Dynamic duo endorses caring centre Don and Jean Chesworth have been supported by Sunbury Community Health for more than 20 years. Turning 90 and 89 respectively this year, the couple’s story is a dynamic one, marking the challenges and successes facing many as they age. Jo Howard, Health and Wellbeing Manager, shares that story: “In the early stages of ageing, Don and Jean attended a range of allied health services at our centre, including podiatry and physiotherapy, she explains. “They also joined a weekly exercise group to maintain good health and mobility in the years ahead. “Later, the pair asked for a home assessment from an occupational therapist who was able to give advice about kitchen and mobility aids as their physical functions and ability to be independent in everyday tasks became affected. “The Sunbury Community Health volunteer driving service helped them to get to medical appointments when driving out of Sunbury became difficult. As their health and mobility declined in ensuing years, it was time for a Home Care Package – and they chose Sunbury Community Health as their provider. They’d always felt happy over the years with the services and the people so it was an easy decision.

Jo explains what the Home Care Package did for Don and Jean. “They wanted to continue with independent living in their own home, so Don and Jean were allocated a Care Manager to work with them. “Now they get extra services, such as personal care for showers, household cleaning, gardening, physiotherapy, podiatry, transport, community access and shopping. “They play an active role in the management of their Home Care Package and have regular meetings with their Care Manager to review and discuss their Care Plan. Jo concludes by thanking Don and Jean for allowing the centre to be part of their lives over a long time. “We look forward to assisting you both for many more to come. We think you are both amazing members of the Sunbury community! Anyone interested in Home Care Packages needs to first inquire via My Aged Care, by calling 1800 200 422. Sunbury Community Health can also help you with accessing Home Care Packages. Sunbury Community Health, 12-28 Macedon Street, Sunbury. Opens 8.30am-5pm MondayFriday.
