1) When filling this questionnaire, use formulas and principles for calculating the indicators (at marginal) . 2) Fill a separate questionnaire for each business unit. For instance, when a manufacturing company has in its structure a management company and 3 factories, there shall be 4 questionnaires. The same applies to other sectors (telecommunication, trade, finance etc.), a separate questionnaire provides data of a Management Company and a separate questionnaire provides data on regions of representation.

Indicators calculation principles: 1. Average staffing number serves as staff number for the purpose of calculating most indicators. Example

The indicator 'Percentage of employees trained during a year' shall be calculated as follows: (number of employees trained / average staffing number) * 100%

2. For the purpose of some indicators data shall be represented by separate staff categories. Indicators in that case are calculated using average staffing number of that category, not company's average staffing number. Example

Absenteeism indicator (for the category of specialists) shall be calculated as follows: (duration of unplanned absence of specialists at work / specialists' working time rate)*100%

This questionnaire covers 4 staff categories where indicators are to be calculated. 1) Top managers (for instance General Director, Deputy General Director) Each company has its own list of positions referred to the category of top managers. For the purpose of this Survey we suggest to be guided by following descriptions: • responsible for activities of the entire company or its part (direction). For instance, HR Director responsible for HR; • reports to owners or General Director • independent to make decisions, especially financial ones 2) Managers. In this category you include employees who have direct reporters (for instance Director of Department, Chief Accountant, Head of Division, Head of Section etc.) 3) Support and administrative (office) staff – employees of support and administrative divisions, clerks and service staff. In this category you include employees whose work is to support activities of general staff (for instance accountants, legal advisors, HR service, office managers, secretaries, drivers etc.) 4) General staff – 'company's hands'. In this category you include workers who are directly involved in production of company's goods and services (having direct contact with goods produced by a company, or clients who are provided services. Here you include employees whose activity directly 'earns' money for the company (for instance sales managers, engineers, geologists, drillers etc.). Manufacturing: workers Insurance: insurance agents, acquirers Banks: transaction officers, tellers. Trade: trade operators (sellers, loaders, warehousers) and cashiers.

Attention! For all sectors: if a company has a call-center, operators of call-center are related to general staff. 4.

For the purpose of consistent interpretation of values in your questionnaire please use the following symbols for your numerical indicators:

«0» - value of the indicator equals to zero.

2)

«х» - it is not possible to measure the indicator while it exists in the company.

3) «–» - This indicator does not exist in the company. This is a dependent indicator.
