Long-Term Disability (Employer Paid)

Accident Insurance

PRUMC provides all eligible employees with Long -Term Disability coverage through Unum. If you become disabled due to an accident or illness, the plan pays 60% of your monthly earnings up to a maximum amount of $5,000 per month. Benefits begin after 90 days of disability and are payable up to Social Security Normal Retirement Age.

You have the option to purchase Accident Insurance with Allstate. Benefits are paid directly to you based on a flat schedule when an accident occurs off the job.

Some examples of covered accidents include: fracture, dislocation, burn, concussion, coma, torn cartilage in knee, laceration, broken tooth, eye injury, ambulance, hospital admission, paralysis, dismemberment, etc. There is also a benefit for accidental death. Employee Assistance Program

Your Employee Assistance Program is offered through Unum. It is designed to help you lead a happier and more productive life at home and at work. You may call and talk to a Licensed Professional Counselor who can help you. You can also reach out to a specialist for help with balancing work and life issues. Online/phone support is unlimited, confidential, and 24/7. You may have up to 3 face-to-face visits at no additional cost to you.

Critical Illness Insurance

PRUMC provides all eligible employees with a $10,000 Critical Illness policy through Allstate. Employees have the option to buy-up to a $20,000 Critical Illness policy and pay for the difference in premium. The Critical Illness policy provides a financial cushion with a lump sum benefit if you are diagnosed with a critical illness or cancer. This benefit does not have any pre-existing conditions limitations, and there is a 30-day waiting period to use the benefits. Allstate pays a wellness reimbursement of $100 per covered member per year for providing proof of preventive screening tests rendered. The policy is portable. See Beacon portal for costs.
