Chronic Disease Prevention & Management
Chronic disease prevention and management programs are designed to help individuals prevent and manage chronic conditions such as diabetes, hypertension, and arthritis. These programs often include lifestyle modification strategies, educational sessions, and support groups to help participants make healthier choices and manage their conditions effectively.
The YMCA’s Diabetes Prevention Program
Centers for Disease Control and Prevention (CDC) program that helps adults with prediabetes reduce their risk of developing type 2 diabetes
Year-long program with weekly one-hour meetings for first 16 weeks and bi-monthly or monthly for remaining 8 months
Goals include losing 7% starting body weight and increasing physical activity to 150 min. per week
The YMCA’s Blood Pressure Self Management Program
Work with a trained Healthy Heart Ambassador in this evidence-based program
Attend 4 monthly nutrition education seminars to learn about healthy eating habits and other factors that may impact blood pressure Attend 2 personalized consultations per month
LIVESTRONG at the YMCA
Participants meet with a certified instructor to increase flexibility and endurance, build muscle and improve balance
12-week program meets 2 times per week for 75 minutes
Participation in this program can decrease chances of recurrence of cancer
Fall Prevention
Our evidence-based fall prevention programs are part of a broader effort to promote healthy aging and improve the quality of life for individuals with chronic conditions like arthritis. The implementation of evidence-based fall prevention programs was associated with $14,600 in net avoided costs per 1000 patientdays according to the National Institute of Health.

Evidence based walking program from the Arthritis Foundation
Reduce joint pain and discomfort
Increase balance, strength, and walking pace
Group exercise program that helps adults managing chronic conditions like arthritis gain strength and independence
Easy to learn movements that motivate participants to be consistently active
Four-month program that meets 3 times per week for 60 minutes in person
Moving for Better Balance
12-week evidence-based group program that meets 2 times per week
Improve strength, balance, flexibility, and mobility through the slow and therapeutic movements of Tai Chi
Participants report fewer falls, and risk of falling is decreased by 55 percent
