Dance Your Way to a Fitter Life Local dance instructors Edgar Mendez and Amanda Gonzales recently opened Madison Dance Studio in Minglewood Plaza. The studio has a variety of dance and fitness classes offered throughout the week for kids, teens and adults. Edgar Mendez, owner of Salsa Memphis and Rumba Room, has been teaching salsa in Memphis for the last eight years. Amanda Gonzales, a native Memphian, is a Hip Hop and Salsa Instructor. They offer classes they feel are outside of the norm. “We wanted to give Memphis a place where everyone can feel comfortable learning something new and meeting people,” says Gonzales. Madison Dance Studio offers classes in Salsa, Hip Hop, Zumba, Belly Dance and Cardio Ballet. In February they will begin children and teen classes in Salsa and Mexican Folkloric Dance. The 2100 square foot studio, with hardwood floors and a modern loft feel, will also serve as a venue for parties and social dancing. HE A LT H Y L I V ING Ask Dr. Kishore Arcot “VARICOSE VEINS ARE MAKING EXERCISING AND EVEN WALKING PAINFUL. WHAT CAUSES THEM AND WHAT CAN BE DONE TO RELIEVE THE SWELLING?” Varicose veins are abnormal veins that can occur in the legs. They can be small thin purple-colored lines (called “spider veins”) that lie just below the surface, or they can appear as thick, bulging or knotted veins. Many people think varicose veins are simply a cosmetic issue, but actually it’s a medical condition affecting 15% of men and 25% of women in the United States.

WHAT ARE SYMPTOMS OF VARICOSE VEINS?

WHAT CAUSES VARICOSE VEINS? Veins bring the blood from the legs to the heart. Varicose veins result from problems in the veins. When you stand, one-way valves in your veins close to prevent your blood from rushing to your feet. Overtime, the blood backs up in the veins and pressure builds up. This increased pressure leads to a sensation of heaviness and causes the veins to enlarge.

HOW VARICOSE VEINS CAN BE TREATED? — Conservative therapy — Compression stockings — Anti-inflammatory medication (Motrin) — Venous Intervention — Laser Treatment (Endovenous Laser Treatment): a catheter is inserted and passed through the vein up to the leaking valve, usually found at the groin. The laser is activated and slowly pulled out of the vein, closing it down so it will not leak. The procedure lasts less than an hour and is relatively painless. Kishore K. EXPLORING THE TREATMENT AND COSTS OF LOW BACK PAIN Next to the common cold, low back pain (LBP) is the most common reason individuals visit a physician’s office, resulting in billions of dollars in medical expenditures and lost labor costs each year. Low back pain is one of the most common reasons patients visit the emergency room in the US. In 2008 US hospitals had roughly 3.4 million emergency department visits—an average of 9,400 a day— specifically for back problems.

ARE WE TREATING BACK PAIN EFFECTIVELY? Currently, the US ranks 46th among industrialized nations in health care outcomes, yet it ranks #1 in costs per capita. Medicare data illustrates that from 1996 to 2004 (risk adjusted), there was:

• 629% increase in expenditures for epidural steroid injections

• 423% increase in opioids prescribed

• 307% increase in spinal fusions

• I t is also noted that there is a direct correlation between MRI/CT rates and surgeries.

All of this increase in invasive treatment of musculoskeletal problems may be justified if patients’ quality of life and independence was preserved, however that isn’t always the case. In fact, the evidence paints a much different picture. Social Security disability statistics suggest that disability from musculoskeletal disorders is rising, not falling, from 20.6% to 25.4% in 2005. About 80% of health care and social costs related to LBP are attributed to the 10% of patients with chronic pain and disability. This implies that meaningful interventions to reduce the recurrence of LBP would allow significant cost savings in the current health care system.

Physical therapy has been shown to be an effective cost saver and has been shown to have good outcomes:

• I n a 2007 Annals of Internal Medicine systematic review of non-pharmacologic therapy, the authors demonstrate the benefit of manual physical therapy and exercise in both sub-acute and chronic LBP, as well as moderate evidence for the use of spinal manipulation in acute LBP.

• A review article published in the February 2009 Journal of the American Academy of Orthopedic Surgeons recommended that in most cases of symptomatic lumbar degenerative disc disease, a common cause of LBP, the most effective treatment is physical therapy combined with antiinflammatory medications.

RECENT ECONOMIC STUDIES HAVE SHOWN PHYSICAL THERAPY TO BE AN EFFECTIVE TREATMENT CHOICE FOR PATIENTS WHILE SAVING THEM SIGNIFICANT MONEY. The Virginia Mason Hospital system in Washington State was under significant pressure from local employers and health insurance companies to reduce the costs of its services. One of the hospital solutions was to use physical therapy for LBP treatment prior to specialty care. The outcome of this procedural change has just been published in Health Affairs (Sept. 2011). The results were significant.

• Costs were reduced per episode by 55%.

• There was an overall reduction in utilization—less medications, less physical therapy visits, and reduced number of physician visits.

• Patient satisfaction improved, indicating better outcomes from treatment. Health Services Research (Oct 2011) has just published a study that compared physician referral to physical therapy, with patient self-referral to physical therapy (direct access).

• The total number of physical therapy visits were reduced by 14% when patients accessed physical therapy directly.

• The physician referred group received significantly more concurrent care—more medications, more imaging, more interventions.

• There was a staggering 87% reduction in episodic costs when the patient came directly to physical therapy.

Physical therapy ranks among the top treatment choices of Consumer Reports readers with back pain, according to a 2009 survey of more than 14,000 Americans with back pain. Physical therapist treatment and other “hands-on” therapies outranked treatment by medical specialists and primary care providers, a result that should prompt more patients to seek conservative options as a first line of treatment for their back pain. At a time when people are responsible for paying a greater percentage of their health care services, it makes sense to reevaluate how we treat back pain to assure we maximize value. A conservative trial of 2-3 weeks of physical therapy may be your best and cheapest road to recovery. You can now come to physical therapy without seeing a physician first. You should find a physical therapy company where you see the licensed physical therapist at every visit, not just at the evaluation. HARDING ACADEMY STUDENT AND “FIT KID” UNDRA WILSON IS A LION ON THE FIELD

Maybe you’ve already seen 14-year-old Undra Wilson playing football on YouTube. The 8th grade running back and defensive safety has garnered a lot of attention during games for his skill and athleticism on the playing field— observer Lee Parker described him during a recent match against St. George as, “making moves like a rabbit, scrambling, spinning, juking, taking each turn as if it was his last, accelerating with the speed of a cheetah, and breaking tackles like a freight train while holding firmly to the football.” Undra has been playing sports since the age of three. His natural ability was nurtured by his mother Carolyn Crouthers and father Jeff Wilson, himself a former player in the semi-pro All-Army Basketball League. Says Jeff, “He really makes me proud as a father. Not only because he’s an exceptional athlete, but because he has awesome character and has never met a stranger. I truly admire his serious work ethic, which he applies to every aspect of his life.” We spoke to Undra to find out more.

WHAT INITIALLY SPARKED YOUR INTEREST IN SPORTS, AND WHAT HAS INSPIRED YOU TO CONTINUE AND EXCEL? Playing football as a little kid with my dad—catching and throwing to him got me started, and I would spend hours playing with my football and basketball. I just have a love for the game and I want to continue to excel and become the best person and player I can be. WHAT HAS YOUR EXPERIENCE AT HARDING ACADEMY BEEN LIKE? ARE YOU INVOLVED IN ANY EXTRACURRICULAR ACTIVITIES OUTSIDE OF FOOTBALL? I enrolled there in Pre-Kindergarten, so I’ve been with pretty much the same people since I was six. They’ve become some of my best and closest friends. I began playing football for the Lions in sixth grade, but I also run track and play basketball for Harding and the AAU Summer League. In 2010 and 2011 I was awarded the Presidential Award for Fitness. As far as academics go, my least favorite subject is math, but I love to read because it takes me to places that I haven’t traveled to—yet. HOW DO YOU BALANCE THE DEMANDS OF FOOTBALL WITH ACADEMICS AND TIME WITH YOUR FAMILY AND FRIENDS? It’s not easy. When I get out of school, I start on homework before basketball, football or track practice. After practice, I go to the Lifetime Fitness Center three times a week to

do a light weight workout. My dad and I practice my three-point jump shot and shooting the basketball off the dribble, along with basic free throw shooting. Then I come home and my mom takes over—she rides my back to make sure my homework is done correctly. I do 50 pushups, eat and take a bath. Then it’s bedtime. Also, my family is very competitive, so that lets us spend a lot of time together. On my dad’s side, we play ping pong, and everyone is good, including my Granddaddy. My uncle, Reggie Wilson, was recently named National Ping-Pong Champion in Hawaii, and he was in Memphis Magazine last November. We also run 100-yard dashes against each other. My grandfather fished competitively, and my dad played basketball overseas. He still beats me and my friends on the basketball team, but only because we let him. My mom’s family is into sports, too. She ran track and played basketball, and my cousin’s a star wide receiver for MUS. WHAT IS YOUR TRAINING REGIMEN LIKE? WHAT ABOUT DIET AND NUTRITION? My mom makes sure I have balanced meals. I’ve been eating them so long I don’t really like sweets or junk food, but I do love Gatorade. During the past summer, I did almost 500 push-ups a day to build my upper body strength. My dad has a 3-acre pond which I run laps around before I do my sprint drills. Then my dad and I practice playing ball outside on our basketball court. Before bed, I do 200-300 crunches. Then, once my dad thinks I’m asleep, I’m on the XBOX 360. SO YOU’RE A GAMER AS WELL. My mom doesn’t let me play any Playstation or Xbox 360 during the week. But on the weekend I go on XBOX 360 binges at my dad’s house! Right now I’m playing Madden 12 and NBA 2K12 on XBOX Live. DO YOU PLAN TO PLAY FOOTBALL AT THE COLLEGE LEVEL? WHAT ABOUT YOUR PROFESSIONAL ASPIRATIONS? I do want to play college ball, but I’m not sure where at this moment. I just know it has to be a school that can offer me a high-quality education so I can get a good job one day. I would love to play in the NFL or NBA, or run in the Olympics, but eventually I intend to own my own business. I want to be an anesthesiologist—the first practicing doctor in my family—with my own practice.

WHO DO YOU FIND INSPIRATIONAL AND HOW DO YOU FEEL ABOUT SERVING AS AN INSPIRATION TO OTHERS? I love my parents. They’re the people I look up to most. My seven-year-old little cousin, Nehemiah— he emulates everything I do. He’s good at football and basketball like me, and he’s super smart—he can spell words that I can’t even spell. I also have a two-year old sister, which is fun, but sometimes little kids can get on your nerves. I love the way she jumps up and down on the trampoline, though. Her balance is unbelievable! Shortly after Gabriella Arellano was born, she was diagnosed with a hole in her heart, or patent foramen ovale (PFO). A fairly common diagnosis, doctors assured Gabriella’s family that this condition did not pose any serious problems or require any additional follow-up. PFO is a failure of the foramen ovale to close after birth. The foramen ovale is the opening between the atria that prevents blood from going to a fetus’s lungs before birth. According to experts, this closure does not happen in about a quarter of all people. Essentially there are millions of people with PFO who live normal lives, even though blood is leaking from the right atrium to the left. Problems only arise when that blood contains a clot.

Fast-forward 26 years. Gabriella, a former high school cheerleader and member of the track and volleyball team, now has a degree in exercise physiology from the University of Memphis. She was encouraged by a former swim coach to compete in triathlons—which she did, racing the Memphis in May triathlon and a handful of marathons—all while working as a personal trainer. Misfortune struck in the form of a serious car accident, and Gabriella spent time in the hospital. She recovered and went back to life as she knew it—but was soon plagued by migraine headaches and periods of brief paralyzation on the left side of her body. In tune with her body and concern for her health, Gabriella sought medical attention. With the increasing intensity of her migraines and stroke-like symptoms, in addition to tightness and pressure in her chest, Gabriella was diagnosed with everything from anxiety to fluid retention and over-caffeination. Discontent with the dismissal of her symptoms by her doctors, she continued to seek alternate opinions. Gabriella saw a total of 9 different Memphis-area cardiologists over the course of two years. Though she knew something was wrong, she began to give up her quest to find answers about what was causing her symptoms. Meanwhile, back at work, Gabriella had given away three personal training sessions as a promotion. By chance, the recipient of those free sessions, upon learning of Gabriella’s symptoms, referred her to his cardiologist at Vanderbilt University in Nashville. Though her experience with doctors over a two-year period resulted only in frustration and distrust, Gabriella knew her symptoms were not normal and insisted there was something wrong. So she saw the doctor in Nashville. He ran a bubble test—where they inject saline into the bloodstream and track its progression through the body via ultrasound. The test showed the bubbles

of saline were travelling to Gabriella’s brain, indicating there was a portal in which the bubbles could reach the brain. Blood coming from the veins from all over the body enters the vena cava and then the right upper chamber of the heart. From there, blood is pumped into the right ventricle, from where it enters the lungs. Usually, the lungs filter out clots (or in the case of the test, saline bubbles) and oxygenate the blood. The freshly oxygenated blood enters the left atrium, then the left ventricle. From the left ventricle the blood is pumped out into the miles of blood vessels that feed oxygen and nutrients to every cell in our bodies. The bubbles travelling to Gabriella’s brain indicated that her blood was travelling from the right to the left side of the circulation, and directly to the brain, rather than getting filtered out through the lungs. With the indication of such a portal in Gabriella’s heart, her doctor ordered a transesophageal echocardiography, or TEE, which is a minor surgical procedure where the doctor goes in from inside the mouth with a scope to look at the interior side of the heart. It’s the only test that shows exactly what’s currently happening in the heart. The TEE indicated a pin-sized hole in Gabriella’s heart. The doctor diagnosed her with PFO and prescribed blood thinners for the duration—the aim of the drug treatment being to prevent blood clots that can cause strokes. With PFO, the main concern is that the opening or hole can occasionally allow a clot to bypass the lungs, where it would be filtered out and cause a stroke. The “mini-strokes” that Gabriella experienced were TIAs, or transient ischemic attacks. These “warning strokes” produce stroke-like symptoms but no lasting damage. The usual TIA symptoms are the same as those of stroke, only temporary. The short duration of these symptoms and lack of permanent brain injury is the main difference between TIA and stroke. Gabriella returned to work, relieved there was a reason for her symptoms, but still unsatisfied. At 26, she didn’t want to remain on blood thinners for the rest of her life knowing she had a hole in her heart.

While patent foramen ovale (PFO) is a common and generally benign condition found on autopsy in 25% of the population, approximately 40% to 50% of patients younger than age 55 with cryptogenic stroke (CS) have PFO on TEE. Approximately 40% of all strokes are classified as cryptogenic, meaning that the cause is unknown despite an extensive work up. While patent foramen ovale (PFO) is a common and generally benign condition found on autopsy in 25% of the population,

Three months later, Gabriella was training a client when she had a small stroke—more intense than the TIAs she experienced previously. Luckily, that client was a nurse and recognized what was happening and acted quickly, calling an ambulance. Gabriella was admitted to hospital in Memphis, and scheduled surgery with the Vanderbilt doctor who diagnosed her. He performed a PFO closure: closing the hole in her heart with a closure device placed by a catheter threaded through the groin through the veins to the heart. Now Gabriella has none of the prior symptoms, and has since been cleared to continue training and working out at a normal level. She continued to work and train, then went on to have a normal pregnancy (though considered high risk). She had a healthy baby girl whom she has since had tested for PFO by a paediatric cardiologist. “After the closure I felt what it was like to be normal. No more chest pain, TIAs and migraines. It’s a whole different ballgame when you’re dealing with your heart and brain. Sure, it’s ok to tell people this is normal or that it’s really not a big deal because [these symptoms are] a common occurrence. But when you’re getting the feelings of TIAs, chest pain and migraines, it’s a really scary experience.”

approximately 40% to 50% of patients younger than age 55 with cryptogenic stroke (CS) have PFO on transesophageal echocardiography (TEE). This suggests that paradoxical emboli (i.e. venous emboli that gain access to the arterial circulation through a PFO) are a major cause of CS. Thus, many physicians will recommend PFO closure in patients, especially younger patients, who have had CS. Recognizing and treating TIAs can reduce your risk of a major stroke.

Gabriella now focuses on her true love and vocation: helping people stay healthy. As a personal trainer, she specialises in rehab for stroke patients and other clients with cardiac illnesses. She loves what she does and her message is simple: be your own health advocate. Even when she wanted to give up, she persevered and listened to her body—which ultimately kept her alive. Gabriella urges her clients to pay attention to their bodies: awareness is everything. You know your body better than anyone else, and it’s up to you to be aware of anything out of the ordinary. We don’t do crazy diets, but we do watch what we eat and try to eat healthy. We stay away from chips, cookies, sweets, soft drinks, coffee, etc. However, we do treat ourselves on the weekends, especially after a long run! IDEAL DATE NIGHT: We are VERY low key. Sometimes we go to Huey’s or Buckley’s on a Friday night for dinner, then just come home and watch a movie! Pa nk o-C rust e d Sa lmon for Tw o Salmon tends to be the go-to fish for dinner at my house, and there’s good reason for it. Salmon has a very strong flavor that makes it a good match for many other strong flavors, like the rosemary used in this recipe. Salmon is widely available, affordable and loaded with omega-3 fatty acids. It’s also an excellent source of selenium and B-12. No wonder it’s among the many fish that are recommended for twice weekly consumption. Most people I know (including myself) don’t eat enough fish. This recipe is for two, so you can have a lovely romantic and healthy Valentine’s dinner at home this month! (And it’s pink!) I usually like to smoke my salmon, but for this recipe I decided to go for a little more texture in the form of a panko crust. Panko bread crumbs are more coarsely ground than regular store-bought bread crumbs, so they provide a sturdier crunch. I mixed the crumbs with a little hearthealthy olive oil, minced garlic, lemon juice (for an acidic balance) and salt and pepper. I had some beautiful organic rosemary that my friend Melanie brought me (thanks, Melanie!), so I put a few sprigs of it underneath the salmon while it roasted in the oven at a high temperature. By the time the salmon was done, the heady scent of rosemary had permeated through the filets and added a nice warmth to the fish. Any herbs you like could be substituted for the rosemary, especially thyme. If you don’t have fresh herbs, you could always sprinkle about ¼ teaspoon of dried herbs into the panko mixture and just rest the salmon on a few lemon slices in the baking dish.

PANKO-CRUSTED SALMON – – – – – – –

4 sprigs of rosemary or a small bunch of thyme 2 filets of salmon the juice of ½ lemon 4 tablespoons of panko bread crumbs 1 clove of garlic, minced 1 tablespoon olive oil salt and pepper to taste

Preheat the oven to 400 degrees. Make a bed of herbs for the salmon in a small baking dish, and place the fish skin side down on top of the herbs. Squeeze a bit of lemon over the salmon, and squeeze the rest of the lemon juice into a small bowl. Season the salmon with salt and pepper to taste. To the lemon juice, add the panko, garlic and olive oil then salt and pepper to taste. Mix the bread crumbs with the other ingredients until well combined, then spread the mixture over each filet of salmon. Roast the salmon in the oven until it reaches 130 degrees, about twelve minutes. Enjoy!
