Honors And Awards
We believe that students’ efforts should be recognized. Honors and awards are given four times a year to students who work to their ability and achieve good grades. We also recognize students for consistent good behavior, for outstanding effort, cooperation and punctuality.
Honors Assemblies are held at the end of each quarter. The following awards are presented:
Primary Achievement Award
This award is given to students in grades 1-3 whose grades are “O” or “S” with adequate effort.
Intermediate and Junior High Awards
First Honors Straight A’s
Second Honors More A’s than B’s or equal A’s and B’s
Third Honors More B’s than A’s A student may have two C’s if they have two A’s to replace them.
Cooperation Award
The Cooperation Award is given by the classroom teacher to students for outstanding cooperation in the classroom and school in the quarter.
Effort Award
The Effort Award is given to students who have shown outstanding effort to improve their schoolwork and/or conduct in the quarter.
Punctuality Award
The Punctuality Award is given to students who are seated and ready to begin the school day at 8:00 every day in the quarter.
Principal’s Award
At the end of the school year, the Principal’s Award is given to students who remained on the Honor Roll for all four (4) quarters of the school year.
Outstanding Peace Builder Award
At the end of the school year, an Outstanding Peace Builder Award is given to students who maintain an “O” grade in conduct for all four (4) quarters of the school year.
Scholar Athlete Award
The Scholar Athlete Award is presented to students who are on the honor roll for academic honors and who participate in athletics.
Art, Music, Computer, Physical Education and Spanish Awards
At the end of the school year, a special award is presented to a male and female student in each grade for their hard work, contribution to and cooperation in these special classes.
Service Award
At the end of the school year, the Service Award is presented to students for outstanding service to the school community on a regular basis throughout the school year.
Student of the Year Award
Each year, an eighth grade student is chosen to receive the Student of the Year Award. The students are nominated by the eighth grade class and chosen by the collaborative decision of the eighth grade teacher and the administration.
The criteria for this award is as follows:
● Academic excellence
● Positive contribution to the St. Francis School community
● Witness of Christian spirit
● Positive role model to other students
Notre Dame Distinguished Graduate Award
The Notre Dame Distinguished Graduate Award is given by the Sisters of Notre Dame throughout the world to a graduating eighth grade student who gives the most authentic witness to the cornerstones of Notre Dame education. A Notre Dame education is marked by:
● Academic excellence
● Respect for the dignity of the teacher
● Respect for the dignity of the student
● The centrality of religion
● Witness to Christian living and outreach to those in need
Preschool addendum: How to obtain copies of inspections for preschool
Copies of inspection reports may be obtained at anypoint by requesting them from the school office or directly from Mrs. Grace at gracec@stfranciscleveland.org. They may also be observed at any point in viewing the family communication boards.

HealthcheckServicesforChildrenYoungerthanAge21
Healthchek is Ohio's Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program. It is a service package for babies, kids, and young adults younger than age 21 who are enrolled on Ohio Medicaid.
The purpose of Healthchek is to discover and treat health problems early. If a potential health problem is found, further diagnosis and treatment are covered by Medicaid.
Healthchek covers ten check-ups in the first two years of life and annual check-ups thereafter and offers a comprehensive physical examination that includes:
§ medical history
§ complete unclothed exam (with parent approval)
§ developmental screening (to assess if child's physical and mental abilities are age appropriate)
§ vision screening
§ dental screening
§ hearing assessment
§ immunization assessment (making sure child receives them on time)
§ lead screening; and
§ other services or screenings as needed
If your children are enrolled on Ohio Medicaid, Healthchek services are available to them. If you are younger than age 21 and are also enrolled, you can receive Healthchek services, too.
Management Of Communicable Disease
Preschool staff is trained on the management of communicable diseases.
The following precautions shall be taken for children suspected of having a communicable disease:
1. Immediately notify the parent or guardian of the child's condition when a child has been observed with signs or symptoms of illness.
2. A child with any of the following signs or symptoms of illness shall be immediately isolated and discharged to his parent or guardian: (a) Diarrhea (more than one abnormally loose stool within a twenty-four-hour period); (b) Severe coughing, causing the child to become red or blue in the face or to make a whooping sound; (c) Difficult or rapid breathing; (d) Yellowish skin or eyes; (e) Conjunctivitis; (f) Temperature of one hundred degrees Fahrenheit taken by the auxiliary method when in combination with other signs of illness; (g) Untreated infected skin patch(es); (h) Unusually dark urine and/or grey or white stool; or (i) Stiff neck; or (j) Evidence of lice, scabies, or other parasitic infestation.
3. A child with any of the following signs or symptoms of illness shall be immediately isolated from other children. Decisions regarding whether the child should be discharged immediately or at some other time during the day shall be determined by the director and the parent or guardian. The child, while isolated at the program, shall be carefully watched for symptoms listed in (2) of this rule as well as the following: (a) Unusual spots or rashes; (b) Sore throat or difficulty in swallowing; (c) Elevated temperature; or 32 (d) Vomiting
4 St Francis shall follow the Ohio department of health “communicable disease chart" (September 2009, odjfs.state.oh.us/forms) for appropriate management of suspected illnesses.
5. A child isolated due to suspected communicable disease shall be: (a) Cared for in a room or portion of a room not being used in the preschool program; (b) Within sight and hearing of an adult at all times. No child shall ever be left alone or unsupervised; (c) Made comfortable and provided with a cot. All linens and blankets used by the ill child shall be laundered before being used by another child. After use, the cots shall be disinfected with an appropriate germicidal agent, or, if soiled with blood, feces, vomit, or other body fluids, the cots shall be cleaned with soap and water and then disinfected with an appropriate germicidal agent; (d) Observed carefully for worsening condition; and (e) Discharged to parent, guardian, or person designated by the parent or guardian as soon as practical.
6. If a student has been isolated/sent home, they may not return to the program for 24 hours unless a doctor’s note is presented.
7. Parents of children who are mildly ill will be notified and are at liberty to decide whether they would like to pick their child up. No note is needed to return to the program. "Mildly ill child" means a child who is experiencing minor common cold symptoms, but who is not exhibiting any of the symptoms specified in signs and symptoms of isolated illness (2) or a child who does not feel well enough to participate in activities, but who is not exhibiting any of the symptoms specified in signs and symptoms of isolated illness (2).
