Ombudsman of Ontario
Certificates: Birth, Death or Marriage The Office of the Registrar General provides copies of birth, death, and marriage certificates which occurred in the Province of Ontario. If you were born or married in another province or country, you must write to the proper authorities there. There is a cost attached to the issuance of certificates. If you are applying for a social insurance number, passport or for a pension, you need your birth certificate for identification. Certain vacation spots do not require passports but do require a birth certificate—please ensure you check with your travel agent when making your arrangements. You can apply to any M.P.P.’s constituency office for certificates, but you must allow a minimum of 6 to 8 weeks for receipt.

The Ombudsman of Ontario is an officer of the Provincial Legislative Assembly, independent from government and political parties. The Ombudsman investigates complaints about services provided by the Government of Ontario and its organizations. He also conducts systemic investigations on important issues about government services, which can affect large numbers of people. The Ombudsman cannot deal with matters regarding federal or municipal governments (except in the case of closed meetings), courts or judges. The Ombudsman is a place of last resort when other approaches to resolve complaints have been tried. The Ombudsman represents neither the government organization or the person complaining. SERVICES | Government Eligibility for the Old Age Security pension and the Guaranteed Income Supplement The Government of Canada proposes to gradually increase the age of eligibility for the OAS pension and the Guaranteed Income Supplement (GIS) between the years 2023 and 2029, from 65 to 67. People currently receiving OAS benefits will not be affected by the proposed changes. The Government of Canada also proposes to gradually increase the ages at which the Allowance and the Allowance for the Survivor are provided, from 60-64 today to 62-66.
Guaranteed Income Supplement (GIS) People getting the Old Age Security pension who need financial help may be eligible for the Guaranteed Income Supplements (GIS). To find out if you are entitled for GIS you must fill out a GIS application form. An application for GIS is mailed, at your request, once your Old Age Security Pension is approved for payment.
Voluntary deferral of the Old Age Security pension The Government of Canada also proposes a voluntary deferral of the Old Age Security (OAS) pension that will give people the option to defer take-up of their OAS pension by up to five years past the age of eligibility, and subsequently receive a higher, actuarially adjusted pension.
may be eligible for full or partial CPP retirement benefits, provided you have contributed to the plan. A person who has turned 60 years of age may choose to begin receiving CPP benefits at that age. The CPP adjusts the amount of your pension by 0.5% for each month before or after your 65th birthday from the time you begin to receive your pension. The adjustment is permanent. This means that if you choose to start your pension early, the payment does not increase when you reach 65. For example, if you start your pension at 60, your monthly payment is 30% lower than if you wait until you’re 65. However, by starting it sooner, you will likely receive it for a longer time. If you start your pension at 70, your monthly payment is 30% higher than if you had taken it at 65. You may be able to share your Canada Pension Plan benefits with your spouse or common-law partner. You must apply to get these benefits.

The Allowance Benefits A 60 to 64-year-old spouse or common-law partner of a person who is getting OAS may be able to get Allowance Benefits. The spouse or common-law partner must meet residence requirements and the couple’s income must be within set limits. You must fill out an application each year if you still need Allowance Benefits, otherwise payment will be discontinued.
Allowance for Survivors Financial help is offered to the legal or commonlaw spouse; the survivor must be 60 to 64 years old and meet both residence and income rules.

Passports Passports are valid for 5 years now. Effective July 1, 2013, a new 10-year ePassport will be introducd with new fees for 5 and 10-year passports. Persons born in Canada must submit an original birth certificate. Those born outside of Canada must submit an original certificate of citizenship. Unlike the General Application Process, the Simplified Renewal Application Process does not require applicants to submit documentary evidence of citizenship (birth certificate, citizenship card, etc.) or provide supporting identification. The application form is shorter and no guarantor declaration is necessary. The Government of Canada continues to recommend that travellers carry a valid Canadian passport for all visits abroad, including the United States. A passport is the only universally accepted identification document and it proves that you have a right to return to Canada. Canadians are encouraged to plan ahead to avoid peak periods. Canadian citizens require a passport to enter the United States at land ports of entry as of June 1, 2009. Your Community Care Access Centre (CCAC) connects you with the care you need at home and in your communnity by offering information, referral and access to services to help you maintain health, independence and quality of life. Services are available for people of all ages.
The Care Coordinator works closely with individuals, their caregivers and service providers to develop an ongoing care plan which may include a range of services and resources.

Para-Med Home Health Care
The Community Care Access Centre (CCAC) provides one-stop access to home and communitybased health and personal support services to help people live as independently as possible. If a person’s health is such that they cannot remain in their home, a CCAC case manager will help them make the transition to a residential care setting such as a long-term care home.
Para-Med Home Health Care provides in-home health care that maintains and enhances the clients’ independence and quality of life. Full range of services including home support workers and nurses. Services are available 7 days a week, 24 hours a day.

A Living Will is a document that states your wishes regarding consent to or refusal of specific treatment. A Living Will is different from a Last Will and Testament because a Living Will addresses your health care, whereas a Last Will and Testament deals with matters of property. Both documents are essential for every adult. The Living Will allows you to direct your family, physician, and others about your health care choices, based on your own values and beliefs. A completed Living Will should be very specific as to what type of health care choices you wish in end of life situations. An example of what a Living Will addresses includes whether the person wishes to be hooked to a ventilator or respirator. The relationship between grandparents and grandchildren is priceless! This bond is special, providing a sense of heritage and continuity in today’s rapidly changing society. Yet today, factors such as broken marriages, divorce, death and other circumstances often destroy this mutually important relationship. Many grandparents lose access to their grandchildren in which case both the grandparents and children suffer. G.R.A.N.D. works to: • Provide emotional support to grandparents who, for one reason or another, have been denied access to their grandchildren. • Make the public and professionals aware of this ill in our society. • Influence Provincial Family Law to recognize the interest that grandparents have in their grandchildren’s well-being. At the 73 Community Health Centre’s across Ontario, the health and well-being of every individual, every family, every community matters! In our area, four Community Health Centres (CHCs) work closely together to provide a full range of primary health care and health promotion programs and services. They are: Country Roads Community Health Centre (in Portland); Merrickville District Community Health Centre (in Merrickville); the North Lanark Community Health Centre (located in Lanark Village); and the Smiths Falls Community Health Centre in Smiths Falls. Community Health Centres are an effective and unique model of primary health care where an inter-disciplinary team of health care professionals work together to respond to people’s individual health concerns. At all CHC’s in our area, this team includes: doctors, nurse practitioners, nurses, dietitians, social workers, chiropodists, respiratory therapists, health promoters and community developers. In addition to primary care services, CHC’s also offer a wide range of free health promotion, education and prevention programs to address the broader health needs of the people and the communities they serve. In most cases, you do not have to be a client of the CHC in order to access these programs. While each CHC has their own specific group programs based on the unique needs of their community, they often include groups where people: learn how to manage their diabetes, asthma and other chronic conditions; get support to quit smoking; participate in community-based exercise classes, walking groups or yoga classes; learn relaxation, meditation and other techniques for stress reduction; and learn how to cook and purchase healthy foods on a budget.

The purpose of Day Hospital is to improve the client’s general condition, health and independence. Clients receive therapy from
Nurses Physiotherapists, Physiotherapists Occupational Therapists Therapiists t Nurses, and Speech and Language Pathologists. A team of health care professionals work with people who have complicated health care needs. The composition of the team depends on the type of rehabilitation required. The rehabilitation programs are doctor referred and include both neurological and pulmonary. The Pulmonary Rehabilitation Program is an educational and exercise program for people living with COPD. The program runs every Tuesday and Thursday 1:30-3:30 for 8 weeks. The Day Hospital also provides a Heart to Heart Program twice a year for people living with heart disease.

Alzheimer Disease & Related Dementias Alzheimer’s Disease is a degenerative brain disorder that destroys vital brain cells. There is no cure at this time however there are medications and treatments that can slow the progression down. Dementia can affect people as young as 40 years of age. The risk factor for dementia doubles every five years after the age of 65. There are 747,000 Canadians living with a dementia. It is estimated that in the next 20 years there will be close to 1.4 million cases of Alzheimer’s disease or a related dementia in Canada.
Is it Alzheimer’s Disease? 10 warning signs: 1. Memory loss that affects day-to-day function 2. Difficulty performing familiar tasks 3. Problems with language 4. Disorientation of time and place 5. Poor or decreased judgment 6. Problems with abstract thinking 7. Misplacing things 8. Changes in mood or behaviour 9. Changes in personality 10. Loss of initiative There are 2 local chapters of the Alzheimer Society in Lanark, Leeds and Grenville, each providing a variety of services for persons with Alzheimer’s disease and their caregivers. Support & Education is available to individuals who have been diagnosed with Alzheimer’s Disease or a related dementia, their family members and caregivers. We can offer this by phone, email or in person. Support groups are provided in the local communities of Lanark, Leeds and Grenville. The goal of this program is to provide opportunities for socialization and stimulation for individuals who still reside at home, while providing the caregiver with respite relief.
Learning Series and Workshops The Alzheimer Society offers monthly learning series for people who have been diagnosed in the early stages of Alzheimer disease or a related dementia. These series are also for the ones that care for people with the disease and health care professionals. It covers a wide variety of topics such as What is Dementia, Coping Strategies, Legal and Financial Issues, Long Term Care and Home Care. We also offer Workshops, Forums and Expo’s that cover all topics related to Alzheimer’s disease and a related dementia as well as promoting a healthy lifestyle with our Heads Up for a Healthier Brian.

Simple procedures such as nail cutting can usually be performed by caregivers. Elders with diabetes or circulation problems, however, should have a podiatrist or chiropodist attend to their complete foot care. Most Home Support Programs and Para-Med Health Services sponsor weekly footcare clinics. For clients who cannot attend the clinics, agencies can arrange in-home footcare.

SERVICES | Medical Services Geriatric Programs Specialized Geriatrics (SG) (formerly Southeastern Regional Geriatric Program (RGP) Comprehensive, short-term assessment and treatment/rehabilitation for seniors (65+) with complex health needs who are likely to benefit from a multi-disciplinary approach. Services include in-home (including long term care) outreach and community hospital assessment, specialized clinics, a Day Hospital, a Geriatric Rehabilitation In-Patient Unit, and acute care consultation at Kingston General Hospital. All SG clinical services are provided in association with each client’s family physician and covered by OHIP. Referrals accepted from family doctors, other health care professionals, seniors themselves, family members and/or other informal caregivers. Some flags for referral: recent decline in function, more difficulty getting around, recent falls, concern about medications, recent change in ability to cope, several recent admissions to hospital or visits to emergency or family physician, unexplained change in memory or cognition. SERVICES | Medical Services es Palliative Care Brockville & District Hospice Pallative Care Service The goal of the palliative care service is to provide the highest quality of life possible for individuals living with a life-limiting illness. The Hospice Palliative Care Team includes professionals and volunteers who have extensive training in palliative care. The team provides assessment consultations, counseling, advice in pain and symptom management, as well as physical, emotional and spiritual support. The program also provides volunteer visiting and a Day Hospice Program offered on a weekly basis that provides social interaction, therapies, activities and nursing consultations for individuals. Bereavement Support is offered through group or individual counseling for those who have experienced the death of a loved one. The Hospice Palliative Care service offers support at home, in hospital or in a long-term care or retirement facility in Brockville and surrounding area. There is also a six-bed Palliative Care Unit at the Brockville General Hospital Garden Street Site. The service accepts self-referral or referral from a health care professional. Oncology/Palliative Care Age Group: Adult Our Oncology/Palliative staff work in monthly Oncology clinics organized by the Ottawa Regional Cancer Centre and the Kingston Regional Cancer Centre. Leeds & Grenville Volunteer Hospice Service The Hospice Service provides support to patients and families who are coping with a lifechallenging illness. Support is available through Support Groups, the Day Hospice (a weekly day program) and Home Visiting. Tips for Vision Problems Proper Lighting • Ensure bulb wattage is high enough for the particular area. • Have several, evenly distributed light sources around all rooms. • Nightlights should be placed in hallways, the bedroom and the bathroom. • Consistent lighting minimizes shadows. • Dimmer switches and tri-light bulbs will control light intensity. • It is easier if switches are on the outside of the room so that you do not have to fumble in the dark. • Put intense light at the top and bottom of staircases and steps. Eliminate Glare • Use low-gloss wax on furniture and do not wax floors. • Minimize use of fluorescent lights as they produce more glare than incandescent lights. • Blinds or sheer curtains should be used in brightly sunlit rooms. • Shiny furniture or counter surfaces should be removed or covered. • Paint walls with matte-finish paint.

Daily car care: A good daily care routine is essential for maintaining a Hygienic denture. Dentures are made of acrylic(plastic), and can harbor odour and bacteria over time. My experience is that some commercial cleaning solutions can be harsh on a denture so I recommend these solutions for only a 15-20 minute soak, or the length of time it takes to bathe or shower. Removing the denture at night is usually recommended in order to give the gums a much needed rest. The denture should be stored in water during this period or any time it is out of the mouth. FEATURE | Community Home Support Lanark County Community Home Support Lanark County is a not-forprofit, volunteer-based agency for seniors, adults with physical disabilities and persons who have been diagnosed with a life-threatening illness and/or are dying. We strive to provide programs and services that help our clients continue to reside in their own homes and remain a part of their community. Volunteers are the heart and soul of our agency. Without them our programs and services would not exist. New volunteers are always needed. If you can help, please call the office nearest you.
Services offered: • Transportation to medical appointments – Rides to out-of-town medical appointments or essential services are provided by volunteer drivers using their own vehicles. Escorted transportation for local appointments is also available for those who qualify. A nominal cost applies. • Meals On Wheels – Volunteers deliver nutritious and delicious meals right to your home. Most special diets can be accommodated. The service is available up to 7 times per week and can be delivered hot, chilled or frozen. A nominal cost applies. The delivery of the meal provides social contact and a safety check for the client. • Diners Club – Enjoy a delicious and nutritious noon meal and a short social or educational program. A great way to meet old and new friends. Rides are available. • Foot care – A qualified nurse provides professional foot care. No charge. • Home help/home maintenance – This service is a brokerage arrangement offering assistance with housekeeping, homemaking chores, grounds-keeping and home maintenance. The services providers are screened by the agency and the service provider and client determine the fee. • Client intervention and assistance – A support service providing crisis intervention and assistance in critical situations to vulnerable and at risk seniors and adults with physical disabilities and/or their caregivers. After her mother slipped while getting into the bath last year, Mary began losing sleep worrying how her mom, Anne, was managing on her own. Do you often need to turn up the volume on the TV or radio? Do you avoid conversations over the phone because they are just too frustrating and difficult? Hearing loss is the most prevalent chronic healthcare problem in Canada, after hypertension and arthritis. More than 10 percent of Canadians suffer from some degree of treatable hearing loss, and more significantly, 50 percent are over the age of 65. FEATURE | Smiths Falls Audiology Clinic helps all adults with hearing challenges 2. Frequent requests for repetition • Hearing impaired people cannot follow the rapid flow of normal conversation. They may hear parts of sentences and lose the rest.
Audiology is the science of hearing and translating that knowledge through hearing tests, teaching, protective measures and, as the need is established, through rehabilitation with amplifying devices and usage of visual clues.
3. Confusion or inappropriate responses to questions • Individuals have been inappropriately labeled “senile” because of serious hearing problems which made adequate communication impossible.
I had never heard of audiology as a profession until my sister, who was a guidance counselor at a high school in Montreal at the time, told me about a professor, Dr. Dan Ling, who had come to give a lecture about the School of Human Communication Disorders at McGill University. It was a program leading to a Master’s degree in Applied Sciences. Almost immediately, I obtained an interview with the head of the program, Dr. Donald Doehring, who reviewed my Bachelor’s degree courses and said that I would be accepted if I did a year of pre-requisite psychology courses involving courses at a Bachelor’s level from year one through four. I pursued this at the University of Toronto and then returned to McGill to study for my Master’s degree. I had superlative teachers along the way, academically and then in the work place, in rehabilitative settings, in large city hospitals, working with general medical practitioners, ear nose and throat specialists and was given enormous support from acoustical engineers and hearing aid manufacturers as well as hearing scientists, colleagues, private industry, District Health Units, and family.
4. Monopolizing the conversation • It is easier for the hearing impaired patient to do the talking as this lessens the burden of trying to listen. 5. Imperfect speech • Sounds in speech which cannot be heard may begin to deteriorate. With presbycusis, high frequency hearing loss occurs first and sounds like “s’ or “sh” may be slurred or dropped completely. 6. Non-verbal responses to questions • This may take the form of nodding or shrugging. The patient may be guessing or pretending to understand in an attempt to conceal a hearing deficit. continued...

continued from previous page. 7. Complaints of noises or ringing in the ears • Tinnitus is almost always a sign of hearing loss. It is also the initial complaint of people with acoustic neuromas. It can also occur as a side effect of medications. 8. Complaints of plugging or pressure in the ears • May be a sign of Eustachian tube dysfunction, middle ear fluid, cerumen, or less commonly, endolyphatic hydrops or acoustic neuroma. 9. Comments that others are mumbling • Patients with hearing loss hear speech at a low level with many of the sounds missing. Sounds are lost at an unequal rate because hearing loss occurs in different amounts at different frequencies. 10. Complaints of difficulty using the telephone • There may be difficulty understanding speech or even hearing the telephone ring.
11. Recent disinterest in social situations • Avoiding parties, abandoning bridge clubs or church activities may occur as noise levels make understanding too difficult. 12. Expressed disinterest in television • Patients may indicate that there is nothing worthwhile watching when, in fact, they mean that they cannot understand anything they want to watch. Patients exhibiting any of these signs or behaviours may have hearing loss and should be referred for audiology tests. The ear is an intricate system and the more information gleaned from testing, the easier the task becomes for a doctor to rule out certain difficulties medically and focus in on the actual site of the problem. This involves sophisticated equipment and procedures, not possible with a simple screening.
