$10,000 increments to a maximum of the lesser of 5 times pay or $150,000.
$5,000 increments to a maximum of $50,000, not to exceed 50% of employees Optional Life Benefit.
Child under 15 days: $100 Child 15 days to 6 months: $500 Child over 6 months old: $10,000
*Child(ren)’s Eligibility: Dependent children ages from birth to 25 years old, are eligible for coverage. In TX, regardless of student status, child(ren) are covered until age 25.

You have the option to purchase Optional Term Life Insurance. Listed below are your monthly rates (based on your age as of your last birthday on the Plan Anniversary each year) as well as those for your spouse (based on your age of your last birthday on the Plan Anniversary each year). Rates to cover your child(ren) are also shown.

THE EMPLOYER DOES NOT BECOME A SUBSCRIBER TO THE WORKERS’ COMPENSATION SYSTEM BY PURCHASING THIS POLICY, AND IF THE EMPLOYER IS A NON-SUBSCRIBER, THE EMPLOYER LOSES THOSE BENEFITS WHICH WOULD OTHERWISE ACCRUE UNDER THE WORKERS’ COMPENSATION LAWS.
