Dr. Usha Parthasarathi is one of 700 health care professionals and staff who are re-imagining care at the new St. Joe’s West 5th Campus. Like Warren, Dr. P (as her patients call her) is also looking forward to tomorrow. Over 190,000 people in the region comprised of Hamilton, Niagara, Haldimand and Brant will be affected by mental illness at some point in their lives. Up to 280,000 more will have a substance use disorder. What’s more, we are not alone. The rise in mental illness and addiction in the rest of Canada and around the world, especially among aging baby boomers and young adults, is clearly documented. The impact on individuals, their families and society at large is profound. Mental illness will soon become the most common reason for disability in Canada. But while more care is needed, it is not the only solution. For too long, mental illness, those who suffer from it and even the way we provide care has been steeped in stigma – a widely-held bias that must be challenged in the full light of day to be overcome. To rethink the journey to mental health and addiction treatment, we must start by focusing on early diagnosis, patient recovery and new ways of caring with the community to address the pressing needs of our region. In the process, we can open doors to new thinking for the rest of the world. Join us at St. Joseph’s Healthcare Hamilton as we re-imagine care for the 2 1st Century starting at our West 5th Campus. ACCEPT THE CHALLENGE TO START AGAIN. A first-year university student, suffering from depression so severe that she can’t get out of bed, takes her own life. She is just one in the troubling increase of youth at risk in Canadian schools. A nearretirement age millwright loses his job of 30 years along with his wife and family, primarily as a result of substance abuse. They find him homeless a few months later, just one story in the growing narrative of troubled and aging baby boomers. It is for thousands upon thousands of people like them in our communities that we must reevaluate care for those living with mental illness and addiction. At St. Joseph’s Healthcare Hamilton, we are doing this as we renew the promise of health care for the 21st century in our community and beyond. It is time to challenge the status quo – to seek transformational answers to the real and difficult issues we are facing in health care in Ontario.

St. Joe’s new Margaret & Charles Juravinski Centre for Integrated Healthcare gives us the opportunity to find these answers with a view toward integration and collaboration – to change the dialogue; from one focused on capacity to one focused on research, diagnosis, treatment, recovery and most important, collaboration across the health sector and across communities. The vision for St. Joe’s new West 5th Campus began to take shape over a decade ago. With the need for mental health and addiction care growing rapidly, we focused on early diagnosis and patient recovery – a highly practical response to a growing concern for thousands of people in our community. The result of this vision is the Margaret & Charles Juravinski Centre for Integrated Healthcare – the first facility in Canada designed specifically for the future of mental health and addiction care blended with medical services, research, education and treatment.

Imagine yourself in a bright room. With a window of your own. Personal privacy, the ability to lock your door – the freedom to move about safely, walk outside, talk with friends and share stories. It’s what you would expect at home, or even while traveling. You’d be comfortable – more at ease. That is what care and dignity feel like for the patients in the Margaret & Charles Juravinski Centre for Integrated Healthcare. Re-imagining care started with creating an inpatient living experience that promotes personal dignity. The Inpatient Zone and unit designs allow freedom without compromising patient or staff safety. Single rooms and personal facilities for all support the independence and privacy that is so important to recovery.

We filled the new St. Joe’s West 5th Campus building with natural light and surrounded it with trees and walking trails. Patients can shop for clothes, get their hair done, read a book and share a coffee in the coffee shop. They can be a community – designed with intent to foster hope, healing and compassion. What’s more, the building was designed to be more flexible than buildings in the past, allowing the St. Joe’s team to adapt to changing needs and what they learn through research and clinical practice. Central to the spirit of innovation in the new building are the concepts of openness, access and hope. Dignity by design is the new model for transformative care in mental health and addiction.

PATIENT EXPERIENCE FEATURES • 100,000 ft2 of courtyards and gardens located in the building provide outdoor space to all patients. • Four transition apartments will help ease patients back into community living. • The hair salon, clothing store and coffee shop among other amenities contribute to positive community interactions. • Onsite outpatient medical services such as diagnostics will integrate care and reduce stigma in the process.

The historical and Hollywood mythologies of mental health institutions sometimes overshadows the reality of the amazing care and compassion that has been available in this region and others for generations. In Hamilton, our staff and professionals have gone above and beyond to transform care starting as far back as the mid-1800s. The new St. Joe’s West 5th Campus design enables staff and professionals to continue this tradition started here by the very earliest Superintendent.

Now 22-years sober, Ron spends his spare time helping others. “If you’re alone with this you’re doomed,” he says. Counseling others in recovery is part of Ron’s own continuing therapy.

Can one recover from mental illness and addiction? Hope is not a passive emotion. Hope inspires change. Giving a patient the hope that life can be better, perhaps for the first time, motivates the will to recover. To that end, the St. Joseph’s Healthcare Hamilton’s Mental Health and Addiction Program is guided by the principle that individuals should be provided with assistance and tools to optimize their individual potential for recovery. To that end, we help them see the potential of a balanced

and productive life, and give them the support and humane care they need to make the journey. In addition to the standard psychotherapeutic approaches and medications, we’ll now be able to add new modalities such as virtual reality therapy and transcranial magnetic stimulation. It will be home to a variety of rehabilitation activities including occupational and recreational therapy, vocational assessment and counseling.

As a centre for learning, research and practice, in one integrated space, we will have the capacity to create and evaluate innovative new treatments thanks to world-class teams of academic clinicians. After researching similar facilities from around the world, our talented architects have designed a space that will support collaborative, innovative and ground breaking work. Our people will act as one team, inspiring hope and treating for recovery.

ALL TOGETHER NOW Mental Health, Addiction and Medical Services The new Margaret & Charles Juravinski Centre for Integrated Heathcare really is integrated. We’re starting with a blend of inpatient and outpatient mental health and addiction services and medical services such as diagnostics and outpatient medical care. This purpose-designed integration brings the community at large into the facility and opens the door to reduced stigma as a result. It also gives inpatients the opportunity to receive specific medical procedures without leaving the campus, an important step in re-building dignity. But integration doesn’t stop there at St. Joe’s on West 5th. Nature and aerobic activity are now part of mental health and addiction treatment by design. Community engagement is more welcome. Spaces like the auditorium and conference centre and the pedestrian plaza will invite inclusion and improve understanding. Researchers, students and clinical professionals, as described more fully in the next section of this St. Joe’s West 5th introduction, will now work side by side, and with shared opportunity to identify and evaluate treatments in practice. By blending academic and clinical pursuits, we’ll be able to push innovation into mainstream care faster than ever before. St. Joe’s West 5th Campus is a new model for advancing mental health and addiction patient care by integrating every aspect of care to focus on recovery.

The new concurrent disorder treatment program means patients no longer have to separate or prioritize treatment for mental illness and addiction.

The YWC will serve 17-24 year olds, a population with one of the highest levels of mental health and addiction issues per capita. The Centre will link the community and system with the goal of rapid assessment, treatment and recovery.

Aerobic exercise has positive impacts on memory, concentration, emotional and overall health. To build on that knowledge and help local youth with the reality of growing mental health issues, St. Joe’s has created Team Unbreakable - a running group for youth that is focused on transforming care for young people who experience anxiety and depression.

Both patients and staff contributed to the design of inpatient unit care desks that provide staff with an unobstructed line of sight down each bedroom corridor to enhance both medical care and patient safety without intruding on patient privacy.

The most effective treatment for anxiety disorders is cognitive behavior therapy that includes exposure to situations that trigger phobia (such as flying or public speaking). When actual exposure is impossible, Virtual Reality technology allows the therapist to create a simulated situation that activates the phobia and allows for therapeutic exposure.

What if an entire hospital also served as a place of discovery? What if discovery and care were connected to inspire the next generation of professionals and staff? The answers to both questions will unfold at St. Joe’s new West 5th Campus. Integrating research, education and clinical care is common at many teaching hospitals throughout North America. What’s unique about St. Joe’s West 5th Campus is that the integration happens in one, connected space, designed specifically to facilitate collaborative science, innovation and care.

Over 200 Graduate and Undergraduate students will share the most sophisticated and innovative learning environment for mental health and addiction care in Ontario.

INNOVATION IS A WAY OF LIFE HERE. • The Peter Boris Centre For Addictions Research will be closely linked to key clinical programs of the hospital including access to research space near the concurrent disorders (addiction) inpatient unit. • The Academic Department will utilize much easier access to Youth Wellness Centre activities and meetings, in particular to support efforts in early detection and intervention. • 86% of hospitalizations for mental health in Canada occur in general hospitals. At St. Joe’s West 5th Campus, those patients will be admitted to an academic health sciences centre where some of this country’s best clinicians and inspired leadership will guide their journey to recovery.

In addition to blending mental health a nd addiction research with medical services, academic and clinical professionals alike w ill be able to work in close proximity to each other and their patients. This will provide greater flexibility in all aspects of science and study to address advances in technology, changing population demographics, changes in clinical practice and evolving research priorities. Working as one, we will be better able to address the needs of many, now and for the future.

All of the clinical units at St. Joe’s newest facility are designed as teaching units including screening and observation rooms and innovative technologies to enhance the learning experience. The close proximity of research and clinical programs make it easier for professionals and staff to meet and exchange ideas. The goal is to promote collaboration, but equally important, to create the most supportive climate possible for clinical research. This should result in a closer relationship between study and reality, and a greater opportunity

to implement research and study recommendations in a timely manner. Thanks to the West 5th Campus, St. Joseph’s Healthcare Hamilton now has a unique opportunity to develop a model for how an academically-focused hospital can support high-quality teaching, research and care in one space, and how each discipline can best reinforce the others.

While a significant percentage of the Canadian population has been touched by mental health and addiction either directly or indirectly, the lack of comfort with and understanding on the topic of mental illness remains a barrier to recovery. At St. Joe’s on West 5th, we are committed to tearing down that barrier. We’ve created an environment that welcomes our neighbours and the community with an invitation to share this new space with patients and staff. Building community bridges will serve to promote understanding, which in turn will contribute to the de-stigmatization of mental illness and addiction. Medical and mental health and addiction patients, staff and visitors will have access to dedicated public space that offers a resource library, walking trails, indoor and outdoor athletic space and an auditorium that seats up to 300 people. Our diagnostic services will expand capacity to treat all patients – not just those with mental illness – for MRI, ultrasound, CT scans and more. Specific medical outpatient services will be available for the first time at St. Joe’s West 5th, completing the new vision of mental health and medical care in one place. Tours, maps and visitor information will be readily available at the Welcome Centre on Level One near the Main Entrance. The Margaret & Charles Juravinski Centre for Integrated Healthcare is 855,000 square feet of LEED-compliant (Leadership in Energy Efficient Design), recovery-focused health care. It will serve 300+ inpatients every year and countless medical services and outpatients over the years. Inside its walls, we’ll build a community of hope as we replace the figurative walls built by stigma with a bright new future of care.

In 1936, according to records at the Hamilton Museum of Mental Health Care, over 1,800 patients (on just 1,300 beds) were cared for by just 84 staff on the old site of mental health care in Hamilton. • The recovery-focused inpatient zone features private patient rooms and unit-specific common rooms and quiet rooms all with clear sight lines to the patient and staff designed Care Desks.

ACHIEVEMENTS IN MENTAL HEALTH IN HAMILTON • One of the first mental health institutions to campaign against the use of restraints. • Applied the principles of moral therapy; treating mental health by treating the entire physical well being of the patient. • Created a specialized nursing school in 1909. • Among the first in Ontario to implement the Clinical Nurse Specialist role. • 1960 - opened the Auchmar Complex; utilizing open space design to create a hospital environment instead of an asylum environment.
