Surgical materials, surgical gowns, dental materials, an intraoral camera, anatomic equipment, medications, are all provided. 13. How can I register? In addition, you may contact our office at 787-781-1831. We included the Schedule for Group 1 & Group 2, each of which contain 20 spaces. You can request a spot in any of the two groups. However, when either group is completed, participants will be registered in the remaining group.

Will appreciate and benefit from your efforts of keeping the room as clean as possible, by putting away any unused products. Hotel reservation and transportation 1. We are offering you a corporate rate at the Best Western plus Condado Palm

Inn Hotel. This is an affordable yet newly remodeled hotel in Condado, a touristic area with lots of amenities. The corporate rate was plus taxes. It includes a nice breakfast buffet, Wi-Fi speed internet, 24 hr. Access to fitness and to Business Center, and a weekly reception, Wednesdays from 5:30pm to 7:30pm. In addition, you will find a nice beach just across the street and many places to eat walking distance that stay open up late. 2. You may make reservations through Reservations@condadopalm.com or you may contact the hotel through 787-721-9501, Sales Department. Ask for the Puerto Rico maxicourse corporate rate! 3. Our recommendation is to stay at the Best Western during the first session in September, and depending on how you feel, plan for the upcoming sessions. 4. However, some participating Doctors are somewhat adventurous and, after the first session, prefer to look for condominiums and share lodging expenses with the new friends from the training program. Look for Airbnb options. 5. Taxis are still an option for transportation and UBER is also available in PR.


Traveling to Puerto Rico 1. Your destination will be Luis Muñoz Marin International Airport, in San Juan, Puerto Rico (SJU). You may take a taxi from the airport to the hotel. It will take around 15 minutes to get from the airport to the the Best Western plus Condado Palm Inn Hotel depending on the traffic, and the same between the airport and the clinic. 2. Since Puerto Rico is a territory of the USA, you will need NO VISAS. We use the same American money, English spoken as a second language, and Puerto Rican people well known for their hospitality and friendliness. 3. Puerto Rico is an attractive touristic destination. Outdoor sports are popular, beaches are necessary to enjoy, Old San Juan, with their historic treasures and touristic attractions, El Yunque rain forest, lots of outdoor sports, nice restaurants, hotels, and nightlife. 4. You are invited for a Welcome Cocktail Party, our dress code is casual. Usually, we find a place that will allow you to feel welcomed and become familiarized with a group of peers and instructors that chances are soon will make you feel at home. We will provide more details. 5. The last day of your training program, a Saturday night during June, the whole group will gather for a nice Graduation Ceremony and Celebration party. The invitation will be good for you and a significant other. Extra tickets for your family will be available at a reasonable cost. That will be the only day you will be required to dress in a dark suit and cocktail dress for the ladies. It will be an enjoyable occasion to cherish a much-deserved success while concluding your training.
