he face is the most frequent place for skin cancer to appear. A more common type of skin cancer is basal cell carcinoma. If you are unfortunate to get it the good news is that it is typically the slowest growing and easiest to treat in the comfort of the physician's office. Other skin cancers are squamous cell carcinoma and malignant melanoma. Each are progressively more invasive so the key is to catch them early. Treating melanoma early can save your life. During her service of eight years in the Air Force as a flight surgeon and dermatologist, Dr. Rodriguez pioneered military usage of lasers in dermatology. After transferring to the Air Force Reserve, she returned to Texas. In 1987, she began her dermatology practice in San Marcos. BATH SALTS: A new and dangerous designer drug

B

ath salts are a relatively new designer drug. They are also called Ivory Wave, Purple Wave, Vanilla Sky and Bliss. The effects of these compounds mimic amphetamines and cocaine, but on a much greater scale, James W. Schlotter, M.D., of Finesse Surgical Solutions, said Based on the effects that users experience, these compounds are acutely more toxic than the amphetamine and cocaine class drugs that they were designed to emulate. They also cause more issues for law enforcement in dealing with users who may have extreme paranoia and be very aggressive and dangerous to control. People on bath salts experience agitation, paranoia, hallucinations, chest pain and suicidality. A second concern is the ongoing suicidality in these patients, even after the stimulatory effects of the drugs have worn off. The DEA made the three compounds commonly used to make bath salts illegal by issuing an emergency ban last year in October. Mammograms: What to know about procedure etting a mammogram every year is important in finding breast cancer early. That’s why the American Cancer Society recommends women age 40 and over get one every year, along with a breast exam by your doctor or nurse and letting your doctor know about any breast changes you find yourself. Where to go: Find a center that specializes in mammograms. The U.S. Food and Drug Administration certifies mammogram facilities that meet high professional standards of quality and safety. When to schedule: It’s best to schedule your mammogram for the week after your menstrual period. What to wear: Wear a

two-piece outfit because you will need to remove your top and bra. Do not apply deodorant, antiperspirant, powder, lotion, or ointment on or around your chest on the day of your mammogram. What to expect: The entire procedure takes about 20 minutes. Each breast is compressed for a few seconds while two or more Xray pictures are taken. Getting the results: You should get your results within 30 days. If you don’t, you should call to ask about them. What you pay: Under the Affordable Care Act, Medicare and almost all private insurance plans now cover annual mammograms, with no out-ofpocket costs. Medicaid also covers mammograms.

CTMC continues to make health care technology its top priority hrough the years, Central Texas Medical Center (CTMC) has demonstrated a consistent dedication to staying on the leading edge of health care technology, and 2012 has turned out to be another big year of commitment. From the da Vinci robotic assisted surgery program to the new balloon sinus surgery program, to the new interventional cardiac catheterization lab, to a new PET/CT scanner, the hospital continues to invest and reinvest in technologies that are safer, faster and overall improving patient outcomes each day. In May 2012, the first da Vinci robotic surgery procedure was completed at CTMC. More than 50 gynecological, urological and general surgical cases have been completed robotically to date. The hospital is the first in Hays County to invest in a da Vinci surgery suite, making CTMC the only operating room between Austin and San Antonio to offer robotic surgery. CTMC’s new Interventional Cardiac Cath Lab officially opened in Septem-

ber 2012. The new lab represents CTMC’s ongoing expansion of its specialty cardiac services and is an important step in providing a higher-level of cardiac care following the opening of a Certified Chest Pain Center in 2011. CTMC is currently seeking certification as a Primary Stroke Center. The ER renovation at CTMC is slated for completion this fall. This year, Live Oak Health Partners, CTMC’s primary and specialty care physician group, began offering balloon sinuplasty surgery to provide relief for severe allergies. The endoscopic procedure uses a small, flexible balloon catheter to enlarge sinus passageways and can typically be conducted in a doctor’s office under local anesthesia. This year, CTMC partnered with Specialists On Call, Inc. (SOC), the nation’s leading provider of clinical telemedicine, to guarantee local stroke patients immediate access to neurologists via telemedicine. This agreement brings board certified, fellowshiptrained neurologists directly to pa-

Kids like variety so look for a sport that provides this or possibly multiple sports to fulfill this important element. Gymnastics is a sport that offers a variety of fun challenges. Each apparatus has it own unique activities and skill sets. Women train on vault, uneven bars, balance beam and floor exercise. Men train on vault, high bar, parallel bars, rings, pommel horse and floor exercise. Most gymnastics clubs also utilize trampolines. The benefits of gymnastics training are broad. In vault, running skills are developed along with hand eye coordination. Skills at bars

develop hand and upper body strength. Balance beam and pommel horse enhance core strength while also improving balance in a variety of dynamic planes. Floor exercise with all the rolls, handstands, cartwheels and other fun skills aids in multiple areas of motor skill development. Trampoline provides an opportunity to enhance overall space awareness. Students may begin gymnastics at any age with the youngest students starting structured classes at 18 months. The majority of gymnastics clubs group students based on age and ability to provide an appropriate peer group. Breast cancer and lifestyle considerations omen who have not had children or who had their first child after age 30 have a slightly higher breast cancer risk. Having many pregnancies and becoming pregnant at an early age reduces breast cancer risk. Pregnancy reduces a woman's total number of lifetime menstrual cycles, which may be the reason for this effect.

Birth control Recent oral contraceptive use: Studies have found that women using oral contraceptives (birth control pills) have a slightly greater risk of breast cancer than women who have never used them. Over time, this risk seems to go back to normal once the pills are stopped. Women who stopped using oral contraceptives more than 10 years ago

do not appear to have any increased breast cancer risk. When thinking about using oral contraceptives, women should discuss their other risk factors for breast cancer with their health care team. Depot-medroxyprogesterone acetate (DMPA; Depo-Provera) is an injectable form of progesterone that is given once every 3 months as birth control. A few studies have looked at the effect of DMPA on breast cancer risk. Women currently using DMPA seem to have an increase in risk, but the risk doesn’t seem to be increased if this drug was used more than 5 years ago. Hormone therapy after menopause Hormone therapy using estrogen (often combined with progesterone) has been used for many years to help relieve symptoms of menopause and

therapy (HRT), and menopausal hormone therapy (MHT). Breastfeeding Some studies suggest that breastfeeding may slightly lower breast cancer risk, especially if it is continued for 1½ to 2 years. But this has been a difficult area to study, especially in countries such as the United States, where breastfeeding for this long is uncommon. The explanation for this possible effect may be that breastfeeding reduces a woman's total number of lifetime menstrual cycles (the same as starting menstrual periods at a later age or going through early menopause). Alcohol Consumption of alcohol is clearly linked to an increased risk of developing breast can-

cer. The risk increases with the amount of alcohol consumed. Compared with nondrinkers, women who consume 1 alcoholic drink a day have a very small increase in risk. Those who have 2 to 5 drinks daily have about 1½ times the risk of women who don’t drink alcohol. Excessive alcohol use is also known to increase the risk of developing cancers of the mouth, throat, esophagus, and liver. The American Cancer Society recommends that women have no more than 1 alcoholic drink a day. Being overweight or obese Being overweight or obese after menopause has been found to increase breast cancer risk. Before menopause your ovaries produce most of your estrogen, and fat tissue

Different policies have different guidelines but in general the insured has to require assistance with two activities of daily living (ADL). Activities of daily living include bathing, toileting, transferring, eating and ambulating. Most people think that they have to be completely debilitated to qualify in two of the ADL categories but the client’s safety to perform the ADLs also plays an important role in qualifying to use their long term care insurance policy. For example, a client might be able to perform the majority of their own shower but require assistance getting in and out of the shower, washing their back, feet or hair. In this example the client would qualify for bathing if they wanted to use their LTC Insurance policy. The first step to initiating the LTC benefit is contacting the insurance

Some important questions to ask ... company. A family member can call the insurance company but the insured should always be present to give verbal consent to speak to the family member. Before calling you need some demographic information including, name, address, phone number, date of birth, social security number and policy number of the person in question. Be armed with the right questions so that you get the answers you need. Below is a list of questions that potential clients should ask: • What is my daily benefit? This is usually a dollar amount per day or per calendar week; for example, $100 per day or $700 per week The per week amount gives us more scheduling flexibility as we can provide more hours during the week when family members are not present to assist the client. • Is there an inflation rider attached

to my benefit? An inflation rider means that if you purchased your policy many years ago, the insurance company will increase the amount each year to take inflation into consideration. I have seen some policies that have a 5% per year increase in the daily amount. This can add up if you purchased your policy a number of years ago. • Do I have to require assistance with ADLs or can I just have a companion/homemaker? Although rare, there are some policies that will cover household management type services like housekeeping and meal preparation. • Do I have an elimination period? Some policies have 0, 30, 60 or 90 day elimination periods. Simply meaning that the insured has to pay for services out of pocket before their benefit will start being paid. This is very important to know for budgeting and can be quite costly if the benefit

period is long. • Does my policy have a waive of premium? Some insurance will stop monthly premiums once the policy has started paying benefits. A great money saver for the insured. • What is my maximum benefit? Some policy’s have a cap in amount that they will pay out. I recently had a gentleman who maxed out his $78,000 benefit. In order for him to qualify to use his insurance again or reinstatement of benefits, there has to be a six months lapse in care and then he can reapply for benefits again. Maneuvering through the long term care insurance maze can be confusing but once the service is in place, then providing care and billing the insurance company is seamless process. Having more fat tissue after menopause can increase your chance of getting breast cancer by raising estrogen levels. Also, women who are overweight tend to have higher blood insulin levels. Higher insulin levels have also been linked to some cancers, including breast cancer. The connection between weight and breast cancer risk is complex, however. For example, risk appears to be increased for women who gained weight as an adult but may not be increased among those who have been overweight since childhood. Also, excess fat in the waist area may affect risk more than the same amount of fat in the hips and thighs. Researchers believe that fat cells in various parts of the body have subtle differences that may explain this.

The connection between weight and breast cancer is complex The American Cancer Society recommends you maintain a healthy weight throughout your life by balancing your food intake with physical activity and avoiding excessive weight gain. Physical activity Evidence is growing that physical activity in the form of exercise reduces breast cancer risk. The main question is how much exercise is needed. In one study from the Women's Health Initiative, as little as 1¼ to 2½ hours per week of brisk walking reduced a woman's risk by 18 percent. Walking 10 hours a week reduced the risk a little more. To reduce your risk of breast cancer, the American Cancer Society recommends 45 to 60 minutes of intentional physical activity 5 or more days a week.

Coping with cancer; new approach to fighting back

ince being diagnosed with Stage 4 metastatic breast cancer in 2004, Kimberli Herring has devoted her life to health and fitness. She had a plan to help cancer patients during and after treatment by coordinating with doctors and creating a fitness plan that helps prevent muscle wasting, combats depression and gets the patients active. That's where Prescription Fitness came from concept to reality. Herring went through the American College of Sports Medicine to get her personal training certification, and is currently pursuing her certification as a Cancer Exercise Trainer (CET). She started Prescription Fitness to help others achieve total wellness through a means of exercise and nutrition under your own doctors supervision.

Herring graduated from Southwest Texas State University in 1995 with a Bachelors Degree in Science and has remained active in the community ever since. reast cancer is sometimes found after symptoms appear, but many women with early breast cancer have no symp-

toms. This is why getting the recommended screening tests (as described in the section, "Can breast cancer be found early?") before any symptoms develop is so important. If something suspicious is found during a screening exam, or if you have any of the symptoms of breast cancer described below, your doctor will use one or more methods to find out if the disease is present. If cancer is found, other tests will be done to determine the stage (extent) of the cancer.

Signs and symptoms Widespread use of screening mammograms has increased the number of breast cancers found before they cause any symptoms. Still, some breast cancers are not found by mammogram, either because the test was not done or because, even under ideal conditions, mammograms do not find every breast cancer. The most common symptom of breast cancer is a new lump or mass. A painless, hard mass that has irregular edges is more likely to be cancerous, but breast cancers can be tender, soft, or rounded. They can even be painful. For this reason, it is important to have any new breast mass or lump checked by a health care professional experienced in diagnosing breast diseases.

Medical history and physical exam If you think you have any signs or symptoms that might mean breast cancer, be sure to see your doctor as soon as possible. Your doctor will ask you questions about your symptoms, any other health problems, and possible risk factors for benign breast conditions or breast cancer. Your breasts will be thoroughly examined for any lumps or suspicious areas and to feel their texture, size, and relationship to the skin and chest muscles. Any changes in the nipples or the skin of your breasts will be noted. The lymph nodes in your armpit and above your collarbones may be palpated (felt), because enlargement or firmness of these lymph nodes might indicate spread of breast cancer. Your doctor will also do a complete physical exam to judge your general health and whether there is any evidence of cancer that may have spread. If breast symptoms and/or the results of your physical exam suggest breast cancer might be present, more tests will probably be done. These might include imaging tests, looking at samples of nipple discharge, or doing biopsies of suspicious areas.

Other possible signs of breast cancer include: • Swelling of all or part of a breast (even if no distinct lump is felt) • Skin irritation or dimpling • Breast or nipple pain • Nipple retraction (turning inward) • Redness, scaliness, or thickening of the nipple or breast skin • Nipple discharge (other than breast milk) • Sometimes a breast cancer can

eing told you have breast cancer can be scary and stressful. You probably have a lot of questions and concerns. Learning about the disease, how it is treated, and how this information might apply to you is a lot to do on your own. You might need some help. Your American Cancer Society can give you general information about this disease and its treatment, but your doctor is the best source of information about your situation. It is important for you to be able to talk frankly and openly with your cancer care team. They want to answer all of your questions, no matter how minor they might seem to you. But it helps if you know what to ask. Here are some questions that you can use to help you better understand this cancer and your options. TAKING ON THE BIG C Kimberli Herring fights back against breast cancer, decides to assist others By Cody Herring In the fall of 2004 Kimberli Herring got the worst news she could have imagined. After discovering a lump in her breast and dismissing it as a fibroid, she finally went to the doctor and the news she got shocked her: Cancer. “I denied the possibility of it being cancer,” Herring said. “I was 34, I was too young to get cancer. I never had a mammogram. They say you don't need to get them until you're 40.” The days following the diagnosis were a blur for Herring and her family. She went directly to the Wonder World Center for Cancer Care where Dr. Jones and his staff helped her get a plan started. Testing of the biopsy showed that the form of cancer Herring had was triplenegative (which is not fueled by hormones), aggressive, and stage 4 (the cancer had

spread to her lymph nodes and lungs). The news at the time couldn't seem to get worse, cancer seemed like an impending death sentence. Herring slipped into severe depression and had a bleak outlook on what little life she thought she had left. “When you have stage 4 breast cancer, you don't hear of a lot of people who have enjoyed long, happy lives. You hear about people who are bedridden and wasting away because of the chemotherapy and radiation treatments they have to get,” Herring said. “Chemo was a life extender, not a cure.” “I thought I could either give up and be another one of those statistics, or I could be my own statistic. I decided to fight. It's a choice you have to make. Are you going to fight the cancer? Or are you going to be overwhelmed?” Even with the facts on

Breast cancer survivor Kimberli Herring stage 4 cancer not in her favor, less than a 15% 5-year survival rate, Herring found the hope and courage to stand up and not let cancer run her life. She remained an active mother who sat out in 100 degree heat taking her

son Cody to horse shows and took care of her 5-acre ranch which housed seven horses and a variety of farm animals. “It's easy to say I can't. It's easy to get overwhelmed. When the doctors said get prepared for the fight for your life, they weren't joking. I was lucky that I had only mild side effects from chemo, the only bad part was the radiation treatment, my skin was raw and cracking. I still have visible burns from that treatment.” When her hair started falling out, Herring decided to shave it all off. She bought a wig, but now reflects that in Texas heat that is a terrible idea. Instead she wore bandannas and sported pink constantly. Joking about it after the fact, Herring said her eyebrows and eyelashes held on for dear life until the last chemo treatment, then promptly fell out. Herring later went on an

oral chemotherapy drug and surprised all the doctors. She has remained cancer free for seven years, without having to have a mastectomy, or any additional treatments. Dr. Kilbourn, who was Herring’s main oncologist, refers to her as his “miracle patient.” In his 25 years of experience he had only seen one other patient recover so quickly and completely. After the treatments were over, Herring’s body showed signs of the battle she went through. She was 100 lbs heavier than when she started chemo and she was completely drained of energy. This is common in chemo patients due to the toxicity of the drug, it's effect on hormone levels and physical inactivity. “I started working out lightly, trying to get some of my strength and muscle back. Back then the doctors warned

She saw exercise as a form of medicine that can help you combat the negative side effects of cancer and the treatments used to fight it. Herring created the concept of Prescription Fitness, a personal training and total-wellness business that emphasizes exercise and nutrition to

prevent illnesses and disease, and help combat side effects in those who are already affected by certain conditions. She offers in-home exercise plans for patients who are confined, group fitness training and one-on-one sessions.
