Australia’s COVID-19 Vaccine Rollout has commenced. As part of the national COVID-19 vaccine rollout strategy, South West Community Care employees will be eligible for COVID-19 Vaccination in Phase 1b.

South West Community Care has strongly encouraged all employees to receive the COVID-19 vaccination to support their well-being and reduce the risk of a COVID-19 outbreak amongst our employees, our consumers and the wider community.

Even as the COVID-19 vaccine rollout begins, it is important for all our employees and consumers to continue with COVIDSafe practices, including: • good hand hygiene • physical distancing • wearing a mask when required.

South West Community Care encourages consumers to talk to their GP or a health professional about the benefits and risks of the COVID-19 vaccination and to clarify when they may be eligible to receive it. The COVID-19 vaccination will be provided free of charge.

South West Community Care will continue to review and monitor our policies relating to COVID-19 and the vaccination to ensure all recommendations by the Department of Health are considered.

Transport Services with SWCC

Approval required

Once you have been approved for transport services we can help you get around.

SWCC has a range of fleet vehicles available at each office for consumer transport needs. Toyota Camrys , Corollas, Yaris’ and buses make up the fleet for your use. PLEASE NOTE: a minimum of 24 hours notice to be given to book or cancel transport bookings. Weekends are excluded from this notice period.

Transport with volunteer drivers

We have a team of volunteers who willingly give of their time to support transport needs. If you have to travel somewhere between 8 am and 4pm, our volunteers can assist you. They collect a vehicle from one of our offices, come to your home and take you to your requested destination. We also transport you home after your appointment.

Transport with care workers

Our care workers can be scheduled to assist you with transport requirements throughout the day. We have care workers who are able to assist if your medical appointment requires an earlier start than 8am or a return after 4pm.

SWCC transport is provided for:

 Any medical appointments– these requests take priority

 A social centre activity day– get transport to the social centre activity days

 An outing with SWCC to join others from our community

 A shopping centre– these appointments may have to be flexible times and dates depending on demand  Other appointments—will depend on availability of vehicles and drivers.

Please note that this service is for your convenience . We cannot include friends or family members in your trips, unless they have been approved by management and form part of your caring support. All transport will be invoiced– no cash payments required. Transport is charged based on distance travelled.
