TeleCare WILL HELP What is “TeleCare”? TeleCare is a FREE telephone paid for by the American Legion, the Disabled American Veterans and the Veterans of Foreign War and located at the Burlington Fire Department.
Why such a project?
Your loved ones – urge them to join “TeleCare” today!
Because many folks in our community live alone and are not in daily contact with friends or relatives…and, because people do get sick or have accidents.
How does “TeleCare” work? You are furnished with the confidential number of a telephone paid for by the American Legion, the Disabled American Veterans and the Veterans of Foreign War and located at the Burlington Fire Department.
What do I do? You agree to call this special number between 9:00 a.m. and 10:00 a.m. each day.
What if I forget to call? If no call is received from you by 10:00 a.m., the operator will attempt to reach you by telephone.
What then? If it is impossible to reach you by telephone, the operator will contact the neighbor or next of kin, as previously agreed upon, to investigate and see if you need help in any way and they will notify the TeleCare number of their findings. Should the neighbor or next of kin find that an emergency does exist, they will be expected to notify the appropriate emergency agency, as well as TeleCare.
Who may enroll? Any resident able to reach the Burlington telephone exchange toll-free.
How much does this service cost? Nothing.
Is this a confidential service? Absolutely. No one will know that you are receiving this service except the operator at the Fire Station.
Application for TeleCare I hereby indicate that I wish to subscribe to the FREE service of TeleCare. I agree to abide by the following rules: 1. I will telephone TeleCare (262-763-2495) each morning between 9:00 and 10:00. 2. In the event I am to be out of town for one day or more, I will notify TeleCare of my absence in advance, including the day I will be leaving and the day I will return. 3. I agree to permit TeleCare to contact my neighbor and/or my next of kin, in the event I fail to telephone during the specified time. My neighbor has agreed to cooperate with me in this program, and in the event that I fail to telephone TeleCare, will, at the request of TeleCare, go to my home to investigate. 4. In the event I fail to telephone TeleCare during the specified time, and in the event my neighbor is unable to contact me, I agree to permit TeleCare to contact my next of kin, who is aware of this program, and/or the local police so that they might investigate. 5.
