Memphis Healthcare Leaders Prepare for New Directions in 2014 By JUDY OTTO While healthcare professionals historically have been adept at accepting and adapting to change, taking policy shifts in stride and embracing new technologies, what lies ahead in 2014 is possibly the most formidable challenge ever faced: the virtually unknown healthcare landscape that will appear in the wake of the implementation of the Affordable Care Act as the new calendar year begins to unfold. Lack of a crystal ball notwithstanding, six of the area’s key healthcare leaders are among those who are poised to help move Memphis into new directions, in part by charting new directions of their own. The group recently shared their plans and perspective with Memphis Medical News:

DR. REGINALD COOPWOOD

Dr. Reginald Coopwood, CEO of the Regional Medical Center, anticipates expanding The MED’s healthcare footprint with the addition of a long-term acute care (LTAC) hospital, the addition of a surgery center, improvements and expansion of their rehab hospital, and expansion of the burn center. “All of this is part of a $30 million investment in our Turner Tower;

all to be brought online if not before the end of 2014, shortly thereafter.” Although committed to leaving its core medical facilities in the medical district, he speaks of extending that footprint throughout the community in the form of touchpoints that afford better access to The MED’s provider and health system. Creating healthier environments is just the beginning, however. As 2014 Chair of the Tennessee Hospital Association, Coopwood identifies the organization’s primary goal as encouraging both the governor and legislature to expand TennCare under the Affordable Care Act provisions. “We will be working with the state to help create new models of payment, helping to shape them in such a way that we can benefit all across the state of Tennessee.” In spite of the “unfortunate bumpy start” of the ACA’s web site, he believes that its underlying premise is sound, and that by covering more, we can provide better care and better programs to improve the health status of the population. The act’s provisions may create financial issues for the Med, however, since nearly 30 percent of its patients are uninsured—and local, state, and federal government subsidies have previously helped to cover the cost of their treatment. “Those subsidies potentially will be decreased because of the law; so it is imperative that we see the expansion of the insured population to be able to increase that 30 percent (of uninsured patients) to something under 10 percent. In order for us to be able to manage without subsidies, we’re going to have to have expanded coverage in the population which we serve,” he points out. Dr. Stephen Edge, Director of the Baptist Memorial Health Care Cancer Center, points to progress not just linked to the Center, but changes now

sweeping the nation. “Cancer care is changing dramatically with the recognition of new opportunities for treatments, being able to target treatments to individuals who will be specifically able to benefit from them, and the need to do research into understanding who those patients will be. There are enormous changes going on in the way that oncology care is paid for and how it must be organized to address those changes.” Edge’s Cancer Center is reshaping oncology with a program that is directed by the patient’s doctor, but supported by a whole team prepared to help with other aspects of care. And although the Center will be housed in a physical structure to be built in 2014, it encompasses a large network across a broad geographic region with a diverse patient population. “One of our goals will be to extend support services out from our center to our networks,” notes Edge. “We have a lot of people on our campus here in one of the buildings temporarily and we have a building opening in Jonesboro in a few weeks; we have a building in Oxford and we have quite extensive space in the Golden Triangle, to say nothing of the many infusion centers and treatment centers.”

Collaboration and advocacy are the new directions endorsed by Dr. Gary Kimzey of the Medical Anesthesia Group, P.A., and 2014 President of the Memphis Medical Society.

“If we doctors don’t sit at the table and know what’s going on, then we’re going to be on the menu,” he warns. “Doctors have to have a voice—a united voice.” He stresses the importance of working together through the Medical Society to promote the professional interests of physicians as well as to advocate for the rights and privileges of patients. He identifies the Affordable Care Act as a “moving target that makes it tough for us to figure out what we need to do as far as regulations, taking care of people, knowing who to take care of.” Such changes in healthcare should be physician-driven rather than government-driven, he feels, and to address that, and to ensure that the physician-patient relationship is enhanced and healthcare costs are being prudently managed, he urges physicians to become active in the political and legislative process: “Get involved. Get to know your local legislators, establish long-term relationships, and create opportunities where you can share information with your colleagues and also with legislators. They listen to their friends and constituents probably more than they do lobbyists.” “It’s tough enough to do this alone,” agrees Dan Hein, CEO of Memphis Orthopaedic Group, COO of

MSK Group, and 2014 President of the Tennessee Medical Group Management Association. “But if you have some folks you can depend on, that you can bounce ideas off of and share knowledge, it elevates everybody’s performance. “We’re trying to correlate the value of TMGMA to all of this uncertainty; and if there were ever a time to be a part of it, this is really that time. There’s a lot of angst, a lot of anxiety and pent-up fear, because there are so many unknowns. It’s a very intense environment. You’ve got to be involved, you’ve got to participate, and you’ve got to develop a strong network of peers to help shepherd through the process,” he advises. As for new directions, he observes, “Private practices are really just trying to hunker down. We don’t want to go too far too fast in terms of planning and strategies, because we don’t know what the healthcare landscape is going to look like.

“Stay above the fray, don’t become too reactive—and don’t get pulled in any one direction too frequently, and things will be fine,” he advises. Dr. C. Diane Knight, CEO of the VA Memphis Medical Center, recognizes that the Affordable Care Act has the potential to impact customer utilization, and is developing new strategies: “We know that we are competing with the private sector, and we are going to continue our focus on customer service, not only continuing to align our facility resources responsibly to enhance access for the veterans so that we can

continue to expand our telehealth programs and have community clinics and rural outreach and so forth, but also to add some programs. “More than 50 percent of our veterans now enrolled here are eligible to receive vouchers to go elsewhere— but we are convinced that we do serve the veterans with the ‘best care anywhere’. Our veterans appreciate those services that we offer—and we do not anticipate losing a large part of our enrollees to community providers.” In partnership with Catholic Charities, the VA’s coordinators are working to eliminate homelessness. Knight has also consolidated all post traumatic stress disorder (PTSD) functions to create a “one-stop shop” which addresses homeless issues, marital and related problems. The new women’s health center is open, and already serving a growing number of the 5,000 enrolled women veterans; new technology continues to be added to update the hospital’s ability to provide topnotch service. Knight’s team is currently shaping a strategic plan based on a vision to guide the VA through the next decade. Michaelia (Mike) Sturdivant, RN, is COO of The Church Health Center, whose new directions for 2014 include a long-awaited move to the historic Crosstown Sears Tower. “It has been our dream to have all of the Church Health Center under one roof, particularly as it relates to our clinical services and our wellness facility,” says Sturdivant, “because it will improve that handoff of the patient from either side. If we can walk a patient over and

InCharge introduce them to their new therapist, we think that it will greatly increase the chances of their actually following through and developing a relationship with that person.” Construction on the shell and core areas of the building will begin in early 2014, with completion expected in 2016. Meanwhile, the CHC will get an early toe in the door when their resource production staff moves into a bay in December (2013), along with other Crosstown Development neighbors.

As departing (2013) President of the Mid-South Group Managers Association, Sturdivant has focused on preparing members for coming change – ranging from the ACA to the October 2014 implementation of ICD-10 coding. “The Affordable Care Act presents all kinds of possibilities,” she said. “We don’t yet fully know what the impact will be, but we do know that, as we have always done, we will stand in the gap and meet the needs of the people who remain to be served.” And that’s a philosophy all healthcare professionals can share; because, as Hein points out, “At the end of the day, two things will always exist: there will be patients that need physicians, and physicians are the only ones who can fix them. So, regardless of rules, regulations, and environment— although business models may have to adapt and overcome—there will always be opportunity.”

Hey Doc, It’s the Year to Connect By TIM NICHOLSON This year you’ll have thousands of additional codes to choose from when you’re commenting on a patient file. You’ll have the challenge of meeting somewhat subjective patient performance expectations to maximize insurance reimbursements. And you’ll have more checkboxes to click in the EHR on your way to proving meaningful use. Sounds awful. So, how will you make this a great year? You’ll connect. Oh, I don’t mean connect to technology, though it’ll be there to facilitate the connections. I mean you will connect to people in ways that have proven challenging for some but profitable for others. And by ways, I mean social media, smartphone apps, and iPads. Still it’s not about the ways, it’s about the connection. It’s about the patient. So, I thought you might enjoy three ideas that could help you achieve your purpose – the best health for your patients – and frankly help you suffer the codes, surveys and systems. Choose any one of them and you’ll be back to medicine. First, use Social Media. By now you’re aware that the tools we call social media have permeated our culture. They’ve raised expectations for openness. And they created expectations of connectedness. Here, however, are three specific platforms and ways to improve connections. Facebook – yeah, the growth of teens has slowed but they’re not your patient. Make Facebook a place to share information that is helpful to others but not about others. Do this one thing, find a trusted content source and wrap your brand around it. Pinterest – women are the primary decision makers when it comes to healthcare. Start a Pinterest account and make three boards. One should be the role of diet on those who have health issues related to your specialty. The second, a board that features exercise tips that benefit your patient community. And the third board, inspiration. Offer encouraging quotes, images of famous survivors or champions, and images that rest the soul. Instagram – yeah, you’re right it’s for pictures. It’s the fastest growing and most popular app among the next generation of decision makers. Know how to take a picture of anything from equipment, to your staff, to things that give others a glimpse into what you find relevant. really not necessary. There are hundreds of health apps that can make a difference in the wellbeing of your patient and might even prove helpful to you. And you can put them to work for your practice. This idea is especially useful if your patients have chronic illness (i.e. diabetes), potentially life threatening diseases (i.e. cancer) or are expecting children (i.e. obstetrics, gynecology or pediatrics). It starts by imagining that you’re sitting with a patient to tell them what you do, what they might expect through the time you spend together, who your referring specialties are, what technologies you’ll use, resources that others find helpful and are willing to do so in a manner that deepens the relationship instead of sending them around the web finding these things on their own. It will improve the nature of the dialogue that occurs when you’re with a patient or trying to explain an issue to the patient’s family. Do it because it creates a deeper connection between you and the patient. A connection that new codes and checkboxrich EHR systems don’t understand but that you know will lead to better health outcomes. Do it because you’re a care giver; a healer. You’re all about the patient. Do it because in this age of systems and policy changes, the one thing that can always change for the better is your relationship with those who count on you; the patient. Yep, this year you’ll be asked, heck required, to do a lot of things. So, why not do at least one thing you like?

Thought, Passion, Action One year ago, I was especially touched by my devotional reading for January 1, 2013. It began with a scripture that was so optimistic, promising, sharing and caring. “For I know the thoughts that I think toward you,” says the Lord, “thoughts of peace and not of evil, to give you a future and a hope.” — Jeremiah 29:11 Little did I know that this scripture would follow me throughout the year. It is remarkable how a simple seed in a message can be planted in your mind and spurs you to help others. The seed creates a thought, one so powerful it creates passion, and the passion is so strong it leads to action. BAM!!! Thought, Passion, Action. My niece, Lanie, a junior at Auburn, is studying education for children with special needs. I decided to introduce her to my friend, Dr. Elizabeth DuPont, who has been executive director for the Hope House for 19 years. As you may know, Hope House is the only facility in the state of Tennessee designed to meet the unique needs of HIV-affected children. It is a friend to people of all ages and strives to help parents in any way possible – whether it is a ride to the doctor’s appointment, help with GED assignments, or just a listening ear. Currently, Hope House serves 16 children, ages 6 weeks to 5 years. Because of funding cuts Hope House, has had to cut back. Hope House was staffed to serve 36 children. Even with the funding cuts, Hope House has managed to achieve a number of successes including: 121 clients served, 62 child and adult graduates, 116 counseling sessions, more than 3,200 individual services provided, $101,300 in rental assistance paid and 85 support group participants. Lanie and I arrived, and Dr. DuPont showed us around. It just happened to be nap time. We saw all these little people lying on sleeping mats on the floor, asleep or partially asleep. I believe if you looked up “innocence” in the dictionary, this would be the picture shown. BAM!!! Thought, Passion, Action. I have been an active volunteer and will be serving on the board for Hope House this coming year. Today, mention Alzheimer’s disease and you will hear from others whose

parents, other family members or friends who have been touched. Last May, my mom was diagnosed with Alzheimer’s. Although I have worked healthcare for the past 20-plus years, I had not really seen all the different aspects of this dreaded disease. One day, I was book browsing and picked up Moving Miss Peggy, by Robert Benson. The review on the cover said, “Moving Miss Peggy, tells a story that many others are now facing, bringing strength and wisdom and inspiration to anyone dealing with dementia or Alzheimer’s in a parent or loved one.” I highly recommend it. This New Year, Baptist Memory Care Center will open. This center will provide memory screenings as a free service to the community to help with early identification of memory problems. In addition, it will provide assistance in educating and supporting, not only those diagnosed, but their caregivers. The center will employ experienced licensed clinical social workers to link caregivers with the multiple resources available in our community; legal, home care, respite care, day care, driving safety, specialty physician’s services, educational literature, community events, spiritual and emotional guidance and direction, acute care needs, long-term care facilities and more. BAM!!! Thought, Passion, Action. Baptist Memory Care is an example of an organization that saw a need and filled it. I have an affinity for canines. This past year I have read more and seen more about service dogs. I have noticed more

service dog groups being formed with the purpose of helping human companions. These special canines service children with autism and military veterans and are used in grief counseling, anger management, and other physical and medical conditions. They can sense when something is about to happen with their human companion such as seizures, diabetes and panic attacks. I have seen these special dogs cheer up elderly in nursing homes, visiting patients in nursing homes and help children who either do not read or are not comfortable reading in front of an adult. But put one of these specially trained dogs and a child together and the child can read aloud to the dog. If the child stops reading too soon, the dog will put its paw on the child’s leg and the reading will start up again. I have seen these animals sense when a little affection may be helpful, offer a doggie kiss, a nuzzle, or a helpful paw. BAM!!! Thought, Passion, Action. Someone saw a need and filled it. Earlier this year I flew to Washington, DC, to meet with congressmen and others to remind them that they are our representatives and we expect them to represent and eliminate the obstacles presented when “our heroes” a.k.a. veterans come home after all their sacrifices. I took a 25-yr-old veteran on the trip with me. During the cab ride from the airport to the hotel, traffic was bumper to bumper. The driver said that there were a lot of visitors that week. He said that with the traffic being so heavy, he was going to take an alternate route to get us to the hotel sooner. The weather was sunny and beautiful and the trees and flowers were blooming. I was enjoying taking it all in. All of sudden I saw something that gave me a combination of warmth and chills. There hanging on a building was a banner with Jeremiah 29:11 on it. I immediately asked the cab driver to stop so I could take a couple of pictures. Was I following this verse in the Bible or was it following me? It doesn’t really matter. It was there. Positioning for the New Healthcare Environment Experts of American healthcare have been saying for years that healthcare providers cannot continue doing what they have been doing and be successful in the new healthcare environment. While focusing on “Obamacare” as the main challenge facing physicians, we have missed the most important change coming - the changing patient population – age, race and social status. An entirely new patient population with different insurance patterns, different payor mixes and different kinds of wants/needs outweighs the impact of Obamacare or anything else. Increasing emphasis will be placed on keeping patients well (and, to the extent possible, out of the doctor’s office), managing patients for the long-run with emphasis on chronic as opposed to acute care. Future services will be driven by patient needs reinforced by demands by policy makers and payors that may not align with traditional practice goals. Physician practices will have to determine the needs of the population being served (and the desires of payors and other stakeholders) and then determine the mix of services to be offered, appropriate practice locations, and marketing strategies. One way in which physician practices can position themselves is to develop an understanding of the patients they will be serving in the future. The fact is: tomorrow’s patient is going to be different from today’s patient. Understanding the nature of

this next generation of healthcare consumers and anticipating their needs is going to be a prerequisite not only for success but for survival. Tens of millions of baby boomers are aging into their 60’s and will require an increasing amount of health services. These are not your grandmother’s “elderly” but a relatively youthful and well-insured cohort. Women will increase as a proportion of the patient population, and women traditionally use more health services than men. Further, women make most of the decisions related to healthcare for their significant others. Ethnic patients will continue to grow in numbers and diversity and the 2010s may be remembered for the dissipation of the predominately white patient population. Increasingly diverse healthcare consumers will have new and different requirements and expectations. Finally, millions of healthcare consumers are expected to eventually enroll in a health insurance exchange or Medicaid under “Obamacare”. Many of these newly “enfranchised” consumers will come equipped with quite different needs and perspectives. Progressive physician practices should consider these challenges as opportunities, and as a revenue focused practice management group, we (ADF Medical - Practice Development Strategies), believe that physicians can be “winners” in this new environment by meeting the needs of these emerging patient populations while maximizing revenue.

As an adjunct to our traditional revenue generation methodologies, we are adding capabilities for assessing the emerging market, identifying opportunities for practice growth, and developing strategies for capturing a meaningful share of emerging markets. While there are different potential approaches to positioning for the new environment, we and other healthcare specific development groups plan to interface practice repositioning services with our existing revenue enhancement techniques, providing seamless solutions to the challenges of the new healthcare environment. Physicians MUST know the accurate data about the patients/clients they will have – how else can they possibly plan correctly?
