lthough it might not be immediately obvious, often the smallest change can make a big difference – something worth bearing in mind when modern life has us feeling overwhelmed or time-poor! It might be as simple as having a glass of water instead of a cup of coffee. Taking the stairs instead of the escalator. Listening instead of talking. Practising gratitude.

In this issue of IT, Dr Peter Mackereth and Ann Carter look at how the simple act of introducing different fabrics and textures into your treatments can enhance the therapeutic process, from helping to create a sense of security in clients with body image issues, to evoking memories of comforting rituals, routines and valued experiences (page 12).

or me, spring is a really positive, uplifting time of year and a great point in the calendar to take a fresh look at where I am heading, both personally and professionally. Am I taking good care of myself and is my work/life balance as I would like it to be? Are there any new skills I could learn to benefit my clients and my business? Does my therapy room need a fresh lick of paint or new towels?

If you’re asking yourself similar questions, hopefully some of the articles in this issue of IT will prove useful, such as Dr Peter Mackereth and Ann Carter’s article, The Material Therapist (page 12), which looks at the role of fabrics and textures in therapy practice, along with Green Salon, which offers some tips on how to make your therapy practice more eco-friendly (page 24).

Training advice I attended the FHT Conference in November and noted that at the end of Anita Mehrez’s presentation (pictured), a delegate asked for more information about essential oil blends that could be used in cancer care and whether it was possible to share any of the policies created for the therapy service at The Christie NHS Foundation Trust.

As someone who has completed a number of training courses and developed an existing complementary therapy service for an NHS specialist palliative care hospice, I just wanted to share some personal recommendations that this delegate, and other members, might find useful.

For information about using aromatherapy in a hospital or hospice setting, I found the courses offered by Rhiannon Lewis at the Royal Marsden extremely informative (royalmarsden.nhs.uk/ studydays). I would also recommend attending the complementary therapy coordinators course at The Christie (bit.ly/the-christieintegrative-therapies-training-unit), as this will again provide invaluable information and enable therapists to connect with other coordinators, which will provide a network of support.

I would also suggest contacting other local hospice coordinators, to set up a network where they can share information and benefit from a necessary support system.

I would like to say just how much I enjoyed the 2018 FHT Conference. It was well organised and really good value for money, and some of the speakers were inspirational – hearing Sir Sam Everington and Dr Michael Dixon (pictured) brings hope to our profession. Listening to Anita speak about the incredible work being done at The Christie hospital is always good to hear. I have so much respect for everything they do there, and how the complementary therapy profession feels the many ripple effects of their work. I left feeling inspired and motivated.

Since winning a 2012 FHT Excellence in Practice award for research and development, I have been fortunate to teach and present around the world. The award provided a platform to showcase my work and since then it has snowballed

organically into something quite special.

I am the lead practitioner for the renal unit at Salford Royal Foundation Trust, where I recruit and train other complementary therapists to carry out treatments while the patients receive their dialysis.

Our team ended 2018 by eagerly awaiting news of two prestigious awards that we had been shortlisted for. It was a very exciting time. We had been nominated by our peers for the Northern Care Alliance Staff Awards 2018, where judges heard some really wonderful things about all the hard work and commitment of our volunteer complementary therapy team.

We have 25 volunteer complementary therapists based in the renal dialysis units across five sites (Salford, Wigan, Bolton, Oldham and Rochdale). We help make the dialysis patients’ lives a little more tolerable and deliver a multitude of therapies including massage, aromatherapy, reflexology and reiki. Patients have said the treatments are fabulous. They help lift their spirits, ease muscular and bone pain, muscle spasms and boost their health. Some therapists have also assisted patients and their families at the end of life and provide much needed support.

To be nominated for an award by your peers is a fantastic achievement. The awards were to highlight and celebrate the tremendous efforts, contribution and achievement of staff at all levels across the hospital. It’s fabulous to be appreciated for delivering a level of care and service that your colleagues appreciate and want to recognise.

There were over 600 nominations and the standard of the nominations had been exceptionally high, which meant that some very tough choices had to be made to narrow down the final shortlist. This was followed by 4,000 votes to choose the winners.

At the awards ceremony the team were represented by myself, Janet Cairnie, and four other therapists: Janet Johnson, MFHT; Clare Gibbon; Cindy Vernon, MFHT; and Musarah Hussain.

The evening was held at the Sheridan Suite in Manchester and was a very glamorous event. We didn’t win the award in our category, but it was still great to experience the evening.

The second awards ceremony was a week later – the Heart of Salford Awards 2018, hosted by Salford CVS and Volunteer Centre. Established in 2003, these awards were an opportunity to thank volunteers for their dedication, hard work and continued commitment.

Yet again we were shortlisted among 22 other teams for the Volunteering Team Award. We didn’t bring the trophy home but received a wonderful certificate to show our hard work. What a wonderful week.

I take this opportunity to thank every therapist who has taken part in the project since it started in 2016. Without their help, enthusiasm, dedication and commitment, being nominated for these awards would not have been possible.

Exercise can help reduce blood pressure Regular exercise can help reduce blood pressure, according to a recent review of randomised controlled trials (RCTs).

A study, published in the British Journal of Sports Medicine, compared the effects of exercise regimens and medication for reducing systolic blood pressure, selecting 391 RCTs, of which 197 evaluated exercise interventions and 194 looked at medication for hypertension.

While no trials directly compared

exercise with medication, both interventions performed favourably when compared to control groups (no intervention). However, antihypertensive medications achieved higher reductions in blood pressure than exercise interventions.

The findings suggest that modest but consistent reductions in systolic blood pressure can be achieved with exercise.

High-fibre diets linked with lower heart disease risk Eating plenty of fibre reduces the risk of heart disease, stroke, type 2 diabetes and cancer, according to a landmark review commissioned by the World Health Organization (WHO).

People on high-fibre diets were found to be between 15% and 30% less likely to experience heart disease and early death than those who don’t eat much fibre.

The study, which used data involving more than 4,500 participants, will inform forthcoming WHO guidance and was led by a team at the University of Otago in New Zealand, whose last major review paved the way for sugar taxes across the world.

The researchers recommend a daily minimum intake of 25g of fibre, similar to the 2015 UK government guidelines of 30g each day.

Yoga could help with depression Setback in Spain for during pregnancy therapy integration Yoga-based therapies can help manage antenatal depression, according to a review published in Complementary Therapies in Clinical Practice.

A team of scientists in the UK and Singapore conducted a systematic review of six clinical studies, involving 405 pregnant mothers, that examined the effects of yoga on depression during pregnancy.

All six studies showed reductions in depression scores, indicating that yoga is a ‘promising non-pharmalogical modality’ for improving the psychological health of expectant mothers.

Participants recruited for the trials reported mild depressive systems, therefore larger studies may be needed to examine the effects of yoga on severe prenatal depression.

The Spanish government announced last year plans to stop health centres and universities from offering treatments it considers ‘pseudo-therapies’, such as homeopathy and acupuncture.

Health minister Maria Luisa Carcedo said: ‘Public and private establishments that include the therapies will not be able to call themselves health centres.’

The Spanish government argues that treatments ‘lack therapeutic use’, ‘negatively affect health by perpetuating illnesses or causing others’ and ‘even increase the risk of death’.

It says patients must be told that therapies ‘are not effective’. However, apart from mentions of homeopathy and acupuncture, the government haven’t yet made it clear which therapies they are referring to.

A report says that reading could help halt the loneliness epidemic facing the UK. The report from Demos and The Reading Agency, entitled A Society of Readers, says that reading books can significantly reduce feelings of loneliness for people aged 18 to 64 and it is also associated with having close relationships.

Market research agency Mintel has identified ‘sub-zero waste’ as the key trend to impact the global beauty and personal care markets within the next few years.

The trend follows the gaining momentum of a ‘zero-waste philosophy’, where wellinformed consumers are increasingly seeking out environmentally friendly, sustainably produced products to negate the effects of global warming and damage caused by singleuse plastics.

Mintel highlights how irresponsible practices have led to ‘egregious waste like the

Great Pacific Garbage Patch’, which covers an area of ocean twice the size of the state of Texas.

Consumers will make sustainable choices where they can, including in the beauty and self-care market, and will expect brands to be held accountable for their actions. Some brands are already considering eliminating waste altogether, especially independent brands that have built their businesses around ethical and sustainable practices.

High-intensity exercise effective for weight management Small amounts of high-intensity interval training (HIIT) could be more effective for weight loss than regular moderate exercise, a study suggests.

Scientists at the Federal University of Goiás, Brazil, analysed data from 41 studies, involving 1,115 participants of mixed ages and abilities, from sedentary to elite athletes, comparing moderate-intensity continuous training (MICT) with HIIT. MICT is defined as continuous effort that elicits 55% to 70% of the maximal heart rate, while HIIT is an intermittent period of effort interspersed by recovery periods, performed at a maximal heart rate of 80% or more.

While both methods led to weight loss, HIIT was found to be nearly 29% more effective in reducing total body fat.

Organic product market grows by 14% Sales of certified organic beauty and wellbeing products have risen by 14%, says the latest report by Soil Association Certification (SAC).

In its Organic Beauty and Wellbeing Market 2019 report, SAC reveals that UK sales for natural and organic products have continued to rise, marking eight consecutive years of growth.

The market was worth £86.5m in 2018, compared with £75.9m in 2017 and just £27.7m in 2010.

The trend towards organic and natural products is said to be driven by a number of key factors, such as a greater awareness of selfcare and wellbeing among consumers; more consideration towards the planet, including moving away from plastic; and ‘savvy shopping’ led by millennials and Generation Z, where consumers want full transparency of ingredients and environmental claims.

THE MATERIAL THERAPIST DR PETER MACKERETH AND ANN CARTER EXPLORE THE ROLE OF FABRICS AND TEXTURES IN THERAPY TREATMENTS

W

hen we were children, it is likely that we had a favourite soft toy, duvet cover or even a ‘security’ blanket, which often became a worn piece of fabric that accompanied us everywhere. You may recall going to bed, being tucked in by a family member, cuddling your toy and being read a bedtime story. As adults, associations

with fabric textures from childhood may have been replaced by stroking a wellloved pet such as a cat, dog or even a horse.

These sensations can continue to provide feelings of personal comfort and wellbeing (Jorgenson, 1996). The stroking of their warm coats is usually a pleasant experience for both animal and human, creating a special bond.

Our skin is an amazing organ, capable of tactile sensation, with touch and textures playing a major role throughout

our lives, although we often take these sensory experiences for granted.

Unlike our pets, we don’t have an outer coat made of fur; warmth and protection is achieved by the wearing of clothes. The fabrics that we wear may vary in weight, texture and the way in which they interface with our bodies as we move. Remember those heavy duffel coats, light summer dresses or cosy scarves and jumpers, perhaps knitted by family members. Interestingly, for those who can

access visual experiences, the colour of fabrics can also affect our sense of mood and wellbeing.

GIVING FABRIC A ROLE IN THERAPY We postulate that offering fabric choices in terms of textures and colours can enable the client to associate with positive earlier memories, perceptions and sensations, which can all contribute to the therapeutic process. While acknowledging the functional role that the traditional use of towels has to play in contributing to the client’s warmth, modesty and safety, we would like to explore other aspects of the use of fabrics. First, how could we skilfully utilise other materials to enhance the therapeutic process and, second, how could we incorporate fabrics that have a personal meaning during a hands-on treatment? In this article, we offer some suggestions and practical applications.

THE ROLE OF COLOUR Therapists often use towels of a single colour in private practice. This colour is often the therapist’s personal choice and most will have chosen it with great care to help promote a calm and relaxing atmosphere in the treatment room. If a therapist works in a spa, salon or clinic, the towels may be in the corporate colours of the company.

Interestingly, as visual experience is

considered a dominant sense, a client will usually ‘register’ a colour and will ‘assess’ it before experiencing the texture. They may associate a colour with clothes that they would choose to wear, or a person, place or event. It is this association that may help to create the mood and feelings which relate to a positive or negative experience. However, we are not suggesting that therapists change the colours of their towels and linen to offer personalised treatments, but a range of smaller towels, scarves or shawls could be available in a variety of colours and textures.

Smaller towels that represent the colours of the rainbow can also be used to enhance the role of the larger, more functional towels.

CASE STUDY 1 On her first visit for a reflexology treatment, Sandy noticed a basket of scarves of different colours and textures. The therapist asked her if there was a colour she particularly liked. Sandy immediately pulled out a bright yellow chiffon scarf. ‘This is so lovely,’ she said. ‘It reminds me of my little girl – we didn’t want to dress her in pink, so we chose this bright, sunny yellow – and she looked so lovely.

smaller towels that could be used on specific areas of the body. The therapist could say, ‘I have a lovely range of coloured towels for you to choose from – which colour would you like?’ When the client has chosen their preferred colour, the therapist can ask where they would like the towel placed. It is likely that the chosen colour will have some meaning for the client and it enables them to have some choice over and above the standard towels provided.

A second option could be to have a range of ‘cover fabrics’ in different textures and colours which could have a role in the therapy, as described in case study 1 above.

Another use for a set of rainbow towels is to have them in a clearly visible place in the therapy room or reception, to send a message of LGBT inclusivity in addition to offering colour options to clients.

SENSORY EXPERIENCE When covering a client, do we take time to consider how the texture of the cover feels on their skin? Different fabrics can offer a variety of tactile experiences that can engage skin curiosity and possibly evoke memories of comforting rituals, routines and valued experiences. Other forms of cover can include a lightweight blanket that can be easily washed, or even a cool sheet, depending on the therapy, the season of the year and the context in which you work.

Through anecdotal feedback on courses, we have learned that some clients (and therapists) don’t like the feeling of towels, bearing in mind that towels can have many different textures. We are also aware that if working in a clinical setting, there may be no choice in the coverings we use. In some settings, the family may like to bring a favourite blanket from home – this may be particularly useful if you are working with someone with memory loss or dementia.

TEMPERATURE AND LAYERS Another property of the material we use for covering is the degree of warmth or coolness that it offers. In some situations, one layer may not be enough for warmth, but adding a second layer of a blanket may be too much in terms of weight. Rather than using just one layer throughout the whole of a treatment, the weight of the fabric could be used to support the client. For example, when a client is experiencing hot flushes and is anxious about this happening during a treatment, would you use the same covering as for someone who says, ‘I’m always cold – especially my hands and feet’?

A SENSE OF SECURITY The way in which the fabric is used can help to create a sense of security by prompting childhood memories of being tucked in at bedtime, wrapped in a warm towel after a bath, or even being swaddled. There may be a downside to this practice where the positioning of the fabric creates a feeling of constraint and is therefore restrictive.

Examples are tucking in the covers so tightly that the individual can hardly move or taking the covers over the individual’s throat, right up to their chin.

Clients may be reluctant to tell the therapist if something isn’t ‘quite right’.

They will very often say ‘It’s fine’, which means ‘It’s not how I would like it, but it is acceptable.’ One way to avoid this ‘acceptable response’ is to try out two comparable situations and then ask a closed question, such as ‘Is this less or more comfortable for you?’ each time you change the position of the cover. You will receive a more direct and purposeful answer from your client.

THE MEANING OF THE COVERS The covers we normally use are part of our standard routine, but that doesn’t mean to say we can’t add to them. Many of our clients have a fabric at home that has meaning or significance for them. Often there is a favourite blanket, scarf or dressing gown. We like to encourage clients to bring these favourite items to the treatment; there is usually a story attached, which the client is often very willing to share.

Using fabric in this way enhances the treatment for the client in a personal way and enables them to associate the fabric with the enjoyment of the treatment when back at home. This approach is particularly useful when working with people with long-term and life-limiting conditions. We also suggest that relatives could bring in a

‘fabric of significance’ and let the therapist and staff know of its pleasant associations for the client. The fabric can then become an integral part of the complementary therapy session.

THE USE OF TEXTURED OBJECTS Textured items can also play a role.

Increasingly, textures are being used in dementia care and we can learn much from these innovative techniques. Memory boxes are very popular and can have many different contents. For example, in terms of textures, some memory boxes can contain favourite items of clothing (often from the past).

Other suggestions include colourful handmade patchwork or quilted blankets, and objects that can be handled during treatment, such as twiddle muffs. These are knitted tubes in which the hands can be placed for comfort (see picture above).

They are often adorned with tactile objects, such as large buttons. Twiddle muffs are particularly useful with clients who find focusing difficult, as they offer an ongoing source of interest.

HANDS-ON THERAPIES, TEXTURES AND COVERINGS In the UK, two of the most popular therapies are massage and aromatherapy, which are practised using a lubricant such as an oil or cream. Often the contact between therapistt and client is directly between the therapist’ss hands and the client’s skin.

However, there are complementary therapies where the client does not have to disrobe and the work is carried out either n through the clothes or a lightweight cotton

CASE STUDY 2 Michael had Parkinson’s disease.

After resting or sleeping, he needed support and guidance to sit up and to initiate walking with a frame. Dressing was challenging for Michael, and he preferred clothed treatments over his t-shirt and trousers. Michael requested that a blue towel be placed on the pillow as it reminded him of blue skies in the garden in summer. The therapist placed his ‘special blanket’ over the towelling covers and worked gently though the fabric using a variety of fl owing strokes, gentle pressures and holding techniques.

Michael quickly relaxed, and for a while the shaking stopped, giving him some relief. After around 20 minutes of treatment, Michael was guided into the left lying position (recovery) and provided with a soft ‘hug pillow’ covered with his chosen blue towel, with additional pillows placed behind his back and between his knees. After five minutes of resting Michael appeared sufficiently alert and he was then guided from the side lying position to sitting up. He was then able to dismount from the couch and mobilise using his walking frame.

cover. Examples include reflexology, shiatsu, Thai massage, chair massage, craniosacral therapy and reiki. If you are concerned about the fabric reducing the degree of sensitivity of the skin, then don’t be.

Rantala (2017) suggests that the skin can detect 5mg of weight on the face, which is equivalent to the wing of a fly dropping from a height of 3cm onto the skin. To put this another way, the skin can detect the gentlest of breezes on a summer day, when there is no obvious physical contact with the skin.

There are situations where the client would like some hands-on work but may be reluctant to disrobe for a variety of reasons.

Where clients are particularly vulnerable – for example, through illness, body image issues or ageing – it can be a useful option for the use of fabric to play a major role in the treatment.

Rather than regard the covering as purely functional fabric and having to remove it to establish hand-to-skin contact, the covering can become a resourceful interface between the hands of the therapist and the skin of the client.

A therapeutic approach that is based on the skilful use of touch and is always applied through a fabric of some kind is the HEARTS Process (Carter, 2017). While working in a cancer care setting, Ann Carter developed this therapeutic approach, which deliberately focuses on using hands-on work through textures.

Her motivation for developing this approach was to find techniques that could be used where the skin of patients with cancer was very friable, where skin

texture had been altered by prolonged drug use, and where the patient was reluctant to remove any clothing. HEARTS (which stands for Hands-on, Empathy, Aromas, Relaxation, Textures and Sound) can be used in conjunction with conventional complementary treatments or as a therapeutic approach on its own.

Treatments using this approach can last from five to 25 minutes and are designed to bring about a state of calm or relaxation quickly and easily.

Case study 2 opposite illustrates the use of coverings and safety aspects of working with a client living with a longterm condition. Michael always brought a ‘special’ multicoloured blanket, which had been knitted by his partner, to the treatment. His partner had died and he welcomed the opportunity to bring positive memories into the treatments.

CONCLUSION When we were novice massage therapists, we followed patterns of working as we

decided on the appropriate coverings (usually towels) for our clients. We also decided when and how to arrange the towels so we could expose the areas of the client on which we could work, often with little dialogue with the client or commentary about what we were doing.

This article originated from wanting to develop our practice to accommodate the needs of our clients and to find ways of enhancing the therapeutic experience.

Increasingly, our clients may have complex health challenges, and we hope that our suggestions will offer ways of enhancing your therapeutic work.

Q

I was excited to read NHS England will be recruiting 1,000 link workers to support social prescribing. How can I connect with these link workers, so they can refer patients to me?

Dr Michael Dixon, chair of the College of Medicine and NHS England lead for social prescribing, says: As you’ve highlighted in your question, NHS England is planning to recruit 1,000 link workers to support social prescribing this year (for more information, visit fht. org.uk/blog-linkworkers). The role of a link worker, who may also be called a social prescriber, is to find out what really matters to patients so that they can support a holistic approach to improving their health and wellbeing by connecting them to community groups and statutory services for practical and emotional support.

Over the next few months, GP practices will be coming together to form primary care networks, with a population of between 30,000 and 50,000 patients. They will then need to deliver plans by 1 July for how they might employ a link worker and additional members of staff in primary care, such as pharmacists.

The priority for FHT members like yourself will be to find out exactly which of your local GP practices are linking to form these new primary care networks and to start building a relationship with whoever is in charge, which in many cases, will be a GP and practice manager working together. Let them know either directly or via a local clinical champion – perhaps a GP or nurse that you are friendly with – about the value that your service can offer to individual patients, the local population and in developing local

health in general. Make sure that you are also in sufficient contact with them to know when they have appointed a link worker, so you can introduce yourself.

The new world will be about relationships.

It was ever thus, but today, as we move to a less biomedical model of care, there will be increasing recognition that the treatment of every practice has to go hand in hand with the relationships and compassion that we give and our ability to activate patients in their own health and healing.

This presents an enormous opportunity for

complementary therapists to redefine their worth to patients, commissioners and the local community.

The creation of link workers during 2019 will destabilise forever the message that medicine is just about drugs and procedures. It now encompasses a far wider world that should link arms with complementary practitioners and end the historical dichotomy of conventional versus complementary as social prescription and personal budgets begin, at last, to centre care around the patient.

A client has asked to see what information I hold about them.

Do I need to provide this?

FHT registrar and compliance manager Julie McFadden says: Yes, everyone has the right to access a copy of their personal data, which is commonly referred to as ‘subject access’.

Your client can request this verbally or in writing, and you have one month to respond to their request.

The Information Commissioner’s Office (ICO) recommends that ‘it is good practice to have a policy for recording details of the requests you receive, particularly those made by telephone or in person. You may wish to check with the requester that you have understood their request, as this can help avoid later disputes about how you have interpreted the request. We also recommend that you keep a log of verbal requests.’

Furthermore, ‘GDPR requires that the information you provide to an individual is in a concise, transparent, intelligible and easily accessible form, using clear and plain language’.

Of course, before you provide any personal data, it is important to make sure that you are supplying this to the client in question (for more guidance, see bit.ly/ICO-rightof-access).

The ICO advises that in most cases, you cannot charge a fee to comply with a subject access request, unless the request is ‘manifestly unfounded or excessive’, in which case you can charge a ‘reasonable fee’ to cover administrative costs.

You can also charge a reasonable fee if your client requests further copies of their data following a request.

A workshop I completed advertised that it was worth 12 CPD points, but you said it will only count towards seven of my 10 points required annually as an FHT member. Why?

FHT education executive Heidi Hinton says: Unfortunately, there is no universal continuing professional development (CPD) point system used by everyone working in the therapy industry.

As different organisations allocate CPD points differently, it may be that the training provider based the number of CPD points for their workshop on the point system used by another training provider or professional association.

For a formal half-day or full-day workshop or seminar that meets the FHT’s CPD criteria, we allocate one point per hour, so it’s likely that your workshop involved seven hours of learning.

If you are concerned about how to make up the other three points to reach your 10-point quota for the year, there are a number of ways you can do this that won’t involve any further training or cost. For example, you could complete three of the spiral quizzes featured in International Therapist (one point per spiral) or read and reflect on an article in this or any other issue of the magazine (three points for 300 words of reflection – see page 54 for some CPD questions that you may find helpful).

I provide treatments from a purpose-built unit in my garden. Will my insurance through the FHT cover the building and my stock and equipment?

Beverly Bartlett, FHT membership and insurance services manager, says: If the insurance you are referring to is FHT’s medical malpractice, public and product liability policy, then the answer is no. This policy is designed to cover your work as a therapist, and any potential damage that occurs to your client or their property. It does not cover your own business premises, or your stock and equipment.

Similarly, it’s worth highlighting that any buildings and contents insurance you have in place for your home is unlikely to cover your business premises, or your business stock and equipment, unless you have spoken to your insurance provider specifically about this and they have arranged suitable cover.

The good news is that the FHT can help to find you suitable insurance cover for your business premises and stock and equipment, whatever your needs. So, whether the building in your garden is made of bricks and mortar, is a converted barn or summerhouse, we will be able to put you in touch with a suitable insurance provider to help you find a policy that is right for you.

I

n 2007, I was one of the first students to complete a BSc in complementary therapy in practice at Wigan and Leigh College. I wanted to support those who needed therapies as a necessity, not just a luxury, and this led me to join the team at Willowbrook Hospice, working as a complementary therapist.

Willowbrook is an independent charity offering specialist palliative care (SPC) for patients with lifelimiting illnesses living in the boroughs of St Helens and Knowsley, which are hard-hit by high cancer death rates, respiratory diseases and deprivation. These factors lead to high demand for services within the locality.

I have helped develop the complementary therapist role from part-time to full-time, become an integrated member of the multidisciplinary team (MDT) and undertaken further courses to enhance my skills as a therapist within SPC. One of the most difficult challenges was to integrate complementary therapy into the SPC setting and to raise its profile. It has taken and will continue to take patience, determination and perseverance to champion it, and to increase staff knowledge and understanding so that it’s fully utilised.

Supporting emotional health and wellbeing can be a huge challenge in our society. I am passionate about developing holistic approaches to wellbeing, self-awareness and selfcare techniques that can be utilised by all. I try to embed holistic approaches within every aspect of my practice and look for creative ways to assist with wellbeing. To truly adopt a holistic approach, effective teamwork, support and networking is vital.

IMPROVING SERVICE DELIVERY I assisted with the development of a ‘4 Ease’ patient education programme within our day hospice unit. This includes delivering a ‘holistic toolbox’ of self-management techniques that aim to promote empowerment, encouragement, engagement and enjoyment. The peer support programme is delivered by myself and other key members of the MDT and enables our patients to live life to their fullest potential for as long as possible.

BEREAVEMENT SUPPORT Anticipatory bereavement support is a key component of good palliative care and normalising death and dying for young children can be extremely challenging. As an extension to complementary therapies, I have used an established therapy intervention, story massage, on the inpatient unit to help break bad news and reduce fears around the dying process.

It has offered interactive ways of communication through compassionate touch techniques and personal storytelling.

The non-invasive approach can give a sense of calmness amid tension and offers space and time for the children to feel included.

With the recent support of a national grant from Hospice UK, I have helped set up a children’s bereavement service called ‘The Willows’, along with family support.

This has enabled me to support children through the use of complementary therapies. I do this by calling it ‘slothfulness’. The children are able to identify with this name and know that this means slow, calm and relaxing

The children actively engage in slothfulness through mindfulness techniques, grounding exercises, creative projects such as collecting memories/moments in ‘bee happy’ jars, positive affirmations, self-care activities, guided relaxation, and the use of singing bowls. Slothfulness provides quiet reflection time at the end of the session before the children go home.

The itinerary of the group includes ‘hello’, ‘fun time’, ‘fill up time’, ‘me time’ and ‘downtime’ (slothfulness) – this is a developing service with growing links to the young carers centre.

INTEGRATING COMPLEMENTARY THERAPY INTO THE COMMUNITY As an extension of complementary therapies with end-oflife care (EOLC), I work in partnership with our community integrated advanced care planning team and have input as a facilitator for the EOLC forum for over 30 homes in the boroughs of St Helens, Knowsley and Halton. As part of a national programme for care homes called Six Steps for Success, I provide training to support and develop staff skills and knowledge. The project provides insight into complementary therapies, potential benefits, how they can enhance quality of life (QOL) within their settings and how they can be utilised in EOLC. Following this, additional training is offered via a practical workshop consisting of basic, adapted touch therapies and guided relaxation for those wanting to enhance the care and environment.

The six steps are: 1 Discussions as EOL approaches 2 Assessment, care planning and review 3 Coordination of care 4 Delivery of high-quality care in care homes 5 Care in the last days of life 6 Care after death.

This project helps to support steps four and five of the Six Steps for Success programme.

Residents have responded positively to therapies, reporting reduced agitation after treatments. After spending time in our relaxation room and receiving a hand massage, one lady needed less medication to manage her agitation.

The complementary therapy training is very much enjoyed by the care home staff who attend, with lots of positive d evaluations and a demand for further training.

WELLNESS FOR YOU WELLN With compassion fatigue, staff welfare and burn burnout of those working in EOLC services in increasingly hitting the headlines, lleading healthcare organisations are declaring a public health crisis. While working in palliative care can be rewarding, it can often be challenging and healthcare workers are at high risk.

Following a hospice literature review in 2015, the idea of a dedicated wellness and resilience programme w was conceived called Wellness for

Programmes that combine multiple strategies can generally reach more people, so an extension to the WFY programme was devised in April last year – an annual staff and volunteer safety and wellbeing planner, utilising some tailored national awareness events and representing champions across all departments to facilitate a team approach to health and wellbeing, ensuring sustainability.

The wellness strategy continues to grow and be successful.

Over time, the WFY programme has proved to be flexible and adaptable. As another extension, I have taken elements of the programme into the wider community to better support members of the public with their health and wellbeing in our community-based venue, The Living Well.

HOLISTIC SUPPORT SERVICES

You (WFY). With increasingly high staff turnover, high sickness levels and poor morale, the drive to support staff became our focus. This is a supportive wellbeing strategy, focusing on improving health and energy, satisfaction in the workplace and reducing compassion fatigue. The aim of the WFY programme is to identify necessities for us to feel good, look at our perceptions of situations, and provide coping strategies.

It does this through a series of fun, informal and supportive self-care sessions, to help people: Regain a sense of happiness Become more self-aware Find confidence to take charge/combat fears Have courage and reach goals/ambitions See what they can nurture to help get to the next level.

Sessions include focusing on happiness, mindfulness, grounding techniques, easy ways to feel good, not comparing and opening up to new possibilities.

Overall evaluation has shown the programme to have been a great success in building resilience and improving wellbeing. There have been positive impacts on staff sickness, morale and working culture as staff feel more supported and valued. A poster application for this was successful at the Hospice UK Conference in 2016 and the abstract was published in BMJ Supportive and Palliative Care (2016).

I have developed a non-clinical volunteer-led service called Holistic Support Services (HSS). This is a 10-week programme that offers different activities through a creative wellbeing group, WFY workshops and a complementary therapy group.

The sessions are delivered in a relaxed and supportive environment, which can be attended as a full day or individually and costs just £2 per session or £5 for a full day.

This helps to provide additional income and exposure for the hospice. The Living Well also generates income through a bespoke charity shop, café and room hire.

Sessions are open to anyone aged 18 or over living in Knowsley and St Helens. I carefully recruit, train and supervise our volunteers. Using the Likert scale to measure outcomes, the group has shown significant improvements in sense of wellbeing, anxiety/stress, relaxation, general mood and feelings of positivity.

We network with local services, including the local council, with a key aim of reaching out further into the community and with a vision of developing the HSS further, through joint partnership, to become a model to support patients and carers in their own homes.

The work I do is highly rewarding, and the evaluations and feedback demonstrate that complementary therapies are effective and make a positive impact across multiple settings.

The last few years have been overwhelming with the amount of support shown for complementary therapies here at the hospice. I hope to continue to be innovative in my practices and deliver the best care possible for all.

I

n the past couple of years, the public has started to turn its back on plastic and single-use products and is embracing the principles of sustainability and zero-waste living like never before. Some experts have referred to this unprecedented shift in public opinion as the ‘Blue Planet II effect’, with the final episode of the BBC series and Sir David Attenborough’s stark warnings about climate change and pollution triggering a grassroots demand for action.

Bringing the debate a little closer to the salon door, a recent report published by Mintel warned beauty and personal care

(BPC) businesses that ‘consumers expect brands to take responsibility for their waste, and will be attracted to manufacturers, companies and brands that not only do so, but also facilitate waste reduction processes for consumers’ (Mintel, 2018). The report goes on to highlight that some of the key reasons consumers want a zero-waste lifestyle are that it’s top of the news agenda, it helps them to feel like they are ‘making a difference’ by taking their own steps to reduce waste, they feel it’s healthier to seek natural solutions, and it inspires others to follow.

In the same vein, a report published by Soil Association Certification (SAC, 2018) identifies that sustainable shoppers have driven the market for certified organic and natural beauty products to an all-time high, with a 14% growth in sales in 2018.

The report highlights that awareness around sustainability, ethical working practices, brand integrity and avoiding toxicity are key factors driving this rapid growth, and that millennials and the younger generations in particular are pushing for sustainable change in the industry, meaning this trend is likely to continue.

We’re thrilled to report that our 2019 survey showed that more than half of FHT members (52%) already use sustainable and environmentally friendly products. For others wanting to take some steps to join this positive movement and show their clients that they too are doing ‘their bit’ to create a more ethical, environmentally friendly business, we hope you find this article useful.

REDUCE – REUSE – RECYCLE As you look across the whole of your business, these three little words will help you to address matters in a logical order and minimise the amount of materials, resources and energy potentially used or wasted.

Reduce. The first step is to simply cut back.

The less we purchase or consume, the less we waste. For example, while bulk buying products can seem cost-effective – and potentially environmentally friendly, if it means less packaging and fewer deliveries – if you don’t use or sell all of the product before its expiry date, it will go to waste.

Could you switch single-use products for a reusable alternative, such as washable mitts or flannels instead of disposable cotton pads?

Or perhaps you could reduce the heating in your therapy room by one degree?

Recycle. When you can’t reduce or reuse something, this is where the third ‘R’ comes into play. However, while recycling is better than sending waste to a landfill, breaking down textiles, paper, plastics, glass and metal requires time, energy, materials and cost, so should be a final resort.

ENERGY MATTERS When it comes to conserving energy, there are a few areas worth considering, such as how this energy is sourced, how you use it and how much you use.

Green energy Green energy is produced from renewable sources that are naturally replenished, such as solar power, wind power, hydroelectric power, tidal power and biofuels. These are also considered green because they produce a smaller carbon footprint compared to nonrenewable sources (for example oil, coal, gas and uranium).

According to government research, renewable sources produced 29.3% of the UK’s electricity in 2017, which was a record high (DBEIS, 2018a). Green energy tariffs are also becoming more competitive, which is potentially due to the public’s growing support for renewable energy (85%) and concern about climate change (74%) (DBEIS, 2018b).

If you are looking to switch to a green supplier, check the current tariffs available and keep an eye out for some of the smaller providers, as these may be offering deals that compete favourably against the more mainstream suppliers, such as British Gas, EDF, SSE and Scottish Power.

Lightbulb moment For any electric lighting in your salon or clinic, using energy-efficient lightbulbs is a key way to save both energy and money.

Compact fluorescent lamps (CFLs) are considered the best option, as they use 75 to 80% less electricity than traditional lightbulbs and last up to 10 times longer. If you are using spotlights or dimmable lights, then LED (light-emitting diode) bulbs are a better choice.

TOWELS, TEXTILES AND COUCH ROLL Our 2019 survey showed that more than 70% of FHT members regularly use towels and couch roll. We also know that many therapists use sheets, either to protect their client’s modesty or for hygiene purposes, as an alternative to couch roll.

Whatever product you use – or you are thinking of introducing to your treatments, after reading Peter and Ann’s article (page 12) – here are a few things worth considering: It may be wise to spend a little extra money on high-quality towels, sheets and other material-based products, as these are likely to be more durable and last longer than cheaper rivals.

Are there eco-friendly or more ethical alternatives you could use when it’s time to buy new couch roll or reuse, upcycle or recycle your tired towels and sheets? For example, fair-trade 100% natural textiles, such as organic cotton or bamboo?

Where possible, only put the washing machine on when you have a full load to reduce the number of cycles and use ecofriendly products.

Can any of your towels, sheets and other textiles be separated out and washed at a slightly lower temperature than others, without compromising hygiene and cleanliness?

Lots of salon owners and therapists tumble-dry their towels for that ‘soft feel’.

Try air-drying your therapy textiles as much as possible first, to save electricity.

BRANDS WITH INTEGRITY If you are in the process of reviewing which skincare range to stock, sales in certified organic, natural and vegan products are reaching an all-time high.

Conscious consumers are seeking a holistic approach to their beauty and personal care routines, using products that align with their ethical and environmental values.

If you’re concerned that natural or organic products may not perform as well as the big beauty brands, SAC highlighted this in its 2018 market report: ‘In the past, there have been misconceptions about the sophistication of organic vs. conventional beauty, but the idea of having

WHOLE BUSINESS APPROACH...

While some actions and swaps will be easier to identify and implement than others, once you get the ball rolling and do a little research online, you’ll probably start to look at every aspect of your business with a fresh pair of eyes. Here are a few other pointers...

Could you swap glossy magazines in your reception or relaxation area for health-related books or mindful colouring pads? They last longer and will inspire your clients.

Go paperless – could you use online client records and booking systems through software suppliers such as ClinicSense?

If you provide your clients herbal tea, consider suppliers who support fair trade and use plastic-free tea bags, or use loose tea.

Can you buy locally to reduce your carbon footprint?

If you’re redecorating your salon or treatment room, consider using eco-friendly paints.

Key things to look at are the ingredients, manufacturing process and breathability.

Are the cleaning products you use eco-friendly?

Can you encourage your clients to join in? If you sell products, could you offer a small discount if they return their empty bottles to you, or suggest how they could reuse, upcycle or recycle these?

Remember: one small change is a positive change. Not all the suggestions in this article will be practical or affordable for everyone. At the FHT, we’re taking one step at a time – from looking at alternative wrapping and paper options for International Therapist magazine, to our daily practices at FHT HQ – but we’re pleased that we’re making a more conscious effort to join the movement.

Tell your clients what steps you have taken to support sustainability and zero-waste, perhaps in a little sign on the wall or on your website – they will appreciate your efforts and you could attract more clients with the same values.

CHOOSE CANDLES WITH CARE Candles and tea lights help to create a relaxing treatment environment without drawing on electricity. The downside, however, is that some candles – and paraffin ones in particular – can be potentially damaging to both the environment and our health, due to the toxic chemicals they release.

One option is to switch to fake batteryoperated candles but for those who prefer the real thing, experts consider natural wax candles to be the best option. Soy candles are reported to be the cleanest, producing around a 10th of the soot created by a paraffin wax candle (Wynarczyk, 2019).

Where possible, always try to open the window and circulate the air in your treatment room on a regular basis. You could also consider introducing a houseplant or two to help improve air quality (see below).

HOUSEPLANTS FOR HEALTH Houseplants not only help to improve air quality by trapping and removing pollutants – an article published on the Royal Horticultural Society’s website highlights that these leafy allies offer a range of psychological and physical health benefits too (RHS, 2019).

Studies have shown that in terms of mental wellbeing, indoor plants can help to improve mood, reduce stress levels and increase productivity at work, while on a physical level, they can help to reduce blood pressure, fatigue and headaches and improve breathing problems (created by indoor air quality).

Five easy-to-grow houseplants that are recommended by the RHS to improve air quality are: 1 Madagascar dragon tree (Dracaena marginata) 2 Indian rubber tree, rubber plant (Ficus elastica) 3 English ivy, common ivy (Hedera helix) 4 Boston fern (Nephrolepis exaltata ‘Bostoniensis’) 5 Mother-in-law’s tongue (Sansevieria trifasciata).

ESSENTIAL SINGLE-USE ITEMS If your treatment requires single-use items that it would be impractical or unhygienic to find a greener alternative for, have a look at whether you can source organic, biodegradable or fair-trade options instead. For example, did you know you can buy cotton buds that are made from bamboo and cotton, and organic and unbleached cotton balls?

Joint I approaches

n the UK, more than 10 million people have arthritis or similar conditions that affect the joints. The most common type of arthritis is osteoarthritis, affecting around eight million people.

This is the ‘wear and tear’ form of arthritis affecting the smooth cartilage lining of the joint that usually develops in individuals in their late 40s who have a family history of the condition. However, it can occur in much younger people and may be associated with other joint-related conditions. The joints most commonly affected include the hands, spine, hips and knees.

Rheumatoid arthritis differs from osteoarthritis by occurring when the body’s immune system attacks its own tissues, with the outer covering of the joint being the first place to be affected. In the UK, around 400,000 people are affected by rheumatoid arthritis, which tends to occur between the ages of 40 and 50 and affects women three times more commonly than men. This type of arthritis can also cause problems in other organs and body tissues.

More than 200 forms of different musculoskeletal conditions have been identified, which in addition to degenerative and inflammatory arthritis include connective tissue diseases such as systemic lupus erythematosus, scleroderma, dermatomyositis, soft tissue musculoskeletal pain such as bursitis, and back pain.

The symptoms of arthritic conditions range from mild aches and pains in and around the joints to swelling, weakness and muscle wasting, and the inability to undertake daily tasks because of pain or limited range of movement. Other less obvious joint symptoms may also include general tiredness, fatigue, loss of appetite and skin rashes.

RELIEVING PAIN We not only have an incredibly wide range of essential oils at our disposal, but nowadays we have an increasing awareness of the potential therapeutic properties and actions of these at a chemical component level.

Numerous research studies have demonstrated how many essential oils can act as anti-inflammatory and pain-relieving agents through interaction with various enzymes, carrier proteins, receptors and proteins involved in signalling pathways, thereby slowing down or inhibiting associated biological processes (Bahr et al, 2018; Cino, 2014; Sritoomma et al, 2014; Yip and Ying Tam, 2008).

As there is no cure for arthritic conditions, the aim is to help clients ease symptoms of pain and stiffness, and to address any psychological aspects by attempting to alleviate changes in their perception of and ability to cope with pain. Once we are aware of our clients’ conditions and concerns, we have the potential to create a tailor-made blend to address and alleviate those issues.

Once all relevant medical history, lifestyle and other key information has been collected from the client, it’s important to ascertain what we are trying to achieve from our treatment and therefore what the purpose of our blend is. In many cases, we will not be able to address every client concern, so it’s best to focus on one or two main concerns and attempt to deal with these. This will provide focus and help us with the best method of application, appropriate essential oil selection and concentrations of individual oils in the blend.

Many essential oils that offer painrelieving properties should only be used short-term and can act as potential skin irritants. It’s also important to keep updated

For example, oils that contain a high percentage of the monoterpenoid alcohol menthol exert a cooling sensation when applied diluted to 2%, producing a calming and numbing sensation on the skin.

However, when applied at 5% this acts as a rubefacient counter-irritant inducing warmth and a burning sensation. The phenylpropanoid ether eugenol acts as a counter-irritant, inducing a sensation of heat at low concentration and is an effective component for pain relief, but it should not be used with anyone taking anticoagulant therapy.

Research on massage has shown positive benefits. Although most studies tend to be relatively small in scale and over short periods of time, they show promising outcomes

(Bervoets et al, 2015; Field et al, 2014).

In terms of providing massage for the arthritic client, it’s imperative to consider client comfort and good support, especially of the affected area. The use of bolsters, pillows or other props should be employed where indicated; it may be, for example, that the best position for a back massage is in a seated position, rather than face down on the couch.

In terms of techniques, the use of slow, broad massage strokes with even pressure, which can be modified depending on the client’s needs, can help clients feel safe, soothed and comforted. Never massage over areas where there is any acute inflammation, such as a local flare-up, and ensure the treatment is never painful. Treatment times need to be flexible and a session may be shorter than for a regular client. It’s better to provide a shorter and effective treatment than to over-treat the client.

HOT AND COLD TREATMENTS The application of heat and cold has also been found to be an effective treatment modality for both osteo- and rheumatoid arthritis. In general, cold applications should be used for the treatment of active arthritis and heat for non-inflammatory disorders. In individuals with osteoarthritis

of the knee, ice massage has been found to improve range of movement, function and knee strength (Aciksoz et al, 2017).

The use of cold packs has been found to be less effective than ice massage; although swelling is decreased, there is no significant effect on pain. Both heat and cold can easily be incorporated into an aromatherapy treatment session.

In January, NHS England published its Long-Term Plan outlining how the health service will evolve over the next 10 years.

This plan recognised the growing prevalence of arthritis and musculoskeletal conditions in England, and their impact on physical and mental health services. Since the majority of knee and hip replacements carried out by the NHS are for people with osteoarthritis, delays in access to such operations for those who need them can lead to deterioration in health and worse overall outcomes. Aromatherapy and complementary therapies, in general, may help play a supportive role for clients waiting for joint replacement surgery.

Jean is a 64-year-old client who I have been seeing on and off for more than three years.

She was diagnosed with polymyalgia rheumatica by her GP after complaining of pain and stiffness, mainly in her shoulder and thigh muscles. Originally, she thought this was the after-effects of a virus as she had a severe cold that left her feeling rundown and lacking in energy. She then noticed that in the mornings her shoulders were particularly stiff, making it difficult to get out of bed and get dressed. However, once she was up and moving around, she felt better.

She was referred to the local rheumatology department and prescribed a course of prednisolone, which she took for almost two years.

Jean came to see me once this had finished as she felt slightly depressed by the whole episode and was being monitored for the development of a condition called temporal arteritis, which is often associated with her condition. She had not previously tried

any complementary therapies and was slightly apprehensive about how aromatherapy could help her, but was determined to take some positive action to help manage her condition.

As the muscles of the shoulders were very tense, tight and painful, we decided to make up a blend to address this as well as her stress. That original blend consisted of true lavender (Lavandula angustifolia), sweet marjoram (Origanum marjorana), peppermint (Mentha piperita) and basil (Ocimum basilicum) blended into a stock carrier base oil blend, consisting of calendula, St John’s wort and arnica.

Jean had her very first aromatherapy massage to her shoulders, while in a seated position, and found the treatment so beneficial that she booked a course of treatments and came weekly for the

next six weeks. Over that period, the combinations of oils changed depending on her progress. I provided shower gels for use first thing in the morning and a muscle rub to use at home between treatments.

Her shoulder muscles gradually became less tender and she was happy for the treatment to include massage around the muscles of the thigh and lower back.

Today, Jean comes to see me for a short course of ‘top and tail’ sessions.

She’s feeling much better, as she has also made other changes to her lifestyle, exercise regimen and general outlook on life.

Having used essential oils and massage to see her through a challenging time in her life, she is now much more open to other forms of complementary therapy, believing that if you look, the right thing will come along to help you at the right time.

hen thinking of yoga, many people imagine a young woman bending into pretzellike positions, or someone dressed in orange chanting ‘om’. I doubt many people think it is suitable for older people, including those living with dementia and other forms of cognitive impairment.

Older people often say to me, ‘Oh, I’m too stiff to do yoga.’ To me, that is like saying ‘I’m too dirty to have a shower,’ or ‘I’m too

overweight to go on a diet.’ Yoga is highly adaptable and suitable for all people.

In this article, I will look at how yoga can benefit clients living with dementia, how it can be adapted, and give a few examples of techniques that can be used with this type of client group.

dementia. These diseases result in a loss of brain cells and affect how the brain functions.

One of the most common risk factors with dementia is age, with people over 65 most likely to be affected. The progression of dementia will vary according to what type of dementia someone has, when they are diagnosed and many other factors. Therefore, the experience of working with someone living with dementia can vary greatly.

POTENTIAL BENEFITS OF YOGA Although there is no known cure for dementia, it does present a number of common symptoms that can be alleviated

with yoga. In a pilot project I ran, the clients appeared more awake, alert and happy after practising the yoga sequences.

One of the most compelling studies (Eyre et al, 2016) showed that a yoga exercise, which took just 12 minutes each day, was more effective than memory enhancement exercises in minimising cognitive decline.

Studies have also shown that meditation and yoga have wider anti-ageing effects, and those who meditate have younger, healthier-looking brains with more brain blood flow at rest.

The Alzheimer’s Association estimates that 20 to 40% of people with Alzheimer’s also have depression. Yoga, and in particular yogic breathing, has been shown to have a significant impact on depression by taming the stress response.

The way this works is that yogic breathing helps to switch on the parasympathetic nervous system, which helps to slow the heart rate and increase the production of feelgood hormones.

Yoga and mindfulness-based practices have also been shown to reduce feelings of anger and anxiety. When a person feels anxious or angry, their body goes into a fight-or-flight response. Hormones such as cortisol and adrenaline are released, the heart rate rises and blood flows away from the vital organs to the muscles as the body prepares itself for action. As well as this physical response happening when the incident first occurs, it can also repeat when we remember or retell an incident. Yoga and mindfulness can help us be more in the present and help us deal with these ruminations.

awareness, known as proprioception.

Yoga can also help with balance, posture, breathing and flexibility. All of these can help people improve overall wellbeing.

ADAPTED TRADITIONAL YOGA PRACTICES When I first started volunteering in my local nursing home, I treated a range of different clients with age-related conditions, including dementia. As the abilities within the classes were so varied, I was keen to make my classes as inclusive as possible. When designing the sequences, I therefore included a range of different movements, but also practices from the wider yogic traditions, including chanting, awareness practices, guided visualisations and meditation.

These wider practices, in particular singing or chanting, proved to be very popular. When yogic singing or chanting, we focus on repetitive sounds, which anyone can experience. Chanting in a group helps people to relax and feel connected to one another. Furthermore, research shows that singing connects neural pathways and fires up the right temporal lobe of the brain, which releases endorphins.

Much of my research and experience has been in residential care settings. One of the key points with those living with dementia is to adapt to the person you are working with, not what they used to be like, or your idea of what a yoga position should look like. It is important to remember also that yoga can be highly beneficial for family and other carers.

PRACTICES Here are three short practices that are useful for those living with various stages of dementia, or who are worried about cognitive decline. These practices and photos are taken from my book Yoga for Dementia (Jessica Kingsley Publishers).

WOOD-CHOPPING ANGER PRACTICE The wood-chopping exercise is a fantastic practice for releasing pent-up energy and frustration. It can be done from three positions: sitting in a chair with the legs apart (this may be the preferred option for older people), standing with the legs wide, or squatting. I would not recommend this exercise if you have any pre-existing back problems.

1 Interlace your fi ngers and keep your palms strongly together while sitting with your back straight. Imagine the thing you are angry with on a chopping block in front of you.

Next, inhale and take your arms straight above your head with your forearms by your ears.

2 3 4 When you are ready to exhale, swiftly bend forward at the waist and take your arms down making a strong ‘haa’ sound as you exhale. The movement is like axeing a tree or swinging the arms down – hence the name wood chopping.

Repeat the exercise five to 15 times, building up strength and stamina each time you do the exercise. After doing the exercise a few times you will probably find your heartbeat will have increased, and you might feel a bit warmer.

At the end of the exercise, rest back in your chair or lie down in a comfortable position. Then when your breath has returned to normal, sit up slowly and take a few minutes to notice how you feel.

THREE-PART BREATH Three-part breath is a fantastic calming practice.

It can be done lying down or sitting up straight.

1 Place your hands right down on your lower belly so that your little fingers touch the top of the hips. Spread the fingers wide and have the tips of the middle fingers lightly touching. As you breathe in and out, see if you can direct the breath into your belly. As you breathe deeper into your belly, see if you can feel the tips of the middle fingers 1 move apart.

2 After two to three minutes like this, move your hands to either side of your ribs.

Spread your fingers and press your hands gently into the side of the ribs so you can feel them moving as you breathe in and out. Be

aware of the breath moving the ribs, and see if you can feel the way the breath not only lifts the ribs but moves them out to the side. Try to imagine that the breath is filling up the lungs like a balloon. Stay like this for two to three minutes.

3 Next, bring your 2 hands up to your collarbone, spread the fingers wide so the little fingers almost touch the breastbone and the thumbs the inside edge of the shoulders, and focus on the breath moving in the upper chest.

Feel how the breastbone lifts as you inhale, and as you exhale allow the shoulders to move out and down.

Again, stay like this for two to three minutes.

Tania has been a certified yoga teacher for many years and piloted a therapeutic yoga programme for people with dementia with a UK-based care provider.* *The pilot project was supported by a Patients First grant from the Foundation of Nursing Studies and the Burdett Trust.

KIRTAN KRIYA – 12-MINUTE MEMORY BOOSTING PRACTICE This singing exercise can be used by anyone interested in improving memory loss and brain function.

The practice involves singing or whispering the sounds ‘sa’, ‘ta’, ‘na’ and ‘ma’. The thumbs touch the index fingers on ‘sa’, the middle fingers on ‘ta’, the ring fingers on ‘na’, and the little finger on ‘ma’. Both hands perform the same movements at the same time to help with coordination.

Use a timer to softly chime on every second minute. Sit in a comfortable position with your spine straight.

For two minutes, sing at a normal level while moving your thumb along the tips of your four fingers. For the next two minutes, whisper the sounds, again with the hand movements.

For the next four minutes, say them silently. Next, whisper them for two minutes and then for the final two minutes sing them out loud. To finish the exercise, inhale deeply, lift your hands above the head, and then bring them down slowly in a sweeping motion while exhaling.

Sit still for a while and take a moment to observe how you feel.

Feel the breath first rising in the belly, next the ribs and then into the upper hand.

Then feel the breath leave your upper chest, mid-chest and belly.

It can be helpful to imagine that the torso is an empty vessel and we are drinking in the breath through 3 a straw and filling the torso from bottom to top. On the outbreath, you could imagine the breath like a gas as it evaporates from the lungs. Spend five minutes just focusing on this full deep yogic breath.

Return to a natural breath for a minute or so and take some time to tune in with yourself and note how you feel.

If you are working with a person or a group, encourage them to come out of this practice gently, open their eyes and stretch.

The benefits of full yogic breath include engaging the three main parts of the lungs and helping to correct poor breathing patterns. It also helps to release stress and activates the body’s rest and restore mode.

PAINFUL MATER GINA REINGE PROVIDES A CASE STUDY OF A LADY WITH ADHESIVE ARACHNOIDITIS

ADHESIVE ARACHNOIDITIS IS NOT a well-known condition, and its true prevalence remains largely unknown because of poor recognition and inaccurate client records.

The condition is caused by injury to the arachnoid layer of the cranial meninges, which protect the spinal cord, often because of rupture, trauma or infection. Injecting dye into the spine for myelography purposes is one cause of injury, resulting in chronic inflammation of the arachnoid membrane of the meninges in the brain.

Myelography was a common protocol prior to the use of MRI scans, which have now rendered the technique obsolete.

The meninges are membranous coverings of the brain and spinal cord, consisting of the dura mater, the arachnoid mater and the pia mater.

Between the arachnoid mater and pia mater is the subarachnoid space, containing cerebrospinal fluid that acts as a cushion between the brain and skull, while at the same time delivering nutrients to the nervous system and removing waste products.

In adhesive arachnoiditis, the cerebrospinal fluid becomes sticky, causing the nerves to glue together.

As the sticky fluid affects the spinal cord, it creates considerable nerve issues throughout the body. There are several types of arachnoiditis, with the adhesive version being the most severe.

The symptoms of adhesive arachnoiditis include numbness and tingling; stinging, burning pain in the lower back and legs; muscle cramps and spasms; balance issues; weakness; and eventually paralysis.

Currently, there is no known cure and only limited treatment options. Adhesive arachnoiditis is widely recognised as an untreatable condition, and the main aim of treatment is to manage symptoms. Pain

relief options achieve only limited success, and people with this condition have the added complication of responding badly to medication. It is considered a progressive condition, with most patients ending up in a lying wheelchair as pain in the coccyx area makes sitting in a wheelchair impracticable.

PHYSICAL AND MENTAL STRUGGLE I have been working with a client with this condition for the last three years.

She is a 74-year-old lady who underwent a failed myelography procedure in 1969.

In 2002, following subsequent surgery (a laminectomy, which involves removing the back of one or more vertebrae to relieve pressure on the spinal cord or nerves), she began to develop symptoms consistent with arachnoiditis. A scan confirmed the diagnosis.

When we first met, she was seeking help for knee pain, which was making it

very difficult for her to walk. But as we discussed her history, it was clear that she was struggling both mentally and physically with her condition, which left her bedbound for most of the day. She couldnâ&#x20AC;&#x2122;t cook for herself, leave the house for long periods or drive. She felt her life was over and doctors were telling her there was nothing more they could do, suggesting she was likely to be in a wheelchair within the year.

She was not prepared to accept this and decided to see if I could do anything for the worst of her symptoms, the knee pain.

We treat many clients with complex health needs in our clinic and always start by putting the medical complications to one side for a moment and assessing the biomechanics of the person in front of us.

All too often, people get caught up in the disease and miss what can be very simple secondary reasons for the pain they are presenting with.

The reason for the knee pain quickly became apparent. My client had a posteriorly positioned pelvis with a rotation and a lateral tilt. Her feet were very weak and severely pronated. She had no ability to balance, and leg circumference measurements showed weakness in the right side, confirming an overuse of the left leg, the same side as her knee pain. The knee itself was swollen, hot and painful. Putting her arachnoiditis to one side, any client presenting with these biomechanical issues would be experiencing pain because of the forces acting around the overused knee.

My initial treatment plan, therefore, was to rebalance her pelvis and leg strength.

During the first session, she could only tolerate 10 minutes of hands-on treatment.

In that time, I used light trigger-point work to release the gluteus maximus muscle, allowing her pelvis to return to a more neutral position. She reported significant

ALTERING PAIN She kept notes for me over that first week and when she returned a week later she had the biggest smile on her face. ‘What did you do?’ was the first thing she asked, to which I replied rather nervously, ‘I straightened your pelvis – why?’ She then told me the pain in her coccyx, which had made sitting intolerably painful for over a decade, had all but disappeared and her knee now didn’t hurt at all. We were both rather stunned. She had always put her coccyx pain down to the arachnoiditis but here was strong evidence that we could alter her pain. I asked her if

she would like me to continue rebalancing her, to which I got a resounding ‘Yes!’ Over the next year, every fortnight, that is exactly what we did. She gave me copies of her MRI scans and I studied them, looking for any biomechanical reasons for her pain.

In her spinal canal, the spacing between the vertebrae had narrowed and the spinal cord was perilously close to compromise. The spine was showing signs of biomechanical scoliosis as the muscles dragged the spinal column away from neutral.

I started with her core muscles and spinal stabilisers, directing her to sit on a gym ball (with support) and performing a pelvic rocking exercise to encourage the spine to mobilise. We progressed to isolated core activation while lying supine, performing a standing cobra exercise against the wall, sitting on the ball without support, then resupporting and lifting a foot off the floor, and finally lifting the foot off the floor unaided. We strengthened the core until the spine could hold a neutral position.

Using soft tissue techniques – mainly trigger point work and myofascial release (MFR) – I continued to realign the pelvis until the muscles were strong enough to maintain the correct position. I worked mostly on quadratus lumborum and gluteus maximus and used MFR work on the thoracic spine.

I then worked on the deep spinal stabilisers, strengthening muscles such as interspinales and rotatores in order to increase the spacing between the vertebrae.

The clinical rationale was that this would give the nerves an increased space in which to move and hopefully reduce some of the tingling she had in her feet. The objective feedback from our client was that she could move more easily, was less stiff and had reduced tingling.

However, she responded surprisingly well and the pelvis was considerably straighter. I decided not to follow up with any exercises at this point as she was clearly unable to tolerate anything further.

BALANCE AND BAND-WORK Next, I worked on the larger muscles of the spine. She had significantly protracted shoulders and a forward head posture, which was affecting her balance and overall spinal health. I used trigger point work and MFR techniques to release the pectoralis major muscles, long head of biceps and sternocleidomastoid muscles to realign the protracted shoulders and neck. I strengthened the rhomboids, mid trapezius, rotator cuff and triceps muscles until she could maintain a correct shoulder position.

It was slow going, but results were fantastic.

I then started to work on her balance and leg strength. Years of being bed-bound had left her with little leg strength and virtually no balance. One of the characteristics of adhesive arachnoiditis is the inability to plantarflex and dorsiflex the ankle, so I gave her a wobble board and directed her to first push her toes down, then her heels, to get this movement back and create some strength in these areas. It worked perfectly.

I broke down the component movements of walking, beginning with encouraging the heel-to-toe movement we had just gained with the wobble board. We worked on her ability to balance through one leg and alongside isolated band-work with leg extension and flexion, we managed to get basic balance back into the legs and create some strength. Later, we began to work on her ability to perform sit-to-stand and stand-to-sit exercises on a chair using exaggeratedly slow movements to ensure correct alignment of the leg and to maximise the number of muscle fibres used. Over time, I managed to progress her on to mini squats and then full squats with the gym ball for assistance. Finally, I got her to balance on one leg with the wall and ball for assistance.

Eventually she was sitting on the gym ball, lifting one leg and balancing, standing on a wobble board and performing basic squats. Her posture was now within normal levels and her pain had hugely reduced. She could walk, drive, head out with friends, and look after herself at home. None of this was easy, but the general trend was of steady improvement. To this point, our treatment aim was all biomechanical; we were still to look specifically at the underlying medical condition.

MYOFASCIAL RELEASE Now she was in a better physical state, I started to consider the sticky fluid supply in the subarachnoid space. She still experienced numbness in her legs and despite a straighter, stronger spine, frequent neurodynamic sliders and exercise, this was still not abating. Neither was the daily exhaustion she felt, even though she was fitter than she had been in years.

I wondered if the technique of MFR could help to draw fluid back and reduce adhesion.

With my client’s consent, we tried five minutes of MFR on the most superficial area at the base of the skull. During the first treatment, she experienced feelings of extreme heat in her skull, not unpleasant but very strong.

Over the following two weeks, her energy levels improved dramatically, and she was able to cook meals, go out for lunch with friends and weed the garden; she even moved furniture around in her house and did some decorating – activities usually impossible because of exhaustion. The effects began to wear off towards the end of the second week, but we both knew we should continue with MFR, given such a positive result.

Six months after we started, she reported her foot numbness to be of lower intensity, an astonishing thing to report. Eight months after, she began to lower her morphine dose under the supervision of

her doctor and 16 months later, she started to lower the dosage of her pain medication, sodium valproate.

I currently give her 10 minutes of MFR at the end of each treatment to the base of the skull; she is now reporting that the tingling in her feet is intermittent. We have clearly affected her nerves in some manner and are getting a positive response, something that is not thought to be possible with this condition. I would be fascinated to have her rescanned to gain an objective measure of the adhesiveness of the cerebrospinal fluid, for indications on whether the fluid has been affected or whether there is another reason for the improvement in her symptoms.

Recently, my client was admitted to hospital with shaking, twitching muscles and memory issues. Doctors concluded this was caused by her medication and weaned her completely off two of her pain medications. This stopped her symptoms and sent a firm message that she didn’t need these pain-relieving drugs any more. They are now working on reducing the rest of her medication.

This lady had constant pain and the very real prospect of a life in a lying wheelchair.

Three years later, she is on significantly reduced pain medication, has massively increased energy, very little pain, less numbness and a huge zest for life. She isn’t in a wheelchair; in fact, she even takes part in a mainstream exercise class.

I submitted this case study to the 2018 Complementary Therapy Awards and won the Pain Management, Injury Prevention and Rehabilitation award. In the judges’ words: ‘This approach could be replicated in other complex conditions and by other therapists – it is highly transferable.’

Going forward, I am interested in identifying a consultant or trust with an interest in this field that would consider collaborating to help establish what effect MFR is having on the cerebrospinal fluid.

2 Why do we dream?

Most dreaming occurs during REM [rapid eye movement], a phase of sleep in which certain parts of the brain are highly active. There are many theories as to the purpose of dreams, but the whole truth still evades us. We do know, though, that dreaming improves our ability to be creative and solve problems.

3 Why do we feel

When we haven’t slept, the emotional brain goes

into overdrive. The brain’s amygdala – the alarm system responsible for triggering emotions such as fear, sadness, anger or rage – becomes significantly more sensitive.

4 Why do we struggle to concentrate when we’re tired?

The prefrontal cortex is the rational brain’s CEO. It calls the shots, making all the sensible decisions. You can think of your amygdala and your prefrontal cortex as being in a tug of war.

When you haven’t slept well, the amygdala is much more reactive and stronger, which means the prefrontal cortex becomes diminished.

5 Why does lack of sleep make us put on weight?

Sleep deprivation encourages you to overeat by changing the levels of two hormones that are critical to maintaining

weight. Leptin is the satiety hormone, high levels of which signal that you’re full. Ghrelin is the hunger hormone, high levels of which signify that you’re hungry. Sleep deprivation increases ghrelin, so you feel hungry, and decreases leptin, so you never feel full. This is hardly the best combination!

6 Why is it hard to sleep in hotel rooms?

Many of us know that feeling – our sleep is restless and broken. On an evolutionary level, it’s because a new place to sleep might contain a threat and we don’t know for sure if it will be safe.

Our clever brains therefore resist going into slow-wave sleep, which is critical for replenishment and restoration. It’s thought that this is the brain’s way of keeping us in lighter sleep so that we can be on the lookout for threats.

1 Why do we sleep?

Although many of the reasons we sleep remain mysterious, we know that sleep is critical for clearing out the accumulation of waste that occurs during the day. Think of it like a bin man coming round every night to remove your wheelie bin full of physical and emotional rubbish.

TREAT YOURSELF TO SLEEP If you’ve had a bad day, rather than sitting on the sofa eating pizza, washing it down with a half bottle of wine and staying up late on social media, gorge yourself on a generous portion of extra sleep. Light some candles. Read a magazine or book (studies show that brain activity during sleep is better regulated after reading a book, compared with a screen).

When you start feeling the long fingers of melatonin crawling seductively through your system, round it all off with a hot bath. When you get in, all your blood comes to the surface of your body so, when you step out, your core temperature starts to drop. This is a stimulus to sleep.

Getting a large dose of relaxing sleep is one of the most effective ways to take you out of stress state and launch you into thrive.

Norwich LSG explores integrated health in inaugural meeting I run the Norwich LSG with Beverly Chilson and we recently had our first meeting, writes group coordinator Jackie Hamilton.

After we all introduced ourselves, Beverly opened the meeting with an overview of the incredibly motivating FHT Conference we attended last November. Two GPs who run large social prescribing surgeries – one in Devon and one in Bromley by Bow, London – were speakers there. These are very different areas, but both serve the local community effectively.

We talked about joining our own GPs’ patient participation group, which is a requirement for each practice. If we can get into these groups, we have more chance to influence those who make the decisions.

One of the things that came out of the FHT Conference is that it is winning hearts that is important. This came from Sir Sam Everington, the GP who runs the Bromley by Bow Centre. We need research and statistics, but we also need to get to know the people first. They won’t be won over with paperwork alone.

Our 5 December post to that group gave an overview and a link to an FHT blog post about the conference.

We also talked about using MYCAW (Measure Yourself Concerns and Wellbeing), a questionnaire that can be used by therapists to provide an evidence base for complementary therapy treatments.

MYCAW is easy to implement, is a useful way to keep track of changes, and something to show other than your notes.

We were so lucky to have the 2018 FHT Complementary Therapist of the Year, Jane Sheehan, come to give a truly inspiring talk on working in a hospice and making a difference.

She managed to turn around the way therapists were treated regarding pay as well as their inclusion in the medical meetings, so they could have input and receive referrals.

Jane read out encouraging messages from former patients. Having a simple back and shoulder massage gave one lady the strength to organise her life to make it easier for others to cope with things after she is gone. She hadn’t wanted to leave behind her young children and not see her grandchildren, but the treatments helped her cope and feel more positive.

Another patient was so overwhelmed with the care received from therapies that she bought a brilliant treatment couch for the hospice. She was too ill to have a treatment by the time it arrived but saw her mum enjoying it.

Jane had many experiences to relate, but the thing that stood out was that she kept on trying. The medical establishment within the hospice took a lot of convincing.

Jane was refused a job there initially, and received many knock-backs, but with perseverance she was able to finally make a difference and win over hearts and minds.

Well done, Jane!

Successful start for the Horsham and Crawley group On a very wet and windy Monday night, I made my way to Barns Green near Horsham for Gemma Underwood’s launch of the Horsham and Crawley LSG, writes 2018 FHT LSG Coordinator of the Year, Alison Brown.

I run the Worthing LSG and was happy to offer Gemma support at the first meeting of this new group.

The venue was in the back of beyond, but a lovely large village hall was brightly lit

up to greet the therapists who joined us.

It was a lovely evening and the meeting was well attended. We started off with each of us saying who we are, what we do, what we love about our business and what we dislike.

The consensus seemed to be that we loved helping people and therapies, but we disliked book-keeping, accounts and marketing.

Gemma then went on to talk about and share her

vision board and how it helps her with her planning, dreams and goals for her personal life and business.

A few of us had brought magazines and spent a lovely time over refreshments cutting and sticking our images onto paper to create our own board, all the while chatting and making friends.

A lovely evening was enjoyed by all with fabulous feedback, contacts made and all looking forward to more meetings in 2019.

Manchester North coordinator promotes LSGs in new video Manchester North LSG coordinator Martin Thirlwell (pictured, right) has recently created an informative video to highlight the benefits of the FHT and being part of an LSG. The video gives us an interesting glimpse of a recent meeting, where group members attended a workshop on first aid, learning potentially life-saving skills.

Medical A-Z NAUSEA Nausea is the sensation of feeling sick or needing to vomit (NHS, 2019; Peters, 2008).

CAUSES There are many different causes of nausea. Other symptoms experienced alongside nausea can often indicate the root cause. Someone with diarrhoea or vomiting, for example, may have a virus or food poisoning; a high temperature and headache may suggest an infection; heartburn or bloating after eating could relate to acid reflux; sensitivity to light or sound with a headache may imply a migraine; and dizziness could be due to vertigo or labyrinthitis.

Other causes of nausea include anxiety, alcohol, medicines, motion sickness, pregnancy or recent surgery.

ORTHODOX TREATMENT Treatments can vary depending on the cause. However, the NHS says the following suggestions may help eliminate nausea: • Getting plenty of fresh air • Sipping a cold drink • Distracting yourself – for example with a film or music • Eating foods containing ginger, such as ginger biscuits or drinking ginger tea • Drinking peppermint tea • Eating smaller meals, more often.

The NHS also recommends avoiding certain foods when nauseous, including hot, fried, greasy or strong-smelling food and large drinks with meals. Those experiencing nausea should also avoid eating too quickly, lying down immediately after eating and wearing clothes that are tight around the waist or tummy.

COMPLEMENTARY THERAPY Aromatherapy could be used to help manage postoperative nausea and vomiting, according to a new study published in Complementary Therapies

in Medicine (Karaman et al, 2019). A randomised controlled trial assigned 184 patients to four groups to receive aromatherapy with lavender essential oil, rose essential oil, ginger essential oil or pure water (placebo). Following surgery, the patients were given a pad with their assigned scent to inhale for five minutes and were asked to rate how nauseous they felt after 15 and 40 minutes. After 15 minutes the postoperative nausea scores improved in 83% of the lavender group, 65% of the ginger group, 48% of the rose group and 44% of the placebo group. This indicates that lavender and ginger essential oils could be particularly effective in helping to reduce nausea after surgery.

The efficacy of ginger is also supported by a systematic review involving 918 patients who experienced postoperative nausea and vomiting (Tóth et al, 2018). Using the visual analogue scale to assess outcomes, 10 trials found that pharmacological doses of ginger taken orally could have a significant effect on the severity of postoperative nausea and vomiting, reducing its incidence, as well as the demand for antiemetic medication.

Postoperative nausea and vomiting may also be reduced with acupuncture and acupressure treatments, nine systematic reviews suggest (Lederer et al, 2018). One acupressure study compared 47 women who had treatments with 50 who had medication 12 hours after gynaecologic surgery (Ünülü et al, 2018). The acupressure was effective in preventing vomiting, reducing nausea intensity and enhancing patient comfort, comparing favourably with the control group.

In another trial, acupuncture used as a complementary therapy performed better than standard care alone in reducing nausea, pain and anxiety in 30 women who

had surgery for breast cancer (QuinlanWoodward et al, 2016).

Nausea caused by chemotherapy could be alleviated by complementary therapies, according to a recent trial (Zorba et al, 2018). Seventy-five patients with breast cancer were randomly assigned to three groups to receive a 20-minute aromatherapy foot massage, three-minute inhalation of an essential oil before three chemotherapy cycles or standard treatment alone (control). Feelings of nausea were significantly more prevalent in the control group than both other groups, with the best treatment outcomes recorded in the aromatherapy foot massage group.

Aquatic exercise aids mobility in people with haemophilia HAEMOPHILIA AFFECTS THE BLOOD’S ABILITY TO CLOT, CAUSING INTERNAL BLEEDING THAT CAN CAUSE PAIN AND STIFFNESS AROUND THE JOINTS (NHS, 2019) Aquatic exercise could increase joint range of movement in people with haemophilia, according to a review published in Complementary Therapies in Clinical Practice (Neelapala et al, 2019).

Researchers at the Manipal Academy of Higher Education conducted a review of literature, identifying four studies involving a total of 79 participants from leading scientific databases. These examined the effects of water-based exercise on patients with haemophilia, comparing outcomes with land-based exercise and no treatment.

Massage and reflexology reduce pain in bladder cancer patients

A RADICAL CYSTECTOMY INVOLVES REMOVING A CANCEROUS BLADDER AND THE NEARBY LYMPH NODES (CANCER RESEARCH UK, 2019) Patients who have had a cystectomy following bladder cancer may benefit from massage and reflexology treatments to reduce pain and improve wellbeing, according to a pilot service evaluation (Silverdale et al, 2019).

This pilot service provided 38 cystectomy patients with one hour of massage, reflexology or a combination of both the day after they had surgery. This was then followed by another one-hour treatment two days later.

Self-reported concerns, wellbeing and pain were measured before and immediately after each treatment and in the evening of each therapy day, using Measure Yourself Concerns and Wellbeing (MYCAW) and the Pain Thermometer (PT). Common concerns among patients included pain at the wound site, anxiety, nausea and bloating.

Self-reported concerns and wellbeing (via MYCAW) and pain (via PT) were significantly reduced after treatments. These results were maintained on the evening of day one but not on day three.

There were no significant differences between the outcomes of reflexology, massage or a combination of the two.

elbow, knee and ankle; knee strength; and oxygen uptake.

Eight weeks of aquatic exercise significantly improved range of motion in the elbow, knee and ankle and increased knee strength when compared with no treatment. Furthermore, it also led to greater outcomes on pain than land-based exercise after four weeks, and increased oxygen uptake by more than 50% following three weeks of treatment.

Researchers invite more trials to further validate the effectiveness of treatments.

Inhaling lavender may improve quality of life in postmenopausal women MENOPAUSE IS A TIME OF GREAT TRANSITION IN A WOMAN’S LIFE, LEADING TO EMOTIONAL AND PHYSICAL CHALLENGES THAT CAN SIGNIFICANTLY AFFECT QUALITY OF LIFE (PAULOSE ET AL, 2018) Aromatherapy using lavender essential oil could improve the quality of life (QOL) of women who have been through the menopause, a study suggests (Bakhtiari et al, 2019).

Researchers recruited 62 postmenopausal women who were referred to health centres in Isfahan, Iran, to take part in a randomised controlled trial, comparing aromatherapy (intervention) with a placebo (control). Women in the intervention group inhaled 2% lavender essential oil for 20 minutes before bedtime each day for one month, while the control group did the same with distilled water.

Data was collected using menopause-specific QOL and demographic characteristics questionnaires, and QOL measured across vasomotor, psychosocial, ial, physical, sexual and overall QOL. Prior to the study, there were ere no significant differences between en the two groups. However, by the end of the study the intervention group showed much greater improvements in QOL, reporting rting reductions in the severity of physical and d psychological symptoms.

A DAY IN THE LIFE OF...

A DAY IN THE LIFE OF… BRIAN PAUL JAUNCEY, BSc MFHT, COMPLEMENTARY THERAPIST, PGCE STUDENT AND 2018 FHT STUDENT OF THE YEAR with today’s focus on elitism, funding and financial barriers within education.

12.30PM Break for lunch. Time for a healthy bite to eat and to catch up with fellow students and friends to discuss the morning’s lectures. This break also gives me time to visit the university library where I can gather and evaluate relevant and current research for the afternoon sessions.

1.30PM Afternoon sessions are usually based around presentations and debate, depending on the topic. My course has many people enrolled, with a wide variety of subject backgrounds.

I’m the only complementary therapist, which leads to some interesting views and debates. These sessions are very studentled, expanding on the theories or concepts from the morning session and debating how these may influence, impact on and shape our classroom dynamics or teaching approaches within our own educational setting. I gain my actual teaching hours by delivering sessions within the Glyndŵr University clinic.

most of her issues arise from posture and office ergonomics. As a medical secretary and audio typist within a local hospital, my client spends many hours in the same position at her computer. Shoulder pain for typists or computer workers is characteristically caused by muscles and tendons being held tensed in one posture for long periods which can result in muscle/ tendon fatigue, imbalance and pain.

We decide to apply a range of techniques including some realignment, muscle energy techniques/resistance stretching, and deep tissue massage to the affected areas. As I live with a chronic back condition myself, I have to be aware of my own body mechanics and posture throughout a treatment. I tend to use a lot of hands-free techniques and employ martial arts stances where possible, which helps me to manage my own condition and not place undue stress or fatigue in my lumbar area.

Following the treatment, we have an in-depth discussion about home exercise and rearranging of her workspace to help lessen the muscle fatigue, and devise an ongoing treatment plan.

6.30AM Alarms go off for the whole 3.30PM The university day is over: house, promptly followed by the obligatory fight for the bathroom. While I wait my turn, I usually make coffee, prepare my mid-morning smoothie, run through the scheme of work and do some prep for the day’s lectures.

The morning commute.

As my wife and daughter both work nine to five in the vicinity of my university, I tend to brave the traffic and drop them off on my way in.

9.30AM

I arrive at Glyndŵr University with enough time to grab a coffee and a snack, then make my way to the lecture theatre ready for a day of PGCE study, including debate, group work and projects. At the moment we are studying contemporary issues within education,

I head home for a shower, check my client list for this evening’s clinic, and prepare my kit accordingly.

4.30PM I head off to the clinic.

I like to arrive at least an hour before my first client to prepare the space, make sure it is warm enough and grab a quick herbal tea. Because of my university schedule, my clinics are predominately run between 5pm and 9pm. I run Heka Therapies from within an established physiotherapy and acupuncture clinic owned by former FHT President Paul Battersby, whose support and mentoring has been fantastic.

My client arrives with upper back and shoulder pain that is affecting her sleep and work. After a full consultation and postural assessment, it would seem that

Beyond trauma DR MARC JOHNSON TALKS ABOUT HIS CAREER PATH FROM A MILITARY MEDICAL ADVISER TO A THERAPIST SUPPORTING SURVIVORS OF TERRORISM

A

s a shy youngster, I joined the Air Training Corps at 13, and I quickly advanced after finding I could switch off my shyness while teaching others the skills I had learned. By 14, I was given an instructional role and found a love of teaching first aid skills to new recruits.

Three years later, I began work for the army medical services, both as a civilian and combat medical technician in the Royal Army Medical Corps. I was later transferred to another branch of the military, although retained my honorary title of ‘Doc’ among my peers. In 1997, I was made redundant from the Ministry of Defence and wondered what I would do next with my skills.

I began working as an instructor, teaching everything from basic CPR to advanced cardiac life support, and started my own business providing not only courses but medical staff for large events and tours.

This blossomed rapidly, and beyond expectations, into providing medical support for international A-list musicians and celebrities. Along the way somewhere,

I had also picked up something just as important – a strong sense of compassion that military medicine never really embraced. I found that language skills made a massive difference to patient outcomes and had always relished using language correctly and intelligently now it was paying dividends. But another turn in my professional career was ahead of me.

IRAQ INSURGENCY By 2004, my work had become monotonous. Working daily with divas and vast numbers of intoxicated members of the public who seemed to target anyone resembling medical staff, I took an opportunity to work for the US military as a civilian medical adviser in Iraq. In 2004, the insurgency in Iraq was raging, and incredibly dangerous. There

was an urgent requirement for medical staff, especially those with teaching, management and liaison skills.

After a short predeployment course in the US, I flew to Iraq to become the regional medical adviser in Mosul, Iraq’s largest reconstruction project.

I was dealing with combat trauma almost daily as I volunteered at a combat support hospital, sometimes seeing mass casualty events with more than 50 casualties arriving at once. My language skills were used to reduce anxiety and stress in casualties, and I passed on some of these skills to my fellow US military medics. I became fascinated by the strength of character and resilience in those with horrible trauma, and this was to come in useful later in my career.

It was in Mosul that I was critically injured in a terrorist attack. Despite being potentially fatally injured, I survived. After

three days in an intensive care unit and 18 days in a US Air Force hospital in Germany, I was flown back to the UK to begin my recovery. Because of the severe nature of my physical and mental injuries, I would never work in frontline emergency medical care again. Despite a brain injury from several pieces of shrapnel passing through my skull, many serious physical injuries and two subsequent strokes, I still loved what I did, and I was determined to continue to offer help to those who needed it.

ROAD TO RECOVERY

My recovery was not an easy one. I was left to my own devices and, having fallen between the cracks of primary and secondary care, I drifted into post-traumatic stress disorder (PTSD). I didn’t realise the severity of the condition, and it wasn’t until 2014 that my GP referred me to Veterans NHS Wales. Here, I received psychotherapy and eye movement desensitisation and reprocessing therapy, which reduced the severity and frequency of symptoms enough for me to function far better mentally and emotionally.

Before that, from 2009 onwards, I used my recovery time to study, initially with the Open University in the peace and quiet of home, and then at a traditional university setting, where I learned more about psychology and studied traditional Chinese medicine. My passion for learning had returned. I studied a university programme at the same time as completing four additional diplomas, including a practitioner diploma in hypnotherapy and a diploma in clinical acupuncture to supplement the traditional acupuncture I was also learning.

I had discovered hypnotherapy purely by accident while researching psychology and was surprised to find a hypnotherapy school nearby in Wrexham. It was so thrilling to see rapid change happening in the volunteers my mentor arranged for us, just through the use of language, and I wondered now if perhaps I had unknowingly been using hypnotherapy for years with the injured and sick. This was as much of a buzz as frontline primary care – and no one was shooting at me.

A NEW PATH In late 2014 I was doing much better and, not being one to hesitate too much, I rented a room to practise my therapies. My superbly supportive wife helped me set up a therapy room that was warm, inviting and suitable

BENEFIT TO SOCIETY I give regular talks to groups about how complementary therapies can benefit society, and to local university therapy students about integrative healthcare, how modern medicine can benefit from complementary therapies, and how the NHS is gradually changing to adapt and accept a new era, including ‘social prescription’, as discussed by Sir Sam Everington and Dr Michael Dixon at the 2018 FHT Conference.

I believe that, as therapists, our reach into the community is just as important as getting clients through our doors. We benefit everyone by going out there and talking about what we do, whether to small groups or at huge events. Effective communication continues to be key for me, and goes back to my belief that the language we use can change outcomes positively.

Keep up the positive dialogue, including the way you talk to yourself, and you will succeed in your goals to help others.

I began with a Facebook page and a website and, with money scarce to begin with, taught myself the basics. The phone started ringing within a week, much to my surprise. I had done something right, clearly, and word of mouth spread within the community. I soon contacted local newspapers and got a full-page spread regarding a national sleep campaign I was keen to help with.

A local journalist began calling and emailing me for more information on a variety of subjects he knew I could help with.

My PTSD was still lingering, but instead of allowing this to become a limitation I used my insight to help others. It became one of four clinical areas that I took a special interest in, the others being clinical weight management, irritable bowel syndrome and pain management. I really wanted to work more with people post-trauma and in the years following two major conflicts, and looking at society in general, there was clearly a requirement for this. Events in 2017 made this requirement abundantly clear.

AFTER MANCHESTER The May 2017 suicide bombing at the Manchester Arena, which killed 22 innocent victims, and physically injured at least 139, many of them children, brought together all modalities of therapists, first across the UK, and then globally. Fellow hypnotherapist Karen Riley approached a small number of therapists via Facebook to administer a new network that would offer free support to victims of major incidents who were suffering with mental and emotional aftereffects. The response from the therapist community was overwhelming, and the group remit was expanded to help those involved in major incidents internationally.

This group is now known as the Post Major Incident Therapist Network and can be found on Facebook.

My journalist contact heard of my involvement in this network after reading reviews and comments from those who had sought my help following the Manchester attack and offered to get the word out that help was available. Soon enquiries were

coming in via phone and email regarding survivors, mainly children and young people who desperately needed help but had been placed in long queues by struggling NHS mental health services. I offered free support to these survivors on an ongoing basis, for as long as required.

As a result of the press coverage, survivors of other major incidents discovered the clinic and made contact. A survivor of the Las Vegas mass shooting commenced therapy, and word continued to spread. The strength of these people inspired me, and we continue to work together on a variety of psychological issues and physical symptoms that have arisen since their trauma.

I use a variety of therapies to reduce symptoms, and each session is tailored to the very individual needs of the client, their presenting symptomology and their own preferences for therapy. Sometimes I use single modalities and at other times a mix of therapies, including hypnotherapy and auricular acupuncture, combined in a single session.

Psychological therapy is no longer limited to the therapy room. The internet has made therapy accessible to many more people who may not have had access to it before.

Enquiries are now received from around the world, with some signposted by the Post Major Incident Therapist Network for more local assistance. I firmly believe that accessible mental healthcare is difficult to achieve, but is a fundamental human right, and together we can all help it happen, if we each play a small part.

A website is now an essential part of any therapy practice yet many therapists describe themselves as ‘technophobic’ which can lead to their website becoming outdated, unsecured and potentially at odds with current legislation. Another big consequence of the mobile internet was that it facilitated online booking systems. Big hotels have had online booking for years but it was impractical for, say, the owner of a small holiday cottage to synchronise their paper diary with online bookings until smartphones replaced the diary and everything was in one place. Of course we all value up to date availability when booking a holiday cottage and expectations are now shifting towards this being expected. Restaurants followed suit and we are just now getting to the point where it is commonplace for people to use online booking which is why it will soon become the norm for therapy websites.

Internet adoption by therapists is similar in many regards to that by any other sector though perhaps a little slower than more progressive sectors such as online shopping. Key events such as the rise of the mobile internet trigger new waves of change for all of us. Some responded quickly yet until it became the norm to search for a therapist using a smartphone it made more sense to wait until mobile friendly technologies became mainstream (and cheaper) rather than rush in and incur a lot of expense. Current levels of mobile internet usage mean that “responsive” mobile friendly design is now essential for your therapy practice website.

TELL US A BIT ABOUT YOUR BACKGROUND…

Originally from the North East, my family moved to Leeds just in time for me to start school. From there we moved around a lot because of my dad’s job, which is quite difficult when you’re young. But on reflection, it probably served me well and contributed to my much-needed communication and life skills.

From an early age, I’d shown an interest in beauty therapy and hairdressing, and during the early 1980s my local college offered newly combined hair and beauty therapy programmes, which was a dream come true.

After qualifying, I worked in various salons before opening my own in 1993, offering both hair and beauty treatments.

In 1995 I discovered reiki and have studied many other complementary therapies since. Meanwhile, I had

also started teaching and went on to teach a wide range of therapies in further and higher education, including reflexology to students in Cyprus.

WHAT MAKES A GOOD THERAPIST?

Good communication skills and a personable, caring approach is of the utmost importance. The ability to listen, understand, empathise and show compassion regarding the health and wellbeing of each client is essential, as well as retaining integrity, respect and a professional attitude at all times.

WHAT ATTRACTED YOU TO THE ROLE OF FHT VICE PRESIDENT?

During my years as both a therapist and therapy tutor, the broad range of services FHT membership offered, including the all-encompassing advantages of insurance, CPD courses and regular magazines, has been second to none. I didn’t waver when I was given the opportunity to join the Governing Council.

WHAT KEY CHANGE WOULD YOU LIKE TO SEE IN THE INDUSTRY?

The integration of western medicine and holistic therapies is vital and has seen a significant increase throughout recent years, especially with the care of

cancer patients. However, I’d like to see therapy treatments more accessible in general for other illnesses, such as mental health problems.

TELL US A BIT ABOUT YOUR INTEREST IN FENG SHUI AND YOUR VISIT TO THE YUN LIN TEMPLE…

Feng shui, known as ‘therapy for the environment’, is a passion of mine. Last year I was fortunate enough to visit the primary Yun Lin Temple in Berkeley, California, where its founder, the late Grandmaster Lin Yun Rinpoche, brought the ‘Black Hat Sect’ philosophy from Asia to the western world.

My ultimate aspiration is to complete its two-year certified programme in feng shui.

DO YOU USE THERAPIES TO IMPROVE YOUR OWN HEALTH AND WELLBEING?

Holistic therapies are my favourite form of relaxation. Meditation can be invaluable for clearing the mind and provoking positive thoughts. I self-treat (which is also beneficial and important) using reflexology techniques and channelling reiki energy; how can we, as therapists, relax our clients if we are suffering from stress or anxiety ourselves? Visiting a spa is my absolute guilty pleasure – in addition to having treatments, I love long walks, swimming, hydrotherapy and exercise classes. Overall, keeping fit and healthy is very important to me.

TELL US A BIT ABOUT JUDITH…

I share a love of travelling with my husband Gary and have been extremely lucky to visit some remarkable and extraordinary places, including attempting the Inca trail to Machu Picchu, Peru (I was so disappointed to experience altitude sickness!); the sulphur springs in Rotorua, New Zealand; and the overwhelming Viet Cong tunnel system and beautiful Halong Bay in Vietnam.

WHAT IS THE BEST ADVICE YOU HAVE BEEN GIVEN?

Thoughts are very powerful, and by trying to remain optimistic and think positively, you will gain much more from life.
