A Family of Companies


It began with a farm… In 1956, Ken and Nancy Tack purchased an old farm in Armstrong county that had fallen into disarray. The property and farmhouse, which were built in 1830, were restored to their original country charm… …and a dream was born.

Desiring to serve others, the Tack’s originally considered building a youth treatment center on the farm. That dream, though, was quickly replaced with the vision of building a nursing center for the elderly.

At that moment, no one could have imagined the impact that would come from that dream and desire to serve others… …on the family, on the community and on the lives of thousands in need of care.

This was the birth of what would eventually become Quality Life Services.


Sugar Creek Rest Sugar Creek Rest, the flagship home, was constructed on the 120-acre property of the old family farm in 1973. This 21-bed, intermediate nursing facility was truly a family affair – invested in, constructed by and operated by a number of Tack family members.

Four years later, a 60-bed skilled nursing addition was built, followed by the construction of an independent living community, Armstrong Retirement Village, one year later.

The campus continued to grow to meet the needs of the community and in 1995, sub-acute and specialized memory care “neighborhoods” were added.

With its tranquil country setting, Sugar Creek Rest was a healing destination for the mind, body and soul… …and the Tack family’s commitment to providing quality rehabilitation and long-term care was only just beginning.


Emlenton’s Laurel Manor As the Tack family developed their investment and interest in healthcare, the dream to serve others continued to grow as well. Over the next several years, the family built a personal care home in Venango county – Emlenton’s Laurel Manor. Operating for nearly 20 years, the home was eventually sold as the Tack family sharpened its focus on skilled nursing care.

The Tack family had also purchased a campground and headquarters of a religious organization and converted the property into the first private jail in Pennsylvania. In an effort to maintain their focus on helping others physically heal, the Tack family sold the renovated jail to a non-profit, charitable organization that currently uses the property to provide short and long-term housing to homeless veterans.

Generation of Leadership and Countryside The Tack family would see its next shift – in the exchange of ownership from the 1st generation to the 2nd – in the early 1990’s. With deep roots of service and success in skilled care, the family moved forward in faith and focus in growing the company. In 1992, the 2nd generation of Tack’s, brothers Ken and Phil, purchased Countryside Convalescent Home in Mercer county. After renovations, including the addition of a state-of-the-art sewage treatment plant, Countryside now provides skilled nursing and personal care to both the young and mature in need of rehabilitation and long term care. Countryside is also proud to offer a specialized memory care neighborhood that features a unique town square atmosphere and café dining, providing those residents with a comfortable reminder of a familiar life.


Chicora Medical Center In 1999, the Tack family added Chicora Medical Center to its portfolio of homes. Located in the heart of Chicora, this campus provides both skilled nursing care and personal care services. Like all of our homes, residents of Chicora Medical Center are treated to a home-like environment that not only provides for their medical needs but also seeks to engage residents in lifeenriching activities, allowing for continued sense of purpose during their healing or long-term residency with us. The Tack family has always infused its beliefs in providing for the whole person into its homes and programs. This person-centered approach provides for successes beyond the measurable clinical outcomes that so many healthcare companies choose to solely focus on. Success in our homes is also measured in treasured memories created, in relationships developed between residents and staff and in meeting the social and spiritual needs of our residents.


Trinity Living Center Realizing the need for quality services in the Mercer county region, the Tack brothers would purchase another property in Mercer county, in the college town of Grove City. The former Grove City Hospital had been converted to a nursing home and in 2001, Trinity Living Center became the latest member of the Quality Life Services Family of Companies.

One year later, the home underwent a major renovation which included the addition of a 20-bed personal care neighborhood and a state-of-the-art, 48-bed specialized memory care neighborhood. In 2003, independent living apartments were added to the offering of services and Trinity Living Center claimed its mark as the first and only Continuing Care Retirement Community in Grove City.

Generation of Leadership

In 2007, the Tack family and Quality Life Services would see its most transforming shift yet – the exchange of ownership from the 2nd generation to the 3rd. The dynamic energy of a generation that was born into a family of service and commitment to healthcare would drive the company forward with fever and confidence. Steve Tack (Chief Executive Officer,) Susan Tack-Beardsley (Chief Administrative Officer) and Mary Susan Tack-Yurek (Chief Quality Officer) are the current 3rd generation owners of Quality Life Services. Under their leadership, Quality Life Services would acquire another 5 homes over the following 7 years.


Quality Pharmacy Also in 2006, Quality Life Services made the move to open its own retail and institutional pharmacy, located on the Chicora Medical Center campus. Quality Pharmacy serves the general public, a number of other local care homes and provides for the pharmaceutical needs of all of the Quality Life Services homes and residents.


Fair Winds Manor and South Fayette Nursing Center Under the new leadership team, Quality Life Services would purchase Fair Winds Manor in Butler County and South Fayette Nursing Center in Fayette County in 2007. Fair Winds Manor, with its rural location, overlooking a sprawling pond and country setting, is reminiscent of the Sugar Creek Rest flagship property. Offering both skilled nursing care and personal care services, Fair Winds Manor combines the old time charm of a rich, antique-decorated dÊcor with progressive therapies and medical care that is revered by local physicians and hospitals. South Fayette Nursing Center, located in Markleysburg, PA, has consistently been named one of America’s Top Nursing Homes. Because of its exceptional reputation for providing quality care to a specialized market, South Fayette receives referrals from hospitals and medical services throughout Pennsylvania.


West Haven Manor and Henry Clay Villa The company continued to grow in 2010 with the acquisitions of West Haven Manor and Henry Clay Villa. The largest home in the Quality Life Services’ portfolio is West Haven Manor, located in Westmoreland county – offering both skilled nursing and personal care services, including specialized memory care.

Henry Clay Villa is our other property located in Fayette county, in the heart of the beautiful Laurel Mountains. Like its sister home, South Fayette Nursing Center, this home specializes in skilled nursing and provides services for local residents and those from the neighboring states of West Virginia and Maryland.


Golden Hill Nursing & Rehab The ambition and vision of the new owners’ group moved the company to purchase another facility in 2014. Golden Hill Nursing and Rehab, located in Lawrence county, joined the Quality Life Services family as our largest, solely skilled nursing home with a capacity of 204 residents. Having operated as a skilled facility since 1965 under one owner, Golden Hill was purchased by Quality Life Services along with its solid reputation for exceptional care. It continues to be named “Best of the Best in New Castle� under the direction of Quality Life Services.


Culture of Family Throughout the years, the company has undergone shifts in perspective and progress but at the core of its operation has always been the value of family. This theme has been a focus of each generation to lead Quality Life Services. Begun with the intent to serve others and create an extension of family in a care environment, the theme continued and strengthened through the 2nd and 3rd generations.

Today, Quality Life Services prides itself on creating a family and home-like environment for both its residents and employees.
