Community Health Centers: A Key to Accessing Affordable Care

At PCC Community Wellness Center, we are more than just a healthcare provider—we are a vital part of the communities we serve. As a Federally Qualified Health Center (FQHC), also known as Community Health Center, we are deeply committed to providing high-quality, accessible, and compassionate care to underserved populations. Our role as a Community Health Center goes beyond medical treatment; we are woven into the fabric of the communities of Chicago, Oak Park, Berwyn, and Melrose Park, working alongside local residents to address their healthcare needs and improve overall well-being.

In 2024, FQHCs nationwide served 32.5 million individuals, providing essential care to low-income, uninsured, and Medicaid-insured populations. By focusing on primary care and prevention, these centers significantly reduce the need for costly emergency and hospital-based services, saving billions annually. PCC Community Wellness Center contributes to this vital work, meeting the unique needs of the communities we serve through programs that focus on early intervention, wellness, and access to care.

What we do

COMPREHENSIVE SERVICES FOR HEALTH AND WELLNESS AT PCC

At PCC Community Wellness Center, we offer a wide range of services designed to meet the diverse needs of our community. Our family medicine team provides comprehensive care for individuals of all ages, ensuring that every member of the family receives the medical attention they need. Our perinatal and women’s health services offer specialized care to pregnant and parenting people throughout every stage of life, from prenatal care to reproductive health. We also offer pediatric and children’s health services, ensuring that our youngest patients receive the best care from infancy through adolescence.

In addition to primary care, PCC is proud to provide essential behavioral health services, including counseling and mental health support, as well as the Austin Recovery Connection to help patients navigate recovery journeys. Our dental services ensure that our community has access to quality oral healthcare, while our specialty care offerings—such as our diabetes programming and newly introduced cardiology services—address more complex health needs. With a holistic approach that integrates physical, mental, and social health, we are committed to improving health outcomes and empowering individuals to take control of their well-being.
