*The provision of routine examinations and hygiene visits is subject to receipt of the required consecutive monthly payments
**Exlusions apply to orthodontic treatment, implant surgery, smile makeovers (composite bonding, veneers, crowns), cosmetic treatment, wisdom tooth removal and facial aesthetic treatment
