In September this year, IBMS Congress will once again take over Birmingham’s International Convention Centre and provide 2023’s biggest and best biomedical science event. I am certainly looking forward to attending the exhibition hall, seeing the stalls and talking to the representatives from our diagnostics industries – all of whom you will find in these pages.

The Congress Exhibition

is a great opportunity for us to combine business and networking, and enable the different branches of the biomedical science community to come together – to view new equipment and products, to build a greater awareness of techniques and technologies, and make contact with a broad range of exhibiting companies.

The IBMS is committed to building bridges that lead to the best products and services and, through that, improved patient care. We are exploring how to help the system with

the new IVD-R regulations, have taken part in consultations for the Coronavirus Test Device Approvals and continue to have conversations with the Medicines and Healthcare products Regulatory Agency, to ensure our members’ needs are taken into consideration.

We have proactive and positive relationships with the British In Vitro Diagnostic Association and the Association of British HealthTech Industries – and we try to ensure that our presence and input helps both our members and our Company Members to thrive.

The UK spends £16 per person on in vitro diagnostics, compared with an average of £20 among countries in the European Union. This equates to 1% of total health expenditure – even though the results of in vitro diagnostic tests influence many clinical decisions.

Diagnostic activity forms part of more than 85% of clinical pathways. The same diagnostic technologies and workforce are used in national screening programmes, but do not count as diagnostic activity as they are used to screen asymptomatic patients.

Each year, the NHS undertakes more than 1 billion diagnostic tests. This accounts for about 6% of the NHS budget.

Demand is rising across almost all aspects of diagnostics, with 4–7% increases in activity seen for tests such as colonoscopies and magnetic resonance imaging (MRI) scans each year between 2014/15 and 2018/19.

INTRODUCING THE COMPANY MEMBERS

In 1992, the IBMS launched the Company Membership scheme and the Company Members Liaison Group, which comprised Company Members and Institute Officers. The latter held its first meeting in 1992, which provided an excellent forum to discuss issues at the Liverpool Triennial and work with exhibitors to transform the event.

Fast-forward to the present and the scheme connects a members’ network of over 20,000 biomedical staff worldwide to many of the leading organisations in biomedical science, covering disciplines across the sector.

Delivering success

One of the Institute’s central aims for the scheme is to ensure that members are up to date with the latest developments in technology and advances in scientific techniques.

Working with the Company Members also enables the delivery of the highly successful annual Congress, while the companies also support local IBMS regional and branch scientific events and recognise members’ achievements through the award of the Company Members Prize, which is presented to the IBMS member who achieves the highest mark in each specialist

Who are the Company Members and what is the aim of the scheme? We give an outline of their role and responsibilities.

DIAGNOSTIC TRENDS

The pandemic has thrust biomedical science into the limelight and increased public and political awareness of the role that diagnostics play in disease detection, prevention and management.

At the end of last year, The King’s Fund – an independent think tank that focuses on health and social care policy – published a report Why do diagnostics matter? Maximising the potential of diagnostic services.

It looks at the increasing

pressures on the sector. For example, demand is rising across almost all aspects of diagnostics, with 4–7% increases in activity seen for tests such as colonoscopies and MRI scans each year between 2014/15 and 2018/19, it states.

As well as exploring the role that diagnostics play in underpinning activity in the health and care system, the report also looks to the future and where attention is needed to ensure diagnostic capacity and capability survives and thrives.

Here we will look at some of the key ideas, concepts and innovations that are put forward in the report.

Point-of-care testing

As the report states, simple point-of-care testing (POCT) diagnostics have been around for a long time. However, advances in technology over recent years have led to an increase in the number of diagnostic tests and the potential for the increased prominence in diagnostics. This is

demonstrated by the fact that IBMS Congress two years ago introduced a POCT stream and has also launched an IBMS Certificate of Expert Practice in Point of Care Testing.

The report goes on to state that there is now the potential for more accurate and timely diagnosis of diabetes, cardiovascular disease, various cancers and other high-prevalence conditions, such as urinary tract infections. An example of this is the D-dimer blood test, which is used to assess possible deep vein thrombosis and pulmonary embolism.

In the pandemic, the drive to keep people out of hospitals to avoid the spread of infections led to a number of innovations. One that the report flags up is a drivethrough phlebotomy service launched in Sheffield. Virtual wards

Virtual wards allow patients to get the care they need at home, rather than in hospital. The NHS is increasingly introducing virtual wards to support people at home (including care homes). Patients are supported through the remote monitoring of physiological variables with the use of technology platforms and diagnostic medical devices,

such as pulse oximeters. The report states: “Virtual wards are currently being used to support patients with COVID-19, acute respiratory infections, urinary tract infections and chronic obstructive pulmonary disease, but the ambition expressed in the 2022/23 operational planning guidance is for all integrated care systems to have 40 to 50 virtual beds per 100,000 population by the end of 2023.”

Diagnostic centres

The roll-out of new community diagnostic centres (CDCs) across England is very welcome, says the report. However, government restrictions on the building of new facilities and the so-far limited range of tests on offer at these centres risk limiting

their potential to increase diagnostic capacity.

The idea was first raised back in 2021, when the government announced it planned to establish a series of testing sites that would be more convenient for patients. “Millions of patients will benefit from earlier diagnostic tests closer to home, thanks to 40 new community diagnostic centres set to open across England in a range of settings from local shopping centres to football stadiums,” the government announced. “The new one-stop shops for checks, scans and tests will be backed by a £350m investment from government to provide around 2.8 million scans in the first full year of operation.”

While the reality of CDCs is currently far short of what was planned, they do have the potential to help clear the COVID-19 backlogs.

In April 2022, the government announced that “73 centres [were] already open and carrying out 30,000 extra tests a week” and the aim is to have 160 centres across England by 2025. BMS

A GUIDE TO ISO 15189:2022

Here is your quick guide to the updated standard ISO 15189. It covers implementation and transition for the new standard and practical advice for laboratories.

What is ISO 15189?

It is the accreditation that underpins confidence in the quality of medical laboratories through a process that verifies their integrity, impartiality and competence. Assessments under UKAS accreditation ensure labs meet the relevant requirements including the operation of a quality management system and the ability to demonstrate that specific activities are performed within the

What disciplines does it cover?

Clinical biochemistry

– toxicology – endocrinology

Haematology – blood

transfusion

Microbiology – virology

– parasitology – serology

Immunology

Genetics

Andrology

Histocompatibility and immunogenetics.

Why was the standard being revised?

There were two key reasons for the review. Firstly, because the parent standard – ISO/IEC 17025:2017 – had been updated to comply with the structure of the normative ISO 9001:2015. Secondly, as ISO/IEC 17025:2017 is a normative reference of ISO 15189, there was a requirement for the documents to be aligned. Also, ISO 15189:2012 had reached its periodic review date. A vote was taken by the international community, and it was agreed that the standard required review and an update to reflect the changing demands of the medical laboratory community.

What is the revised standard like?

The standard is written as

a minimum requirement and not a maximum. There is often a misconception that if the standard does not specifically require or reference a process/ activity, then it cannot be offered by an accredited organisation. This is not the case. If there is an opportunity to improve the quality of the service offered to patients and other users, then this inherently meets the requirements of the standard (old and new), so long as adequate justification and documentation is available. A decision was made very early in the revision that the updated version needed to be patient-focused and risk-based

as this is a clinical standard for a service that is key to the patient pathway. There was an understanding that, as effective as the current version is, there is a high degree of prescription and over-reliance on interpretation of the standard in terms of perceived accreditation requirements.

Are there any useful reference standards?

Yes, there are several quoted in the updated ISO 15189, none of which are compulsory, but all of which add value to any quality system. These include: ISO 15190:2020: Medical laboratories – Requirements for safety

GUIDANCE TO HELP WITH IMPLEMENTATION

The IBMS, Royal College of Pathologists (RCPath) and Association for Clinical Biochemistry & Laboratory Medicine (ACB), in collaboration with the United Kingdom Accreditation Service (UKAS), have produced an overview of the key changes included in the updated ISO 

The overview is designed to guide healthcare professionals when implementing the new requirements in their service.

The IBMS has been involved with revising the standard from the outset and is hugely supportive of the newly published document.

IBMS President Debra Padgett said: “Whilst the majority of the standard remains unchanged and the terminology is consistent with that currently described in local quality management systems, I welcome the focus on better understanding the risks of the services we provide to the patient, mitigating those risks and ensuring that we put the patient at the heart of everything we do.

“The IBMS looks forward to supporting members during the transition period, working collaboratively with UKAS, RCPath, ACB and colleagues in other professional bodies.”

What about risk management?

During an accreditation assessment, we anticipate that there will be significant discussion between the laboratory representatives and the UKAS assessment team regarding how the accredited organisation has considered their management of risk. It is important to acknowledge that every accredited service will have service-specific risks, and that there is no “one-size-fits-all” approach to risk management.

Does it cover point-of-care testing (POCT)?

Requirements for a POCT service are now incorporated into the body of the new ISO 15189 standard, to replace the current ISO 22870:2016, although there is a POCT-specific annex in the updated ISO 15189, to provide additional clarity and guidance. There is no longer a specific requirement for a multidisciplinary POCT team, however, this does not mean that an accredited organisation can’t have one. If an organisation continues with a

multidisciplinary POCT team, this group could be used, for example, to agree and monitor the service agreements.

How does it impact decision-making?

There is a greater emphasis on risk to the patient and clinical decision-making. For example, there is no longer a prescriptive requirement on how samples are labelled, however, these still need to be traceable to the patient. Also, the decision to process or not to process a potentially compromised sample should be linked to the risk to the patient before deciding on how to action the request. Consider the risk of not processing an inadequately labelled, potentially cancerous, skin biopsy versus not processing an inadequately labelled urine sample.

How has verification and validation changed?

The definitions remain

unchanged, and the requirement for verification data to show how the laboratory can demonstrate that a validated method is acceptable in their hands is still in place. However, the requirements for method validation and verification have been updated, to ensure a focus on the clinical question being asked and assurance that the methods are fit for that purpose.

What happens about implementation?

There is a transition period in which all organisations accredited to ISO 15189:2012 worldwide must transition to accreditation to the new standard. UKAS has been delivering training sessions in 2022 and it will continue to do so over the coming months. Get familiar

All four UK health services have united to commit to net zero carbon emissions. There are a range of strategies and targets across the countries. Make sure you are familiar with all the relevant documents.

Use resources

There are guides out there to help you when it comes to going green, such as the My Green Lab programme and LEAF, which are increasingly branching out into clinical lab sustainability.

Green suppliers

If you are able to shop around, then many equipment and consumable companies have been increasingly paying attention to sustainability. Ask about green credentials when collecting quotes.

Speak to your supplier

If you are already working with a supplier and you aren’t happy with their packaging, for example, as they use an excess of non-recyclable plastics – speak to them.

Speak to other labs

Many labs are trying to reduce their environmental impact and have dedicated Green Champions. They have already done the hard work, so speak to them and draw on their knowledge and expertise.

Fine tune your lab

Once you have all your equipment in place, make sure it is working for you in as sound and as sustainable a way as possible – defrost fridges, turn off equipment and lights when not in use and recycle and reuse.
