Unity, Collaboration, and Cooperation Keys to City’s Story: Moving Healthcare Future Cover the Memphis Medical Community Forward

By JONATHAN DEVIN 2012 was a year of preparing for the uncertain in the Memphis medical community. Whether it was questions about the future of insurance under the Affordable Health Care Act, concerns about cuts to Medicare and sequestration, or navigating businesses through a period of alignment, medical leaders have had their hands full while waiting on answers that have yet to come. As 2013 approaches, eight of the city’s foremost healthcare leaders responded to questions from the Memphis Medical News. The group, featured on the cover of this publication’s InCHARGE issue, calls for unity, collaboration, and cooperation as the road ahead becomes clearer. “Perhaps our biggest challenge is continuing to adapt to change,” said Jason Little, executive vice president and chief operating officer for Baptist Memorial Health Care. “We are witnessing an evolution in our healthcare Jason Little delivery system. We must be willing to adjust, so we can continue to thrive.” But what it takes to lead doctors, administrators and patients through the reorganization of medical systems that have been in place for decades is largely subjective. Those who have emerged as leaders this year, though, agree on certain qualities of leadership, which are necessary in facing future challenges. “Leadership involves more than a title,” said Sarah Maurice, COO of Campbell Clinic. “Rather, a demonstrated commitment to the greater whole is needed. Leadership carries with it the responsibility to rise above circumstanc-

es and lead by example. “Anyone in our organization can be a leader by working to develop themselves and others, being proactive, and owning the issues and problems associated with their role while seeking positive solutions. Leaders demand more from themselves by setting the bar higher for themselves, but more importantly they help others reach that bar by engaging each person to work SARAH MAURICE together as a team toward a given goal.” Accepting responsibility for decisions and actions, whether they succeed or fail, is another quality of leadership in the medical community. “I expect leaders on my team to be self-starters who have a passion for the work, and who welcome challenges, which is a daily reality in healthcare,” said Audrey Gregory, COO of Saint Francis Healthcare. “I do not do well with excuses from the leadership team, so leaders on my team know, and are clear, about my AUDREY GREGORY expectations around problem solving and accountability.” And one challenge facing healthcare leaders is quite clear: remaining the same in an ever-changing field will not lead to success. Rather, leaders are focusing on how to make the necessary changes to their structures palatable and even preferable to all involved. Perhaps the most notable change healthcare providers are making is adapting to a system in which fees are tied heavily to qualitative outcomes, and

in which outcomes are determined more precisely, scrutinized more purposefully, and are available to patients more quickly and easily. “Employee pride and enthusiasm for the organization and the work we perform have been cornerstones of our corporate culture for the past 40 years,” said Dawn Fitzgerald, CEO QSource. “This passion derives from a strong sense of mission and purpose. However, with the diversification of our work and the everdawn fitzgerald changing dynamic of new business opportunities, we’ve considerably expanded our core competencies and, subsequently, our associates’ skills. In this new environment we need to continue to evolve and to empower our associates to find more efficient and effective ways to conduct our work. “In 2005, we adopted the ISO 9000:2001 standard to help us better understand our processes and outcomes. With this framework now firmly in place we use the results of process and procedure reviews to create ongoing organizational improvement.” In some cases, healthcare organizations have expanded rapidly, reaching out to physicians groups, which changed the scope and feel of their operations altogether for both patients and employees.“Baptist continues to make the transition from a hospital-centered organization to a hospital and physiciancentered organization,” said Little. “During the past year, Baptist Medical Group, our multispecialty physician practice, has grown into one of the MidSouth’s largest groups with 350 physicians in specialties ranging from internal medicine to oncology, cardiology and pulmonology. “These doctors chose to

align with Baptist, because we promised them a strong voice on an integrated team. They are actively involved in our future plans, including our new Baptist Cancer Center and a number of other projects. The president of Stern Cardiovascular Foundation, Dr. Steven Gubin, has even volunteered to lead our flagship hospital’s Well4Me Personal Challenge, where colleagues have begun six-month journeys toward healthier lifestyles.”But at the heart of the culture is developing the curiosity that leads to new break-throughs in treatments for patients.“St. Jude Children’s Research Hospital has unprecedented resources to fight childhood cancer and other catastrophic pediatric diseases,” said Dr. Richard Gilbertson, director of the Cancer Center at St. Jude. “Our science is amazing. Daily we are making new discoveries about these disease processes. Translating these discoveries rapidly dr. richard and appropriately to gilbertson the clinic is our greatest focus and challenge.“We have an extensive healthcare system that serves diverse groups of citizens with markedly different healthcare models. Working together and learning from each other will be crucial if we are to bring the very best medicine to the people of the MidSouth.” Rising to the challenges of impending system change oftentimes means new innovations in the technology that facilitates communication and procedures like electronic medical records and using the internet to decrease waiting time in the emergency room. In facing challenges, leaders have had the opportunity to think creatively and experiment with ideas that before would have seemed unrealistic. In the changing medical landscape, it seems that nothing which can be accomplished is off the table. “The shifting dynamics within the healthcare industry have demanded that providers and organizations think out of the box and adopt new models of care and business,” said Maurice. “Technology is developing rapidly, as are regulations, and we must stay ahead of the curve in order to continue to provide the highest quality of care, while lowering cost and increasing access. “As healthcare administrators,

it’s important to analyze the options and make smart decisions for all stakeholders including physicians, patients, employees, and our community. Everything has a domino effect. It’s our job to anticipate outcomes, then mitigate or leverage as appropriate.” There’s a certain amount of watching and waiting involved in rising to meet a challenge and those who can predict which issues will rise up next are often the ones who land on top. “I think that keeping up with what’s happening across the entire healthcare landscape is our biggest challenge,” said Fitzgerald. “Given our role in supporting Medicare, TennCare, Health Information Technology (HIT) and quality improvement in general, it’s important that we maintain a solid grasp of current trends. “These trends include federal and state legislation that might impact our work, implementation of new programs such as Accountable Care Organizations, Meaningful Use of HIT, Patient Centered Medical Homes and Health Insurance Exchanges, and changes in population health priorities such as reducing readmissions. These are all areas where we have an obligation to remain current and develop expertise to continue to be a valued resource.” New technology, new systems, new solutions always come with a cost, but avoiding investments in the future means avoiding accountability to some. In the case of The Med, that means going to the extreme of rebuilding almost every building on its campus, a plan that was announced this year. “We (at The Med) have made significant investments within the organization to create a culture of accountability,” said Dr. Reginald Coopwood, CEO of The Med. “Every individual within the organization from the top down is being held accountable to manage our environment such that the patient experience is one that is second to none. “We all face the DR. REGINALD COOPWOOD challenge around the uncertainties of healthcare reform. We are all evolving as we face changes in payment reform, health information exchange and the new focus on quality and transparency. These are all good changes, even

though they are different from how we do business today.” But as these leaders considered the healthcare picture in Memphis, all agree that in order to move forward, there must be calculated and strategic cooperation among systems at all levels from government to nonprofit to healthcare organizations. Hospitals and physicians’ groups will not be able to stand alone against the changing environment of healthcare. “I would propose that one of the most important steps the city can take is a coordinated, innovative and disciplined approach towards preventative care,” said Gregory. “We must redouble efforts to collaborate with all our health care partners,” said Laura Talbot, dean of the College of Nursing, The University of Tennessee Health Science Center. “Building bridges, working in tandem, recognizing more of our similarities Laura talbot and minimizing our differences will help us continually find ways to make a positive difference for all those who so desperately need our skills and the talents of the professionals we educate and train.” “The most important step Memphis can take toward moving forward in healthcare is to take responsibility of the entire city’s health outcomes and invest in partnerships that will improve the health status of the communities in which we live,” said Kevin Spiegel, CEO of Methodist University Hospital. “For example, The Church Health Center, founded by Dr. Scott Morris, provides primary health care to low-income, uninsured working people of Memphis. Another example is Christ Community Health Services, founded by Drs. David Pepperman, Rick Donlon, Karen Miller and Steve Besh, also providing high-quality health care and other services in Memphis. Such invested partnerships as these two agencies have with the KEVIN SPIEGEL Memphis community are commendable. “However, it is vital that more Memphians rise up and take responsibility for the health status of the communities in which we live.”

FIVE WISHES During the past two years I have had friends or loved ones who have dealt with aging with dignity and also have developed a greater understanding of Hospice. I do not think I could understand or say it any better than two of my friends: “Death is something that is difficult and some find it hard to face – especially if it is a loved one or family member. Yet, who wants to die in an ICU, in an induced coma, with strangers, surrounded by technology and not the people you love. To die in a peaceful setting, in comfort with your family is the way most would like to end their lives. My mother died earlier this year at the Methodist residential hospice and that was our experience. She was there for 10 days. During that time, she was constantly with friends and family. Reliving memories, holding my father’s hand and talking about the wonderful 64 years they had been together. “The setting was like a home with professionals who cared for her and for our entire family. They made sure she was never in pain and made her as comfortable as possible. She had a chronic disease that resulted in her being in and out of the hospital and nursing home for six months. “When she first became ill, we wanted everything possible done for her in an effort to extend her life and hopefully improve to the point of getting life back to normal. Over time, we accepted the fact that despite great care, terrific nurses and wonderful physicians, her life would never be normal again. While we, as a family, had never discussed care options and end of life plans in a formal way, we fortunately had talked enough to know that she would not want heroic measures futility implemented in her final days and hours. It took a wise, thoughtful and honest assessment of her situation by her physician to help us make the decision, with her, to transfer from the hospital to hospice care. “We also were fortunate to get the counsel and advice of a team of hospice professionals. It was a decision that we all agree in retrospect was the best for her. She ended her journey

and began her eternal life at peace. She took her last breath with my father holding her hand and telling her he loved her, it was okay to go and that he would be with her again. My sister and I were at her bedside and so was our minister reading a prayer; sending her off with soothing words from scripture – an experience none of us will ever forget and one which we are all thankful. It was possible because we were not in a hospital setting where the mission is to treat and heal. The hospice staff not only provides comfort but guidance. They can tell from experience what is happening to a patient and why and can see when death is within hours. “It was such a blessing to be able to have all of us spent hours with my mother those last few days. A blessing to be with her, holding her, talking to her, loving her as she breathed her last breath. A blessing made possible thanks to the Methodist hospice. A beautiful place — but more importantly an experienced staff of outstanding professionals dedicated to helping all families experience the need of life with dignity, love and peace of mind.” —Gary Shorb “It takes great courage to face and even welcome death, but that is exactly what my Uncle Buddy did. After his heart transplant 11 years ago, he had a few good years but then the antirejection medicine began attacking his body. Buddy put up a remarkable fight through back surgeries, shingles, dialysis, skin cancer, and loss of motor skills. He was always upbeat and never complained. The last two years, he was in and out of hospitals and rehabilitation centers more than he was at home. “After he and his wife Jerrie moved to Memphis in April to be close to family, Buddy entered Baptist Memorial Hospital in July with a broken hip. He spent the next eight weeks at Baptist before being transferred to a rehabilitation facility. The doctors, nurses and aids who cared for Buddy during his stay at Baptist were exemplary. He made many friends while there and knew everyone by their

first name. ‘Two weeks after being moved to rehab, Buddy was transported back to Baptist. There he underwent two more major procedures. After discussing the decision with his wife Jerrie and doctors, Buddy made the decision to give up the fight. He was disappointed that he would not be able to return home, but thanks to the Baptist Trinity Hospice House, he was able to spend his last days in a beautiful home-like setting. His room looked more like a bedroom than a hospital room. It had French doors so that he could see the outdoors – something he had not been able to enjoy during the preceding three months. “The Baptist Trinity Hospice House was a soothing environment for not only Buddy, but the Family as well. Family and friends were always welcome to visit and they even provided a playroom full of books and toys for the children. Most importantly, Buddy was able to take back control of his final days and to end his struggle pain-free and in a dignified manner.” — Kenna Cagle There is no way I could add anything to these two experiences. It is comforting to know that Baptist Memorial Health Care, Methodist Le Bonheur Healthcare and St Francis Hospital are bringing a collaborative effort to further educate our community on life planning. Coordinated Comprehensive Care Can Improve Your Quality Of Life by: Dr. Avinash Reddy As the science and technology of medicine improves, Americans are living longer than ever. Unfortunately, a longer life doesn’t always mean a healthier life. An increasing number of older Americans suffer from chronic medical conditions as they age and the management of multiple chronic diseases presents a significant challenge for both physicians and modern medicine. Older patients suffer from diabetes, hypertension, osteoporosis, arthritis, heart disease and a multitude of other medical conditions. Often, these patients see multiple specialists, in addition to their primary care physician, all of whom prescribe drugs or treatments. Unfortunately, these medications and treatments can have unpredictable interactions. In addition, a medicine prescribed for one medical problem can worsen another. For example, a medicine prescribed for diabetes may worsen heart failure. Another medication prescribed for hypertension may worsen diabetes. This makes the treatment of patients with multiple chronic illnesses complex and sometimes even dangerous. In an effort to improve treatment of patients with multiple chronic conditions, or multimorbidity, the American Geriatrics Society recently convened a panel of 11 geriatrics experts to develop a better approach for the care of patients with multiple chronic illnesses. The resulting document, “Guiding Principles for the Care of Older Adults with Multimorbidity,” was published in the recent Journal of the American Geriatric Society. It contains a set of recommendations for clinicians in the treatment of multimorbidity. The article recommends that clinicians focus on two separate aspects of patient care when treating patients with multiple chronic illnesses. First and foremost, the article recommends that doctors shift from a single-disease focus, to a more comprehensive approach. Clinicians should not blindly follow treatment guidelines

for management of a single condition when that treatment could worsen other conditions. Second, the article also recommends that clinicians place a stronger emphasis on patient preference when they prescribe treatments for various conditions. Patients should be educated on potential risks and benefits prior to initiating any new treatment and their preferences should be taken into account before starting treatment. The list below contains the panel’s recommended approach to the evaluation and management of multimorbidity.

If the patient’s prognosis is poor, there may not be a compelling reason to start a new treatment and it may even be worthwhile to consider discontinuing some of their medications. Finally, clinicians should reassess patients periodically to see if prescribed treatments should be reduced or discontinued.

1. Inquire about the patient’s primary concern (and that of family and/or friends, if applicable) and any additional objectives for the visit. 2. Conduct a complete review of a care plan for the person with multimorbidity OR focus on a specific aspect of care for the person with multimorbidity. 3. What are the current medical conditions and interventions? Is there adherence/comfort with the treatment plan? 4. Consider patient preferences. 5. Is relevant evidence available regarding important outcomes? 6. Consider prognosis. 7. Consider interactions within and among treatments and conditions. 8. Weigh benefits and harms of components of the treatment plan. 9. Communicate and decide for or against implementation or continuation of intervention / treatment. 10. Reassess at selected intervals: for benefit, feasibility, adherence, alignment with preferences. In conclusion, physicians should focus on treating the patient instead of focusing on an individual medical condition. Patient preferences and expectations should be assessed and taken into
